There is a strong evidence supporting the hypothesis of synaptic dysfunction as a major contributor to neural circuit and network disruption underlying emotional and mood dysregulation in psychiatric disorders(Simmons et al.,2024).Diverse sets of distinct molecular signaling pathways converge on the synapse to regulate synaptogenesis,synaptic function,and synaptic plasticity in brain regions and circuits through complex interactions organized by numerous multivalent protein scaffolds,including the family of proteins known as A-kinase anchoring proteins(AKAPs).

Primary sclerosing cholangitis(PSC) is a rare cholestatic liver disease characterized by progressive fibroinflammatory destruction of the intra-and/or extrahepatic biliary ducts. While its features and disease course can be variable,most patients with PSC have concurrent inflammatory bowel disease and will eventually develop liver cirrhosis and end-stage liver disease, with liver transplantation representing the only potentially curative option. Importantly,PSC is associated with a significantly increased risk of malignancy compared to the general population, mainly cholangiocarcinoma, gallbladder carcinoma,hepatocellular carcinoma, and colorectal cancer, with nearly 50% of deaths in patients with PSC being due to cancer. Therefore, robust surveillance strategies are needed, though uncertainty remains regarding how to best do so. In this review, we discuss the epidemiology, prevention, and surveillance of cancers in patients with PSC. Where evidence is limited, we present pragmatic approaches based on currently available data and expert opinion.

Point-of-care ultrasound(POCUS)is a core component of emergency medicine(Em)residency training used to guide medical decision making and to enhance procedural competence.[1-3]many repetitions of POCUS exams are required to achieve competency,and it is difficult to quantify a minimum needed to achieve mastery.[4,5]For Em residents whose career goals require greater depth of knowledge and skill in POCUS,educators can consider expanded training through a professional development track.

Primary sclerosing cholangitis(PSC)is a chronic,progressive,hepatobiliary disease characterized by inflammation and fibrosis of the intra-and extra-hepatic bile *** natural history is one that generally progresses towards cirrhosis,liver failure,cholangiocarcinoma,and ultimately disease-related death,with a median liver transplantation-free survival time of approximately 15-20 ***,despite its lethal nature,PSC remains a heterogenous disease with significant variability in outcomes amongst different regions of the *** are also many regions where the outcomes of PSC have not been studied,limiting the overall understanding of this disease *** this review,we present the geoepidemiologic variations in outcomes of PSC,with a focus on survival pre-and post-liver transplantation as well as the concurrence of inflammatory bowel disease and hepatobiliary neoplasia.

In chemical product design,the aim is to formulate a product with desired *** and internal product structure are two key drivers of product performance with direct impact on the mechanical,electrical,and thermal ***,there is a keen interest in elucidating the dependence of product performance on ingredients,structure,and the manufacturing process to form the *** of product structure,particularly microstructure,is an intrinsically complex problem that involves different phases of different physicochemical properties,mass fraction,morphology,size distribution,and ***,computational methods have emerged that assist systematic microstructure quantification and *** objective of this paper is to review these computational methods and to show how these methods as well as other developments in product design can work seamlessly in a proposed performance,ingredients,structure,and manufacturing process framework for the design of structured chemical *** begins with the desired target properties and key *** is followed by computation for microstructure and then selection of processing steps to realize this *** framework is illustrated with the design of nanodielectric and die attach adhesive products.

Climate change can accelerate infrastructure deterioration in coastal areas because increased temperature and humidity can promote steel *** study(1)projects corrosion rate changes for reinforced concrete and steel structures in 223 coastal counties,(2)assesses the impact of corrosion rate changes on the useful life of structures,and(3)evaluates direct economic losses due to shortened useful life of highway bridges over the period *** results show that the useful life of concrete structures may decrease by 1.7-2.7%under the representative concentration pathway(RCP)8.5 and decrease by 0.7-1.1%under RCP 4.5 by the end of the 21st *** useful life of steel structures may decrease by 7.9-15.9%under RCP 8.5 and 3.3-6.7%under RCP *** bridges may suffer an average loss of$6.5-11.7/m 2 under RCP 8.5 and$3.3-16.5/m 2 under RCP 4.5 due to shortened useful *** bridges may suffer an average loss of$73.4-111.3/m 2 under RCP 8.5 and$46.9-81.2/m 2 under RCP *** both climate scenarios,10%of counties may have negative losses and 10%of counties may have losses greater than$20 million due to corrosion rate changes for concrete and steel *** results reveal the spatial difference of climate change impacts on infrastructural deterioration and suggest the importance of developing regional specific adaptation strategies.

In the last decade,the detection and attribution science that links climate change to extreme weather and climate events has emerged as a growing field of research with an increasing body of *** paper overviews the methods for extreme event attribution(EEA)and discusses the new insights that EEA provides for infrastructure *** found that EEA can inform stakeholders about current climate risk,support vulnerability-based and hazard-based adaptations,assist in the development of cost-effective adaptation strategies,and enhance justice and equity in the allocation of adaptation *** engineering practice shifts from a retrospective approach to a proactive,forward-looking risk management strategy,EEA can be used together with climate projections to enhance the comprehensiveness of decision making,including planning and preparing for un-precedented extreme ***,attribution assessment can be more useful for adaptation planning when the exposure and vulnerability of communities to past events are analyzed,and future changes in the probability of extreme events are *** large uncertainties inherent in event attribution and climate projections,future research should examine the sensitivity of engineering design to climate model uncertainties,and adapt engineering practice,including building codes,to uncertain future *** this study focuses on adaptation planning,EEA can also be a useful tool for informing and enhancing decisions related to climate mitigation.
