External Validation of SENIC and NNIS Scores for Predicting Wound Infection in Colorectal Surgery

Objective: We aimed to identify the ratio of Surgical Site Infection (SSI) and also the validity of the National Nosocomial Infection Survellance (NNIS) and Study on the Efficacy of Nosocomial Infection Control (SENIC) risk indexes in colorectal surgery, among Turkish population. Background: Some problems have been reported with the power of NNIS risk index to predict the risk of surgical site infection. We aimed to validate theNNIS and SENIC risk indexes in colorectal surgery. Methods: Between January 2003 and December 2006, surgical site ?nfection survellance was performed to 107 patient who undergo colorectal surgery with NNIS and SENIC risk scales. The mean patient age was 48 years (range, 17 to 86), and 61.7% of the group (66) was female. For this patient cohort, 6 (5.6%) were diagnosed with incisional SSI. While the mean Body Mass Index (BMI) of all patients was 26.6;mean value of BMI among the patiens with SSI was ***: 6 insicional surgical site infection were observed during the study. According to Receiver Operating Characteric (ROC) curve analyze neither NNIS with avalue of 0.70, nor SENIC with a value of 0.67 are perfect risk indexes. Conclusion: As a result both NNIS and SENIC ?s a good risk indexes but not perfect. Scarcely when NNIS and SENIC is used together to predict the SSI they forecast the development of infection better. But there is a lot of other factors that effect the development of SSI, so for excellent surveillance risk index those factors known by everyone must be added to risk index scales.

Preparation and characterization of iron-ore-imbedded silicone rubber materials for radiation protection

Iron-ore-imbedded silicone rubber materials were produced for radiation shielding. Samples were tested against a Co-60 gamma source, which is widely used in nuclear technology and medicine. Decreasing the particle size of iron ore resulted in better gamma radiation protection owing to more homogenous distribution. In addition, the materials had flexible properties up to the addition of 60 wt% iron ore content. Further, 0.5 mm Pb E gamma protection was provided by using 2.06-mm-thick SDT-60 as the Co-60 source. Iron ore–silicone rubber composites are candidate materials for lead-free flexible radiation protection systems owing to their relatively inexpensive and easy production.

Learning distance effect on lignite quality variables at global and local scales

Determining scale and variable effects have critical importance in developing an energy resource *** study aims to explore the relationships in heterogeneous lignite sites using different scale models,spatial weighting as well as error-based pair-wise *** a statistical learning framework,the relationships among the quality variables such as geochemical variables and the contributions of the coordinates to quality measures have been exhibited by generalized additive *** this way,the critical roles of spatial weights provided by the coordinates have been specified at a global *** experimental studies reveal that incorporating the geological weighting in the models as the additional information improves both accuracy and *** relationships among lignite quality variables and sampling locations are spatially non-stationary,the local structure and interdependencies among the variables were analyzed by geographically weighting *** local analyses including spatial patterns of bandwidths,search domains as well as residual-based areal dependencies provided not only the critical zones but also availability of pair-wise model alternatives by calibrating a model at each point for location-specific parameter *** results completely show that the weighting models applied at different scales can take spatial heterogeneity into consideration and these abilities provide some meta-data and specific information using in sustainable energy planning.

Comparative New Insight into the Tectonic Origin of Folds and Thrust Faults of an Extensional Basin: Soke-Kusadasi Basin, Aegean, Western Turkey

The Aegean area of the western Anatolian region of Turkey,controlled by the low-angle detachment normal fault system,forms an extensional province,the West Anatolian Extensional Province(WAEP).The tectonic deformation which occurred in the Miocene Period,including the Plio–Quaternary Period has created different structures in both the basement rocks and intra-basin deposits of the *** of these structures,high-angle normal faults,controls the supradetachment Soke-Kusadasi Basin(SKB).Within this basin,there are folds with different axes and thrust faults with a north-northwestnortheast(N,NW,NE)*** folds and thrust faults in the SKB deformed the sedimentary structures of intra-basin *** folds and thrust faults,which caused the rotation of beddings and imbrications in the SKB,are mainly associated with the tectonic process of the low angle detachment normal fault,which affected the SKB and the Aegean part of western *** the SKB,during the process of extensional deformation associated with primary low angle detachment normal faulting,the ramp-flat and inversion geometry observed in the basement rocks and basin deposits of the crust caused folds and thrust faults in only intra-basin *** the WAEP,it is determined for the first time that the folds and thrust faults causing limited shortening deformed the Plio–Quaternary sediments.

Interfacial Microstructure of Diffusion Bonded Inconel 738 and Ferritic Stainless Steel Couple

In this study, Inconel 738 alloy was diffusion bonded to a ferritic stainless steel. The effect of bonding temperature on the microstructural development across the joint region was investigated. Following the diffusion bonding, conventional characterization techniques such as scanning electron microscopy （SEM）, energy dispersive X-ray （EDX） and microhardness were used to examine the interracial microstructure. It was seen that bonding temperature was effective on the diffusion of Ni from Inconel 738 to ferritic stainless steel that affected the microstructure of the interface. Austenite phase was formed at the interface as a result of Ni diffusion from the Inconel 738 to the interface.

Is enteroscopy necessary for diagnosis of celiac disease?

Celiac disease （CD） is an autoimmune inflammatory disease of the small intestine as a result of reaction to wheat protein, gluten. Exclusion of dietary gluten is the mainstay of the treatment that necessitates a precise diagnosis of the disease. Serological screen ing may aid in identifying patients with suspected CD, which should be confirmed by intestinal biopsy. It has been shown that duodenal biopsies are good for de- tection of the disease in most patients. However, there is a group of patients with positive serology and incon- clusive pathology. As a result of the widespread use of serology, many patients with equivocal findings grow quickly. Unfortunately current endoscopic methods can only diagnose villous atrophy, which can be present in the later grades of disease （i.e., Marsh m）. To diag- nose CD correctly, going deeper in the intestine may be necessary. Enteroscopy can reveal changes in CD in the intestinal mucosa in 10%-17% of cases that have negative histology at initial workup. Invasiveness of the method limits its use. Capsule endoscopy may be a good substitute for enteroscopy. However, both tech- niques should be reserved for patients with suspected diagnosis of complications. This paper reviews the cur- rent literature in terms of the value of enteroscopy for diagnosis of CD.

Prognostic value of claudin 18.2 expression in gastric adenocarcinoma

BACKGROUND Claudin 18.2(CLDN18.2)is a cell surface protein expressed by gastric cancer *** monoclonal antibody zolbetuximab binds CLDN18.2-positive cancer cells and causes cancer cell death.A few studies researched the prognostic effect of CLDN18.2 expression in metastatic gastric *** To identify the prognostic value of CLDN18.2 expression in patients with metastatic gastric *** This study was conducted with 65 patients over the age of 18 who were diagnosed with metastatic gastric *** investigated the effect of CLDN18.2 expression on clinicopathological characteristics(age,sex,histological grade,Lauren classification,family history,metastatic site,HER2 expression)and prognosis for patients with metastatic gastric *** CLDN18.2 expression was positive in 73.8%(48)of the *** the median 17.7-mo follow-up period,89.2%(58)of the patients *** progression-free survival and overall survival(OS)were 6 mo(95%confidence interval:1.6-10.4)and 12 mo(95%confidence interval:7.5-16.5).There was no statistically significant correlation between CLDN18.2 expression and clinicopathological characteristics of the *** univariate and multivariate Cox regression analysis,there was no correlation between clinicopathological characteristics of patients and progression-free survival or *** CLDN18.2 expression was quite high in patients with gastric adenocarcinoma,identifying the proportion of the patients in whom zolbetuximab would be *** is no statistically significant correlation with clinicopathological characteristics and ***18.2 is not a prognostic marker in patients with gastric adenocarcinoma,although it is predictive.

Acoustic beam splitting in a sonic crystal around a directional band gap

Beam splitting upon refraction in a triangular sonic crystal composed of aluminum cylinders in air is experimentally and numerically demonstrated to occur due to finite source size, which facilitates circumvention of a directional band gap. Experiments reveal that two distinct beams emerge at crystal output, in agreement with the numerical results obtained through the finite-element method. Beam splitting occurs at sufficiently-small source sizes comparable to lattice periodicity determined by the spatial gap width in reciprocal space. Split beams propagate in equal amplitude, whereas beam splitting is destructed for oblique incidence above a critical incidence angle.

How helpful is capsule endoscopy to surgeons?

Capsule endoscopy is a new technology that, for the first time, allows complete, non-invasive endoscopic imaging of the small bowel. The efficacy of capsule endoscopy in the diagnosis of suspected small bowel diseases has been established. Important applications for surgeons include observations of obscure gastrointestinal bleeding and small bowel neoplasms.
