Background Clopidogrel was superior to aspirin in patients with previous manifestations of atherothrombotic disease in the CApRIE study and its benefit was amplified in some high risk subgroups of patients. We aimed to assess whether addition of aspirin to clopidogrel could have a greater benefit than clopidogrel alone in prevention of vascular events with potentially higher bleeding risk. methods We did a randomised, double blind, placebo controlled trial to compare aspirin (75 mg/day) with placebo in 7599 high risk patients with recent ischaemic stroke or transient ischaemic attack and at least one additional vascular risk factor who were already receiving clopidogrel 75 mg/day. Duration of treatment and follow up was 18 months. The primary endpoint was a composite of ischaemic stroke, myocardial infarction, vascular death, or rehospitalisation for acute ischaemia (including rehospitalisation for transient ischaemic attack, angina pectoris, or worsening of peripheral arterial disease). Analysis was by intention to treat, using logrank test and a Cox’ s proportional hazards model. Findings 596 (15.7% ) patients reached the primary endpoint in the group receiving aspirin and clopidogrel compared with 636 (16.7% ) in the clopidogrel alone group (relative risk reduction 6.4% , [95% CI - 4.6 to 16.3]; absolute risk reduction 1% [- 0.6 to 2.7]). life threatening bleedings were higher in the group receiving aspirin and clopidogrel versus clopidogrel alone (96 vs 49 ; absolute risk increase 1.3% [95% CI 0.6 to 1.9]). major bleedings were also increased in the group receiving aspirin and clopidogrel but no difference was recorded in mortality. Interpretation Adding aspirin to clopidogrel in high risk patients with recent ischaemic stroke or transient ischaemic attack is associated with a non significant difference in reducing major vascular events. However, the risk of life threatening or major bleeding is increased by the addition of aspirin.

The benefits of regular physical activity are well ***,few studies have examined the effectiveness of integrating physical activity(pA)into curricula within a post-secondary *** investigate the incorporation of pA into medical curriculum,we developed a series of optional exercise-based review sessions designed to reinforce musculoskeletal(mSK)anatomy course *** synchronous sessions were co-taught by a group fitness instructor and an anatomy *** fitness instructor would lead students through both strength and yoga style exercises,while the anatomy instructor asked questions about relevant anatomical structures related to course material previously *** the sessions,participants were asked to evaluate the classes on their self-reported exam preparedness in improving mSK anatomy knowledge,pA levels,and mental *** participants completed surveys;a majority agreed that the classes increased understanding of mSK concepts(90.0%)and activity levels(97.7%).many(70.0%)felt that the classes helped reduce *** majority of respondents(90.0%)agreed that the classes contributed to increased feelings of social ***,medical students saw benefit in pA based interventions to sup.l.m.nt mSK course *** with increasing activity levels and promoting health behaviours,integrating pA into medical curriculum may improve comprehension of learning material,alleviate stress and foster social connectivity among medical students.

Toward an understanding of dwell fatigue damage mechanism of bimodal Ti-6Al-4V alloys

Dwell fatigue effect is a long-standing problem threatening the long-term service reliability for fan blades and fan disks of an aircraft *** understand the basic mechanism of dwell fatigue damage,pure fatigue and 60 s dwell fatigue properties of bimodal Ti-6Al-4V alloys with different volume fractions of the primaryα(α_(p))phase were examined *** results showed that both pure fatigue and dwell fatigue life decreased with increasing the volume fraction of theα_(p)phase and the dwell fatigue life was lower than the pure fatigue *** quasi-in-situ test results and the quantitative characterization of damage behaviors of the local microstructure units defined by theα_(p)-secondaryα(α_(s))combination reveal that theα_(s)phase close to theα_(p)phase with extensively slip activities was gradually damaged under dwell fatigue loading,while that under pure fatigue loading was undamaged,demonstrating that the dwell loading induced the damage of theα_(s)phase,and further reduced the fatigue life.A stress relaxation-based model is proposed to describe the physical mechanism on dwell fatigue damage of the bimodal Ti-6Al-4V alloy,*** elastic deformation of theα_(s)phase caused by the strain incompatibility would be gradually transformed into plastic deformation during the dwell stage,and thus promotes fatigue *** model provides new insights into the microscopic process of stress/strain transfer between the soft and hard microstructure units under dwell fatigue loading.

Invariance of specific mass increment in the case of non-equilibrium growth

The invariance of specific mass increments of crystalline structures that co-exist in the case of non-equilibrium growth is grounded for the first time by using the maximum entropy production principle. Based on the hypothesis of the existence of a universal growth equation, and through the dimensional analysis, an explicit form of the time-dependent specific mass increment is proposed. The applicability of the obtained results for describing growth in animate nature is discussed.

ImpROVING THE mECHANICAl pROpERTIES OF COppER AllOYS BY THERmO-mECHANICAl pROCESSING

Systematic physical simulation of thermo-mechanical processing routes has been ap-plied on a Gleeble 1500 simulator to four copper alloys (mass %) Cu-0.57Co-0.32Si,Cu-0.55Cr-0.065p, Cu-0.22Zr-0.035Si and Cu-1.01Ni-0.43Si aimed at clarifying theinfluences of processing conditions on their final properties, strength and electricalconductivity. Flow curves were determined over wide temperature and strain rateranges. Hardness was used as a measure of the strength level achieved. High hard-ness was obtained as using equal amounts (strains 0.5) of cold deformation beforeand after the precipitation annealing stage. The maximum values achieved for theCu-Co-Si, Cu-Cr-p, Cu-Zr-Si and Cu-Ni-Si alloys were 190, 165, 178 and 193 HV5,respectively. A thermo-mechanical schedule involving the hot deformation-ageing-colddeformation stages showed even better results for the Cu-Zr-Si alloy. Consequently,the processing routes were designed based on simulation test results and wires of 5 and2mm in diameters have been successfully processed in the industrial scale.

Observations on the Formation of Ultrafine Ferrite Grain Size in Steels by physical Simulation Routes

Some observations are reported on the simulation of two thermomechanical routes to produce ultrafine ferrite grainsize in steels. One C-mn grade and Nb, Nb-Ti and Nb-high Ti bearing steels were used in the tests performed ona Gleeble simulator and a laboratory rolling mill. The routes included severe hot deformation of prior grain-refinedaustenite at the temperature close to Ar3 (DIF) and static recrystallization of fine-grained cold-rolled martensite(SRm). It was observed that the hot deformation induces the formation of ferrite above the Ar3 temperature of thesteel, but severe reductions are required for the complete transformation. Strain of 1.2 can result in about 70% offerrite with the grain size of about 1.4～2μm in all the studied steels. Similarly, in short annealing of cold-workedmartensite, the static recrystallization can also lead to a grain size of about 1.5 μm. The distribution of carbonvaries in the microstructures, carbon being in the second phase in the DIF route and in carbide particles in the SRmroute, which may have a significant influence on the mechanical properties and the thermal stability of ultrafine grainstructure.

Geometrical Scale-Sensitive Fatigue properties of Ti-6.5Al-3.5mo-1.5Zr-0.3Si Alloys With α/β lamellar microstructures

Fatigue properties of the Ti-6.5Al-3.5mo-1.5Zr-0.3Si alloy sheets containing different numbers of α/β Widmansttten colonies in the thickness direction of the sheets were investigated by tensionetension fatigue testing. It is found that fatigue properties of the Ti alloy either in low- or high-stress amplitude regimes become more sensitive to the sheet thickness of the Ti alloy as the sheet thickness is comparable to the length scale of the Widmansttten colonies. The basic mechanism of such length scale-sensitive fatigue properties in the Ti alloy was elucidated.
