背景输注红细胞（RBC）与心脏手术后死亡率升高有关。我们分析了在住院期间接受了1或2个单位RBC输注的心脏手术患者的长期生存情况。方法将2001年至2004年期间9079例，在新英格兰北部地区8个医疗中心中接受了冠状动脉旁路移植术、瓣膜手术或同时进行了冠状动脉旁路移植术和瓣膜手术的患者纳入本研究。研究将纳入地区与2006年6月30日社会保障署死亡档案确定的死亡率进行概率性配对。调整的风险比采用Cox比例风险和倾向评分法进行计算。结果36％的患者（n=3254）接受过1或2个单位的RBC。43％的RBCs是在手术中被输注的，分别有56％和1％的RBCs是在手术后和手术前输注的。输血的患者更可能是贫血、高龄、体表面积小、女性和同时患多种疾病的患者。那些在心脏手术住院期间接受了1或2个单位RBCs的患者的存活率显著低于未接受过RBCs的患者（P〈0．001）。调整了患者和疾病特征之后，接受1或2个单位RBCs仍使远期死亡风险提高16％（调整的风险比=1．16，95％CI：1．01—1．34，P=0．035）。结论输注1或2个单位的RBCs可使心脏手术术后存活率降低的风险提高16％。

在近期的《麻醉与镇痛》杂志中，Ezaru等。给出了15年问有关颈内静脉置管和意外颈动脉置管的数据。在经历了一次警讯事件之后，Ezaru等作者制定了一项强制性政策，即在放置大口径导管之前，对延长管内回血的压力进行测量以确认套管针是否在颈动脉内。据作者报道，15年间，在9859例颈内静脉置管的患者中，无一例发生意外动脉置管和动脉损伤的不良事件。因此，作者建议将这种简单的测压法常规用在颈内静脉导管置入之前，以避免动脉置管和损伤。

Neuroendocrine tumors(NET)previously called carcinoid tumors are neoplasms of enterochromaffin/neuroendocrine cell origin which display neurosecretory capacity that may result in the carcinoid *** annual incidence of patients with NET is 8.4 per 100000;yet many NET remain asymptomatic and clinically undetected.A maj.rity of NET follows a benign course;however,some will display malignant *** most commonly occur in the gastrointestinal tract(67%)and bronchopulmonary system(25%).Gastrointestinal NET occur within the stomach,small intestine,liver,and *** report a retrospective study of 11 subj.cts:Eight with benign carcinoid tumors:duodenal bulb(n=2),terminal ileum(n=1),sigmoid colon(n=2),and rectum(n=3);three with malignant carcinoid:liver(n=1)and intra-abdominal site(n=2).The diagnosis,endoscopic images,outcome,treatment and review of the literature are presented.

背景有少量的病例研究提倡静脉输注丙泊酚来治疗慢性每日头痛（chronicdailyheadache；CDH），然而，尚无该疗法的随机对照试验。在本随机双盲对照试验中，我们的目的是观察单剂量的静脉丙泊酚输注（2．4mg＆g）能否显著降低CDH患者临床不适及接下来30天的疼痛。方法符合标准的CDH成人患者，分别静脉输注丙泊酚（n=20）或安慰剂咪达唑仑（n=20）对照组。主要的结果评估包括：①治疗30天后头痛失劳动力调查评分（HeadacheDisabilityInventory；HDI）；②头痛指数（HeadacheIndex，HI），治疗期间30天的头痛强度总评估；③采用用药量化评分表（MedicationQuantificationScale，第1II版）评估镇痛药消耗量。结果输注丙泊酚30天后，HDI降低了9．47点（标准差14．1）（P=0．009），但是其降低程度比认为头痛相关失劳动力调查指数（HDI）有临床意义的降低程度更小。与对照组相比，HDI无显著统计差异。在降低平均头痛强度（用头痛指数评估）或用药量化表（MedicationQuantification Scale）（第111版）方便，组内及两组间无统计学差异。结论单剂量的丙泊酚静脉输注（2．4mg/kg）产生了有统计学意义的变化，但是在输注药物后30天CDH失劳动力方面的改变无临床意义。丙泊酚也不能减轻头痛强度和减少镇痛药的应用。本研究不支持静脉输注丙泊酚临床治疗CDH。

Capacitively coupled shortwave radiofrequency fields(13.56 MHz)resistively heat low concentrations(~1 ppm)of gold nanoparticles with a thermal power dissipation of~380 kW/g of *** diameter gold nanoparticles(j.ule heating model has been developed to explain this phenomenon and provides critical insights into the rational design and engineering of nanoscale materials for noninvasive thermal therapy of cancer.
