We read with great interest the recent article entitled “Hepatocellular carcinoma review: Current treatment, and evidence-based medicine” by Raza et al, published in World Journal of Gastroenterology. Authors evaluated treatments for early and advanced stage hepatocellular carcinoma based on an extensive review of the relevant literature. They reported that radiofrequency ablation is the most effective local ablative therapy. They concluded that RF ablation is equivalent to surgical resection in well selected patients with early stage hepatocellular carcinoma. In addition, we want to mention microwave ablation besides RF ablation.

TO evaluate the effects of abdominal trauma on hemorrhagic shock-induced acute lung injury in rats. METHODS： Five groups were allocated （n = 8） in the study. Group Ⅰ was taken as the control group, group Ⅱ as the hemorrhagic shock group, group Ⅲ as hemorrhagic shock ＋ laparotomy, group Ⅳ as hemorrhagic shock ＋ splenectomy and group V as splenectomy ＋ omentectomy ＋ hemorrhagic shock group. Hemorrhagic shock was induced by drawing blood and reducing mean arterial pressure （MAP） to 40 mmHg within 10 min. After a hypotensive period of 1 h, animals were resuscitated. Bronchoalveolar lavage （BAL） was performed to recover cells from the alveolar space with 40 mL of BAL fluid after resuscitation malondialdehyde （MDA） and L-γ-glutamyl-L-cysteinylglycine （GSH） levels were measured in serum, erythrocytes and lung tissue. RESULTS： Serum, erythrocyte, lung tissue MDA and GSH levels were significantly increased in hemorrhagic shock groups Ⅱ-Ⅴ （P 〈 0.05）. Lymphocyte, neutrophil and alveolar macrophage counts in BAL fluid indicated a significant difference between control and shock groups （P 〈 0.05）. CONCLUSION： The degree of trauma increases hemorrhagic shock-induced acute lung injury.

AIM： To evaluate role Cadmium （Cd） exposure line Hep3B cells. of midkine secretion during in the human hepatocyte cell METHODS： Different dosages of Cd （0.5-1-5-10 μg/mL） were applied to Hep3B cells and their effects to apoptosis, lactate dehydrogenase （LDH） leakage and midkine secretion were evaluated as time dependent manner. Same experiments were repeated with exogenously applied midkine （250-5000 pg/mL） and/or 5 t^g/mL Cd. RESULTS： Cd exposure induced prominent apoptosis and LDH leakage beginning from lower dosages at the 48^th. Cd induced midkine secretion with higher dosages （P 〈 0.001）, （control, Cd 0.5-1-5-10μg/mL respectively： 1123 ± 73, 1157 ± 63, 1242 ± 90, 1886 ± 175, 1712 ± 166 pg/mL）. Exogenous 500-5000 pg/mL midkine application during 5 μg/mL Cd toxicity prevented caspase-3 activation （control, Cd toxicity, 250, 500, 1000, 2500, 5000 pg/mL midkine＋ Cd toxicity, respectively： 374 ± 64, 1786 ± 156, 1545 ± 179, 1203 ± 113, 974 ± 116, 646 ± 56, 556 ± 63 cfu） LDH leakage and cell death in Hep3B cells （P 〈 0.001）. CONCLUSION： Our results showed that midkine secretion from Hep3B cells during Cd exposure protects liver cells from Cd induced cellular damage. Midkine has anti-apoptotic and cytoprotective role during Cd toxicity. Further studies are needed to explain the mechanism of midkine secretion and cytoprotective role of midkine during Cd exposure. Midkine may be a promising theurapatic agent in different toxic hepatic diseases.

AIM: To investigate the tolerance development against the relaxant effect of nitric oxide donating drug isosorbide dinitrate (ISDN) and sodium nitropruside (SNP) in internal anal sphincter (IAS) smooth muscle. METHODS: Relaxation responses of ISDN, and electrical fi eld stimulation (EFS) were obtained before and after tolerance induction by ISDN incubation. RESULTS: ISDN (10-7-10-4 mol/L) and SNP (10-8-10-4 mol/L) caused a concentration-dependent relaxation on the basal tonus of the isolated rabbit IAS strips. After a period of 2 h incubation of the 6 x 10-4 mol/L ISDN the relaxation effects of ISDN and SNP did not change compared to control strips. EFS evoked frequency-dependent relaxation in internal anal sphincter smooth muscle and Emax obtained from control strips were not changed in ISDN tolerance-inducing condition. In this study nitrate tolerance was not observed in rabbit IAS smooth muscle. CONCLUSION: This result shows that nitric oxide donating drugs relaxes the internal anal sphincter of the rabbits without the development of tolerance.

Retrorectal or presacral tumors are rare lesions located in the presacral area and considered as being derived from multiple embryological *** tumors are classified as congenital,neurogenic,osseous,inflammatory,or *** most common among these are congenital benign lesions that present with non-specific symptoms,such as lower back pain and change in bowel *** congenital and developmental tumors occur in younger patients,the median age of presentation is reported to be 45 *** resonance imaging plays a crucial role in treatment management through accurate diagnosis of the lesion,the evaluation of invasion to adjacent structures,and the decision of appropriate surgical *** usefulness of preoperative biopsy is still debated;currently,it is only indicated for solid or heterogeneous tumors if it will alter the treatment *** resection with clear margins is considered the optimal treatment;described approaches are transabdominal,perineal,combined abdominoperineal,and minimally *** retrorectal tumors have favorable long-term outcomes with a low incidence of recurrence,whereas malignant tumors have a potential for distant organ metastasis in addition to local recurrence.

As an indispensable energy source, lignite is almost exclusively used in power generation m TurKey, To assess me quality level of Turkish lignite, a multivariate statistical analysis was conducted. The relationship among the lignite quality parameters has been investigated using a response method that is the logistic regression method. The analysis determines the effect of multiple predictor variables such as moisture, ash and sulphur presented simultaneously to predict membership of the two calorific value categories. By this way, a reliable binary response regression structure was constructed considering all the lignite fields in Turkey. Both the experiments on identifying the influential measurements and the measure of goodness of fit indicated that the overall model has a big capability to exhibit the relationship among the parameters of the Turkish lignite.

AIM： TO investigate the prevalence of celiac disease serologic markers （antigliadin IgA, IgG, and antiendomysial IgA） in patients with reflux esophagitis and to detect the relationship between reflux esophagitis and celiac disease （CD）. METHODS： This study was performed prospectively between January 2003 and January 2004. Sixty-eight adult reflux esophagitis patients and 40 people as control group for symptoms related with gastrointestinal system were enrolled in this study. The diagnostic work-up induded an accurate medical history with gastrointestinal symptoms, routine laboratory measurements, the detection of antibodies against gliadin （IgA and IgG） and endomysium （IgA）, and an upper endoscopy with postbulbar biopsy. RESULTS： IgA-AGA and IgG-AGA were positive at 8.8% and 10.3% in patients with reflux esophagitis. In control group, it was found that 10% people had positive IgA-AGA, and 7.5% people had positive IgG-AGA. There was no significant relationship between patients and control group regarding positive IgA-AGA and IgG- AGA. The patients and persons in control group had no positive IgA-EMA. On postbulbar biopsies, no finding was detected concerning celiac disease. There were no symptoms and signs for gluten enteropathy in patients and control group. CONCLUSION： This review supports that an association does not exist between celiac disease and reflux esophagitis. We think these diseases exist independently from each other.

Coagulation of a giant hemangioma in glans penis with holmium laser

A 21-year-old man presented with an enlarged giant hemangioma on glans penis which also causes an erectile dysfunction （ED） that partially responded to the intracavernous injection stimulation test. Although the findings in magnetic resonance imaging （MRI） indicated a glandular hemangioma, penile colored Doppler ultrasound revealed an invaded cavernausal hemangioma to the glans. Surgical excision was avoided according to the broad extension of the gland lesion. Holmium laser coagulation was applied to the lesion due to the cosmetically concerns. However, the cosmetic results after holmium laser application was not impressive as expected without an improvement in intracavernous injection stimulation test. In conclusion, holmium laser application should not be used to the hemangiomas of glans penis related to the corpus cavernosum, but further studies are needed to reveal the effects of holmium laser application in small hemangiomas restricted to the glans penis.
