Survival improvement and prognostic factors in recent management of extrahepatic cholangiocarcinoma:A single-center study

Background:Cholangiocarcinoma was considered as a dismal disease with very poor prognosis until *** is increasingly found due to increased life *** surgical and medical management were advanced recently,data on the prognosis,especially extrahepatic cholangio-carcinoma(ECC),were *** study aimed to identify clinicopathologic features and prognosis of patients with ***:Patients followed up and diagnosed with ECC between January 2014 and December 2016 at a tertiary hospital were included,whereas those with intrahepatic cholangiocarcinoma,gallbladder cancer,and ampullary cancer were ***:A total of 83 patients were followed after the treatment(49 men and 34 women;median age 73.3 years).Cancer location was classified as distal common bile duct(25 patients),proximal com-mon bile duct(24 patients),common hepatic duct(20 patients),and hilar(14 patients).About 14.5%of patients had history of another malignant neoplasm,and 24.1%patients had chronic ***-cal resection was performed in 54 patients(65%)and dysplasia was combined in 63%(34/54).Adjuvant chemotherapy was performed in 54%(29/54),but only 7 underwent palliative chemotherapy in 29 non-surgical *** median overall survival in all patients was 30.9 *** analyzing the treatment modality,median survival of adjuvant chemotherapy,surgery only,palliative chemotherapy,and support-ive care groups were 42.9,30.9,12.0,and 8.9 months,respectively(P<0.05).In the Cox regression analy-sis of survival,age,surgical resection,chemotherapy,and comorbidity were significant prognostic factors,and the comorbidity was the only significant prognostic factor in the multivariable analysis(hazard ratio[HR]=2.80;95%CI:1.32–5.95;P=0.007).In a subgroup analysis of surgical patients,the presence of dysplasia was a favorable prognostic factor in the multivariable analysis(HR=0.29;95%CI:0.09–0.91;P=0.033).Conclusions:The overall survival of patients with ECC wa

A unique and well-designed 2D graphitic carbon nitride with sponge-like architecture for enhanced visible-light photocatalytic activity

Preventive effect of Angelica gigas Nakai extract oral administration on dry eye syndrome

Effects of corner modifications on wind loads and local pressures on walls of tall buildings

In this study, the aerodynamic characteristics of tall buildings with corner modifications (e.g., local wind force coefficients, mean pressure distributions, normalized power spectrum density, and extreme local pressure) were examined. Wind tunnel experiments were conducted to measure the wind pressures on building models with different heights and recessed corners of different ratios. At a wind direction of a = 0° (i.e., wind blowing on the front of a building), corner modifications effectively reduced wind forces in all cases. Specifically, small corner modification ratios reduced wind forces more effectively than their larger counterparts. However, corner modifications resulted in extreme local pressure on building surfaces. In addition, for small corner modification ratios, the probability of extreme local pressure occurring at a = 0° was high. This probability was also high for large corner modification ratios at a = 15° (i.e., wind blowing slightly obliquely on the front of a building) because wind blowing obliquely creates substantial vortex shedding on one side surface and extreme negative pressure over one building side surface. Results of computational fluid dynamic modeling were adopted to determine details of the aerodynamic characteristics of tall buildings with corner modifications.

Seafood,especially sea cucumbers and oysters are an expensive delicacy in several Asian *** sun-drying of these products takes 3 to 4 days and fetches a lower market *** industrial drying machines used for seafood drying are unable to dry sea cucumbers and oysters without texture and color degradation as they are delicate,temperature-sensitive and have longer drying *** economical drying system that does not cause texture or color degradation is the heat pump drying system and is commonly applied for agriculture and fabric *** this study,the heat pump technology is applied to develop two hybrid dryer models(bottom discharge and front discharge)to dry shellfish and sea cucumbers in large scale for storage and *** model is tested in a laboratory-scale with wet sponges as input material for its dehumidification capacity and the power consumed to attain the target *** front discharge model is found to have rapid drying capability and *** fluid dynamic tools are used to study the hot air flow behavior and flow uniformity inside the drying *** front discharge model showed smooth uniform flow over the trays with fewer losses in the flow *** front discharge model with higher drying rate,lower power consumption and uniform airflow distribution over the trays is chosen for the industrial-scale design.

Identity Formation and Cosmopolitan Vision in Asian-American Literature

Asian-American literature is often identified as foreign by Asians and considered inauthentic American literature by the American ***,this minority group literature is unique in its characteristics so that it cannot be easily judged by either Asian or American *** order to better understand Asian-American literature,it is necessary to study how the members of Asian immigrant communities tend to form their own unique identities as Asian-Americans and to examine their literature using a new prism in light of cosmopolitanism,and not with any of the conventional *** formation has never been easy and natural for the bewildered and suffering Asian immigrants in the United States of America,especially for the second *** the study of the Korean American writer Chang-rae Lee’s Native Speaker and the Chinese-American writer Maxine Hong Kingston’s The Woman Warrior,we hope to explore the possibilities and impossibilities in cultural and social identification represented in those *** study clearly reveals that the social and psychological conflicts that Asian immigrants experience are frequently projected on the works of the immigrant writers.A cosmopolitan prospect is often envisioned in their works as a way out for both the locals and the diasporas in today’s increasingly globalized world.

There have been numerous works proposed to merge augmented reality/mixed reality(AR/MR)and Internet of Things(IoT)in various ***,they have focused on their specific target applications and have limitations on interoperability or reusability when utilizing them to different domains or adding other devices to the *** paper proposes a novel architecture of a convergence platform for AR/MR and IoT systems and *** proposed architecture adopts the oneM2M IoT standard as the basic framework that converges AR/MR and IoT systems and enables the development of application services used in general-purpose environments without being subordinate to specific systems,domains,and device *** implement the proposed architecture utilizing the open-source oneM2M-based IoT server and device platforms released by the open alliance for IoT standards(OCEAN)and Microsoft HoloLens as an MR device *** also suggest and demonstrate the practical use cases and discuss the advantages of the proposed architecture.

Clinical assessment and identification of immuno-oncology markers concerning the 19-gene based risk classifier in stage Ⅳ colorectal cancer

BACKGROUND Genomic profiling of tumors has contributed to the understanding of colorectal cancer(CRC), facilitating diagnosis, prognosis and selection of treatments,including targeted regimens. A report suggested that a 19-gene-based risk classifier(TCA19) was a prognostic tool for patients with stage III CRC. The survival outcomes in patients with stage IV CRC are still poor and appropriate selection of targeted therapies and immunotherapies is *** To assess clinical implication of TCA19 in patients with stage IV CRC, and to identify TCA19 with involvement in *** A retrospective review of the medical records of 60 patients with stage IV CRC was conducted, assessing clinicopathological variables and progression-free survival(PFS). TCA19 gene expression was determined by quantitative polymerase chain reaction(qPCR) in matched normal and tumor tissues taken from the study cohort. Expression of potential immune-oncology regulatory proteins and targets was examined by immunohistochemistry(IHC), western blot, immunofluorescence staining in tissues from a validation cohort of 10 patients, and in CRC cell lines co-cultured with monocyte in *** In the patients with TCA19 score higher than the median, the PFS rates of eight patients who received the targeted regimens were significantly higher than the PFS rates of four patients who received 5-fluorouracil-based regimen(P = 0.041).In multivariate analysis, expression of signaling lymphocytic activation molecule family, member 7(SLAMF7) and triggering receptor expressed on myeloid cells 1(TREM1) was associated with PFS in the 60-patient cohort. After checking another 10 validate set, the expression of the IHC, the level of real-time qPCR,and the level of western blot were lower for SLAMF7 and higher for TREM7 in primary and metastatic tumors than in normal tissues.
