Manometric findings in patients with isolated distal gastroesophageal reflux

AIM:To analyze manometric abnormalities in patients with isolated distal reflux and compare these findings in patients with erosive and non-erosive disease. METHODS:Five hundred and fifty patients who presented to the outpatient clinic of Turkiye Yuksek Ihtisas Hospital with gastroesophageal reflux disease-like symptoms were *** individual was evaluated with esophageal manometry,24-h ambulatory pH monitoring,and upper gastrointestinal endoscopy. Manometric findings for the patients with isolated distal reflux were compared to findings in controls who were free of reflux disorders or hypersensitive esopha- *** for isolated distal reflux patients with and without erosive reflux disease were also compared. RESULTS:Of the 550 subjects enrolled,97(17.6%, mean age 48 years)had isolated distal reflux and 100 had no abnormalities on ambulatory pH monitoring (control group,mean age 45 years).There were no significant differences between the isolated distal reflux group and control group with respect to age,body mass index,and esophageal body contraction amplitude (EBCA).Mean lower esophageal sphincter pressure was significantly higher in the control group(12.7±10.3 mmHg vs 9.6±7.4 mmHg,P=0.01).Fifty-five(56.7%) of the 97 patients with isolated distal reflux had erosive reflux *** were no statistical differences be- tween the erosive reflux disease and non-erosive reflux disease subgroups with respect to mean EBCA,lower esophageal sphincter pressure,or DeMeester ***,13%of patients with gastroesophageal reflux disease had distal wave amplitudes≤30 mmHg, whereas none of the patients with non-erosive reflux disease had distal wave amplitudes in this low category. CONCLUSION:Patients with erosive and non-erosive disease present with similar manometric abnormalities. The only striking difference is the observation of very low EBCA exclusively in patients with erosive disease.

Brain metastasis of hepatocellular carcinoma:A case report and review of the literature

Hepatocellular carcinoma (HCC) is one of the most frequent malignancies in the world. It is more common in far eastern countries and relatively rare in the United States and western European countries where at autopsy it accounts for only 1-2% of malignant rumors, The disease is usually manifested in the the 6^th and 7^th decade of life. HCC is one of the highly malignant neoplasms, Extrahepatic metastases are seen in 64% of patients with HCC. The lungs, regional lymph nodes, kidney, bone marrow and adrenals are common sites of HCC metastasis^[1-3], But, metastasis to brain and skull is extremely rare. Table I shows some of the reported cases of HCC with brain metastasis. These case reports reaffirms the complex and multidisciplinary care of these patients^[4-5].

Etiology and portal vein thrombosis in Budd-Chiari syndrome

AIM: To research the etiology, portal vein thrombosis and other features of Budd-Chiari syndrome (BCS) patients prospectively. METHODS: A total of 75 patients (40 female, 35 male) who were diagnosed between January 2002 and July 2004 as having BCS were studied prospectively. Findings from on physical examination, ultrasonography, duplex ultrasonography and venography were analyzed. Hemogram and blood chemistry were studied at the time of diagnosis and on each hospital visit. Bone marrow examination and immune phenotyping were performed by a hematologist when necessary. Protein C, S, antithrombin Ⅲ, activated protein C resistance, and anticardiolipin antibodies, antinuclear antibodies, and anti ds-DNA were studied twice. The presence of ascite, esophageal varices, and portal thrombosis were evaluated at admission and on every visit. RESULTS: At least one etiological factor was determined in 54 (72%) of the patients. The etiology could not be defined in 21 (28%) patients. One etiological factor was found in 39, 2 factors in 14 and 3 factors in 1 patient. The most common cause was the web (16%), the second was Hydatid disease (11%), the third was Behcet’s disease (9%). Portal vein thrombosis was present in 11 patients and at least one etiology was identified in 9 of them (82%). CONCLUSION: Behcet’s disease and hydatid disease are more prominent etiological factors in Turkey than in other countries. Patients with web have an excellent response to treatment without signs of portal veinthrombosis while patients having thrombofilic factors more than one are prone to develop portal vein thrombosis with worse clinical outcome.

基于星载积分路径差分吸收(IPDA)激光雷达探测数据对全球二氧化碳柱浓度(XCO_(2))进行反演并利用全球CO_(2)柱浓度观测网(TCCON)、轨道碳观测者2号(OCO-2)卫星以及碳追踪器三者的CO_(2)数据产品验证了反演结果的准确性。结果表明:IPDA激光雷达的反演结果与TCCON的观测结果一致性较好,两者的平均偏差为0.3×10^(-6),决定系数(R2)为0.952,均方根误差(RMSE)为0.584×10^(-6)。与OCO-2和CT的XCO_(2)数据产品的对比结果也显示出IPDA激光雷达反演结果与TCCON数据产品的吻合程度均高于OCO-2卫星与碳追踪器,进一步表明IPDA激光雷达能够获得更准确的全球XCO_(2)观测数据。IPDA激光雷达对于不同地表类型上空的XCO_(2)探测精度存在一定差异,在空间分辨率为50 km(陆地)和100 km(海洋)下,其探测精度主要分布在(0.80~0.82)×10^(-6)与(0.76~0.78)×10^(-6),均满足1×10^(-6)的探测精度要求。星载IPDA激光雷达的全天时、高精度探测技术将在今后全球碳源碳汇研究中发挥重要作用。

Median Nerve Somatosensory Evoked Potentials in Patients with Chiari Malformation

Abnormal SEP reflects dysfunction of the medial lemniscus and posterior cervical cord. These structures are likely to be affected in Chiari malformation. Therefore, SEP abnormalities may provide valuable information in patients with CM. However, the consistency of SEP abnormality or normality with the damage is a matter of research. Knowing whether median nerve somatosensory evoked potential (SEP) is useful in revealing subclinical damage in patients with Chiari malformation is important in the treatment and follow-up plan of the disease. The aim of this study was to investigate the relationship between median nerve SEP values and the severity of cerebellar ectopia in patients with Chiari type 1 malformation. Median nerve SEP values were obtained from 30 healthy individuals and 146 individuals with Chiari malformation. The cerebellar ectopia degree and McRae line length were measured. SEP values were not significantly different between groups. The McRae line was found to be significantly shorter in the control group than in the Chiari malformation group (p = 0.031). There was no correlation between the degree of cerebellar ectopia and the length of the McRae line (r = 0.002, p = 0.979). Neither cerebellar ectopy degree nor McRae line length had a relationship with SEP values (r = -0.153, p = 0.066;r = -0.056, p = 0.500, respectively). There was no difference in cerebellar ectopy degree or SEP values between the groups with cerebellar ectopy with and without a syrinx (p = 0.899;p = 0.080, respectively). Likewise, McRae line length was not found to be related to the presence of a syrinx (p = 0.139). Median nerve SEP examination was not beneficial for diagnosing asymptomatic-oligosymptomatic Chiari malformation as a subclinical injury, whether accompanied by syringomyelia or not.

Hollow blocks are used widely in buildings for their lightweight construction and high thermal resistance *** used with a phase change material(PCM),these blocks can reduce the heating and cooling loads of buildings compared to conventional hollow ***,it is essential that the block inner forms and PCMs should be selected based on climate *** this study,hollow blocks containing PCM in different climate types are thermally *** the first step,ten different formed blocks are thermally *** these blocks,the effects of hollow ratio(HR),inner form,and the number of cavities on heat flux and temperature distribution are *** inner form of the hollow block with the least heat flux is determined in this *** second step consists of optimization for the PCM melting temperature and layer thickness for the different *** optimization results are evaluated on a building in terms of primary energy consumption and global *** best performing locations of the PCMs within the block are defined for all provinces in the last *** results of the study are discussed in terms of the block surface temperatures and liquid *** best PCM locations are found close to the block’s centerline in hot and warm climates and close to the indoor side in cold climates.

Frequency of celiac disease and distribution of HLA-DQ2/DQ8 haplotypes among siblings of children with celiac disease
