LARCs after unsafe abortion in Libreville (Gabon): Women accept quickly but the time interval for the insertion is longer than that of injectable contraceptives

Objectives: To determine the percentage of women who accepted a LARC after abortion, specify the interval time for the insertion of the LARC and compare such interval time with that of injectable and oral contraceptives. Patients and Methods: This is a prospective, descriptive and comparative study conducted in Libreville (Gabon), from February 1, 2013 to August 31, 2013. After receiving post-abortion care (PAC), all patients with stable general states were counseled about three types of contraceptive methods: combined oral contraceptive pills, LARCs (Jadelle? implants, Copper IUDs) and injectable Medroxyprogesterone Acetate (DMPA). Results: 231 women received PAC and among them 215 (93%) wanted to use one of the three proposed contraceptive methods. At the end of the study, only 193 women (89.7%) used contraception and 22 others (10.3%) abandoned their intentions. LARCs were used by 31 (16.0%) women. The average period for the insertion of LARCs was 15.4 ± 15.5 days with extremes of 1 to 53 days. This period for the insertion of LARCS was significantly longer than that of other methods (p < 0.001). LARCs were inserted more often after the 2nd day than immediately (same day) with a highly significant difference (p < 0.0001). Conclusion: LARCs could constitute an appropriate approach to reducing unintended pregnancies in Gabon. However, in order to increase their rates of use, it is necessary to improve women awareness.

Influence of Prenatal Surveillance on Maternal and Perinatal Prognosis: A Prospective Study over 6 Months at the Maternity Ward of the Owendo University Hospital (Gabon)

Introduction: The occurrence of pregnancy in women is a risky situation. Prenatal care is necessary, which is not often the case in our context. Aim: To analyze the influence of antenatal surveillance on maternal and perinatal prognosis. Patients and Method: Preliminary longitudinal and analytical survey at the Owendo University Hospital (OHU) over 6 months. It focused on prenatal surveillance. The study population consisted of parturients who gave birth within 24 hours and we studied sociodemographic characteristics, variables related to antenatal contact, those of delivery as well as maternal and newborn outcomes. Results: 2485 deliveries were recorded and 1300 patients were retained according to the inclusion criteria. No prenatal contact (ANC0) was performed in 93 (7.15%), insufficient (ANCI) in 943 patients (72.5%), and sufficient (ANCS) in 264 patients (20.30%). Patients with low school level were significantly found when the NPC was not performed or insufficient and the same was true for the group of patients who were not employed and those who were single (p < 0.005). The caesarean section rate and perinatal mortality are high in this case. Conclusion: The quality of prenatal contact is insufficient in our context. The absence or inadequacy of the latter has a strong negative impact on maternal and perinatal morbidity and mortality.

Symmetry and conservation laws for tuberculosis model

In this paper, three-dimensional system of the tuberculosis (TB) model is reduced into a two-dimensional first- order and one-dimensional second-order differential equations. We use the method of Jacobi last multiplier to construct linear Lagrangians of systems of two first- order ordinary differential equations and nonlinear Lagrangian of the corresponding single second-order equation. The Noether's theorem is used for determining conservation laws. We apply the techniques of symmetry analysis to a model to identify the combinations of parameters which lead to the possibility of the linearization of the system and provide the corresponding solutions.

替代性评价需要一系列评估方式,包括课题的使用、论文扩展、组合、演示、展览、实验、实地考察等,这些方法能针对传统基于纸笔的评估方法的问题提出解决措施,此外,替代性评价允许学生展示他们能做什么,而不是在学生不知道的领域里发觉他们的错误。该定性研究的目的是调查当前坦桑尼亚伊拉拉市中学生物教师对替代性评价方法的实施情况,运用问卷调查法、访谈法、观察法收集数据,并采用目的抽样、简单随机抽样和分层抽样选取36名生物学教师,18名学术教师和144名学生。调查结果显示,教师对采用自上而下的方法进行替代性评价具有普遍共识,所调查的对象都表明,在普通中学生物教与学的过程中实施替代性评价仍使他们充满不确定感。教师们担心没有足够的时间以满足改革的需求。教师在评估中起着决定性作用,因为他们发现了几个阻碍替代性评价实施的因素,基于此,本研究提出建议和进一步研究领域。

赞比亚河流系统复杂,它拥有丰富和广泛的水体,主要有五个世界著名淡水生态区,被认为是非洲水生植物多样性最丰富的地区之一。河网系统通常会对水生植物居群遗传多样性及遗传结构产生重要影响,但在赞比亚河流系统中却缺乏相关的研究。赞比亚河流系统是非洲海菜花属（Ottelia）植物的主要分布区和物种形成中心。本研究以赞比亚境内广泛分布的水鳖科（Hydrocharitaceae）海菜花属水生植物O.ulvifolia为研究材料,采用微卫星（SSR）分子标记研究该物种的遗传多样性水平及居群遗传结构,探讨O.ulvifolia居群间的历史基因流,评估河流系统对赞比亚O.ulvifolia居群遗传分化的影响。本研究集中在赞比亚当前的三个主要淡水生态区:赞比西河上游漫滩（UZF）、Bangweulu-Mweru（BM）和Luangwa-Zambezi河中游（MLZ）。每个O.ulvifolia居群选取7-23个个体为代表,对其叶片进行了取样。利用10个SSR多态位点对18个O.ulvifolia居群的307个样品进行了基因分型。多重分析用于确定其遗传多样性,同时给出以下估计值:观察杂合度、预期杂合度、等位基因数和有效等位基因数、等位基因丰富度和私有等位基因;采用贝叶斯聚类、主成份分析（PCo A）和未加权算术平均聚类方法（UPGMA）等研究居群间的关系;本研究还进行了以下分析以评估遗传多样性的基本模式:地理距离与遗传距离的相关性、近期的瓶颈效应分析、基因流等;BARRIER软件用于检验河网和流域之间的遗传不连续性;最后,利用全新世中期和末次冰期气候资料对O.ulvifolia居群进行生态位模拟,以推断该种的冰期避难所,并预测该物种未来的适生生境。本研究的主要结果如下:（1）基于10个SSR分子标记对O.ulvifolia居群的研究结果发现其具有较高的遗传多样性水平（HE=0.425,NT=81,I=0.713,Ar=1.974）,本研究认为生物因素,例如:有效的种子传播机制（水力）、居群大小（N）、寿命和繁殖策略（异交）,（FIS=0.035）,可能是该物种具有较高遗传多样性的主要原因。（2）STRUCTURE、UPGMA树和PCo A分析结果都显示O.ulvifolia具有两个明确的遗传簇（K=2）;AMOVA结果显示73.70%的遗传分化（FST=0.263）存在于居群内部;Mantel检验显示地理距离与遗传距离没有明显的相关性（r=0.130,P=0.053）。基于这些研究结果,我们认为有限的长距离花粉传播可能是导致物种居群内部遗传变异高的主要原因。此外,性别等因素也可能会影响育种和授粉,从而导致物种的遗传分化。虽然O.ulvifolia是异交的、昆虫授粉和多年生的植物,但其不同的居群间中存在较高的分化,开花特性上的差异也可能是造成这一结果的原因之一。（3）在Bangweulu-Mweru和赞比亚中下生态区,仅有三个居群间存在基因流的潜在障碍。而对于其它居群来说,基因流相对较高（NM=0.120-1.571）。诸多因素,例如:复杂的河流系统、没有明显的物理屏障、在雨季存在广泛的易受常年洪水影响的洪泛平原等使得赞比亚FEOWs湿地间具有较大的连通性,这可能是导致流域间存在较大的基因流,从而显著减缓了遗传分化。（4）物种分布模拟（SDM）结果表明,O.ulvifolia曾在非洲广泛分布,在南北半球都具有较高的适生性。本研究的结果还表明末次冰期（LGM）之后其物种分布范围存在收缩,在非洲南部和西部有一些潜在的避难所;未来,O.ulvifolia的适生生境有可能比目前范围要大。总之,本研究揭示了O.ulvifolia在赞比亚存在较高的遗传多样性水平,为了解本物种的遗传多样性及分化提供了资料。居群间较高水平的基因流可能归因于昆虫的长距离传播种子及赞比亚漫滩地区的常年洪水;此外,花的特性对O.ulvifolia的居群遗传结构的形成起着重要的作用;然而,赞比亚的复杂的河流系统和连接给O.ulvifolia居群间的连通提供了条件,再加上足够的花粉和种子扩散,导致了居群间存在高的基因流,因此本研究中出现了中到低程度的居群遗传分化;最后,SDM的结果显示了冰川扩张的证据,并表明该种在热带非洲的基因流明显较高。据此,我们建议对这些孤立的居群给予关注,并积极探索可能导致O.ulvifolia物种形成的因素,并制定对该种的切实有效的保护和管理措施。

Public＇s knowledge of the differences between ophthalmologists and optometrists：a critical issue in eye care service utilisation

AIM：To assess the public＇s knowledge of the differences between ophthalmologists and optometrists and identify the factors associated with knowledge.METHODS：The study was a population-based random survey of adults aged 18 years or older conducted in Enugu,south eastern Nigeria,between March and June,2011.Data on respondents＇ socio-demographics,clinical profile,and knowledge of the differences between ophthalmologists and optometrists were collected using a 28-item questionnaire.Data were analysed using descriptive and analytical statistics.Values of P〈0.05 were considered statistically significant.RESULTS：The respondents（P=394） comprised 198 males and 196 females（sex ratio =1.01：1）,aged 18-70（30.9 ±10.8） years.The majority of respondents were single（57.4%）,possessed secondary education（96.9%）,employed（65.2%） and had no health insurance（77.4%）.Their clinical profile showed previous eye exam 54.1%,spectacle wear 41.6%and contact lens wear 5.6%.In the multivariate analysis,participants＇ good knowledge of the differences between ophthalmologists and optometrists was significantly associated with educational status（OR：0.32,95%CI：0.23-0.44,P〈0.0001,β =-0.988）,employment status（OR：1.8,95%CI：1.45-2.25,P〈0.0001,β=0.124）and previous eye examination（OR：1.63,95%CI：1.29-2.07,P〈0.0001,β =0.549）.CONCLUSION：Participants＇ socio-demographic and clinical characteristics are important predictors of good knowledge.The findings may have implications for all stakeholders in eye care delivery.There is need for knowledge enhancement,by the government and eye care providers,through population-based eye health literacy campaigns.

Multi-Mesh-Scale Approximation of Thin Geophysical Mass Flows on Complex Topographies

This paper is devoted to a multi-mesh-scale approach for describing the dynamic behaviors of thin geophysical mass flows on complex topographies.Because the topographic surfaces are generally non-trivially curved,we introduce an appropriate local coordinate system for describing the flow behaviors in an efficient way.The complex surfaces are supposed to be composed of a finite number of triangle elements.Due to the unequal orientation of the triangular elements,the distinct flux directions add to the complexity of solving the Riemann problems at the boundaries of the triangular elements.Hence,a vertex-centered cell system is introduced for computing the evolution of the physical quantities,where the cell boundaries lie within the triangles and the conventional Riemann solvers can be applied.Consequently,there are two mesh scales:the element scale for the local topographic mapping and the vertex-centered cell scale for the evolution of the physical quantities.The final scheme is completed by employing the HLL-approach for computing the numerical flux at the interfaces.Three numerical examples and one application to a large-scale landslide are conducted to examine the performance of the proposed approach as well as to illustrate its capability in describing the shallow flows on complex topographies.
