Objective To compare wound complication rates between orthopedic closure(OC)and plastic multilayered closure(PMC)in patients undergoing primary posterior spinal fusion for neuromuscular scoliosis(NMS).We hypothesize that multilayered closure will be associated with better postoperative *** We collected data on pediatric patients diagnosed with NMS who underwent first time spinal instrumentation between 1 January 2018 and 31 May *** demographics,length of surgery,spinal levels fused and operative variables,wound complication rate,treatments,and need for wound washout were reviewed in depth and *** In total,86 patients were reviewed:46 with OC and 40 with *** was a significant increase in operating room(OR)time with PMC compared with OC(6.7±1.2 vs 7.3±1.3,p=0.016).There was no difference in complication rate,mean postoperative day of complication or unplanned return to the OR for OC and PMC,*** was a slightly significant increase in the number of patients going home with a drain in the PMC cohort compared with the OC cohort(2.1%vs 15%,p=0.046).Conclusions PMC demonstrated longer OR times than OC and did not demonstrate a statistically significant reduction in wound complications or unplanned returns to the ***,other studies have demonstrated statistical and clinical significance with these *** programs should review internal patient volumes and outcomes for spinal fusion in NMS patients and consider if PMC after spinal fusions in pediatric patients with NMS or other scoliosis subtypes is an appropriate option in their institution to minimize postoperative wound complications.

尽管脑血管病的影响广泛且对其病理生理学的理解已取得一些进展，但“卒中”一词在临床实践、临床研究和公共卫生评价中的定义并不统一。经典的卒中定义主要是临床定义，不能反映科学技术的进步。因此，美国心脏协会卒中委员会／美国卒中协会召集了一个写作组，旨在对21世纪的卒中新定义制定一份专家共识文件。中枢神经系统（centralnervoussystem，CNS）梗死被定义为缺血导致的脑、脊髓或视网膜细胞死亡，并且具有永久性损伤的神经病理学、神经影像学和（或）临床证据。CNS梗死具有一系列临床表现：缺血性卒中特指伴有明显症状的CNS梗死，而无症状梗死则顾名思义不引起明显症状。广义的卒中还包括脑出血和蛛网膜下腔出血。卒中的新定义结合了临床和组织学标准，因此能被融入临床实践、研究和公共卫生评价。

Management and prevention of acute and chronic lateral ankle instability in athletic patient populations

Acute and chronic lateral ankle instability are common in high-demand patient populations. If not managed appropriately, patients may experience recurrent instability, chronic pain, osteochondral lesions of the talus, premature osteoarthritis, and other significantlong-term disability. Certain populations, including young athletes, military personnel and those involved in frequent running, jumping, and cutting motions, are at increased risk. Proposed risk factors include prior ankle sprain, elevated body weight or body mass index, female gender, neuromuscular deficits, postural imbalance, foot/ankle malalignment, and exposure to at-risk athletic activity. Prompt, accurate diagnosis is crucial, and evidence-based, functional rehabilitation regimens have a proven track record in returning active patients to work and sport. When patients fail to improve with physical therapy and external bracing, multiple surgical techniques have been described with reliable results, including both anatomic and nonanatomic reconstructive methods. Anatomic repair of the lateral ligamentous complex remains the gold standard for recurrent ankle instability, and it effectively restores native ankle anatomy and joint kinematics while preserving physiologic ankle and subtalar motion. Further preventative measures may minimize the risk of ankle instability in athletic cohorts, including prophylactic bracing and combined neuromuscular and proprioceptive training programs. These interventions have demonstrated benefit in patients at heightened risk for lateral ankle sprain and allow active cohorts to return to full activity without adversely affecting athletic performance.

Background: The effect of the K?hler group dynamics paradigm(i.e., working together with a more capable partner where one's performance is indispensable to the team outcome) has been shown to increase motivation to exercise longer at a strength task in partnered exercise video games (exergames) using a software-generated partner(SGP). However, the effect on exercise intensity with an SGP has not been investigated. The purpose of this study was to examine the motivation to maintain or increase exercise intensity among healthy, physically active middle-aged adults using an SGP in an aerobic ***: Participants(n = 85, mean age = 44.9 years) exercised with an SGP in a 6-day cycle ergometer protocol, randomly assigned to either(a)no partner control,(b) superior SGP who was not a teammate, or(c) superior SGP as a teammate(team score was dependent on the inferior member). The protocol alternated between 30-min continuous and 4-min interval lhigh-intensity session days, during which participants could change cycle power output(watts) from target intensity to alter distance and ***: Mean change in watts from a targeted intensity(75% and 90% maximum heart rate) was the primary dependent variable reflecting motivational effort. Increases in performance over baseline were demonstrated without significant differences between conditions. Self-efficacy and enjoyment were significantly related to effort in the more intense interval ***: Under these conditions, no K?hler effect was observed. Exercise performance during the higher-intensity interval format is more closely related to enloyment and self-efficacy beliefs compared to the continuous sessions.

本研究探讨了2′-脱氧核苷和3′-脱氧核苷对大鼠离体的郎罕氏胰岛细胞分泌胰岛素的影响。2′-脱氧核苷(10 mmol/l)和3′-脱氧核苷(0.1 mmol/l)均可抑制由糖诱导的胰岛素释放,并且降低特异性磷酸二酯酶抑制剂Org 9935或腺苷酸环化酶激活剂Forskolin促进胰岛素分泌的作用,而A_1腺苷受体拮抗剂DPCPX(0.05～1μmol/l)阻滞脱氧腺苷对胰岛素分泌的抑制作用。结果提示,脱氧腺苷对胰岛素分泌的抑制作用是通过A_1受体介导而并非经P-位点产生对腺苷酸环化酶的抑制。

Shade tolerance is a highly desirable trait when breeding new bermudagrass(Cynodon spp.)lines,but current screening methods can take many years to *** is a clear need for large-scale turfgrass breeding programs to reliably,accurately,and quickly predict shade tolerance in their *** objectives of this research were to:(i)build custom chambers to reproducibly estimate photosynthetic characteristics from turfgrass canopies at different light intensities and(ii)determine which photosynthetic characteristics are indicative of past shade performance.A custom-built chamber was constructed to determine average photosynthetic characteristics for the whole plant by studying the turfgrass canopy at natural leaf angles for light *** tolerant(11-T-56)and shade sensitive(Tifway)bermudagrass cultivars were used to examine the effectiveness of the chamber within an array of photosynthetic characteristics when the grasses were grown in full-sun and 73%shade *** compensation point,chlorophyll content,quantum yield,dark respiration rate,and maximum quantum yield of photosystem II were evaluated in this *** on the results of this study,the authors recommend that light compensation point or maximum quantum yield be further evaluated as an accurate indicator of shade tolerance when performed in the field on spring days with grasses grown in full sun or 73%shade *** Rd rates and ambient temperatures on spring days appear to minimize unexplained variance in the data,which would allow researchers to better detect genotypic differences during this season.

BACKGROUND Total joint arthroplasty is one of the most common surgeries performed in the United States with total knee arthroplasty(TKA)being one of the most successful surgeries for restoring function and diminishing *** with the demonstrated success of TKA and a higher prevalence of arthritis and arthritis related disability among minorities,racial and gender disparity remains a constant issue in providing care for the adult reconstruction *** To assess the role of demographics and expectations on differences in perioperative patient reported outcomes(PRO)following *** One hundred and thirty-three patients scheduled for primary unilateral TKA secondary to moderate to severe osteoarthritis were enrolled in this twoinstitution prospective *** PRO questionnaires were collected at four time *** analysis was conducted to determine the impact of gender,ethnic background and expectation surveys responses to assess PRO at these time *** Females were associated with worse preoperative Knee Injury and Osteoarthritis Outcome Scores(KOOS)for symptoms,pain,and activities of daily *** Americans were associated with worse KOOS for pain,activities of daily living,and quality of *** worse preoperative scores,no difference was noted in these categories between the groups ***,all pre-operative psychometric scales were equivalent across groups except Geriatric Depression scale,which was significantly different between groups within the Race and Age Group(Ply,Pain Catastrophizing Scale,was significantly different for all subscales and total score within Age Group(Plplessness subscales as well as the Total score were significantly different between groups for Race and Relationship Status(PlUSION We conclude that female and African American patients have lower preoperative KOOS scores compared to white male *** postoper
