Background Central blood pressure （BP） is pathophysiologically more important than peripheral BP for the pathogenesis of cardiovascular disease. Arterial stiffness is also a good predictor of cardiovascular morbidity and mortality. The effects of benidipine, a unique dual L-IT-type calcium channel blocker, on central BP have not been reported. This study aimed to compare the effect of benidipine and Iosartan on the central BP and arterial stiffness in mild to moderate essential hypertensives. Methods This 24 weeks, multi-center, open label, randomized, active drug comparative, parallel group study was designed as a non-inferiority study. The eligible patients （n=200） were randomly assigned to receive benidipine （n=101） or Iosartan （n=99）. Radial artery applanation tonometry and pulse wave analysis were used to measure the central BP, pulse wave velocity （PWV） and augmentation index （AIx）. We also measured the metabolic and inflammatory markers. Results After 24 weeks, the central BP decreased significantly from baseline by （16.8±14.0/10.5±9.2） mmHg （1 mmHg =0.133 kPa）（systolic/diastolic BP; P 〈0.001） in benidipine group and （18.9±14.7/12.1±10.2） mmHg （P 〈0.001） in Iosartan group respectively. Both benidipine and Iosartan groups significantly lowered peripheral BP （P 〈0.001） and AIx （P 〈0.05）, but there were no significant differences between the two groups. The mean aortic, brachial and femoral PWV did not change in both groups after 24-week treatment. There were no significant changes of the blood metabolic and inflammatory biomarkers in each group. Conclusion Benidipine is as effective as Iosartan in lowering the central and peripheral BP, and improving arterial stiffness.

Gastric cancer(GC) is a highly heterogeneous disease with multiple cellular types and poor ***, the cellular evolution and molecular basis of GC at the individual intra-tumor level has not been well demonstrated. We performed single-cell whole exome sequencing to detect somatic singlenucleotide variants(SNVs) and significantly mutated genes(SMGs) among 34 tumor cells and 9 normal cells from a patient with GC. The Complete Prediction for Protein Conformation(CPPC) approach directly predicting the folding conformation of the protein 3D structure with Protein Folding Shape Code, combined with functional experiments were used to confirm the characterization of mutated SMGs in GC cells. We identified 201 somatic SNVs, including 117 non-synonymous mutations in GC cells. Further analysis identified 24 significant mutated genes(SMGs) in single cells, for which a single amino acid change might affect protein conformation. Among them, two genes(CDC27 and FLG) that were mutated only in single cells but not in the corresponding tumor tissue, were recurrently present in another GC tissue cohort, and may play a potential role to promote carcinogenesis, as confirmed by functional characterization. Our findings showed a mutational landscape of GC at intra-tumor level for the first time and provided opportunities for understanding the heterogeneity and individualized target therapy for this disease.

Impacts of Pig Manure-Based Liquid Fertilizer Agricultural Application on the Water Quality of Agricultural Catchment

This study was conducted to understand the effect of the livestock liquid fertilizer treatment at field-scale up to catchment-scale on the water quality properties. Cultivated paddy rice and upland plots located in Gyeonggi province, Korea were treated with two different liquid fertilizers, SP (Liquid fertilizer with storage process) and SCB (Liquid fertilizer with slurry composting and bio-filtration process). Plots with no fertilizer (control A) and chemical fertilizer (control B) were also prepared for comparison. Water quantity and quality were monitored at the catchment outlet for assessing the effect on water quality of stream water. As a result, the losses of N and P and the values of EC (Electronic Conductivity) in the surface drainage water from paddy rice plots treated with SP and SCB were higher than those from the control plots (A and B). In addition, the losses of N and P by the runoff water from upland plots with SP and SCB treatment were higher than those from control plots (A and B). The nutrient outflow from paddy rice fields and uplands with application of liquid pig manure was higher than those from the control plots (A and B). Particularly, the outflow from uplands may directly affect the water quality in neighboring streams. This caused the major eutrophication problem in stream water. In conclusion, it is necessary to establish the proper management practices to prevent the nutrient losses from agricultural fields and the pollutants against water environments.

Neutron Irradiation of Electronic Materials at HANARO

HANARO, a 30 MW open-pool type multipurpose research reactor, has been operated as a platform for nuclear researches in Korea, and irradiation facilities have been mainly utilized for various nuclear material irradiation tests requested by users. Although most irradiation tests have been related to national R&D relevant to nuclear power, demand for neutron irradiation of electro-magnetic materials is increasing rapidly at HANARO. Another research reactor, which is named the KIJANG research reactor (KJRR), is under construction in Korea. KJRR is dedicated to increasing the national radio-isotopes supply capacity and to the irradiation facilities including Neutron Transmutation Doping (NTD) facilities for power semiconductor production in a large scale and fast neutron irradiation (FNI) facility for fast neutron irradiation. The NTD and FNI facilities in the KJRR reactor can be effectively utilized on the study of separated effect of thermal and fast neutron irradiations on the properties of electro-magnetic materials. HANARO will also specialize more on irradiation research, including neutron irradiation of electronic materials. The research status and possibility of new electro-magnetic materials using neutron irradiation at HANARO are surveyed to ascertain the utilization of neutron irradiation technology in electro-magnetic material researches.

Foreign bodies in the colon are encountered with increasing frequency, but only sporadic reports concerning their management have appeared in the literature. While most ingested foreign bodies usually pass through the gastrointestinal tract uneventfully, sharp foreign bodies such as toothpicks infrequently cause intestinal perforation and may even result in death. We report our experience with a patient with a sigmoid colon pseudodiverticulum formation, a complication of accidental ingestion of a toothpick that was diagnosed and successfully managed colonoscopically.

Differentiation of acute and chronic hepatitis B in IgM antiHBc positive patients

AIM: To identify the factors that differentiate acute hepatitis B(AHB) from chronic hepatitis B with acute exacerbation(CHB-AE).METHODS: From 2004 to 2013, a total of 82 patients(male n = 52, 63.4%; female n = 30, 36.6%) with clinical features of acute hepatitis with immunoglobulin M antibodies to the hepatitis B core antigen(Ig M antiHBc) were retrospectively enrolled and divided into two groups; AHB(n = 53) and CHB-AE(n = 29). The AHB group was defined as patients without a history of hepatitis B virus(HBV) infection before the episode and with loss of hepatitis B surface antigen within 6 mo after onset of acute hepatitis. Biochemical and virological profiles and the sample/cutoff(S/CO) ratio of Ig M anti-HBc were compared to determine the differential diagnostic ***: The multivariate analysis demonstrated that, the S/CO ratio of Ig M anti-HBc and HBV DNA levels were meaningful factors. The S/CO ratio of Ig M anti-HBc was significantly higher in the AHB group, while the HBV DNA level was significantly higher in the CHB-AE group. The optimal cutoff values of Ig M anti-HBc and HBV DNA levels for differentiating the two conditions were 8 S/CO ratio and 5.5 log10 IU/m L, respectively. The sensitivity and specificity were 96.2% and 89.7% for the S/CO ratio of Ig M anti-HBc and 81.1% and 72.4% for HBV DNA levels, respectively. The area under receiver operating characteristic curves of both the S/CO ratio of Ig M anti-HBc and HBV DNA levels were not significantly different(0.933 vs 0.844, P = 0.105). When combining Ig M anti-HBc and HBV DNA, the diagnostic power significantly improved compared to HBV DNA alone(P = 0.0056). The combination of these factors yielded a sensitivity and specificity of 98.1% and 86.2%, ***: The combination of the S/CO ratio of Ig M anti-HBc and HBV DNA levels was a useful tool for differentiating AHB from CHB-AE in patients with positive Ig M anti-HBc.

＜正＞Purpose : One of the most important prognostic factors in colorectal cancer is lymph node metastasis, which predicts a reduced survival time. Although lymph node metastases were not detected by a conventional hematoxylin-eosin stain technique, 20 to 30 percent of patients fail long-term survival on account of a local or systemic recurrence. Recurrent disease in these patients is believed to develop from occult tumor in lymph nodes. We have conducted an immunohistochemical study with two different antibodies against cytokeratin to identify occult micrometastases in lymph nodes which were diagnosed as tumor negative by conventional histopathology.

Efficacy of Simple Assessment System of Oocyte Maturity in IVF-ET Cycles

Objective To establish and evaluate the efficacy of the simple assessment system of oocyte maturity. Methods A total of 251 couples were enrolled in this study and female patients were divided into two groups. In group Ⅰ, oocytes were inseminated at 6 h after ovum pick-up. In group Ⅱ, oocyte maturity was rapidly categorized by simple assessment system. Mature oocytes were inseminated at 3-4 h after ovum pick-up when oocyte-corona complexes （OCC） exhibited clear thick ring-like halo （RLH） and expanded cumulus cells （CC） or 5-6 h when OCC exhibited RLH and a few clumped and/or dark CC, respectively. Intermediate oocytes were inseminated at 7-8 h when RLH was not found around the OCC and CC were compacted and clumped and/or dark. Results Normal fertilization rate was higher in group Ⅱ （76.5%） than that in group Ⅰ （58.0%）（P〈0.001）. However, abnormal fertilization rate was higher in group Ⅰ（11.3%） than that in group 11 （3.6%）（P〈0.001）. The cleavage （82.6% vs 90.0%）, chemical pregnancy （4. 8% vs 3.9%）, twin pregnancy （6. 7% vs 3.9%） and implantation rate （8.4% vs 10.6%） had no statistically differences between group I and Ⅱ. Rate of clinical and singleton pregnancy was higher in groupⅡ （35.3% and 31.4%） than those in group Ⅰ （24.8% and 18.2%）（P〈0.05）. Conclusion This simple assessment system is useful and effective for evaluation and categorization of the oocyte maturity with better reproductive outcomes.
