Triple Negative Breast Cancer (TNBC) is a malignant form of cancer with very high mortality and morbidity. Epithelial to Mesenchymal Transition (EMT) is the most common pathophysiological change observed in cancer cells of epithelial origin that promotes metastasis, drug resistance and cancer stem cell formation. Since the information regarding differential gene expression in TNBC cells and cell signaling events leading to EMT is limited, this investigation was done by comparing transcriptomic data generated by RNA isolation and sequencing of a EMT model TNBC cell line in comparison to regular TNBC cells. RNA sequencing and Ingenuity Pathway Software Analysis (IPA) of the transcriptomic data revealed several upregulated and downregulated gene expressions along with novel core canonical pathways including Sirtuin signaling, Oxidative Phosphorylation and Mitochondrial dysfunction events involved in EMT changes of the TNBC cells.

Round spermatid injection into human oocytes:a systematic review and meta-analysis

Many azoospermic men do not possess mature spermatozoa at the time of surgical sperm *** study is a systematic review and meta-analysis evaluating outcomes following round spermatid injection(ROSI),a technique which utilizes immature precursors of spermatozoa for *** electronic search was performed to identify relevant articles published through October *** cohort studies in English involving male patients who had round spermatids identified and used for fertilization with human oocytes were *** rate,pregnancy rate,and resultant delivery rate were assessed following ***-analysis outcomes were analyzed using a random-effects *** were extracted from 22 studies involving 1099 couples and 4218 embryo *** fertilization rate after ROSI was 38.7%(95%confidence interval[CI]:31.5%–46.3%),while the pregnancy rate was 3.7%(95%CI:3.2%–4.4%).The resultant delivery rate was low,with 4.3%of embryo transfers resulting in a delivery(95%CI:2.3%–7.7%).The pregnancy rate per couple was 13.4%(95%CI:6.8%–19.1%)and the resultant delivery rate per couple was 8.1%(95%CI:6.1%–14.4%).ROSI has resulted in clinical pregnancies and live births,but success rates are considerably lower than those achieved with mature *** this technique may be a feasible alternative for men with azoospermia who decline other options,couples should be aware that the odds of a successful delivery are greatly diminished and the prognosis is relatively poor.

线粒体DNA(mtDNA)作为分子标记已被广泛应用于各物种系统发生的研究。mtDNA控制区序列(D-loop)以其较高的突变积累对于研究物种种内的遗传分化具有重要价值。鱼类是脊椎动物中最原始但在种属数量上又最占优势的类群,其物种繁多,分布广泛,起源复杂,研究其系统发生历来是令人饶有兴趣的课题。D-loop在研究鱼类种内遗传分化中具有多方面的重要意义。近年来,已有越来越多的研究工作将D-loop作为分子标记来探讨各种鱼类的种内遗传分化,并且获得了许多有启发性的结果。青海湖是我国内陆最大的咸水湖,湖中主要鱼类为青海湖裸鲤(Gymnocypris przewalskii),D-loop分析初步结果显示青海湖及其周围河流中的裸鲤似乎没有新的种内遗传分化现象。

Access Measures Do Not Reflect Actual Access in Urology Clinics

Introduction: Improvement in new patient access to urologic care is important and is increasingly measured as a reflection of practice quality. Two broad categories affect patient access: provider issues and patient preferences. We chose to study the previously unreported patient choices that affect access delays between the time a urology appointment is requested and the urology appointment itself. Methods: A 3-week real time telephone survey was conducted with callers seeking a urology appointment. If the callers did not choose the first available appointment, they were questioned as to why they did not. This data was then compared to a 3-month access delay graph noting the number of new patients offered each day. Results: Adult and pediatric urologic clinics were evaluated separately. For adult clinics, 14% of patients chose not to accept the first available appointments and 5% sought urologic care elsewhere. For pediatric clinics, 44% delayed access beyond the first available appointment. Of these, 26% were asked by the physician to delay in order to time appointments with imaging studies on the same day, or were advised to wait longer because of the nature of the patient’s diagnosis. Parents’ choice delayed 18% of the appointments beyond the first available appointment. Discussion: These data reveal that about 85% of new patient access falls within the provider’s control. The other 15% of delays rest on the preferences of the patients. These decisions significantly skew the access delay data that is increasingly being used to measure the quality of a urology practice.

Interventional Radiology Procedures after Pediatric Pyeloplasty and Ureteral Reimplantation in Patients with Postoperative Obstruction

Introduction: Obstructive complication after pyeloplasty or ureteral reimplant surgery is a rare though worrisome problem in pediatric urology. These are often complex patients with complicated post-operative courses that at times require interventional radiology procedures. The current literature is lacking in guiding principles to manage these complications. In this study we have reviewed these difficult to manage patients at our children’s hospital over the past 15 years. Methods: A list of patients who underwent interventional radiology procedures to place nephrostomy tubes or internal double-J ureteral stents was compared a list of patients undergoing pyeloplasty or reimplant procedures. These lists were cross-referenced to a list of patients undergoing cystoscopic removal of double-J stents. This small patient group does not represent all complications but those with radiology intervention. Results: At our institution, during the years 1998-2011 we performed 458 pyeloplasties and 3003 open ureteral reimplant procedures. 14 (0.4%) met all of the inclusion criteria. The long term outcome of these problems showed 11 of these patients went on to stability or improvement with either percutaneous drainage or JJ stent placement alone, and three of the reimplant patients ultimately required redo surgery. Of our pyeloplasty patients only three required percutaneous nephrostomy tube, and one went on to JJ stent placement (0.66% of pyeloplasties). No patients in the pyeloplasty group needed surgical revision. Of patients how had undergone ureteral reimplantation, with or without tapering, seven of them underwent interventional radiology procedures (0.23% of reimplant patients). Conclusion: Pediatric urology patients with persistent obstruction after pyeloplasties and ureteral reimplantation surgery with or without tapering who needed interventional radiology rescue procedure resolved or stabilized in 11 of 14 patients.
