Evaluation of Methods for.Dissemination of Pr.state Cancer.Sur.ivor.hip Guidelines: A Sur.ey of Pr.mar. Car. Pr.vider.

Objectives: To assess potential knowledge gaps between pr.mar. car. pr.vider. (PCPs) and cancer.specialists, Michigan Cancer.Consor.ium guidelines wer. developed and disseminated statewide. We evaluated the impact of dissemination of these guidelines on PCP attitudes and beliefs r.gar.ing management of pr.state cancer.(PC) post-tr.atment sequelae. Methods: Guidelines wer. disseminated via har.-copy and web-link mailed to 12,000 Michigan PCPs in 2009, and via 5 CME pr.sentations to 181 PCPs in 2010-2011. 132 initial sur.eys wer. collected fr.m CME attendees and 549 follow-up sur.eys wer. r.ceived 20 months after.the initial mailing. r.sults: Sur.eyed PCPs indicated er.ctile dysfunction (88%), incontinence (84%), anxiety (54%) and fear.of the unknown (50%) as fr.quent post-tr.atment sequelae. A minor.ty of PCPs (≤41%) felt “ver. comfor.able” assessing PC patients for.ten post-tr.atment sequelae at the initial sur.ey, which incr.ased by up to 27% on follow-up sur.ey. 93% of PCPs that r.ceived guidelines by mail and CME pr.sentation had incor.or.ted them into pr.ctice, compar.d with 72% of PCPs that r.ceived guidelines solely by mailing. Similar.y, a gr.ater.pr.por.ion r.por.ed pr.ctice patter. changes (65% vs. 34%, p = 0.0003). A higher.pr.por.ion felt CME events (69% vs. 57%) and exper. pr.sentations (64% vs. 44%) wer. “ver. effective”, when compar.ng attendees and non-attendees. Conclusions: Guideline distr.bution r.sulted in incr.ased comfor. with sur.ivor.hip issues and incor.or.tion into the pr.ctices of the major.ty of PCPs sur.eyed. A gr.ater.impact was obser.ed in PCPs that attended a CME pr.sentation than in those that r.ceived guidelines only by mailing.

It is widely assumed that fetal ischemic br.in injur. dur.ng labor.der.ves almost exclusively fr.m sever., systemic hypoxemia with mar.ed neonatal depr.ssion and acidemia. Sever. asphyxia, however. is one of sever.l causes of per.natal neur.logical injur. and may not be the most common;most neonates diagnosed with hypoxic-ischemic encephalopathy do not have evidence of sever. asphyxia. Sepsis, dir.ct br.in tr.uma, and dr.g or.toxin exposur. account for.some cases, while mechanical for.es of labor.and deliver. that incr.ase fetal intr.cr.nial pr.ssur. sufficiently to impair.br.in per.usion may also contr.bute. Because of bony compliance and mobile sutur. lines, the fetal skull changes shape and r.distr.butes cer.br.spinal fluid dur.ng labor.accor.ing to constr.ints imposed by contr.ctions, and bony and soft tissue elements of the bir.h canal as the head descends. These accommodations, including the incr.ase in intr.cr.nial pr.ssur., ar. adaptive and necessar. for.efficient descent of the head while safeguar.ing cer.br.l blood flow. Autonomic r.flexes mediated thr.ugh centr.l r.ceptor. nor.ally pr.vide ample pr.tection of the br.in fr.m the consider.ble pr.ssur. exer.ed on the skull. On occasion, those for.es, which ar. tr.nsmitted intr.cr.nially, may over.ome the var.ous adaptive anatomical, car.iovascular. metabolic, and neur.logical mechanisms that maintain cer.br.l per.usion and oxygen availability, r.sulting in ischemic br.in injur.. Accepting the notion of a potentially adver.e impact of fetal head compr.ssion suggests that avoidance of excessive uter.ne activity and of r.lentless pushing without steady pr.gr.ss in descent may offer.pr.tection for.the fetal br.in dur.ng par.ur.tion. Excessive head compr.ssion should be consider.d in the differ.ntial diagnosis of ischemic encephalopathy.

The Use of PCA3 Can r.duce the Number.of Pr.state Biopsies Per.or.ed in a Community-Based Ur.logic Pr.ctice

Pur.ose: It is now gener.lly accepted that PSA scr.ening identifies many pr.state cancer. that ar. low-r.sk and may not need tr.atment. PCA3 is a pr.state cancer.specific mar.er.with impr.ved diagnostic accur.cy when compar.d with PSA in r.sear.h studies. It r.mains unknown whether.PCA3 testing can r.duce the unnecessar. diagnosis and tr.atment of pr.state cancer.in r.utine pr.ctice. We evaluated whether.the use of PCA3 in clinical pr.ctice decr.ases the number.of biopsies being per.or.ed in r.sponse to PSA testing. Methods: 64 consecutive patients under.oing PCA3 measur.ment in a single community-based ur.logy pr.ctice wer. analyzed for.r.tes of biopsy per.or.ance and pr.state cancer.detection. r.sults: Median PSA was 5.0 (r.nge: 0.4 - 38.6) and 50% had under.one pr.or.biopsy without evidence of cancer. Median PCA3 scor. was 13.6 (r.nge: 1.6 - 133.0) with 14 patients having an elevated PCA3 (>35). Pr.state biopsy was avoided in 50 of 64 patients (78%). Of the 11 patients under.oing biopsy for.abnor.al PCA3, 7 had pr.state cancer.(64%). At >2-year.median follow-up, 39 of the r.maining 50 patients (78%) avoided subsequent biopsy. Only 5 pr.state cancer. wer. diagnosed dur.ng follow-up. Conclusions: When used in r.utine clinical pr.ctice, PCA3 appear. to r.duce the number.of biopsies being per.or.ed in r.sponse to elevated PSA. Given the incr.asing inter.st in str.tegies to r.duce unnecessar. pr.state cancer.diagnosis and tr.atment, this FDA-appr.ved and widely-available molecular.test appear. to achieve these goals. Fur.her.testing will clar.fy the r.le of PCA3 in initial and subsequent pr.state cancer.scr.ening par.digms.

石油基聚合物的降解以及其火安全性能是当今世界面临的关键挑战。这是由于石油基聚合物应用广泛,在使用后难以降解,会导致严重的环境污染问题。此外,石油基聚合物大多易燃,易引发火灾对人类的生命安全造成威胁。因此解决这些问题显得相当紧迫,需要我们提出能够同时平衡材料环保与阻燃性能的创新解决方案。对于聚合物的降解问题,一种潜在的解决方案是利用和开发同时具有生物安全性和可生物降解的生物基聚合物。然而,相较于石油基聚合物,这些替代品往往在某些方面有性能缺陷（如力学性能et al）。因此,对这些生物基聚合物的研究需要同时兼顾其环境友好的优点并改善其对应性能的缺陷。而在阻燃性能方面,传统的阻燃剂（如含卤阻燃剂）,已被证明具有良好的阻燃效果,但由于其会产生毒副产物,存在环境与健康隐患,因此将开发新型安全无卤的磷系阻燃剂以代替传统的卤系阻燃剂。本研究通过将聚多巴胺（PDA）和磷系阻燃剂应用在聚乳酸（PLA）无纺布,为解决上述问题做出了一定贡献。通过实验发现,PDA涂层与磷系化合物的同时应用可显著提高PLA织物的阻燃性能,且涂层同样具有较高的生物安全性且对环境影响较小。此外,这项工作还凸现了PDA涂层可在PLA中快速高效应用的潜力。主要成果如下:本论文的第一部分是聚多巴胺和磷基化合物THPO作为关键阻燃剂的应用。通过喷雾共沉积方法将THPO引入到聚多巴胺基质中。这种方法显著增强了涂层织物的耐火性。极限氧指数（LOI）从对照织物的18%增加到27.5%。此外,UL-94测试的损坏长度从9.5厘米减少到7.5厘米。值得注意的是,熔体滴落性能得到增强,在测试过程中没有观察到火焰滴落。此外,热重分析显示THPO促进了适度的炭形成。仅存在聚多巴胺涂层时,样品的总烟雾产生量和烟雾产生速率均显着降低,导致Fig r.值从22.2 k W/m2s降低至14.7 k W/m2s;进一步凸显了该方法的有效性。抗紫外线测试显示,20小时内涂层样品的质量平均损失1.5%,而对照PLA织物的质量损失超过15%。本研究的第二部分重点是使用聚多巴胺作为功能涂层和聚磷酸铵（APP）作为磷基阻燃剂,在PLA织物表面形成称为PDA-Cu-APP的金属络合物。这种创新方法证明了进一步增强了织物的耐火性能,熔体滴落性能得到改善,在UL-94垂直燃烧测试中未观察到熔滴。极限氧指数（LOI）从对照PLA的18%显着增加至29.2%。此外,热重分析表明,炭残留物从对照PLA织物的1.5%增加到9.0%。Fig r.值从22.2 k W/m2s降低至14.7 k W/m2s,降低了33%。涂层中的PDA组分显着提高了抗紫外线性能,降低了质量损失率,紫外老化后涂层织物的拉伸强度是对照PLA织物的4倍。总之,本研究将生物基聚多巴胺涂层与磷基阻燃剂相结合以增强PLA织物的防火安全性,保持了环境可持续性目标的可行性。这些发现为未来开发生物基聚合物材料阻燃解决方案的研究提供了有效的途径。
