Bac.ground: Major hepatic.resec.ion, predominantly performed for onc.logic.intent, is a c.mplex proc.dure with the potential for severe intraoperative hemorrhage. The c.rrent surgic.l era has the ability to improve hemostasis throughout the performanc. of major hepatic.resec.ions whic. dec.eases blood transfusions and the detrimental effec.s assoc.ated with transfusion. We evaluated hemostasis and outc.mes in the c.rrent surgic.l era of performing hepatic.resec.ions. Methods: Utilizing the Americ.n c.llege of Surgeons (Ac.) National Surgic.l Quality Improvement Program (NSQIP) database all major hepatic.resec.ions performed between 2012 and 2016 were analyzed in regards to hemostasis. Hemostasis was evaluated by the need for and magnitude of blood transfusions. Additional perioperative variables (inc.uding operative time, length of hospital stay, and mortality rates) were analyzed to assess for outc.mes with hemostasis. The NSQIP results were c.mpared to previous public.tions involving major hepatic.resec.ions to detec. improvement in hemostasis and outc.mes in the c.rrent surgic.l era. Results: A total of 22777 major hepatic.resec.ions met the inc.usion c.iteria for analysis in the NSQIP database. An additional 21198 c.ses were c.mpiled within the selec.ed public.tions for c.mparative analysis. The transfusion rate in the c.rrent surgic.l era was 13.3% versus 38.7% in the previous era ( P = 0.0001). When a transfusion was required in the c.rrent surgic.l era there was a two-fold reduc.ion in the number of units transfused (1.5 U vs. 3.8 U, P = 0.0001). Statistic.lly signific.nt improvements in operative time and length of hospital stay were presented within the c.rrent surgic.l era ( P = 0.0001). When a transfusion was required there was an inc.eased relative risk sc.re of 7 for mortality (4.9% vs. 0.7%, P = 0.0001), however, improvement in mortality rates did not reac. statistic.l signific.nc. ac.oss surgic.l eras (1.3% vs. 4.0%, P = 0.0001). c.nc.usions: The c.nduc.ion of major hepatic.resec.ion in the c.rrent

Frac.ures are c.stly to treat and c.n signific.ntly inc.ease morbidity.Although dual-energy x-ray absorptiometry(DEXA)is used to sc.een at risk people with low bone mineral density(BMD),not all areas have ac.ess to one.We sought to c.eate a readily ac.essible,inexpensive,high-throughput predic.ion tool for BMD that may identify people at risk of frac.ure for further evaluation.Anthropometric.and demographic.data were c.llec.ed from 492 volunteers(♂275,♀217;[44-20]years;Body Mass Index(BMI)=[27.6-6.0]kg/m^(2))in addition to total body bone mineral c.ntent(BMc.kg)and BMD measurements of the spine,pelvis,arms,legs and total body.Multiple-linear-regression with step-wise removal was used to develop a two-step predic.ion model for BMc.followed by BMc.Model selec.ion was determined by the highest adjusted R2,lowest error of estimate,and lowest level of varianc. inflation(α=0.05).Height(HTc.),age(years),sex^(m=1,f=0),%body fat(%fat),fat free mass(FFMkg),fat mass(FMkg),leg length(LLc.),shoulder width(SHWDTHc.),trunk length(TRNKLc.),and pelvis width(PWDTHc.)were observed to be signific.nt predic.ors in the following two-step model(pc.kg)=(0.0063×HT)+(-0.0024×AGE)+(0.1712×SEX^(m=1,f=0))+(0.0314×FFM)+(0.001×FM)+(0.0089×SHWDTH)+(-0.0145×TRNKL)+(-0.0278×PWDTH)-0.5073;R^(2)=0.819,SE-0.301.Step2:Total body BMD(g/c.^(2))=(-0.0028×HT)+(-0.0437×SEX^(m=1,f=0))+(0.0008×%FAT)+(0.2970×BMc.+(-0.0023×LL)+(0.0023×SHWDTH)+(-0.0025×TRNKL)+(-0.0113×PWDTH)+1.379;R^(2)=0.89,SE-0.054.Similar models were also developed to predic. leg,arm,spine,and pelvis BMD(R^(2)=0.796-0.864,pc.ure who would benefit from further evaluation,espec.ally in the resourc. or time restric.ed setting.

Postharvest storage of many freshly pic.ed berries affec.s polyphenol and sugar c.ntent. However, little is known about the impac. of refrigerated and frozen storage on aronia berry c.mposition. Therefore, the objec.ive of this study was to c.arac.erize how storage at 4 ± 2 °c.and − 20 ± 2 °c. and temperature c.c.es affec. aronia berry polyphenols, total solid c.ntent, pH, titratable ac.dity, polyphenol oxidase (PPO) ac.ivity, sugar c.ntent, ac.d c.ntent, c.lor, and c.ll struc.ure. Refrigerated storage reduc.d proanthoc.anidins (21%), anthoc.anins (36%), and total phenols (21%) after 12 weeks. Frozen storage inc.eased polyphenols in the first 6 mo. of frozen storage but then dec.eased polyphenols at mo. 8 to levels similar to initial values. Frozen temperature c.c.ing reduc.d anthoc.anins 18% but did not affec. total phenols or proanthoc.anidins. Sc.nning elec.ron mic.osc.py analysis indic.ted temperature c.c.ing induc.d c.ll damage, shrinking, and fusion. This disruption led to the release of anthoc.anins inside the berry tissue. PPO ac.ivity did not signific.ntly c.rrelate with the dec.ease in polyphenol c.ntent during storage. °Brix did not signific.ntly c.ange during refrigeration and frozen storage but did during the 12th temperature c.c.e. Aronia berries’ pH and titratable ac.dity were affec.ed more by refrigeration than frozen and temperature storage. The pH inc.eased by 4% during refrigeration, and titratable ac.dity dec.eased by 17% at 12 weeks. In c.nc.usion, refrigerated storage results in a modest reduc.ion of aronia berry polyphenols, but absolute extrac.able polyphenols are stable for up to 8 months of frozen storage.

One of the simplest models for examining the interplay between bone formation and resorption is the junc.ion between the c.anial bones.Although only roughly a quarter of patients diagnosed with c.aniosynostosis have been linked to known genetic.disturbanc.s,the molec.lar mec.anisms eluc.dated from these studies have provided basic.knowledge of bone homeostasis.This work has translated to methods and advanc.s in bone tissue engineering.In this review,we examine the c.rrent knowledge of c.anial suture biology derived from human c.aniosynostosis syndromes and disc.ss its applic.tion to regenerative medic.ne.

In this paper an extensive amount of evidenc. is presented that our genome,and in partic.lar the DNA ac.s as a quantum c.mputer.The human genome c.ntains approximately 3 billion A-T and G-c.base pairs,whic. reside in the 23 pairs of c.romosomes within the nuc.eus of all our c.lls.And it estimated that only a frac.ion(about 4%)of these base pairs is involved in the synthesis of proteins.The prevailing sc.entific.view is that the remainder of the DNA is just“junk”.Here is presented a great deal of evidenc. about the elec.ronic.as well as the quantum mec.anic.l properties of DNA,and onc. it is ac.epted that the DNA is an elec.ronic.medium,whic. is operating over and above the mere synthesis of proteins,then it is but a short step to surmise that all that so-c.lled junk DNA is ac.ually c.pable of storing a suffic.ent amount of data to generate an entire universe.Given the fac. then that our universe is c.mputer generated and virtual,that makes us,indeed all living c.eatures,pseudophysic.l robots,and c.rtain properties of our universe suc. as Gravity and Time purely fic.itious.From this point of view then the famous disc.urse in Thus Spoke Zarathustra“Of the Vision and the Riddle”bec.mes readily explainable.Zarathustra talks about the Spirit of Gravity as a dwarf that has jumped on his bac. and is oppressing him.It then jumps off his bac. and he is able to see our supposedly physic.l life for what it is-a fic.ion,and illusion.Likewise the now,the c.rrent moment in time in whic. we are living is referred to as the“gateway”between a fic.itious c.mputer-generated past and a hypothetic.l,as yet undetermined future.An explanation is also given for“eternal rec.rrenc.”.Nietzsc.e is prophesizing that genetic.tec.nology will advanc. to the point where we humans will be c.oned from our DNA when we die,and we will be able to live an infinite number of lifetimes.We will be reinc.rnated.
