Evaluation of participants' feedback after a simulation-based training in neonatal resuscitation using a realistic delivery room

BACKGROUND: Low-frequency and high-risk situations, such as neonatal resuscitation, are the ideal targets for simulation-based learning. The aim of this paper is to present the structure of our internal neonatal resuscitation training program, using a realistic, simulated delivery room, and to present the participants' opinions about teamwork, emotional stress, and their subjective ability to face a resuscitation. METHODS: We administered a training course to 24 doctors and midwives. One of the simulation classrooms was modified to appear similar to a real delivery room. Four scenarios were conducted using a previously designed checklist of primary and secondary goals. Upon completion, all students participated in a debriefing session with the help of a video review. RESULTS: Students rated the achievement of their previously defined goals on a scale of 1 to 5. Grouping together the percentages of the highest ratings (Categories 4 and 5), 83.4% (20/24) of the students considered the course useful for acquiring clinical skills. For 87.5% (21/24) of the students, the scenarios simulated real clinical situations, the room properly simulated a real delivery room, and the course improved the students' ability to work in a team. For 66.6% (16/24) of the students, the course improved their stress in confronting neonatal resuscitation. Initially, only 33.3% (8/24) of the students considered themselves very capable or fully able to cope with a resuscitation. After the course, that percentage rose to 62.5% (15/24). CONCLUSIONS: The incorporation of simulation-based learning into neonatal resuscitation teaching programs, using realistic scenarios, is useful and offers the possibility of acquiring technical skills, but it also allows for the improvement of teamwork and the adoption of different roles and positive attitudes towards emotional stress.

An alternative model to the well-established paradigm of the externally switchable drug delivery systems is herein proposed. In contrast to the on--off archetype, here the amount of released drug is pre-set by the application of an external stimulus, and is gradually released after the withdrawal of the exogenous signal. These attributes are achieved through an innovative approach featuring the integration of plasmonic nanovehicles in a polymer-based film. Such a platform is provided with optically responsive capabilities together with multiple diffusional barriers, allowing for an ＂on-demand＂ time-limited release where light acts as a therapeutic ＂starting shot＂. These nanoarchitectured depots have great potential as implantable drug delivery systems in clinical scenarios where a recurrent, sustained, and yet, on--off administration of medication is required. The application of these hybrid materials may extend the implementation of nanomedicine strategies beyond the point-of-care setting.

Age-adjusted NT-proBNP could help in the early identification and follow-up of children at risk for severe multisystem inflammatory syndrome associated with COVID-19(MIS-C)

BACKGROUND Multisystem inflammatory syndrome in children(MIS-C) has emerged as a new disease associated with COVID-19 that presents in acute critically ill children with acute cardiovascular *** To determine whether the age-adjusted N-terminal pro-brain natriuretic peptide(NT-proBNP) value(Z-log-NT-proBNP) is associated with severe MIS-C and myocardial *** A retrospective study was conducted which included children with MIS-C managed at our institution between April 1,2020,and February 28,*** divided the population into groups depending on severity based on pediatric intensive care unit(PICU) *** compared Z-log-NT-proBNP values across these groups and analyzed Z-log-NT-proBNP dynamics during the onemonth *** We included 17 participants [median age 3(2-9) years] and seven(41%) required PICU ***(100%) of these cases presented very high(Z-log > 4) levels of NT-proBNP at the time of admission compared to only 5(50%) patients with non-severe MIS-C(P = 0.025).NT-proBNP was significantly correlated with highsensitive Troponin I levels(P = 0.045),Ross modified score(P = 0.003) and left ventricle ejection fraction(P = 0.021).CONCLUSION Raised NT-proBNP,specifically very high values(Z-log-NT-proBNP > 4) could help in the early identification of MIS-C patients with myocardial dysfunction requiring inotropic support and PICU admission.

Emerging technologies such as electric vehicles,solid-state transformers,and DC transformers are implemented using the closed-loop bi-directional dual-active-bridge(DAB)*** this context,it is necessary to have average models that provide an efficient way of tuning the proportional integral(PI)compensator parameters for large-and small-signal *** present a novel small-signal model(SSM)for DAB converter with a single closed-loop PI controller and the total elimination of reactive current(IQ=0).The method applies a modulation technique for IQ=0 and introduces a composite function in the controller while reducing the original nonlinear switching model,which allows to decrease the order of the transfer function and analyze the closed-loop *** proposed SSM is analyzed using different response time,load,and DC voltage *** simulation and experimental results demonstrate the ease of implementation and effectiveness of the proposed model with respect to other SSM techniques.

A novel and stretchable carbon-nanotube/Ni@TiO_(2):W photocatalytic composite for the complete removal of diclofenac drug from the drinking water

We present the structural,morphological and photocatalytic properties of stretchable composites made with carbon nanotubes (CNTs),silicon rubber and Ni@TiO_(2):W nanoparticles(TiWNi NPs) with average size of 37±2 *** images showed that the TiWNi NPs decorated the surface of the CNT fibers,which are oriented in a preferential *** NPs presented a mixture of anatase/rutile phases with cubic *** performance of the TiWNi powders and stretchable composites was evaluated for the photocatalytic degradation of diclofenac (DCF) anti-inflammatory drug under ultraviolet-visible *** results revealed that the maximum DCF degradation percentages were 34.6%,91.9%,97.1%,98.5%and 100%for the CNT composite (stretched at 0%),TiWNi powders,CNT+TiWNi(stretched at 0%),CNT+TiWNi (stretched at 50%) and CNT+TiWNi (stretched at 100%),***,stretching the CNT+TiWNi composites was a good strategy to enhance the DCF degradation percentage from 97.1%to 100%,since stretching created additional defects (oxygen vacancies) that acted as electron sink,delaying the electron-hole recombination,and favors the DCF ***/absorbance measurements confirmed the presence of such ***,the reactive oxygen species (ROS) were determined by the scavenger’s experiments and found that the main ROS were the·OH and O_(2)-radicals,which attacked the DCF molecules,causing their *** results of this investigation confirmed that the stretchable CNT/TiWNi-based composites are a viable alternative to remove pharmaceutical contaminants from water and can be manually separated from the decontaminated water,which is unviable using photocatalytic powders.

Metamizole(MZ)is a widely used anti-inflammatory *** to its common use,this contaminant is found in sewage and *** order to reduce the contamination produced by the MZ,we fabricated in this work a photocatalytic composite using recycled polyethylene(RPE)and the CaLaCoO_(9)(LCCO)*** nanoparticles had a microplate-like morphology and sizes of 1.4–5.5μm according to the analysis of *** photocatalytic properties of the LCCO powders were evaluated under ultraviolet-visible(UV-Vis)irradiation and found a removal efficiency of 96%.When the RPE+LCCO composite was employed for the photocatalytic degradation of MZ,a maximum degradation of 92.5%was *** influence of the pH on the photocatalytic activity was also studied and found that an initial pH=3 produced a total degradation of MZ after 240 min of UV-Vis ***,three reuse cycles were carried out for the pure LCCO powders and for the RPE+LCCO composites and found that the maximum loss of degradation was 5%.Furthermore,scavenger experiments demonstrated that the super oxide and hydroxyl radicals are formed during the photocatalytic reaction and were responsible for the degradation of ***,the X-ray photoelectron-spectroscopy and Raman analysis demonstrated the formation of defects(oxygen vacancies),those ones delayed the electron-hole recombination,which in turn,enhanced the degradation of the ***,the studies performed in this work proved that composites made with recycled plastics and LCCO perovskites are a low-cost and feasible alternative for the cleaning of water sources polluted with pharmaceutical compounds.

In this paper we show several mathematical properties of defensive （offensive） k-alliance free sets and defensive （offensive） k-alliance cover sets, including tight bounds on their cardinality.

Clinical presentation and early predictors for poor outcomes in pediatric myocarditis:A retrospective study

BACKGROUND Myocarditis is an important cause of morbidity and mortality in children, leading to long-term sequelae including chronic congestive heart failure, dilated cardiomyopathy, heart transplantation, and death. The initial diagnosis of myocarditis is usually based on clinical presentation, but this widely ranges from the severe sudden onset of a cardiogenic shock to asymptomatic patients. Early recognition is essential in order to monitor and start supportive treatment prior to the development of severe adverse events. Of note, many cases of fulminant myocarditis are usually misdiagnosed as otherwise minor conditions during the weeks before the unexpected *** To provide diagnostic clues to make an early recognition of pediatric *** investigate early predictors for poor *** We conducted a retrospective cross-sectional single-center study from January 2008 to November 2017 at the Pediatric Department of our institution, including children < 18-years-old diagnosed with myocarditis. Poor outcome was defined as the occurrence of any of the following facts: death, heart transplant, persistent left ventricular systolic dysfunction or dilation at hospital discharge(early poor outcome), or after 1 year of follow-up(late poor outcome). We analyzed different clinical features and diagnostic test findings in order to provide diagnostic clues for myocarditis in children. Multivariable stepwise logistic regression analysis was performed using all variables that had been selected by univariate analysis to determine independent factors that predicted a poor early or late outcome in our study *** A total of 42 patients [69% male; median age of 8(1.5-12) years] met study inclusion criteria. Chest pain(40%) was the most common specific cardiac symptom. Dystrophin is a membrane-associated protein responsible for structural stability of the sarcolem-ma in cardiac myocytes and is very sensitive to ischemic *** goal of our study was to determine if ischemic preconditioning could prevent dystrophin breakdown through inhibition of matrix metalloprotein-ase-2(MMP-2)*** rabbit hearts were subjected to global ischemia with or without reperfusion in order to evaluate if dystrophin is preserved by ischemic preconditioning through MMP-2 *** preconditioning significantly reduced the infarct size induced by 30 min of ischemia and 180 min of ***,it also diminished dystrophin proteolysis and attenuated MMP-2 activity after 30 min ***,our study shows a novel protective role of ischemic preconditioning as a mechanism of preservation of plasma membrane integrity by inhibiting MMP-2 activation.
