Comparative Study of Bovine Pericardium and Gore-Tex in Tissual Interaction with Wistar Rats Diaphragm

Introduction: Simple suture isn’t always possible in large congenital diaphragmatic hernia (cdh) defects. Synthetic materials are used for correction such as Silastic®, Gore-Tex® (GT), Teflon® or biological, such as autologous muscle patches. It was shown that bovine pericardium (bp) was effective to correct those large defects with many positive outcomes when compared with syntactic materials. Aim: This study aims to establish an experimental model of correction for large diaphragmatic defect with PB and GT patches to compare histologically the tissue interaction between them and diaphragm in young Wistar rats. Materials & Methods: 15 wistar rats were divided in 3 groups: Rats that used BP was named G1;Rats that used GT was named G2;and rats with only scraping in the diphragm, named G3 (control). Animals were submited to a laparotomy and fixed pathces to diaphragms and harvested 3 weeks later. Area between normal diaphragm and patches were isolated and separated for histological analysis, such as lymphocytic infiltration (inflammation), neovascularization and fibrosis. Results: G1 presented inflammation between BP and Diaphragm In 5 Samples. G2 Presented Neovascularization In 5 Samples, But No inflammation. Fibrotic tissue overlapping GT patches occurred in 3 samples in G2. Comparing G1 with G2 there was a significant statistical difference concerning inflammation (P = 0.0079), in G1. Comparing neovascularization there is no significant statistical difference (P = 0.4444), despite a slight higher incidence in G2. Fibrosis in both groups presented no significant statistical difference (P = 0.4444), despite a slight higher incidence in G2. There were no alterations in G3. Discussion: Despite the statistical difference in the inflammatory process was more frequent in G1 (P = 0.0079), neovascularization and fibrosis were more frequent in G2. 背景:疼痛症在临床上极为常见,目前应用针灸治疗具有较好的效果。然而针灸疗法方法甚多,各有所长,认识不同针法对疼痛症治疗的疗效差异,在临床上具有重要意义。1999年,应厄瓜多尔医学疼痛学会的要求,开展此项观察。目的:观察不同针法对不同疾病疼痛症的疗效差异。设计:随机对照观察。单位:厄瓜多尔三军总医院疼痛科,成都军区总医院中医科、外一科。对象:选择1999-05/2000-11厄瓜多尔三军总医院疼痛科门诊腰肌劳损、增生性脊柱炎患者360例,其中腰肌劳损179例,增生性脊柱炎181例。随机对两类疾病患者进行分组,即腰肌劳损皮肤针组88例,体针组91例;增生性脊柱炎皮肤针组92例,体针组89例。方法:分别运用皮肤针、体针对上述疾病患者进行治疗,并同时对治疗前后患者症状加以观察。隔日治疗1次,腰肌劳损5次为基本疗程,增生性脊柱炎则为7次。治疗结果按Budzynski疼痛分级标准进行评定,统计方法采用MEPS统计软件处理数据。主要观察指标:皮肤针、体针组对腰肌劳损、增生性脊柱炎治疗前后患者疼痛症状缓解情况,并对同种疗法治疗异病、不同疗法治疗同病的疗效、疗程进行相关分析。结果:观察病例366例,剔除病例6例,实际完成完整病例360例。①皮肤针、体针疗法治疗后患者的疼痛分级均较治疗前减轻(P0.05)。④两组患者疼痛分级减少1个等级时,平均治疗次数皮肤针组少于体针组(P<0.05)。结论:皮肤针与体针对增生性脊柱炎、腰肌劳损两类疾病疼痛症的治疗上,均有较好疗效,其中,以皮肤针对软组织损伤疗效明显,且起效迅速;体针对增生性脊柱炎患者疼痛改善上较皮肤针更为彻底。

G protein-coupled receptor kinase 2 （GRK2） is an important serine/threonine-kinase regulating different membrane receptors and intraceUular proteins. Attenuation of Drosophila Gprk2 in embryos or adult flies induced a defective differentiation of somatic muscles, loss of fibers, and a flightless phenotype. In vertebrates, GRK2 hemizygous mice contained less but more hypertrophied skeletal muscle fibers than wild-type littermates. In C2C12 myoblasts, overexpression of a GRK2 kinase-deficient mutant （K220R） caused precocious differentiation of ceUs into immature myotubes, which were wider in size and contained more fused nuclei, while GRK2 overexpression blunted differentiation. Moreover, p38MAPK and Akt pathways were activated at an earlier stage and to a greater extent in K220R-expressing cells or upon kinase downregulation, while the activation of both kinases was impaired in GRK2-overexpressing cells. The impaired differentiation and fewer fusion events promoted by enhanced GRK2 levels were recapitulated by a p38MAPK mutant, which was able to mimic the inhibitory phosphorylation of p38MAPK by GRK2, whereas the blunted differentiation observed in GRK2-expressing clones was rescued in the presence of a constitutively active upstream stimulator of the p38MAPK pathway. These results suggest that balanced GRK2 function is necessary for a timely and complete myogenic process.

What is the prognosis of emphysematous pyelonephritis associated with extended-spectrum beta-lactamases producing microorganisms?

Objective:To describe the microbiological characteristics in emphysematous pyelonephritis(EPN),demonstrate the frequency of extended-spectrum beta-lactamase(ESBL)microorganisms,and determine if these microorganisms are associated with the prognosis of patients with ***:We conducted a retrospective study in patients with a diagnosis of EPN in a tertiary care hospital of the northeast region of Mexico during the period from January 2011 to January *** variables were analyzed to determine association with the presence of ESBL-producing *** significance was set with p<***:A total of 63 patients were included;55(87.3%)of them were females,with a median age of 55(interquartile range:45-65)*** management was indicated in 38.1%;42.9%were treated with ureteral stent;12.7%with open or percutaneous drainage;15.8%with early nephrectomy;and 9.5%with delayed *** mortality was 13(20.6%)cases;23(36.5%)cases required admission to the intensive care *** most frequent microorganism isolated was Escherichia coli(n=34,53.9%).ESBL microorganisms were found in 31.7%of the *** significant association of ESBL was found with admission to the intensive care unit,or with increased ***:To our knowledge,this is the first study that evaluates ESBL microorganisms as a prognostic factor in *** factors associated with a poor prognosis in patients with EPN have been *** microbiological factors,specifically ESBL-producing bacteria,do not seem to influence in the prognosis of these patients.

A lead-free relaxor ferroelectric,Sr_(0.15)Ba_(0.85)Bi_(2)Nb_(2)O_(9),was synthesized via solid-state reaction and the temperature-dependence of the heat capacity was measured in a wide temperature *** dielectric permittivity was also measured between 500 Hz and 5 MHz in the same temperature *** anomaly has been detected in the heat capacity curve for the whole temperature range covered in the present experiments,while broad peaks have been observed in the dielectric permittivity with high frequency dispersion.A typical relaxor behavior has been observed from the dielectric *** Debye's temperature has showed a minimum value near the freezing *** results are discussed considering the spin-glass model and the high frequency dispersion,which has been observed for the studied relaxor system.

Mass Media and Urban Tribes, Analysis of Their Relations by Stafford Beer＇s Viable System Model

This paper discusses the relations between mass media and urban tribes, by Stafford Beer＇s viable system model （VSM）. During 1950-2010, the influence of media and advertising over individual decision-making processes has deepened of age range to which have targeted advertising and expanded for Western world, leading the culture diversity, naturally given, toward only one standardized culture prevailing nowadays. In the 1960s they were the women, then the young people during 1970s, and then children in the 1980s and during the 1990s over early childhood, when we learnt to speak （3 years old）, achieving brand loyalty among very young children and the brands of products for this age group. From this perspective, urban tribes have taken control of the lives of young people, offering a false freedom, at the moment of choosing ＂How to be yourself＂ from options given in the market as a ＂catalogue of predesigned identities＂. A phenomenon is so complex, so it requires a holistic framework able to make explicit its components and relations between them and the relationship with internal and external environment. In particular, the VSM provides a way to analyze communication＇s problems thus clarifying： how, when and where, repair and controlling into the organization, identifying strategies to improve decision-making. Given the recursive nature of the VSM, it is possible to navigate within the phenomenon, through different recursions＇ levels. It will help to define more assertive and effective policies by health authorities, education and government in general, and also help someone among young people to see themselves inside of a dynamic consumption and thus can leave the domain and enter into a recta-domain, with more wisdom and consciousness.

Case Series of 10 Drug-Refractory IBS Patients Who Respond to Oral Serum-Derived Bovine Immunoglobulin/Protein Isolate (SBI)

Aim: The responses of 10 patients with long-standing, symptomatic, intractable drug-refractory histories of irritable bowel syndrome with diarrhea (IBS-D) and with abdominal pain, gas/bloating and distention, termed IBS undefined (IBS-U), were evaluated when administering a medical food product containing serum-derived bovine immunoglobulin/protein isolate (SBI). Methods: Patients in this case series were chosen based on their lack of satisfactory response to a variety of drugs, including antidiarrheal and antispasmodic medications, serotonin 5-HT3 receptor antagonists, selective serotonin re-uptake inhibitors (SSRIs), proton pump inhibitors (PPIs), antibiotics, and antidepressive drugs. Patients met Rome III criteria and were administered 5 g/day of SBI as standard-of-care nutritional support. A scale of 0% - 25%, 25% - 50%, 50% - 75%, 75% - 100% response to SBI was used for patient-reported improvement in overall IBS symptoms following administration for one month. Exact methods for calculating confidence intervals and p-values were used to assess complete management of symptoms and response to therapy. Adverse events were also monitored for this nutritional product. Results: The onset of gastrointestinal (GI) symptom reduction utilizing nutritional management with SBI occurred within an average time of 2-4 weeks with improved or near complete management in all 10 patients who were refractory to previous drug therapies by 4 weeks. When prompted, patients reported significant IBS symptom improvement which averaged between 50% - 100% (p = 0.002) with an average for complete management in all patients of 69%. No side effects were reported after SBI administration even when taken for up to 28 weeks. Conclusion: Based on the safety profile and reported outcomes in this case report, SBI should be considered as a nutritional option for management in IBS-D and IBS-U.

Using molecular and quantitative variation for assessing genetic impacts on Nucella lapillus populations after local extinction and recolonization

The dogwhelk Nucella lapillus is a predatory marine gastropod living on rocky shores in the North *** with many other gastropod species,Nucella was affected by tributyltin(TBT)pollution during the 1970s and 1980s,and local populations underwent *** a partial ban on TBT in the UK in 1987,vacant sites have been *** of genetic diversity and quantitative genetic variation in shell form were compared between recolonized sites and sites that showed continuous population at three localities across the British ***,estimates of genetic diversity were only slightly lower in recolonized populations,suggesting that populations have recovered from previous impacts due to the relatively high levels of migration from non-impacted *** and quantitative analyses are broadly concordant and a positive correlation was observed(although not statistically significant)between molecular and quantitative estimates of genetic diversity,indicating the potential usefulness of quantitative methods to complement molecular population genetics analyses.
