Generally hidden from public view, out of our daily thoughts, and literally under our feet, are myriad urban underground tunnels that make our modern megacities possible. From their ancient beginnings in antiquity, as a means of supplying fresh water and draining waste water from cities, underground tunnels evolved into a means of providing high capacity rail mass transit in our most densely populated urban centers. This paper provides a broad overview of the evolution of urban tunnels across a 6000 year time span and includes the specific engineering formulas/computations for the earliest 19th century subways/infrastructure projects based on the Roman vaulted arch tunnel.

Today, it is well known that small airborne particles are very harmful to human health. For the first time in Hawaii we have conducted an environmental pilot study of fine magnetic particles on the island of Oahu, Hawaii, of particulate matter (PM) PM = 60, PM = 10, and PM = 2.5. In order to do a rock magnetic characterization we have performed low field susceptibility vs. temperature [k-T] experiments to determine the Curie points of small particles collected from exhaust pipes, as well as from brake pads of four different types of car engines using gasoline octane ratings of 87, 89, and 92. The Curie point determinations are very well defined and range from 292°C through 393°C and up to 660°C. In addition, we have conducted magnetic granulometry experiments on raw tobacco, burnt tobacco ashes, as well as on automotive engine exhaust, and brake pads in question. The results of the experiments show ferro and ferrimagnetic hysteresis loops with magnetic grain sizes ranging from superparamagnetic-multidomain [SP-MD], multidomain [MD] and pseudo-single domain [PSD] shown on the modified Day et al., diagram of Dunlop (2002). Thus far, the results we have obtained from this pilot study are in agreement with other studies conducted from cigarette ashes from Bulgaria. Our results could be correlated to the traffic-related PM in rome, Italy where the SP fraction mainly occurs as coating of MD particles originated by localized stress in the oxidized outer shell surrounding the un-oxidized core of magnetite-like grains. The role of radiologic imaging in the investigation of irritable bowel syndrome(IBS) remains a subject of debate and there is some evidence, from recent studies of utilization of imaging in IBS, which focused on associated costs and radiation exposure, that imaging is being used relatively widely in these patients. This review aims to assess current best evidence to accurately define the role of radiologic imaging in IBS patients. Primary and secondary literature searches were performed. Evidence suggests that the lack of "red flag" or alarm features in IBS patients should reassure the clinician that the diagnosis of IBS is correct and United States and United Kingdom guidelines recommend no radiologic imaging for IBS patients if alarm features are not present. In patients presenting with IBS symptoms and alarm features, radiologic testing may be used to exclude an alternative diagnosis and the imaging modality should be chosen based on the most likely alternative diagnosis.

Background:The use of laparoscopic(LLR)and robotic liver resections(RLR)has been safely performed in many institutions for liver tumours.A large scale international multicenter study would provide stronger evidence and insight into application of these techniques for huge liver tumours≥10 ***:This was a retrospective review of 971 patients who underwent LLR and RLR for huge(≥10 cm)tumors at 42 international centers between ***:One hundred RLR and 699 LLR which met study criteria were *** comparison between the 2 approaches for patients with huge tumors were performed using 1:3 propensity-score matching(PSM)(73 vs.219).Before PSM,LLR was associated with significantly increased frequency of previous abdominal surgery,malignant pathology,liver cirrhosis and increased median *** PSM,RLR and LLR was associated with no significant difference in key perioperative outcomes including media operation time(242 vs.290 min,P=0.286),transfusion rate rate(19.2%vs.16.9%,P=0.652),median blood loss(200 vs.300 mL,P=0.694),open conversion rate(8.2%vs.11.0%,P=0.519),morbidity(28.8%vs.21.9%,P=0.221),major morbidity(4.1%vs.9.6%,P=0.152),mortality and postoperative length of stay(6 vs.6 days,P=0.435).Conclusions:RLR and LLR can be performed safely for selected patients with huge liver tumours with excellent *** was no significant difference in perioperative outcomes after RLR or LLR.
