Comparison of the rheological properties of four root canal sealers

The flowability of a root canal sealer is clinically important because it improves the penetration of the sealer into the complex root canal system. The purpose of this study was to compare the flowabilities of four root canal sealers, measured using the simple press method （ISO 6876）, and their viscosities, measured using a strain-controlled rheometer. A newly developed, calcium phosphate-based root canal sealer （Capseal） and three commercial root canal sealers （AH Plus, Sealapex and Pulp Canal Sealer EWT） were used in this study. The flowabilities of the four root canal sealers were measured using the simple press method （n= 5） and their viscosities were measured using a strain-controlled rheometer （n=5）. The correlation between these two values was statistically analysed using Spearman＇s correlation test. The flow diameters and the viscosities of the root canal sealers were strongly negatively correlated （p= -0.8618）. The viscosity of Pulp Canal Sealer EWT was the lowest and increased in the following order： AH Plus〈Sealapex〈Capseal （P〈0.05）. All of the tested root canal sealers showed characteristic time- and temperature-dependent changes in their rheological properties. The viscosities measured using the strain-controlled rheometer were more precise than the flowabilities measured using the simple press method, suggesting that the rheometer can accurately measure the rheological properties of root canal sealers.

AIM: To examine whether the reversibility of chronic cyclosporine A (CsA) nephrotoxicity is associated with apoptotic cell death and its regulatory factors. METHODS: Chronic CsA nephrotoxicity was induced in Sprague- Dawley rats by administering CsA (15 mg/kg, sc) for 5 weeks, and then withdrawing it for 5 or 10 weeks. The effect of CsA withdrawal on apoptotic cell death was evaluated by an in situ TdT-mediated deoxyuridine triphos- phate-biotin nick end-labeling (TUNEL) assay and the expression of pro-apoptotic [(transforming growth factor- beta1 (TGF-β1) and Fas] and anti-apoptotic [epidermal growth factors (EGF) and Bcl-2] factors. RESULTS: Discontinuation of CsA induced significant decreases in TUNEL-positive cells in a time-dependent manner and the reduction in TUNEL-positive cells was correlated with the tubulointerstitial fibrosis score (r=0.919, P<0.01). Upregulation of TGF-β1 and Fas expression in CsA-treated rat kidneys was decreased significantly after with- drawal of CsA. In contrast, downregulated EGF and Bcl-2 expression returned to normal or supernormal levels. CONCLUSION: CsA withdrawal is associated with a decrease in apoptotic cell death and with changes in the expression of pro-apoptotic and anti-apoptotic molecules involved in renal wound repair. This may constitute one of the mechanisms underlying the reversibility of chronic CsA nephrotoxicity.

High prevalence of osteoporosis in patients with gastric adenocarcinoma following gastrectomy

AIM： To evaluate the prevalence and predictive factors of osteoporosis in patients with gastric adenocarcinoma alter gastrectomy.METHODS： The study included 133 patients diagnosed with gastric adenocarcinoma but who did not undergo prior diagnostic work-up for osteoporosis. Bone mineral density （BMD） was measured by dual-energy X-ray absorptiometry （DXA） and vertebral deformity was assessed by plain X-rays. We evaluated the effects of age, sex, body mass index （BMI）, anemia, back pain, vertebral deformity, tumor staging, reconstruction type, and past medical history to determine predictive factors of osteoporosis in these patients.RESULTS： The prevalence of osteoporosis in the lumbar spine was 38.3% （male, 28.9%; female, 54.0%）, and 15.0% in the femoral neck （male, 10.8%; female, 22.0%）. The vertebral deformity rate was 46.6% （male, 43.4%; female, 52.0%）. Age, BMI and hemoglobin correlated with BMD （P 〈 0.01）. In males, anemia and age 〉 64 years were independent predictive factors of osteoporosis in multivariate analysis. In females, back pain was an independent factor for osteoporosis.CONCLUSION： The results of this study revealed that prevalence of osteoporosis and vertebral bone deformity rate were high in gastric cancer patients, regardless of post-gastrectomy duration and operation type. Early diagnosis and a proper management plan must be established in these patients.

Predictors of poor outcomes in patients with wild mushroom-induced acute liver injury

To identify early predictive markers of poor outcomes in patients with acute liver injury from wild mushroom *** observational, retrospective record review involved adults aged ≥ 18 years admitted to emergency department with mushroom intoxication from January 2005 to December 2015. The diagnosis of mushroom intoxication was based on the following: (1) a positive history of recent wild mushroom intake (either raw or cooked); (2) the onset of gastrointestinal symptoms, such as watery diarrhea, vomiting, and/or abdominal pain, after ingestion; and (3) the exclusion of other possible causes of acute liver injury. Acute liver injury was defined by a > 5-fold elevation of liver enzymes or moderate coagulopathy [international normalized ratio (INR) > 2.0]. Clinical and laboratory findings were compared in survivors and *** 93 patients with mushroom intoxication, 23, 11 men (47.8%) and 12 women (52.2%), of median age 61 years, developed acute liver injury. The overall in-hospital mortality rate was 43.5% (10/23). Clinical outcome after pulmonary metastasectomy from primary hepatocellular carcinoma:Analysis of prognostic factors

AIM： To review the surgical outcomes in terms of the surgical indications and relevant prognostic factors. METHODS： Sixteen patients underwent therapeutic lung surgery between March 1999 and May 2006. The observation period was terminated on May 31, 2007. The surgical outcomes and the clinicopathological factors were compared. RESULTS： There was no mortality or major morbidity encountered in this study. The mean follow-up period after metastasectomy was 26.7 ＋ 28.2 （range： 1-99 mo）, and the median survival time was 20 mo. The 1- and 5-year survival rates were 56% and 26%, respectively. At the end of the follow-up, 1 patient died from hepatic failure without recurrence, 6 died from hepatic failure with a recurrent hepatocellular carcinoma （HCC）, and 4 died from recurrent HCC with cachexia. Among several clinical factors, Kaplan Meier analysis revealed as a treatment for the that liver transplantation primary lesion, grade of cell differentiation, and negative evidence HBV infection were independent predictive factors. On Cox＇s proportional hazard model, there were no significant factors affecting survival after pulmonary metastasectomy in patients with HCC. CONCLUSION： A metastasectomy should be performed before other treatments in selected patients, Although not significant, patients with liver transplantation of a primary HCC survived longer, Liver transplantation might be the most beneficial modality that can offer patients better survival, A multi- institutional and collaborative study would be needed for identifying clinical prognostic factors predicting survival in patients with HCC and lung metastasis.

AIM:There is some evidence of functional superiority of colonic J-pouch over straight coloanal anastomosis (CM) in ultralow anterior resection (ULAR) or intersphincteric resection. On the assumption that colonic J-pouch anal anastomosis is superior to straight CM in ULAR with upper sphincter excision (USE: excision of the upper part of the internal sphincter) for low-lying rectal cancer, we compare functional outcome of colonic J-pouch vsthe straight CM. METHODS: Fifty patients of one hundred and thirty-three rectal cancer patients in whom lower margin of the tumors were located between 3 and 5 cm from the anal verge received ULAR including USE from September 1998 to January 2002. Patients were randomized for reconstruction using either a straight (n = 26) or a colonic J-pouch anastomosis (n = 24) with a temporary diverting-loop ileostomy. All patients were followed-up prospectively by a standardized questionnaire [Fecal Inco-ntinence Severity Index (FISI) scores and Fecal Incontinence Quality of Life (FIQL) scales]. RESULTS: We found that, compared to straight anastomosis patients, the frequency of defecation was significantly lower in J-pouch anastomosis patients for 10 mo after ileostomy takedown. The FISI scores and FIQL scales were significantly better in J-pouch patients than in straight patients at both 3 and 12 mo after ileostomy takedown. Furthermore, we found that FISI scores highly correlated with FIQL scales. CONCLUSION: This study indicates that colonic J-pouch anal anastomosis decreases the severity of fecal incontinence and improves the quality of life for 10 mo after ileostomy takedown in patients undergoing ULAR with USE for low-lying rectal cancer.

Effect of the Weaving Density of Aramid Fabrics on Their Resistance to Ballistic Impacts

Two Heracron? woven fabrics, HT600-1 and HT600-2, were fabricated with different weaving densities and their resistance to ballistic impact was investigated. While HT600-1 was inherently stronger along the weft than HT600-2, the latter exhibited a higher tensile strength along the warp. Crimp values indicate that HT600-1, which possesses a relatively larger weft weaving density, induces an excess in the warp crimp ratio, thereby weakening the fabric along the warp. The dimensionless fiber property U*, which is defined as the product of the specific fiber toughness and the strain wave velocity, was calculated for each fabric. The U* values of HT600-1 were lower than those of HT600-2;U* values along the warp of HT600-1 were extremely low. These analyses show that HT600-2 exhibited improved ballistic properties over those of HT600-1. These findings further indicate the existence of an optimal weave that would minimize damage to both yarn and fabric. Establishing these optimal conditions can be crucial in implementing better ballistic properties into fabrics.

Incomplete High Radial Nerve Palsy Due to Schwannoma of Radial Nerve in the Upper Arm—A Case Report

A schwannoma is a benign soft tissue tumor composed of Schwann cells. Clinically, the tumor is slow-growing and usually asymptomatic, rarely developing neurological symptoms. We present a case of a 44-year-old female who was diagnosed with an incomplete high radial nerve palsy, showing a wrist drop and extension of fingers due to schwannoma on the radial nerve of the upper arm. The patient showed excellent result after excision of the schwannoma and neurorrhaphy of the radial nerve.

Clinical characteristics of Helicobacter pylori-negative drugnegative peptic ulcer bleeding

AIM:To investigate the clinical characteristics and outcomes of idiopathic Helicobacter pylori(***)-negative and drug-negative]peptic ulcer bleeding(PUB).METHODS:A consecutive series of patients who experienced PUB between 2006 and 2012 was retrospectively analyzed.A total of 232 patients were enrolled in this *** patients were divided into four groups according to the etiologies of PUB:idiopathic,***-associated,drug-induced and combined(***-associated and drug-induced)*** compared the clinical characteristics and outcomes between the *** the silver stain or rapid urease tests were ***-negative,we obtained an additional biopsy specimen by endoscopic re-examination and performed an *** antibody test 6-8 wk after the initial endoscopic *** a diagnosis of idiopathic PUB,a negative result of an *** antibody test was *** all cases,re-bleeding was confirmed by endoscopic *** the risk assessment,the Blatchford and the Rockall scores were calculated for all ***:For PUB,the frequency of *** infection was 59.5%(138/232),whereas the frequency of idiopathic cases was 8.6%(20/232).When idiopathic PUB was compared to ***-associated PUB,the idiopathic PUB group showed a higher rate of rebleeding after initial hemostasis during the hospital stay(30%vs 7.4%,P = 0.02).When idiopathic PUB was compared to drug-induced PUB,the patients in the idiopathic PUB group showed a higher rate of rebleeding after initial hemostasis upon admission(30%vs 2.7%,P < 0.01).When drug-induced PUB was compared to ***-associated PUB,the patients in the drug-induced PUB were older(68.49 ± 14.76 years vs 47.83 ± 15.15 years,P< 0.01) and showed a higher proportion of gastric ulcer(77%vs 49%,P < 0.01).However,the Blatchford and the Rockall scores were not significantly different between the two *** the patients who experienced drug-induced PUB,no significant differences were found with respect
