Probe-based confocal endomicroscopy is accurate for differentiating gastric lesions in patients in a Western center

Objective： Probe-based confoeal laser endomicroscopy （pCLE） technique may improve the diagnosis of gastric mucosal lesions allowing acquisition of high-resolution in vivo images at the cellular and mierovaseular levels. This study aims to evaluate the accuracy of pCLE for the differential diagnosis of non-neoplastic and neoplastic gastric ***： Twenty gastric mucosal lesions from 10 patients were evaluated during endoscopic procedure and were examined by pCLE. Diagnostic pCLE was followed by biopsies or endoscopic resection of suspected lesions. A senior pathologist evaluated the specimens and was blinded to the pCLE ***： Patients＇ mean age was 68.3 （range, 42-83） years and six were men. Thirteen suspicious flat or elevated lesions （classified as 0-Is, 0-IIa or 0-IIa ＋ IIc） and seven pre-malignant lesions （atrophy and intestinal metaplasia） were evaluated. One patient was studied during his long-term follow-up after partial gastrectomy and presented severe atrophy, intestinal metaplasia, and xanthomas at the stump mucosa. The location of gastric lesions was in the body （n=10 lesions）, the antrum （n=9） and the incisura angularis （n=1）. All neoplastic lesions and all but one benign lesion were properly diagnosed by pCLE. pCLE incorrectly diagnosed one small antrum lesion as adenoma, however the final diagnosis was intestinal metaplasia. The final histological diagnosis was neoplastic in 9 and benign lesions in 11. In this small case series, pCLE accuracy was 95% （19/20 lesions）.Conclusions： pCLE is accurate for real time histology of gastric lesions, pCLE may change the management of patients with gastric mucosal lesions, guiding biopsies and endoscopic resection, and avoiding further diagnostic workup or unnecessary therapy.

Colorectal cancer(CRC)is one of the most prevalent malignancies worldwide,being the third most commonly diagnosed malignancy and the second leading cause of cancer-related deaths *** the progress in screening,early diagnosis,and treatment,approximately 20%-25%of CRC patients still present with metastatic disease at the time of their initial ***,the burden of disease is still expected to increase,especially in individuals younger than 50 years old,among whom early-onset CRC incidence has been *** and early detection are pivotal to improve CRC-related *** is well established that CRC screening not only reduces incidence,but also decreases deaths from *** screening strategies have proven effective in decreasing both CRC incidence and mortality,though variations in efficacy have been reported across the ***,uncertainties persist regarding the optimal screening method,age intervals and ***,adherence to CRC screening remains globally *** recent years,emerging technologies,notably artificial intelligence,and non-invasive biomarkers,have been developed to overcome these ***,controversy exists over the actual impact of some of the new discoveries on CRC-related outcomes and how to effectively integrate them into daily *** this review,we aim to cover the current evidence surrounding CRC *** will further critically assess novel approaches under investigation,in an effort to differentiate promising inno-vations from mere novelties.

Endoscopic management and prevention of migrated esophageal stents

The use of self-expandable metallic stents has increased recently to palliate inoperable esophageal neoplasia and also in the management of benign strictures. Migration is one of the most common complications after stent placement and the endoscopist should be able to recognize and manage this situation. Several techniques for managing migrated stents have been described, as well as new techniques for preventing stent migration. Most stents have a "lasso" at the upper flange which facilitates stent repositioning or removal. An overtube, endoloop and large polypectomy snare may be useful for the retrieval of stents migrated into the stomach. External fixation of the stent with Shim's technique is efficient in preventing stent migration. Suturing the stent to the esophageal wall, new stent designs with larger flanges and double-layered stents are promising techniques to prevent stent migration but they warrant validation in a larger cohort of patients.

Among the impacts of climate change,there is the intensification of phenomena such as the El Niño South-ern Oscillation(ENSO)responsible for El Niño and La Niñ***,understanding their effects on the functional pro-cesses of forests is ***,this study evaluated the effects of ENSO on litter stock and water holding capac-ity(WHC)in a successional forest in eastern *** occurred in periods with the most rainfall in El Niño(2019)and least in La Niña(2021)*** permanent plots were used to sample *** effects were evident for WHC,higher during El Niñ***,this influence was not clear for litter,as only in the rainy season effects were *** was a positive correlation of WHC with precipitation and humidity,while litter stocks were negatively correlated with temperature and wind *** the subject of this study requires long-term assessments,preliminary results suggests that,depending on the intensity of ENSO,forest functional processes can be strongly impacted and *** conclusion reinforces warnings by the scientific community about the impacts of climate change on the maintenance of litter stocks,decomposition and,consequently,the biogeochemical cycle and essential ecosystem services for the maintenance of Amazonia *** need to develop long-term research to understand the effects of climatic change on litter stocks and water holding capacity is highlighted,especially in Amazonia.

Association between mucosal surface pattern under near focus technology and Helicobacter pylori infection

BACKGROUND Many studies evaluated magnification endoscopy(ME)to correlate changes on the gastric mucosal surface with Helicobacter pylori(***)***,few studies validated these concepts with high-definition endoscopy without *** To access the association between mucosal surface pattern under near focus technology and *** infection status in a western *** Cross-sectional study including all patients referred to routine upper *** exams were performed using standard high definition(S-HD)followed by near focus(NF-HD)*** of erythema,erosion,atrophy,and nodularity were recorded during S-HD,and surface mucosal pattern was classified using NF-HD in the gastric *** were taken for rapid urease test and *** One hundred and eighty-seven patients were analyzed from August to November *** those,47(25.1%)were ***+,and 42(22.5%)had a previous *** *** the examination with S-HD,erythema had the best sensitivity for *** detection(80.9%).Exudate(99.3%),nodularity(97.1%),and atrophy(95.7%)demonstrated better specificity values,but with low sensitivity(6.4%-19.1%).On the other hand,the absence of erythema was strongly associated with ***-(negative predictive value=92%).With NF-HD,56.2%of patients presented type 1 pattern(regular arrangement of collecting venules,RAC),and only 5.7%of RAC+patients were ***+.The loss of RAC presented 87.2%sensitivity for *** detection,70.7%specificity,50%positive predictive value,and 94.3%negative predictive value,indicating that loss of RAC was suboptimal to confirm *** infection,but when RAC was seen,*** infection was *** The presence of RAC at the NF-HD exam and the absence of erythema at S-HD were highly predictive of *** negative *** the other hand,the loss of RAC had a suboptimal correlation with the presence of ***.

Systematic Review on Ground-Based Cloud Tracking Methods for Photovoltaics Nowcasting

Renewable energies are highly dependent on local weather conditions, with photovoltaic energy being particularly affected by intermittent clouds. Anticipating the impact of cloud shadows on power plants is crucial, as clouds can cause partial shading, excessive irradiation, and operational issues. This study focuses on analyzing cloud tracking methods for short-term forecasts, aiming to mitigate such impacts. We conducted a systematic literature review, highlighting the most significant articles on cloud tracking from ground-based observations. We explore both traditional image processing techniques and advances in deep learning models. Additionally, we discuss current challenges and future research directions in this rapidly evolving field, aiming to provide a comprehensive overview of the state of the art and identify opportunities for significant advancements in the next generation of cloud tracking systems based on computer vision and deep learning.

Spices are defined as any aromatic condiment of plant origin used to alter the flavor and aroma of foods. Besides flavor and aroma, many spices have antioxidant activity, mainly related to the presence in cloves of phenolic compounds, such as flavonoids, terpenoids and eugenol. In turn, the most common uses of gum arabic are in the form of powder for addition to soft drink syrups, cuisine and baked goods, specifically to stabilize the texture of products, increase the viscosity of liquids and promote the leavening of baked products (e.g., cakes). Both eugenol, extracted from cloves, and gum arabic, extracted from the hardened sap of two species of the Acacia tree, are dietary constituents routinely consumed virtually throughout the world. Both of them are also widely used medicinally to inhibit oxidative stress and genotoxicity. The prevention arm of the study included groups: Ia, IIa, IIIa, Iva, V, VI, VII, VIII. Once a week for 20 weeks, the controls received saline s.c. while the experimental groups received DMH at 20 mg/kg s.c. During the same period and for an additional 9 weeks, the animals received either water, 10% GA, EUG, or 10% GA + EUG by gavage. The treatment arm of the study included groups Ib, IIb, IIIb e IVb, IX, X, XI, XII). Once a week for 20 weeks, the controls received saline s.c. while the experimental groups received DMH at 20 mg/kg s.c. During the subsequent 9 weeks, the animals received either water, 10% GA, EUG or 10% GA + EUG by gavage. The novelty of this study is the investigation of their use alone and together for the prevention and treatment of experimental colorectal carcinogenesis induced by dimethylhydrazine. Our results show that the combined use of 10% gum arabic and eugenol was effective, with antioxidant action in the colon, as well as reducing oxidative stress in all colon segments and preventing and treating genotoxicity in all colon segments. Learning to combine multiple string similarity metrics for effective toponym matching

Several tasks related to geographical information retrieval and to the geographical information sciences involve toponym matching,that is,the problem of matching place names that share a common *** this article,we present the results of a wide-ranging evaluation on the performance of different string similarity metrics over the toponym matching *** also report on experiments involving the usage of supervised machine learning for combining multiple similarity metrics,which has the natural advantage of avoiding the manual tuning of similarity *** with a very large dataset show that the performance differences for the individual similarity metrics are relatively small,and that carefully tuning the similarity threshold is important for achieving good *** methods based on supervised machine learning,particularly when considering ensembles of decision trees,can achieve good results on this task,significantly outperforming the individual similarity metrics.

This study investigated the effects of yerba mate consumption, a South American beverage, on cardiac remodeling in rats. For this purpose, 24 male Wistar rats were divided into Control Group (CG) which received filtered water and a standard diet, and Yerba Mate Group (YM), 6 g of Ilex paraguariensis in 100 ml water and the same diet, for 30 days. The YM group showed a reduction in final body weight and food consumption without altering weight gain. Regarding cardiac remodeling, the YM group exhibited a decrease in the right ventricle weight/final body weight ratio, suggesting cardiac atrophy, without affecting the atria and left ventricle. There was no change in cardiomyocyte area or nuclear fractal dimension in both groups. However, animals that consumed yerba mate showed increased collagen deposition and a smaller fractal dimension in the left ventricle. The consumption of yerba mate at room temperature for 30 days induced changes in cardiac remodeling, as evidenced by increased collagen deposition and alterations in fractal dimension in the left ventricle.
