management of recto-vaginal fistulas after prosthetic reinforcement treatment for pelvic organ prolapse

AIm: To communicate our findings on successful treat-ment of recto-vaginal fistulas (RVFs) after prosthetic reinforcement surgery of pelvic organ prolapse (POP). mETHODS: A retrospective single center study between 1998 and 2008 was performed. A total of 80 patients with RVF were identified, of which five patients (6%), with a mean age of 65 years (range: 52-73), had undergone previous surgery for POP with pros-thetic reinforcement. RESULTS: All patients complained about ongoing vaginal infections and febrile episodes. These symptomswere reported after a mean period of 18 mo after POP repair. As a first intervention, three patients underwent ablation of the prosthetic material (Pm). As a second intervention, open proctectomy with a primary anas-tomosis, an omental patch, and a protective ileostomy were performed in two patients. One patient required a terminal colostomy due to complete destruction of the anal sphincters. In two other patients, ablation of the Pm and proctectomy was performed as a one-step procedure. The postoperative course in all patients was uneventful, with a mean length of hospitalization of 20 d (range: 15-30). Closure of the ileostomy was achieved in all four patients within four months. After a mean period of 35 mo (range: 4-60) of follow-up, no recurrence was observed with normal continence in four ***: In our experience, the definitive treat-ment of high RVFs after Pm repair for POP necessitates ablation of the Pm, proctectomy with a primary colo-rectal anastomosis, an omental patch interposition, and a temporary ileostomy.

Sirolimus,bevacizumab,5-Fluorouracil and irinotecan for advanced colorectal cancer:A pilot study

AIm： To evaluate the efficacy and the safety of combined 5-Fluorouracil, irinotecan, bevacizumab and sirolimus in refractory advanced colorectal carcinoma. mETHODS： We initiated a regimen with at day 1 an injection （iv） of bevacizumab at 5 mg/kg, followed by 180 mg/m^2 irinotecan, followed by Leucovorin 400 mg/m^2, followed by a 5-Fluorouracil bolus 400 mg/m^2 and a 46-h infusion 2400 mg/m^2. Sirolimus was given orally as continuous administration of 2 mg twice a day every days. This treatment was repeated every 14 d. RESULTS： A total of 12 patients were enrolled. All patients presented with metastatic disease that had failed at least three lines of chemotherapy that contained oxaliplatin, irinotecan and bevacizumab. Cetuximab failure was also observed in all K-Ras wildtype patients. The median number of cycles was 8.5 （range 2-20） and clinical benefit was observed in eight patients. The median time to progression was 5 mo and the median survival was 8 too. Grade 3 neutropenia developed in four patients, and grade 3 diarrhea and stomatitis in ***： The combination regimen of 5-Fluorouracil,irinotecan, bevacizumab and sirolimus in advanced colorectal carcinoma after failure of dassical *** is feasible and promising. The set of proteins required for mitotic division remains poorly characterized. Here, an extensive series of correlation analyses of human and mouse transcriptomics data were performed to identify genes strongly and reproducibly associated with cells undergoing S/G2-m phases of the cell cycle. In so doing, 701 cell cycle-associated genes were defined and while it was shown that many are only expressed during these phases, the expression of others is also driven by alternative promoters. Of this list, 496 genes have known cell cycle functions, whereas 205 were assigned as putative cell cycle genes, 53 of which are functionally uncharacterized. Among these, 27 were screened for subcellular localization revealing many to be nuclear localized and at least three to be novel centrosomal proteins. Furthermore, 10 others inhibited cell proliferation upon siRNA knockdown. This study presents the first comprehensive list of human cell cycle proteins, identifying many new candidate proteins.

Drying conditions in Switzerland-indication from a 35-year Landsat time-series analysis of vegetation water content estimates to support SDGs

Exacerbated by climate change,Europe has experienced series of hot and dry summer since the beginning of the 21st *** importance of land conditions became an international concern with a dedicated sustainable development goal(SDG),the SDG *** calls for developing and finding innovative solu-tions to follow and evaluate impacts of changing land condi-tions induced by various driving *** Switzerland,drought risk will significantly increase in the coming decades with severe consequences on agriculture,energy production and *** this paper,we used a 35-year satellite-derived annual and seasonal times-series of normalized difference water index(NDWI)to follow vegetation water content evolution at different spatial and temporal scales across Switzerland and related them to temperature and precipitation to investigate possible responses of changing climatic *** indicate that there is a small and slow drying tendency at the country scale with a NDWI mean decreasing slope of−0.22%/year for the 23%significant pixels across *** tendency is mostly visible below 2000 m above sea level(m.a.s.l.)and in all biogeographical *** Southern Alps regions appear to be more responsive to changing drying conditions with a significant and slight negative NDWI trend(−0.39%/year)over the last 35 ***,NDWI values are mostly a func-tion of temperature at elevations below the tree line rather than *** suggest that multi-annual and seasonal NDWI can be a valuable indicator to monitor vegetation water content at different scales,but other components such as land cover type and evapotranspiration should be considered to better characterize NDWI *** Earth Observations data can provide valuable complementary obser-vations for national statistics on the ecological state of vegeta-tion to support SDG 15 to monitor land affected by drying conditions.

Dynamics of Water Flow in the Atmosphere-Aerial Roots Continuum

The study of the parameters influencing the absorption of atmospheric humidity not condensed by plants with aerial roots is a challenge in the current context of climatic disturbance where methods of adapting plants to water stress have become the concern of many scientific researchers. Based on Poiseuille and Fick laws, leaf water potential depending on solar radiation and atmospheric water potential around aerial roots;the influence of temperature, relative humidity, inclination angle of roots and solar radiation wavelength on the radial and axial water flow through a root segment is simulated. The various simulations show that a low temperature of the air surrounding the root favors a significant entry of water into the root as well as a relative humidity of the high air approaching 100%. The angle of inclination has an influence on the quantity of water entering the root and that rising in the xylem. These different streams of water are optimized for root tilting of 60°. The wavelength of the red radiation received by the leaves favors more entry and rise of water in the root. Future studies for transgenes should be taken into account the influence of all these parameters.

Is it possible to adopt the same oncological approach in urgent surgery for colon cancer?
