AIM: To study the trends of major causes of visual impairment(VI) in adults in Sichuan,China and evaluate the effect of aging on the trends. ·METHODS: We used data from the National Sample Survey on Disabilities(NSSD) in Sichuan province conducted in 1987 and 2006. The age-adjusted prevalence of major causes of VI and the prevalence stratified by age in each cause were calculated and compared. The association between age and each cause of VI was also analyzed.·RESULTS: Retinal disease increased and became the second leading cause of VI in 2006 while blinding trachoma decreased markedly. Cataract and non-trachomatous corneal diseases were among the leading causes of VI in both years. We found associations between age and causes of VI,with age showing the strongest association with cataract and relatively lower associations with other causes. · CONCLUSION: In the last two decades,dramatic changes occurred in the major causes of VI with significantly increased retinal disease and decreased blinding trachoma. Aging of the population might be an important factor accounting for the changed trends of VI. Understanding the prevalence of VI,its major causes and trends over time can assist in prioritizing and developing effective interventional strategies and monitoring their impact.

Background:During human locomotion,a sufficiently stiff foot allows the ankle plantar flexors to generate large propulsive *** foot stiffness(e.g.,via a carbon plate)increases the ankle’s external moment arm in relation to the internal moment arm(i.e.,increasing gear ratio),reduces plantar flexor muscles’shortening velocity,and enhances muscle force *** contrast,when activation of the foot’s intrinsic muscles is impaired,there is a reduction in foot and ankle work and metatarsophalangeal joint *** speculated that the reduced capacity to actively control metatarsophalangeal joint stiffness may impair the gearing function of the foot at the ***:We used a tibial nerve block to examine the direct effects of the intrinsic foot muscles on ankle joint kinetics,in vivo medial gastrocnemius’musculotendinous dynamics,and ankle gear ratio on 14 participants during maximal vertical ***:Under the nerve block,the internal ankle plantar flexion moment decreased(p=0.004)alongside a reduction in external moment arm length(p=0.021)and ankle joint gear ratio(p=0.049)when compared to the non-blocked *** medial gastrocnemius muscle-tendon unit and fascicle velocity were not different between conditions,the Achilles tendon was shorter during propulsion in the nerve block condition(pknown role of regulating the energetic function of the foot,our data indicate that the intrinsic foot muscles also act to optimize ankle joint torque production and leverage during the propulsion phase of vertical jumping.

Experimental Investigation of the Effects of Acute Exercise on Real-World Ecological Memory

Purpose To evaluate the effects of acute moderate-intensity exercise on ecological memory,as assessed from a face-name memory *** A two-arm,parallel-group,randomized controlled intervention was ***(N=40;Mage=20.8 years)were randomized into a seated control task or a bout of acute moderate-intensity treadmill exercise(15-min).Thereafter,participants completed a 3-phase face-name memory task,involving a study phase and two test phases(immediate and delayed recall,with the delay occurring 15 min after the immediate recall).Results For the immediate memory recall,the mean(SD)scores for the exercise and control conditions,respectively,were 6.60(2.5)and 6.20(2.5).For the 15-min delayed assessment,the respective scores were 6.25(2.6)and 5.75(1.9).There was a significant main effect for time(F=4.06,P=0.05,η^(2)_(p)=0.10).However,there were no main effects for group(F=0.33,P=0.56,η^(2)_(p)=0.01)or time by group interactions(F=0.12,P=0.72,η^(2)_(p)=0.003).Conclusions Despite the exercise group having slightly higher immediate and delayed face-name memory scores,we did not observe robust evidence of acute exercise enhancing face-name memory performance.

A Curious Case of Methaemoglobinemia

The most frequent cause of raised methaemoglobin is from recreational drug use, with oxidation of the haem molecule leading to methaemoglobinaemia. Our case describes that of a man in his early thirties who presented with shortness of breath, hypoxia and methaemoglobinemia due to ingestion of impure cocaine mixed with local anaesthetic agents, a common cause of acquired methaemoglobinemia. This case report demonstrates the need for clinicians to consider methaemoglobinaemia as a differential diagnosis for patients presenting with hypoxia.

Esophageal cancer (EC) is a highly lethal disease. Approximately 50% of patients present with metastatic EC and most patients with localized EC will have local recurrence or develop metastases, despite potentially curative local therapy. The most common sites of distant recurrence are represented by lung, liver and bone while brain and breast metastases are rare. Usually patients with advanced disease are not treated aggressively and their median survival is six months. We report a woman patient who developed breast and brain metastases after curative surgery. We treated her with a highly aggressive chemotherapeutic and surgical combination resulting in a complete remission of the disease even after 11-year follow-up. We think that in super selected patients with more than one metastasis, when functional status is good and metastases are technically resectable, a surgical excision may be considered as a salvage option and chemotherapy should be delivered to allow a systemic control.

Nonalcoholic fatty liver disease in patients with inflammatory bowel disease: Beyond the natural history

BACkGROUND Nonalcoholic fatty liver disease(NAFLD)is a frequently reported condition in patients with inflammatory bowel disease(IBD).Both intestinal inflammation and metabolic factors are believed to contribute to the pathogenesis of IBDassociated *** To evaluate the prevalence of steatosis and liver fibrosis(LF)in a cohort of IBD patients and the identification of metabolic-and IBD-related risk factors for NAFLD and *** IBD patients were consecutively enrolled from December 2016 to January ***,anthropometric and biochemical data were collected so as eating *** ultrasound and transient elastography were performed to evaluate the presence of NAFLD and LF *** A total of 178 consecutive patients were enrolled and included in the analysis(95 Ulcerative colitis,83 Crohn’s disease).NAFLD was detected by imaging in 72(40.4%)*** between patients with and without NAFLD showed no significant differences in terms of IBD severity,disease duration,location/extension,use of IBD-related medications(i.e.,steroids,anti-TNFs,and immunomodulators)and *** was significantly associated with the presence of metabolic syndrome[MetS;odds ratio(OR):4.13,P=0.001]and obesity defined by body mass index(OR:9.21,P=0.0002).IBD patients with NAFLD showed higher caloric intake and lipid consumption than those without NAFLD,regardless disease *** the multivariate analysis,male sex,advanced age and high lipid consumption were independent risk factors for the development of *** increased liver stiffness was detected in 21 patients(16%)and the presence of MetS was the only relevant factor associated to LF(OR:3.40,P=0.01).CONCLUSION In this study,we demonstrate that risk factors for NAFLD and LF in the IBD population do not differ from those in the general population.

A Prospective Review： The Significance of Procalcitonin Levels in the Evaluation of Pediatric Patients Presenting to the Emergency Department with Fever （Pedi-SPEED I）

Purpose： This study was formulated to educate physicians and medical residents on procalcitonin （PCT）. The need for this study is based on lack of information and studies in the United States in the pediatric population regarding PCT, and also due to the lack of awareness by the Baylor Scott ＆ White McLane Children＇s residents in the implications and benefits of PCT. There are several instances where it can be valuable in the diagnosis and treatment of bacterial infections, which in turn, can decrease patient＇s length of stay, hospital costs, and patient exposure to unnecessary antibiotics, without increasing morbidity or mortality. Methods： An educational, qualitative analysis of PCT awareness is assessed utilizing pre and post surveys. These surveys will be given to pediatric and emergency medicine residents on paper before and after an educational presentation about PCT and its importance as a detection method for infection. Surveys consist of six closed-ended questions on the pre-survey and eight open and close-ended questions on the post-survey about PCT, how it works, general use, and how it can be beneficial. Results： Pediatric and emergency medicine residents were asked to participate in this study at a random monthly department meeting, selected by primary investigator and department head. Residents were asked to complete the pre-survey, listened to a short presentation, and completed the post survey. Comparing the pre-survey to post-survey, there was an overall improvement for both groups for questions 1-5 which showed statistical significance （P 〈 0.05）. No statistical difference was found between groups comparing emergency medicine vs. pediatric departments, interns vs. residents and each subgroup pre-survey vs. post-survey for questions 6 and 7. The open-ended post-survey question identified factors for obtaining PCT levels with attending physician＇s preference as the most common response. Improved detection Of the MUC1 cancer antigen CA 15-3 by ALYGNSA fluorimmunoassay

Breast cancer is the second leading cause of cancerrelated deaths in women worldwide;a prime cancer biomarker to aid in the diagnosis, directed treatment, clinical management, and reoccurrence of this cancer is a MUC1 peptide fragment: cancer antigen 15-3 (CA 15-3). Herein, an immuno-fluorescence assay for CA 15-3 was developed;this ALYGNSA system consists of a protein biolinker (Protein G’) adsorbed onto Poly (methyl methacrylate) (PMMA). The unique interaction of Protein G’ with PMMA, a thermo-plastic polymer has been demonstrated to improve human IgG capture antibody alignment/ orientation and result in greater assay sensitivity. Indeed a previous report (HEALTH 1 325 - 329, 2009) on the shed extracellular domain of HER-2/neu revealed a 10-fold increase in sensitivity of the ALYGSNA assay over a control ELISA assay. Results from this ALYGNSA assay study revealed that a 16-fold increase in detection (≤0.94 U/mL) of CA 15-3 was found in comparison to a commercial control ELISA kit (≤15 U/mL). In conclusion, this enhanced sensitivity of the ALYGNSA assay for CA 15-3, may provide insights into the role/function of this biomarker in normal, as well as, breast cancer and other epithelial cancers.
