Plastic multilayered closure versus orthopedic surgeon closure after spinal instrumentation in pediatric neuromuscular scoliosis

Objective To compare wound complication rates between orthopedic closure(OC)and plastic multilayered closure(PMC)in patients undergoing primary posterior spinal fusion for neuromuscular scoliosis(NMS).We hypothesize that multilayered closure will be associated with better postoperative *** We collected data on pediatric patients diagnosed with NMS who underwent first time spinal instrumentation between 1 January 2018 and 31 May *** demographics,length of surgery,spinal levels fused and operative variables,wound complication rate,treatments,and need for wound washout were reviewed in depth and *** In total,86 patients were reviewed:46 with OC and 40 with *** was a significant increase in operating room(OR)time with PMC compared with OC(6.7±1.2 vs 7.3±1.3,p=0.016).There was no difference in complication rate,mean postoperative day of complication or unplanned return to the OR for OC and PMC,*** was a slightly significant increase in the number of patients going home with a drain in the PMC cohort compared with the OC cohort(2.1%vs 15%,p=0.046).Conclusions PMC demonstrated longer OR times than OC and did not demonstrate a statistically significant reduction in wound complications or unplanned returns to the ***,other studies have demonstrated statistical and clinical significance with these *** programs should review internal patient volumes and outcomes for spinal fusion in NMS patients and consider if PMC after spinal fusions in pediatric patients with NMS or other scoliosis subtypes is an appropriate option in their institution to minimize postoperative wound complications.

Novel Heat Transfer Issues Associated with the Design and Safe Operation of the MEgAPIE Spallation Source Target

Critical heat transfer problems are discussed in the context of the operation of a spallation source target, which represents a first demonstration of the feasibility of an innovative concept for generating energy using a particle accelerator. Within the framework of the umbrella project MEgAPIE, an R＆D support group was organized to take responsibility for target cooling. This involved the use of advanced numerical methods - Computational Fluid Dynamics （CFD） and Finite Element Method （FEM） - validated against suitable experimental data, and by means of appropriate benchmarking exercises. The design studies using CFD resulted in an optimum flow configuration being defined for the coolant circulation. Flow visualization tests were undertaken using a glass/water test section, with the velocity field mapped using optical and ultrasonic measuring techniques. These were followed by heat transfer tests, using the actual target materials （lead-bismuth-eutectic coolant and steel confinement）. Further CFD/FEM work to analyze operational transients and accident sequences was also carded out, and is described in the paper.

Numerical simulation on inclusion transport in continuous casting mold

Turbulent flow, the transpor＇t of inclusions and bubbles, and inclusion removal by fluid flow, transport and by bubble flotation in the strand of the continuous slab caster are investigated using computational models, and validated through comparison with plant measurements of inclusions. Steady 3-D flow of steel in the liquid pool in the mold and upper strand is simulated with a finitedifference computational model using the standard k-εturbulence rondel. Trajectories of inclusions and bubhles tire calculated by integrating each local velocity, considering its drag and buoyancy forces, A ＂random walk＂ model is used to incorporate the effect of turbulent fluctuations on the particle motion. The attachment probability of inclusions on a bubble surface is investigated based on fundamental fluid flow simulations, incorporating the turbulent inclusion trajectory and sliding time of each individual inclusion along the bubble surface as a function of particle and bubble size. The chunge in inclusion distribution due to removal by bubble transport in the mold is calculated based on the computed attachment probability of inclusions on each bubble and the computed path length of the bubbles. The results indicate that 6%-10% inclusions are removed by fluid flow transport. 10% by bubble flotation, and 4% by entrapment to the submerged entry nozzle （SEN） walls. Smaller bubbles and larger inclusions have larger attachment probabilities. Smaller bubbles are more efficient for inclusion removal by bubble flotation, so Inng as they are not entrapped in the solidifying shell A larger gas flow rate favors inclusion removal by bubble flotation. The optimum bubble size should be 2-4mm.

The role of behavior in tern conservation

Behavioral research has long had an important role in the conservation of terns （Ayes： Stemidae）. Habitat management and restoration of breeding colony sites depends on knowledge of the cues used to select colony and nest sites. For example, conspecific attraction with playback and decoys is commonly used to bring terns to suitable colony sites and habitat modification is often used to increase the availability of suitable nest sites. Tern colonies are interconnected by dispersal, and a metapopulation approach is needed for effective management. Population dynamics are therefore affected by behaviors that influence the frequency of movement among colony sites： site fidelity, natal and breeding dispersal, and group adherence. The monogamous breeding system of terns should keep effective population size similar to census population size, but variation in sex ratios （likely resulting from sex differences in behavior） and in parental quality can result in a smaller than expected effective population size. In addition to the behavior of terns, knowledge of the behavior of predators on terns contributes to management plans, because predator behavior can sometimes be manipulated and predation is often performed by only a few specialized individuals. Other examples of links between tern behavior and conservation are also briefly reviewed, such as behavioral toxicology research and studies of behavioral responses to human disturbance and manmade structures. More work is needed on the behavior of migratory terns at staging sites, stopover sites and wintering grounds, and on the behavior of less well-studied species and species in less well-studied geographic regions [Current Zoology 60 （4）： 500-514, 2014].

Membrane-Transport Systems for Sucrose in Relation to Whole-Plant Carbon Partitioning

Sucrose is the principal product of photosynthesis used for the distribution of assimilated carbon in plants. Transport mechanisms and efficiency influence photosynthetic productivity by relieving product inhibition and contribute to plant vigor by controlling source/sink relationships and biomass partitioning. Sucrose is synthesized in the cytoplasm and may move cell to cell through plasmodesmata or may cross membranes to be compartmentalized or exported to the apoplasm for uptake into adjacent cells. As a relatively large polar compound, sucrose requires proteins to facilitate efficient membrane transport. Transport across the tonoplast by facilitated diffusion, antiport with protons, and symport with protons have been proposed; for transport across plasma membranes, symport with protons and a mechanism resembling facilitated diffusion are evident. Despite decades of research, only symport with protons is well established at the molecular level. This review aims to integrate recent and older studies on sucrose flux across membranes with principles of whole-plant carbon partitioning.

Effect of sedative agent selection on morbidity, mortality and length of stay in patients with increase in intracranial pressure

BACKgROUND: To identify the effects of sedative agent selection on morbidity, mortality, and length of stay in patients with suspected increase in intracranial pressure. Recent trends and developments have resulted in changes to medications that were previously utilized as pharmacological adjuncts in the sedation and intubation of patients with suspected increases in intracranial pressure. Medications that were previously considered contraindicated are now being used with increasing regularity without demonstrated safety and effectiveness. The primary objective of this study is to evaluate and compare the use of Ketamine as an induction agent for patients with increased intracranial pressure. The secondary objective was to evaluate and compare the use of Etomidate, Midazolam, and Ketamine in patients with increased intracranial pressure. METHODS: We conducted a retrospective chart review of patients transported to our facility with evidence of intracranial hypertension that were intubated before trauma center arrival. Patients were identifi ed during a 22-month period from January 2014 to October 2015. goals were to evaluate the impact of sedative agent selection on morbidity, mortality, and length of ***: During the review 148 patients were identifi ed as meeting inclusion criteria, 52 were excluded due to incomplete data. Of those the patients primarily received; Etomidate, Ketamine, and Midazolam. Patients in the Ketamine group were found to have a lower mortality rate after injury stratifi cation. CONCLUSION: Patients with intracranial hypertension should not be excluded from receiving Ketamine during intubation out of concern for worsening outcomes.

Hepatitis A vaccine associated with autoimmune hepatitis

To describe a case of probable relapsing autoimmune hepatitis associated with vaccination against hepatitis A virus （HAV）. A case report and review of literature were written concerning autoimmune hepatitis in association with hepatitis A and other hepatotropic viruses. Soon a^er the administration of formalin-inactivated hepatitis A vaccine, a man who had recently recovered from an uncharacterized but self-limiting hepatitic illness, experienced a severe deterioration （AST 1687 U/L, INR 1.4）. Anti-nuclear antibodies were detectable, and liver biopsy was compatible with autoimmune hepatitis. The observation supports the role of HAV as a trigger of autoimmune hepatitis. Studies in helper T-cell activity and antibody expression against hepatic proteins in the context of hepatitis A infection are summarized, and the concept of molecular mimicry with regard to other forms of viral hepatitis and autoimmunity is briefly explored.

Protective effects of trilinolein extracted from Panax notoginseng against cardiovascular disease

Trilinolein is a triacylglycerol purified from a commonly used traditional Chinese medicine Panax notoginseng. Trilinolein has been reported to provide a number of beneficial effects including reducing thrombogenicity and arrhythmias and increasing erythrocyte deformability. Additionally, trilinolein has been reported to be an antioxidant, which can counteract free radical damage associated with atherogenesis, and myocardial damage seen with is-chaemia and reperfusion. These pharmacologic effects may explain the perceived benefits derived from treating circulatory disorders with the herb over the centuries.

How might epigenetics contribute to ecological speciation？

Speciation research has seen a renewed interest in ecological speciation, which emphasises divergent ecological se- lection leading to the evolution of reproductive isolation. Selection from divergent ecologies means that phenotypic plasticity can play an important role in ecological speciation. Phenotypic plasticity involves the induction of phenotypes over the lifetime of an organism and emerging evidence suggests that epigenetic marks such as cytosine and protein （histone） modifications might regu- late such environmental induction. Epigenetic marks play a wide role in a variety of processes including development, sex dif- ferentiation and allocation, sexual conflict, regulation of transposable elements and phenotypic plasticity. Here we describe recent studies that investigate epigenetic mechanisms in a variety of contexts. There is mounting evidence for environmentally induced epigenetic variation and for the stable inheritance of epigenetic marks between generations. Thus, epigenetically-based pheno- typic plasticity may play a role in adaptation and ecological speciation. However, there is less evidence for the inheritance of in- duced epigenetic variation across multiple generations in animals. Currently few studies of ecological speciation incorporate the potential for the involvement of epigenetically-based induction of phenotypes, and we argue that this is an important omission [Current Zoology 59 （5）： 686-696, 2013 ].
