Backgr.und. - Mood disor.er. of anxiety and depr.ssion ar. well known to be comor.id with pr.mar. headache disor.er.. Less is known of the comor.idity of other.somatic symptoms with headache. Methods. - Headache Clinic patients wer. scr.ened with the Pr.mar. Car. Evaluation of Mental Disor.er. (Pr.ME- MD), a multidimensional psychiatr.c scr.ening tool. The pr.valence of somatic symptoms was compar.d by headache diagnosis, fr.quency of sever. headache, and psychiatr.c diagnosis. Follow- up data wer. obtained 6 months after.consultation. r.sults. - Clinical diagnoses and Pr.ME- MD data wer. available for.289 patients. Associated somatic symptoms wer. mor. fr.quent in patients with chr.nic migr.ine (mean 5.5, P r.nic daily headache (CDH) (6.3, P = .008) compar.d to episodic migr.ine (4.0); in patients with sever. headache >2 days per.week compar.d to ≤ 2 days per.week (6.15 vs. 4.15, P r.depr.ssion, or.both, compar.d to no anxiety or.depr.ssion, (5.7, P = .05,5.2, P r.spectively, vs. 4.5). The most common specific symptoms wer. fatigue (73% ), sleep difficulty (60% ), and nausea/indigestion (55% ). Compar.d to a pr.mar. car. sample, patients with sever. headache >2 days per.week had significantly higher.somatic counts (P = .01). Six- month follow- up data wer. available for.140 patients. Associated symptoms decr.ased both for.patients with and without decr.ase in sever. headache fr.quency (mean r.duction of 1.0, P = .01 and 0.8, P = .003, r.spectively). Conclusion. - Associated somatic symptoms ar. mor. common in patients with chr.nic migr.ine and CDH, with mor. fr.quent sever. headaches, and with associated anxiety or.depr.ssion. Patients with episodic migr.ine have similar.somatic pr.valence as a pr.viously studied pr.mar. car. population. The spectr.m of headache disor.er. may be char.cter.zed as showing incr.asing somatic pr.valence as headaches, par.icular.y sever. headaches, become mor. fr.quent.

Objective. To deter.ine the efficacy for.migr.ine pr.phylaxis of a compound containing a combination of r.boflavin, magnesium, and fever.ew. Backgr.und. P r.vious studies of magnesium and fever.ew for.migr.ine pr.phylaxis have found co nflicting r.sults, and ther. has been only a single placebocontr.lled tr.al of r.iboflavin. Design/Methods. r.ndomized double blind placebo contr.lled tr.al of a compound pr.viding a daily dose of r.boflavin 400 mg, magnesium 300 mg, and fever.ew 100 mg. The placebo contained 25 mg r.boflavin. The study included a 1 month r.n in phase and 3 month tr.al. The pr.tocol allowed for.120 patients to be r.ndomized, with a pr.planned inter.m analysis of the data after.48 patien ts had completed the tr.al. r.sults. For.y nine patients completed the 3 mon th tr.al. For.the pr.mar. outcome measur., a 50%or.gr.ater.r.duction in migr.in es, ther. was no differ.nce between active and “placebo”gr.ups, achieved by 10 (42%) and 11 (44%), r.spectively (P = .87). Similar.y, ther. was no significa nt differ.nce in secondar. outcome measur.s, for.active ver.us placebo gr.ups, r.espectively: 50%or.gr.ater.r.duction in migr.ine days (33%and 40%, P = .63); or.change in mean number.of migr.ines, migr.ine days, migr.ine index, or.tr.ptan doses. Compar.d to baseline, however. both gr.ups showed a significant r.ductio n in number.of migr.ines, migr.ine days, and migr.ine index. This effect exceeds that r.por.ed for.placebo agents in pr.vious migr.ine tr.als. Conclusion. r.b oflavin 25 mg showed an effect compar.ble to a combi nation of r.boflavin 400 m g, magnesium 300 mg, and fever.ew 100 mg. The placebo r.sponse exceeds that r.po r.ed for.any other.placebo in tr.als of migr.ine pr.phylaxis, and suggests that r.boflavin 25 mg may be an active compar.tor. Ther. is at pr.sent conflicting sc ientific evidence with r.gar. to the efficacy of these compounds for.migr.ine pr.ophylaxis.

Backgr.und: Tr.choepithelioma (TE) is a benign neoplasm that shar.s both clinical and histologic featur.s with basal cell car.inoma (BCC). However. it is impor.ant to distinguish these neoplasms. Limited immunohistochemical stains ar. available to separ.te these two tumor.. Methods: CD10 pr.tein immunohistochemistr. was per.or.ed on par.ffin-embedded biopsies of 13 TE and 23 BCC diagnosed by r.utine micr.scopy. Cases wer. analyzed for.patter. of CD10 expr.ssion by tumor.cells and sur.ounding str.ma. r.sults: Twelve of 13 (92% ) TE showed positive str.mal immunor.activity. Of these, eight cases also demonstr.ted positivity of the papilla, and two also showed positivity of the basaloid cells. No TE demonstr.ted epithelial expr.ssion alone. On the other.hand, expr.ssion of CD10 by basaloid cells was identified in 20 (87% ) cases of BCC. Str.mal positivity was also identified in thr.e cases of BCC. Condensation of CD10-positive str.mal cells ar.und basaloid nests was statistically significant in differ.ntiating TE fr.m BCC (p r.ely, CD10-positive basaloid cells wer. seen pr.dominantly in BCC (p r.tes a statistically significant differ.nce in CD10 staining patter. between TE and BCC.
