Late Regularization of the Alveolar Ridge in a Patient with a Prosthesis and Special Needs: A case Report

Residual ridge shape is important for the seating of a prosthesis. The objective of this case report was to illustrate the importance of reduction maneuvers with regard to alveolar bone plates after multiple tooth extractions in patients with prostheses and special needs. In the current patient, there were difficulties during the adaptation and fitting of the upper denture. Bilateral bone resection was indicated in the molar buccal region. Late regularization of the alveolar ridge, which was accomplished in hospital, was necessary for the preparation, adaptation, and use of the denture. The general and systemic health care of the patient should be monitored throughout surgical intervention. Though tooth extractions are common procedures, particular attention is required to ensure adequate alveolar ridge remodeling that is conducive to oral rehabilitation.

The prevalence of torus mandibularis, Eagle’s syndrome and parafunctional activity was reported to be higher in patients with temporomandibular joint dysfunction and might be useful as an indicator of increased risk of temporomandibular disorders. This study case report was to evaluate a 62-year-old female patient, with limited mandibular and cervical movements, articular and muscular pain, ear fullness, irritation of the lingual mucosa, discomfort associated with movement of her tongue, involving temporomandibular dysfunction, torus mandibularis and Eagle’s syndrome. The treatment comprehended the collection of data of Helkimo’s clinical Dysfunction Index, electromyographic activity of the masseter and temporalis muscles in rest position and bite force, masticatory efficiency, speech evaluation and oclusal splint, after the excision of the stylohyoid process and mandibularis torus. The authors found a decrease in electromyographic activity of masticatory muscles, a reduction of painful symptoms throughout the region muscular and joint pains, an increase in the amplitude of mandibular movements, an improvement in masticatory efficiency and an increase in bite force.

Zygomatic Implant-Supported Prosthesis: When the Prosthetic Rehabilitation Affects the Function: A case Report

The oral rehabilitation of edentulous patients can be done in different ways depending on the alveolar ridge morphology and patient expectations. The objective of this clinical case was to report prosthetic and functional adaptation difficulties when conventional complete denture was changed to a zygomatic implant-supported prosthesis. We report a 52-year-old male singer with an atrophic maxilla who was rehabilitated with zygomatic implant-supported prosthesis using 4 implants. However, the thickness of the prosthesis in the palatal region and the space between the prosthesis and soft tissue caused difficulty in speaking and singing. The palatal region of the prosthesis was trimmed and the anterior region of the prosthesis was relined. These procedures were performed to make room for the tongue and minimize the passage of air during speech. Adaptations in the shape of the prosthesis must be made to enable its use, without compromising its strength.
