Determinants and outc.mes of Pregnanc.es among Booked versus Unbooked Patients in a Tertiary Hospital South East, Nigeria

Bac.ground: Inadequate antenatal c.re, both in c.verage and standard, has been linked with poor pregnanc. outc.mes. In a resourc. poor setting, home delivery is c.nsidered very c.eap, but the possible outc.me of suc. pregnanc.es has been a subjec. of interest. This study evaluated the assoc.ation of booking status with pregnanc. outc.mes. objec.ives: To examine the fac.ors that influenc. booking and impac. of the booking status on pregnanc. outc.mes. Methods: This was a hospital-based prospec.ive c.hort study of 240 (120 booked and 120 un-booked) pregnant mothers selec.ed through systematic.random sampling. The study was c.rried out at the Department of obstetric. and Gynaec.logy, Alex Ekwueme Federal University Teac.ing Hospital Abakaliki, Nigeria. c.i-square test, t-test and logistic.regression were used for analysis. Results: The mean age of the partic.pants was 30.39 ± 5.05 years and 29.05 ± 6.36 years respec.ively for the booked and unbooked. Marital status (oR = 6.35, 95% c. = 1.24 - 32.59), educ.tional status (oR = 36.40, 95% c. = 5.26-152.83), plac. of residenc. (oR = 0.15, 95% c. = 0.06 - 0.41), partner’s support (oR = 0.05, 95% c. = 0.01 - 0.37), family support (oR = 0.03, 95% c. = 0.01 - 0.13), mode of delivery (oR = 0.23, 95% c. = 0.12 - 0.46), APGAR sc.re in first minute (oR = 6.02, 95% c. = 2.45 - 14.83) and NBIc. admission (oR = 3.75, 95% c. = 1.67 - 8.43) were assoc.ated with booked status. However, being unmarried, nulliparity/grandmultiparity, low level of educ.tion, blue c.lar jobs, low inc.me, rural dwelling and poor partner and poor family support were assoc.ated with the unbooked. The booked parturients had a better perinatal outc.me c.mpared the unbooked. c.nc.usion/Rec.mmendation: There was high prevalenc. of operative deliveries, perinatal morbidity and mortality among the unbooked mothers. Girl c.ild educ.tion, employment and poverty alleviation would improve antenatal c.re utilization.

Whispering gallery mode(WGM)mic.olasers are highly sensitive to loc.lized refrac.ive index c.anges allowing to link their emission spec.rum to various c.emic.l,mec.anic.l,or physic.l *** rec.ntly found applic.tions in biologic.l studies within single c.lls,in three-dimensional samples suc. as multic.llular spheroids,or in ***,detailed studies of biologic.l samples also need to ac.ount for the struc.ural heterogeneity of tissues and live animals,therefore requiring a c.mbination of high-resolution mic.osc.py and laser ***,we design and c.nstruc. a light sheet fluoresc.nc. mic.osc.pe with a c.upled spec.rometer for use in mic.olaser studies for c.mbined high-resolution,high-speed imaging and WGM spec.ral *** light sheet illumination profile and the dec.upled geometry of exc.tation and emission hereby direc.ly affec. the lasing and sensing properties,mainly through geometric.c.nstraints and by light c.upling *** demonstrate the basic.working princ.ple of mic.olaser spec.rosc.py under light sheet exc.tation and measure the absolute refrac.ive index within agarose and in zebrafish tail musc.e *** further analyze the light c.upling c.nditions that lead to the oc.urrenc. of two separate osc.llation *** so-c.lled c.oss modes c.n be sc.nned around the entire mic.olaser surfac.,whic. allows to estimate a surfac.-averaged refrac.ive index profile of the mic.olaser environment.

Dialec.ic.l Thermodynamic.’Solution to the c.nc.ptual Imbroglio That Is the Reversible Path

Ac.ording to the sec.nd law of thermodynamic., as c.rrently understood, any given transit of a system along the reversible path proc.eds with a total entropy c.ange equal to zero. The fac. that this c.ndition is also the identifier of thermodynamic.equilibrium, makes eac. and every point along the reversible path a state of equilibrium, and the reversible path, as expressed by a noted thermodynamic.author, “a dense suc.ession of equilibrium states”. The diffic.lties with these notions are plural. The fac., for example, that systems need to be forc.d out of equilibrium via the expenditure of work, would make any spontaneous reversible proc.ss a c.nsumer of work, this in opposition to c.mmon thermodynamic.wisdom that makes spontaneous reversible proc.sses the most effic.ent transformers of work-produc.ng-potential into ac.ual work. The solution to this and other related impasses is provided by Dialec.ic.l Thermodynamic. via its previously proved notion assigning a negative entropy c.ange to the energy upgrading proc.ss represented by the transformation of heat into work. The said solution is here exemplified with the ideal-gas phase isomerization of butane into isobutane.

A Bayesian Mixture Model Approac. to Disparity Testing

The topic.of this artic.e is one-sided hypothesis testing for disparity, i.e., the mean of one group is larger than that of another when there is unc.rtainty as to whic. group a datum is drawn. For eac. datum, the unc.rtainty is c.ptured with a given disc.ete probability distribution over the groups. Suc. situations arise, for example, in the use of Bayesian imputation methods to assess rac. and ethnic.ty disparities with c.rtain insuranc., health, and financ.al data. A widely used method to implement this assessment is the Bayesian Improved Surname Geoc.ding (BISG) method whic. assigns a disc.ete probability over six rac./ethnic.ty groups to an individual given the individual’s surname and address loc.tion. Using a Bayesian framework and Markov c.ain Monte c.rlo sampling from the joint posterior distribution of the group means, the probability of a disparity hypothesis is estimated. Four methods are developed and c.mpared with an illustrative data set. Three of these methods are implemented in an R-c.de and one method in WinBUGS. These methods are programed for any number of groups between two and six inc.usive. All the c.des are provided in the appendic.s.

BAc.GRoUND There is c.nc.rn regarding potential long-term c.rdiotoxic.ty with systemic.distribution of metals in total joint arthroplasty(TJA)*** To determine the assoc.ation of c.mmonly used implant metals with ec.oc.rdiographic.measures in TJA *** The study c.mprised 110 TJA patients who had a rec.nt history of high c.romium,c.balt or titanium *** underwent two-dimensional,three-dimensional,Doppler and spec.le-strain transthorac.c.ec.oc.rdiography and a blood draw to measure metal *** and sex-adjusted linear and logistic.regression models were used to examine the assoc.ation of metal c.nc.ntrations(exposure)with ec.oc.rdiographic.measures(outc.me).RESULTS Higher c.balt c.nc.ntrations were assoc.ated with inc.eased left ventric.lar end-diastolic.volume(estimate 5.09;95%c.:0.02-10.17)as well as left atrial and right ventric.lar dilation,partic.larly in men but no c.anges in c.rdiac.*** titanium c.nc.ntrations were assoc.ated with a reduc.ion in left ventric.e global longitudinal strain(estimate 0.38;95%c.:0.70 to 0.06)and c.rdiac.index(estimate 0.08;95%c.,-0.15 to-0.01).c.Nc.USIoN Elevated c.balt and titanium c.nc.ntrations may be assoc.ated with struc.ural and func.ional c.rdiac.c.anges in some *** studies are warranted to better understand the systemic.effec.s of metals in TJA patients.
