Situation of Artificial Depigmentation among Women in 2016 to Bobo-Dioulasso, Burkina Faso

INTRODUCTION: The artificial depigmentation (DA) is a practice that aims to reduce the natural pigmentation of the skin by the use of depigmenting products (DP). In Burkina Faso, few studies have been made on the issue and the last date of 2003. The objective of the study is to realize an overview of the DA phenomenon in women population in Bobo-Dioulasso City. METHODS: This is a descriptive cross-sectional study held from April 16th to April 30th 2016 including 437 women aged from 13 to 55 years old. A multi-stage random sampling was realized A chi-square test was used to compare groups with significant threshold of 5%. RESULTS: The prevalence of the use of DP (67.28% with IC95 = [63.52 - 71.03]). The mean age of users was 27.5 years old;the typical profile was women from 30 to 35 years old (85.7%), with elementary study level, (73.47%), housewife’s (75.19%) and monogamous (71.98%). the products used was those made of hydroquinone (81.6%), EDTA (8.33%), kojic acid (4.86%), mixtures (11.12%) or unknown products (14.58%). The origin of the product was doubtful for almost all the cases (98.96%). The monthly mean cost of this practice for a single user is 1710 CFA. Complications were reported in 62.5% of users, and the most found was dyschromia (85.87%), acne (17.39%) and burns (10.32%). DISCUSSION: The DA is in great progress in Bobo-Dioulasso (2 out of 3 women) compared to half, thirteen years ago. The hydroquinone was markedly the most frequent DP while there was an apparent lack of corticosteroids and mercuries replaced by the new depigmenting agent such as EDTA and kojic acid. This result contrasted with the classic combination hydroquinone-dermocorticoid-mercuries reported by previous studies. CONCLUSION: The low-cost of DP could explain the magnitude of the artificial depigmentation phenomenon. Two Cases of Hallopeau Type of Pemphigus Vegetans in Two Women in Burkina Faso

Pemphigus of Hallopeau (PH) is a mild and pustular form of pemphigus vegetans, termed initially as “pyodermite végétante” described by Hallopeau. We report cases in two black women. Two women aged 32 and 36 respectively (case 1 and case 2) were seen for a pustulosis associated with blisters and erosions. Their history and their review have found the mouth ulcerations. The clinical examination showed pustules on the cephalic end, the trunk, the limbs and the large folds. Lesions had a polycyclic aspect and spread in a centrifugal way, developing towards budding, squamous and scabby erosions quickly surrounded by new pustules. Regression left a pigmented macula. Histological examination of the skin lesion showed images similar to vulgaris pemphigus (PV), together with hyperacanthosis and papillomatosis. Aspects observed in immunofluorescence were identical to those of PV (case1). General corticoid therapy led to a rapid disappearance of lesions in Case 1 but in Case 2 the Lever protocol combining corticoid and methotrexate have been used. PH is rarely described in our regions. Clinical diagnosis is not obvious for the uninitiated. Beyond the clinical presentation, PH has the same paraclinical diagnosis and the same treatment as PV.

Skin Damage and Aesthetic Disadvantage Observed in Women in the Hand Craft Shelling Chain of Cashew Nuts in a Factory to Bobo-Dioulasso, Burkina Faso

Introduction: Occupational dermatitis has long been an unforeseen consequence of industrial progress. Today, most occupational diseases are dermatosis throughout the world. The morbidity and the various prejudices concern different actors, especially those of the cashew industry in Bobo-Dioulasso to Burkina Faso. We considered it appropriate to conduct this study in women who handle cashew nuts in the cashew nut shelling station at a factory. The aim is to contribute to the search for preventive and curative solutions adapted to this health risk. Methods: This was a prospective cross-sectional study with a descriptive purpose in May 2016. The study involved 112 women present in the factory in the shelling unit and having given their approval during our visit. This station is occupied by 118 women. Results: The prevalence of all dermatosis and dermatitis was estimated respectively at 98.21% and 88.39%. The manual shelling station with a shearing machine was exclusively occupied by illiterate women (72.3%) without personal protective equipment, with an average age of 33.74 ± 8.49 years. Among the clinical forms of contact dermatitis, the erythematous contact dermatitis was most observed (86.4%), followed by palmar keratoderma (70.9%), nail disorders (33.6%) and chemical burns (30%). The implication of Cashew Nut Shell Liquid (CNSL) was established with identified promoting factors. Aesthetic damage related to hand injuries had a significant psychosocial impact in 97.32% of cases. Discussion: Few studies exist on dermatitis and the health impacts in the reality of cashew because they reach vulnerable subjects without social welfare and health protection. The cashew nuts shell contains a highly irritating brown oily liquid. Dermatitis occurs during contact with this oily juice. Management of a Severe Form of Verneuil’s Disease in Its Genitoperineal Localisation in Bobo-Dioulasso

The authors report a severe form of genitoperineal localisation of Verneuil’s disease managed in a surgical environment, having evolved favorably with improvement of the patient’s quality of life, but persistence of unsightly skin scars. They insist on the unexceptional nature of this disease whose prognosis is reserved, sometimes leaving severe functional sequelae. The long evolution of the disease with prolonged hospitalization in a professionally active young patient and the extension of suppurations to the perineal, scrotal and inguinal regions was the features of this observation. After wide excision of the lesions with antibiotic associated with local care, evolution had taken to complete wound healing with improved quality of life and a recovery of his professional activity.
