Performance of the Finnish Diabetes Risk Score (FINDRISC) in the Identification of Dysglycemia in an Urban Population in Ouagadougou (Burkina Faso)

Background: Diabetes mellitus is a real public health problem worldwide and is growing in developing countries. Our aim was to determine the prevalence of undiagnosed type 2 diabetes mellitus (T2DM) and to evaluate the performance of the Finnish Diabetes Risk Score (FINDRISC) questionnaire to identify type 2 diabetics in the urban population of Ouagadougou, Burkina Faso. Methodology: This was a cross-sectional study among volunteers aged 18 years and older recruited systematically from November 11 to 16, 2019, in the city of Ouagadougou, Burkina Faso, FINDRISC score was used. Analysis of the Receiver Operating Characteristic (ROC) curve was used to study the diagnostic performance of FINDRISC for the identification of type 2 diabetics. The optimal threshold was determined by the sum of the highest sensitivity and specificity. Multivariate logistic regression was used to analyze the association of each variable used in the calculation of the FINDRISC score. Results: A total of 1276 individuals were included in the analyses, of which 667 (52.27%) were women. The average age was 34.16 years (SD: ±12.42). The prevalence of T2DM was 10.74%. The mean FINDRISC score was 5.85 (SD: ±4.31). The majority (58.54%) of individuals had a low risk of diabetes according to the FINDRISC score 0 - 7, while 3.61% had a score ≥ 15. The FINDRISC score showed good performance (AUC = 0.70) in identifying undiagnosed type 2 diabetics. International Healthcare Terminologies for Morbidity New Era: SNOMED and ICD11

The Information Technology (IT) developments have changed the use of Healthcare terminologies from paper-based mortality statistics with the WHO international classifications of diseases (ICD) to the IT-based morbidity implementations for instance for Casemix-based healthcare funding and managing systems. This higher level of granularity is worldwide spread under the umbrella of several national modifications named ICD10 XM. These developments have met the increased use of the International Clinical Reference Terminology named SNOMED. When the updating of WHO ICD10 to WHO ICD11 was decided a merging was envisaged and a WHO SNOMED CT common work proposed a methodology to create a common formal ontology between the 11th version of the WHO International Classification of Diseases and Health Problems (ICD) and the most used in the world clinical terminology named Systematized Nomenclature of Human and Veterinary Medicine - Clinical Terms (SCT). The present work follows this unachieved work and aims to develop a SNOMED-based formal ontology for ICD11 chapter 1 using the textual definitions of ICD11 codes which is a completely new character of ICD and the ontology tools provided by SCT in the publicly available SNOMED Browser. There are two key results: the lexical alignment is complete and the ontology alignment is incomplete with the validated SNOMED concept model can be completed with not yet validated attributes and values of the SNOMED Compositional Grammar. The work opens a new era for the seamless use of both international terminologies for morbidity for instance for DRG/Casemix and clinical management use. The main limitation is that it is restricted to 1 out of 26 chapters of ICD11.

Prevalence and Factors Associated with Violence among Primary and Secondary School Workers in Ouagadougou, Burkina Faso

Introduction: Violence is a major problem in today’s workplace. It affects workers in all sectors of activity. The aim of the study was to determine the prevalence and factors associated with violence among primary and secondary school staff in Ouagadougou. Materials and Methods: This was a cross-sectional, descriptive and analytical study that took place from April 22, 2020 to February 18, 2021 in primary and secondary schools in Ouagadougou. It focused on the educational staff of these establishments. Results: A total of 362 agents took part in the study. The average age of participants was 36.5 ± 6.89 years. The sex ratio was 1.62 men to one woman. The majority of workers were teachers, with 300 participants (82.87%). The average job tenure was 7 years ± 6.91 years. With regard to violence in the workplace, 87 workers were victims, representing a prevalence of 24.03%. Of these workers, 59 (67.81%) were men and 36 (41.38%) had less than six years’ experience. Acts of violence were predominantly pushing observed in 15 workers (50%), while shouting and aggressive tone dominated psychological violence and concerned 54 cases (36.24%). On univariate analysis, the factors associated with violence were age (p = 0.046) and type of establishment (p = 0.012). On multivariate analysis, only the nature of the establishment was the only factor associated with the occurrence of violence among teaching staff (OR = 2.3690, p = 0.0213). Conclusion: The prevalence of violence is high in these educational establishments. Age and type of establishment are factors associated with its occurrence. It is therefore necessary to develop an effective strategy for preventing this risk in the workplace.

Management of Testicular Cancers in Brazzaville

Introduction: Testicular cancer accounts for 5% of urological tumors, predominantly affecting young men. The aim of our study was to report the diagnostic and evolutionary aspects of testicular cancer cases treated in our center. Patients and Methods: A retrospective study conducted over a 15-year period involving 12 patients treated for testicular cancer at the University Hospital of Brazzaville. Results: The median age was 31 years (range 11 to 49 years), with a median consultation delay of 10.6 months (range 3 to 27 months). Scrotal mass was the most common reason for consultation. Cancer was bilateral in two patients. Two patients were admitted with metastatic disease. Histopathological examination favored germ cell tumors in 7 cases, two cases of non-Hodgkin’s malignant lymphoma, and one case of epididymo-testicular adenocarcinoma. Adjuvant chemotherapy resulted in complete remission in patients with germ cell tumors. However, neoadjuvant chemotherapy was not effective in patients admitted with advanced-stage disease. Conclusion: Testicular cancer is a rare condition that is curable in the majority of cases, but its management is often complicated in our setting due to delayed diagnosis caused by taboos surrounding genital organ pathologies.

AIM： To examine the psychological impact of chronic hepatitis C （CHC） diagnosis in a large cohort of CHC patients as compared with other stressful life events and chronic diseases carrying a risk of life-threatening complications. METHODS： One hundred and eighty-five outpatients with compensated CHC were asked to self-grade, using a 100-mm visual analogue scale （VAS）, the degree of stress caused by the learning of CHC diagnosis and the perceived severity of their disease. Diagnosis-related stress was compared to four other stressful life events and perceived CHC severity was compared to four other common chronic diseases. RESULTS： Learning of CHC diagnosis was considered a major stressful event （mean ± SD scores： 72±25）, significantly less than death of a loved-one （89±13, P〈0.0001） and divorce （78 ± 23, P〈0.007）, but more than job dismissal （68 ± 30, P〈 0.04） and home removal （26±24, P〈 0.0001）. CHC was considered a severe disease （74± 19）, after AIDS （94±08, P〈 0.001） and cancer （91± 11, P〈 0.001）, but before diabetes （66±23, P〈0.001） and hypertension （62±20, P〈0.001）. Perceived CHC severity was not related to the actual severity of liver disease, assessed according to Metavir fibrosis score. In multivariate analysis, diagnosisrelated stress was related to perceived disease severity （P〈0.001）, trait anxiety （P〈 0.001） and infection through blood transfusion （P〈 0.001）. CONCLUSION： Our results show the considerable psychological and emotional burden that a diagnosis of CHC represents, even in the absence of significant liver disease. They should be taken into account when announcing a diagnosis of CHC in order to reduce its negative effects.

Electroreduction of Tungsten Oxide（VI） in Molten Salts with Added Metaphosphate

Tungstate ions WO4^2- are not electrochemically active in chloride melts. Upon introduction of PO3 ions into the melt, two waves appear in the voltammograms at potentials -0.1-- -0.2 V and -0.3-- -0.5 V vs. Pb/Pb2＋ reference electrode. With a PO3- concentration ratio of 0.01〈[PO3-]/[WO42-]〈0.18, the potentiostatic electrolysis product of WO42 at the above mentioned potentials is metallic tungsten; a NaPO3 concentration increase for ratios [PO3-]/[WO42-]〉〉0.18 results in tungsten phosphide in electrolysis product. Cyclic voltammograms and dependence of half-peak potentials on electrode polarization rate indicate the irreversible character of the electrode process. Elec- trode process modeling allows us to state that the first wave in the voltammogram of KC1-NaC1-Na2WOa-NaPO3 sys- tem corresponds to tungsten oxychlorides discharge while the second wave appears due to the discharge of di- tungstate ions. In the voltammograms of Na2WOa-NaPO3 melts, reduction wave was observed at -1.1-- -1.2 V po- tentials. Proportionality of limiting current to NaPO3 concentration, constancy of ld/V1/2 ratio, and IJnFc constant ki- netic value equal to （8.3--9.5）×10-5 cm/s for steady-state wave indicate that electrode process rate is limited by elec- trochemically active particle diffusion to the electrode. Nascent ditungstate ions become electrochemically active in the overall electrode process. Charge transfer stage is reversible.

Larvicidal activities of hydro-ethanolic extracts of three Cameroonian medicinal plants against Aedes albopictus

Objective: To investigate the larvicidal activity of Syzygium guineense(Myrtaceae)(S. guineense), Monodora myristica and Zanthoxylum heitzii(Rutaceae)(Z. heitzii) extracts against Aedes albopictus(Ae. albopictus).Methods: The larvicidal activity of the hydro-ethanolic extracts from these plant species was assessed at three different concentrations(50, 100 and 200 mg/L) on first-instar of Ae. albopictus larvae in comparison with untreated controls. Mortality rate was recorded daily for a period of 12 days. The values of LC_(50) and lethal time killing 50% of the tested individuals(LT_(50)) were calculated using the log-probit ***: The root extract of S. guineense exhibited the best activity with 100% mortality after 8 days of treatment at 200 mg/L, followed by the fruit extract of Z. heitzii with83.33% mortality at the same concentration. Nonetheless, larvae were most susceptible to the fruit extract of Z. heitzii both in terms of LC_(50)(39.89 mg/L) and LT_(50)(145.68 h). A statistically significant difference between the control and the group treated at 200 mg/L was noticed in all the ***: The present study shows that the hydro-ethanolic extracts of S. guineense,Monodora myristica and Z. heitzii tested have significant larvicidal activity. These preliminary results are of great interest and some of these plant species can be proposed for the formulation of new bioinsecticides to control Ae. albopictus populations.
