Objective： To investigate the anti-oxidative stress and preventive effect of modified Gongjin-dan （WSY-1075） in a detrusor underactivity rat model. Methods： Rats were randomly allocated to three groups： sham- operated （control）, bladder outlet obstruction-induced detrusor underactivity （BOO-DU）, and BOO-DU with WSY-1075 （WSY） groups. WSY-1075 was orally administrated to rats 200 mg daily for 2 weeks pdor to the operation and 4 weeks after the operation. Bladder outlet obstruction was surgically induced in rats by ligation around the urethra avoiding total obstruction. Cystometrography was conducted on rats in each group for examination of bladders. Results： Compared with the control group, bladder outlet obstruction led to a significant increase in oxidative stress with consequent changes to molecular composition, and decrease in maximal detrusor pressure （P〈0.05）. WSY-1075 treatment significantly suppressed oxidative stress and prevented degenerative and dysfunctional changes in bladder, as compared with BOO-DU group （P〈0.05）. Conclusion： WSY-1075 had beneficial effect on prevention of BOO-DU.

Aim:To study the effect of arecoline,an alkaloid isolated from Areca catechu,onthe secretion of catecholamines (CA) evoked by cholinergic agonists and themembrane depolarizer from isolated perfused rat adrenal gland.Methods:Adrenalglands were isolated from male Sprague-Dawley rats.The adrenal glands wereperfused with Krebs bicarbonate solution by means of a peristaltic pump.The CAcontent of the perfusate was measured directly using the fluorometric method.Results:Arecoline (0.1-1.0 mmol/L) perfused into an adrenal vein for 60 min pro-duced dose-and time-dependent inhibition in CA secretory responses evoked byacetylcholine (ACh) (5.32 mmol/L),1.1-dimethyl-4-phenyl piperazinium iodide(DMPP) (100μmol/L for 2 min) and 3-(m-choloro-phenyl-carbamoyl-oxy)-2-butynyltrimethyl ammonium chloride (McN-A-343)(100μmol/L for 2 min).However,lowerdoses of arecoline did not affect CA secretion of high K~+(56 mmol/L);higherdoses greatly reduced CA secretion of high K~+.Arecoline also failed to affectbasal catecholamine output.Furthermore,in adrenal glands loaded with arecoline(0.3 mmol/L),CA secretory response evoked by Bay-K-8644 (10μmol/L),an activa-tor of L-type Ca2+channels,was markedly inhibited,whereas CA secretion bycyclopiazonic acid(10μmol/L),an inhibitor of cytoplasmic Ca2+-ATPase,was notaffected.Nicotine (30μmol/L),which was perfused into the adrenal gland for 60min,however,initially enhanced ACh-evoked CA secretory responses.As timeelapsed,these responses became more inhibited,whereas the initially enhancedhigh K~+-evoked CA release diminished.CA secretion evoked by DMPP and McN-A-343 was significantly depressed in the presence of nicotine.Conclusion:Arecoline dose-dependently inhibits CA secretion from isolated perfused rat ad-renal gland evoked by activation of cholinergic receptors.At lower doses arecolinedoes not inhibit CA secretion through membrane depolarization,but at largerdoses it does.This inhibitory effect of arecoline may be mediated by blocking thecalcium influx into the rat adrenal medullary chromaffin ce

Although a number of feasibility studies for sentinel node （SN） concepts in gastric cancer have been conducted since 2000, there remains a debate regarding detailed detection techniques and oncological safety. Two important multicenter phase ii clinical trials were performed in Japan that used different methods and reached different conclusions; one confirmed acceptable results with a false-negative rate of 7%, and the other showed an unacceptably high false-negative rate of 46.4%. The Sentinel Node Oriented Tailored Approach （SENORITA） trial is a multicenter randomized controlled phase 1/I trial being performed in Korea. Patient enrollment is now complete and the long-term results are currently awaited. Recently, an image-guided SN mapping technique using infrared ray/fluorescence was introduced. This method might be a promising technology because it allows the clear visualization of SNs. With regard to the primary tumor, the non-exposed endoscopic wall-inversion surgery technique and non-exposure endolaparoscopic full-thickness resection with simple suturing technique have been reported. These methods prevent abdominal infection and tumor seeding and can be good alternatives to conventional laparoscopic gastric wedge resection. For indications, SN navigation surgery can be extended to patients who underwent non-curative endoscopic resection. Although a few studies have been performed on these patients, sentinel concepts may be beneficial to patients as they omit the need for additional gastrectomy. SN navigation surgery can lead to actual organ-preserving surgery and plays a key role in improving the quality of life of patients with early gastric cancer in the future.

Intestinal endometriosis:Diagnostic ambiguities and surgical outcomes

BACKGROUND Endometriosis is a common disease for women of reproductive age. However,when it involves intestines, it is difficult to diagnose preoperatively because its symptoms overlap with other diseases and the results of evaluations can be unspecific. Thus it is important to know the clinical characteristics of intestinal endometriosis and how to exactly diagnose.AIM To analyze patients in whom intestinal endometriosis was diagnosed after surgical treatments, and to evaluate the clinical characteristics of preoperatively misdiagnosed cases.METHODS We retrospectively reviewed the pathologic reports of 30 patients diagnosed as having intestinal endometriosis based on surgical specimens between January2000 and December 2017. We reviewed their clinical characteristics and surgical outcomes.RESULTS Twenty-three(76.6%) patients showed symptoms associated with endometriosis,with dysmenorrhea being the most common(n = 9, 30.0%). Thirteen patients(43.3%) had a history of pelvic surgeries. Ten patients(33.3%) had a history of treatment for endometriosis. Only 4 patients(13.3%) had a diagnosis of endometriosis based on endoscopic biopsy findings. According to preoperative evaluations, 13 patients(43.3%) had an initial diagnosis of pelvic endometriosis and 17 patients(56.6%) were misdiagnosed as having other diseases. The most common misdiagnosis was submucosal tumor in the large intestine(n = 8, 26.7%),followed by malignancies of the colon/rectum(n = 3, 10.0%) and ovary(n = 3,10.0%). According to the Clavien-Dindo classification, 5 complications were grade I or ii and 2 complications were grade iiIa. The median follow-up period was 26.9(0.6-132.1) mo, and only 1 patient had a recurrence of endometriosis.CONCLUSION Intestinal endometriosis is difficult to diagnose preoperatively because it mimics various intestinal diseases. Thus, if women of reproductive age have ambiguous symptoms and signs with nonspecific radiologic and/or endoscopic findings,intestinal endometriosis should be included in the differential diagnosis.

Transient electronics represent an emerging class of technology comprising materials that can vanish in a controlled manner in response to stimuli. In contrast to conventional electronic devices that are designed to operate over the longest possible period, transient electronics are defined by operation typically over a short and well-defined period; when no longer needed, transient electronics undergo self-deconstruction and disappear completely. In this work, we demonstrate the fabrication of thermally triggered transient electronic devices based on a paper substrate, specifically, a nitrocellulose paper. Nitrocellulose paper is frequently used in acts of magic because it consists of highly flammable components that are formed by nitratil^g cellulose by exposure to nitric acid. Therefore, a complete and rapid destruction of electronic devices fabricated on nitrocellulose paper is possible without producing any residue （i.e., ash）. The transience rates can be modified by controlling radio frequency signal-induced voltages that are applied to a silver （Ag） resistive heater, which is stamped on the backside of the nitrocellulose paper. The Ag resistive heater was prepared by a simple, low-cost stamping fabrication, which requires no harsh chemicals or complex thermal treatments. For the electronics on the nitrocellulose paper substrate, we employed semiconducting carbon nanotube （CNT） network channels in the transistor for superior electrical and mechanical properties.

Highly immunosuppressive tumor microenvironment containing various protumoral immune cells accelerates malignant transformation and treatment resistance.In particular,tumor-associated macrophages(TAMs),as the predominant infiltrated immune cells in a tumor,play a pivotal role in regulating the immunosuppressive tumor microenvironment.As a potential therapeutic strategy to counteract TAMs,here we explore an exosome-guided in situ direct reprogramming of tumor-supportive M2-polarized TAMs into tumor-attacking M1-type macrophages.Exosomes derived from M1-type macrophages(M1-Exo)promote a phenotypic switch from anti-inflammatory M2-like TAMs toward pro-inflammatory M1-type macrophages with high conversion efficiency.Reprogrammed M1 macrophages possessing protein-expression profiles similar to those of classically activated M1 macrophages display significantly increased phagocytic function and robust cross-presentation ability,potentiating antitumor immunity surrounding the tumor.Strikingly,these M1-Exo also lead to the conversion of human patient-derived TAMs into M1-like macrophages that highly express MHC class ii,offering the clinical potential of autologous and allogeneic exosome-guided direct TAM reprogramming for arming macrophages to join the fight against cancer.

Pancreatic adenocarcinoma(PDAC)is one of the most common and lethal human cancers worldwide.Surgery followed by adjuvant chemotherapy offers the best chance of a long-term survival for patients with PDAC,although only approximately 20%of the patients have resectable tumors when diagnosed.Neoadjuvant chemotherapy(NACT)is recommended for borderline resectable pancreatic cancer.Several studies have investigated the role of NACT in treating resectable tumors based on the recent advances in PDAC biology,as NACT provides the potential benefit of selecting patients with favorable tumor biology and controls potential micro-metastases in high-risk patients with resectable PDAC.In such challenging cases,new potential tools,such as ct-DNA and molecular targeted therapy,are emerging as novel therapeutic options that may improve old paradigms.This review aims to summarize the current evidence regarding the role of NACT in treating non-metastatic pancreatic cancer while focusing on future perspectives in light of recent evidence.

The sickness is characterized by fever cough, and trouble breathing, and its severity can range from mild to severe to fatal. It is primarily transmitted through the respiratory droplets produced by an infected person when they speak, cough, or sneeze, but it can also be transmitted through contact with contaminated surfaces [2].
