Abstract： A new low-cost seismic isolation system based on spring tube bracings has been proposed and studied at the Structural and Earthquake Engineering Laboratory of Istanbul Technical University. Multiple compression-type springs are positioned in a special cylindrical tube to obtain a symmetrical response in tension and compression-type axial loading. An isolation floor, which consists of pin-ended steel columns and spring tube bracings, is constructed at the foundation level or any intermediate level of the building. An experimental campaign with three stages was completed to evaluate the capability of the system. First, the behavior of the spring tubes subjected to axial displacement reversals with varying frequencies was determined. In the second phase, the isolation floor was assessed in the quasi-static tests. Finally, a scaled 3D steel frame was tested on the shake table using actual acceleration records. The transmitted acceleration to the floor levels is greatly diminished because of the isolation story, which effects longer period and higher damping. There are no stability and self- centering problems in the isolation floor.

Topiramate（TPM） is a widely used antiepileptic and antimigraine agent which has been shown to exert neuroprotective effects in various experimental traumatic brain injury and stroke models. However, its utility in spinal cord injury has not been studied extensively. Thus, we evaluated effects of TPM on secondary cellular injury mechanisms in an experimental rat model of traumatic spinal cord injury（SCI）. After rat models of thoracic contusive SCI were established by free weight-drop method, TPM（40 mg/kg） was given at 12-hour intervals for four times orally. Post TPM treatment, malondialdehyde and protein carbonyl levels were significantly reduced and reduced glutathione levels were increased, while immunoreactivity for endothelial nitric oxide synthase, inducible nitric oxide synthase, and apoptotic peptidase activating factor 1 was diminished in SCI rats. In addition, TPM treatment improved the functional recovery of SCI rats. This study suggests that administration of TPM exerts neuroprotective effects on SCI.

AIM： To evaluate the sensitivity and specificity of MR colonography （MRC） and CT performance in detecting colon lesions, and to compare their sensitivity and specificity with that of conventional colonoscopy. METHODS： Forty-two patients suspected of having colonic lesions, because of rectal bleeding, positive fecal occult blood test results or altered bowel habits, underwent the examinations. After insertion of a rectal tube, the colon was filled with 1000-1500 mL of a mixture of 9 g/L NaCI solution, 15-20 mL of 0.5 mmol/L gadopentetate dimeglumine and 100 mL of iodinized contrast material. Once colonic distension was achieved, three-dimensional gradient-echo （3D-GRE） sequences for MR colonography and complementary MR images were taken in all cases. Immediately after MR colonography, abdominal CT images were taken by spiral CT in the axial and supine position. Then all patients were examined by conventional colonoscopy （CC）. RESULTS： The sensitivity and specificity of MRC for colon pathologies were 96.4% and 100%, respectively. The percentage of correct diagnosis by MRC was 97.6%. The sensitivity and specificity of CT for colon pathologies were 92.8%, 100%, respectively. The percentage of correct diagnosis by CT was 95.2%. CONCLUSION： In detecting colon lesions, MRC achieved a diagnostic accuracy similar to CC. However, MRC is minimally invasive, with no need for sedation or analgesics during investigation. There is a lower percentage of perforation risk, and all colon segments can be evaluated due to multi-sectional imaging availability; intramural, extra-intestinal components of colonic lesions, metastasis and any additional lesions can be evaluated easily. MRC and CT colonography are new radiological techniques that promise to be highly sensitive in the detection of colorectal mass and inflammatory bowel lesions.

BACKGROUND The coronavirus disease 2019(COVID-19)pandemic had a significant impact on the management of all *** diseases such as cancer have a higher risk of COVID-19-related *** this fact,any delay or alteration in treatment of cancer may have fatal *** carcinoma(HCC)is an aggressive liver cancer that requires multimodality treatment to improve *** To evaluate the impact of COVID-19 on the management of patients with HCC by determining changes in demographic,clinical and histopathological *** Demographic,clinical and pathological variables of patients with HCC who had undergone liver transplantation between March 2020 and June 2021(Pandemic group,n=48)were retrospectively compared with that of the patients with HCC transplanted between November 2018 and March 2020(Pre-pandemic group,n=61).RESULTS The median age of the patients in the study was 56(interquartile range=15).Ninety-seven patients(89%)were male and 12 were female(11%).The most common etiology of liver disease was hepatitis B virus(n=52,47.7%).According to our results,there was a 21.3% drop in the number of patients transplanted for *** was no difference in the demographic,clinical and pathological characteristics of the patients except blood alkaline phosphatase levels(P=0.029),lymphovascular invasion(P=0.019)and type of the liver graft that was transplanted(P=0.017).CONCLUSION It is important to develop a surveillance strategy for liver transplant *** liver transplantation for HCC is justified and safe provided that strict surveillance protocols are applied.

AIM: To demonstrate the effects of two different types of allergic conjunctivitis on severity of keratoconus (KC). ·METHODS: We retrospectively reviewed the medical records of 171 KC patients referred between June 2010 and June 2011. The KC patients were divided into 3 groups as KC (group A), KC with vernal keratoconjunctivitis (VKC) (group B) and KC with allergic conjunctivitis (AC) (group C). Main outcome measures were demographic and ocular clinical features including age at presentation, gender, spherical equivalent (SE), best spectacle corrected visual acuity (BCVA), mean keratometric measurement (Km), central corneal thickness (CCT), and intraocular pressure (IOP). Groups were compared in term of study variables. · RESULTS: The median age at presentation was significantly lower in group B (P <0.001). According to the median SE (P =0.003), BCVA(P =0.022), Km(P <0.001), CCT (P =0.015) and Amsler-Krumeich classification (P < 0.001), KC was more severe in group B. There was no significant difference in terms of IOP and corrected IOP among the groups (P =0.44), however there were 4 patients who had increased corrected IOP developed after topical corticosteroid use in group B. The differences among the groups persisted even after controlling for age and gender. · CONCLUSION: Our findings demonstrated a more severe KC in VKC patients despite their younger age which suggests evaluation of VKC patients as a separate group in keratoconus disease.

The effects of implantable collamer lens implantation on higher order aberrations

AIM: To evaluate the changes in higher order aberrations(HOAs) after implantable collamer lens(ICL;Staar Surgical, Nidau, Switzerland) implantation. METHODS: Totally 30 eyes of 18 patients with myopia were included in this study with an average age of 25.77y(min: 21, max: 40). Refraction, uncorrected distance visual acuity(UDVA), corrected distance visual acuity(CDVA), HOAs(entire, corneal and internal) were evaluated preoperatively and three months postoperatively. Ocular aberrations were measured by using iT race(Tracey Technology, Houston, Texas, USA). SPSS(IBM Corp. Released 2013. IBM SPSS Statistics for Windows, Version 22.0. Armonk, NY: IBM Corp) was used for the statistical analysis and the interpretation of the data. P values of less than 0.05 were considered statistically significant. RESULTS: The preoperative mean spherical power was -9.01 D(min:-5.00, max:-13.00) and the mean cylindrical error was -2.40 D(min:-0.50, max:-4.75). The postoperative mean residual spherical power was -0.73 D(min:-0.20, max:-1.75) and the mean cylindrical error was -0.89 D(min:-0.18, max:-2.09). Analyses were made on root mean square(RMS) values of total HOAs(tH OAs), spherical aberration, coma and trefoil as entire, corneal and internal components. The differences in entire t HOAs and in internal t HOAs were significant. There was no significant change found in spherical aberrations. The differences in entire coma and in internal coma were significant. There was no significant change found in corneal coma. With respect to trefoil, the only significant difference was in internal trefoil. CONCLUSION: The ICL implantation corrects the refractive error successfully and changes entire and internal HOAs of the eye.

Morphotaxonomy and seasonal abundance of dinoflagellates of the genera Prorocentrum and Mesoporos （Prorocentrales） were studied from nutrient-rich waters, Karachi Harbor and the mouth of the Manora Channel, Pakistan during May 2002-July 2003. Using both light and scanning electron microscopy, 13 species of Prorocentrales were identified according to cell shape, size, ornamentation of thecal plates, and architecture of apical platelets, apical pore area, marginal pores, and intercalary bands. P. sigmoides, P. arcuatum, P. scutellum, P. donghaiense, P. balticum, P. minimum, P. emarginatum, P. lima, P. faustiae, and Mesoporos perforatus constitute new records for sindh coast of Pakistan. The most abundant species were P. minimum/P, balticum （4.5x103 cells/L）, P. micans （1.1~103 cells/L）, P. gracile/P, sigmoides （2.5 x 102 cells/L） and P. donghaiense （6.6 x 103 cells/L） at temperatures of 29-31 ~C and salinities of 35-40. Maximum abundance was observed in winter and lower abundance in summer. There was no significant change in the distribution of species between stations except for the benthic species which occurred close to Karachi Harbor waters. Significant positive correlations were observed between Prorocentrum spp. and temperature （R2=0.27） and negative correlations with salinity （R2=-0.32） except for P. minimum and P. emarginatum which has negative correlation with temperature （R2=-0.24） and positive with salinity （R2=0.08, 0.19）. The finding of potential okadaic-acid producing species of benthic Prorocentrum call for monitoring for possible human health problems in this region.

Liver transplantation(LT) for hepatocellular carcinoma is still a hot topic,and the main factor that is associated with the success of treatment is to determine the patients who will benefit from *** criteria have been defined 25 years ago and still is being used for patient selection for ***,in living donor LT,the Milan criteria is being *** criteria for patient selection do not only consider morphologic characteristics such as tumor size and number of tumor nodules but also biologic markers that show tumor aggressiveness is also being *** the present review article,we have summarized all the criteria and scoring systems regarding LT for hepatocellular *** criteria have 5-year overall survival rates that were comparable to the Milan Criteria and ranged between 60%-85%.On the other hand,it was seen that the recurrence rates had increased as the Milan criteria were exceeded;the 5-year recurrence rates ranged between 4.9% to 39.9%.Treatment of hepatocellular carcinoma needs a multidisciplinary *** selection criteria are yet to be *** same is true for treatment *** goal will be achieved by a harmonic interplay between basic science researchers and clinicians.
