EUS diagnosis of ectopic opening of the common bile duct in the duodenal bulb: A case report

Among the various congenital anomalies of the biliary system,an ectopic opening of the common bile duct (CBD) in the duodenal bulb is extremely rare. ERCP is essential for diagnosing the anomaly. A 55-year-old male was admitted to hospital for severe right upper quadrant abdominal pain,followed by fever, chills, elevated body temperature and mild icterus. The diagnosis of ectopic opening of CBD in the duodenal bulb was established on endoscopic ultraso-nography (EUS),which clearly demonstrated dilated CBD, with multiple stones and air in the lumen, draining into the bulb. A normal pancreatic duct, which did not drain into the bulb, was also observed. This finding was confirmed on ERCP and *** far as we know, this is the first case of this anomaly diagnosed by EUS. Ectopic opening of the CBD in the duodenal bulb is not an incidental finding, but a pathologic condition which can be associated with clinical entities such as recurrent or intractable duodenal ulcer, recurrent biliary pain,choledocholithiasis or acute cholangitis. Endoscopic ultrasonography features allow preoperative diagnosis of this anomaly and can replace ERCP as a first diagnostic tool in such clinical circumstances. Embryology of the anomalies of the extrahepatic biliary tree has been also reviewed.

AIM:This paper describes the procedure of detection of Helicobacter pylori(***)in bile specimens in patients suffering from benign diseases of biliary ducts(lithiasis with/without nonspecific cholangitis). METHODS:The group of 72 patients entering the study consisted of 32 male and 40 female(45 % and 55 %, respectively).Bile was obtained during ERCP in 68 patients, and during cholecystectomy in 4 patients.A fast urease test (FUT)to determine the existence of *** in gastric mucosa was carried out for all the patients during the endoscopic *** existence of genetic material of *** was determined by detection of ure A gene by the method of nested *** results of this reaction were shown by electrophoresis on 10 g·L^(-1)agarose gel in a band of 256 bp. RESULTS:The majority of the patients included in our study had biliary lithiasis without signs of cholangitis(48 patients, 67 %),whereas other patients were complicated by cholangitis(17 patients,24 %).Seven patients(9 %)had normal ERCP,forming thus the control *** the group of patients with lithiasis 26 patients(24.2 %)had positive PCR of *** in bile and among the patients with associated cholangitis positive PCR was detected in 9 patients(52.9 %).Among the seven patients with normal ERCP only one(14 %)had positive PCR of ***.A high percentage of *** infection of gastric mucosa was observed(57 patients,79 %).It was also observed that its slightly higher positivity was in the patients with distinct bile pathology:81% FLIT positive patients in the group with choledocholithiasis alone and 76 % in the group with choledocholithiasis associated with ***-one percent of the patients with regular findings had positive ***: The prevalence of H. pylori infection both in bile and in gastric mucosa in patients with benign diseases of biliary ducts does not show a statistically significant difference in relation to the prevalence of the same with the patients

This paper describes severe plastic deformation(SPD)procedures,which are utilized to form an ultrafine-grained structure in metallic *** the SPD process,a solid material sample is subjected to very high loads without a significant change in sample *** the present work,the high-pressure torsion(HPT)process,as one of the SPD techniques,which achieves a high degree of deformation and ensures refinement of the microstructure,will be discussed in more *** that grain size control is accepted as a method to obtain materials with desired characteristics,an overview of the properties of ultrafine-grained titanium-based biomaterials to be used in medicine is ***,particular attention is dedicated to the influences of HPT process parameters,primarily hydrostatic pressure,and number of revolutions during torsion,on the grain size and physical and mechanical characteristics(modulus of elasticity,microhardness,and tensile properties),corrosion resistance,and biocompatibility of the titanium-based biomaterials.A review of the literature indicates that titanium-based materials obtained by the SPD process show improved mechanical and physical properties without losing biocompatibility and corrosion resistance,which suggests that these methods of obtaining implants are something that should be further developed in the future.

We characterize all functions f ： N → C such that f（m^2 ＋ n^2） = f（m）^2 ＋ f（n）^2 for all m, n ∈ N. It turns out that all such functions can be grouped into three families, namely f ≡ 0, f（n） = ±n （subject to some restrictions on when the choice of the sign is possible） and f（n） =±l/2（again subject to some restrictions on when the choice of the sign is possible）.

BACKGROUND： The well-known functions of bile acids（BAs） are the emulsification and absorption of lipophilic xenobiotics. However, the emerging evidences in the past decade showed that BAs act as signaling molecules that not only autoregulate their own metabolism and enterohepatic recirculation, but also as important regulators of integrative metabolism by activating nuclear and membrane-bound G protein-coupled receptors. The present review was to get insight into the role of maintenance of BA homeostasis and BA signaling pathways in development and management of hepatobiliary and intestinal *** SOURCES： Detailed and comprehensive search of PubM ed and Scopus databases was carried out for original and review ***： Disturbances in BA homeostasis contribute to the development of several hepatobiliary and intestinal disorders, such as non-alcoholic fatty liver disease, liver cirrhosis, cholesterol gallstone disease, intestinal diseases and both hepatocellular and colorectal ***： Further efforts made in order to advance the understanding of sophisticated BA signaling network may be promising in developing novel therapeutic strategies related not only to hepatobiliary and gastrointestinal but also systemic diseases.

For the designing of cutting-dies is a complex and experience-based process,it is poorly supported by conventional 3D CAD ***,the majority of design activities,including the(re)modeling of those cutting die-components that are directly responsible for performing shaping operations on a sheet-metal stamping part,traditionally still need to be carried-out repetitively,separately,and manually by the *** eliminate some of these drawbacks and upgrade the capabilities of conventional 3D CAD software,this paper proposes a new methodology for the development of a parametric system capable of automatically performing a(re)modeling process of compound washer dies' *** presented methodology integrates CATIA V5 built-in modules,including Part Design,Assembly Design and Knowledge Advisor,publication mechanism,and compound cutting die-design *** system developed by this methodology represents an 'intelligent' assembly template composed of two modules GAJA1 and GAJA2,***1 is responsible for the direct input of the die-design problem regarding the shape,dimensions and material of the stamping part,its extraction in the form of geometric features,and the transferring of relevant design parameters and features to the module ***2 interprets the current values for the input parameters and automatically performs the modeling process of cutting die-components,using die-design knowledge and the company's internal design and manufacturing *** results show that this system significantly shortens the modeling-time for cutting the die-components,improves the modeling-quality,and enables the training of inexperienced designers.

Differences in heavy metal tolerance among separate populations of the same species have often been interpreted as local adaptation. Persistence of differences after removing the stressor indicates that mechanisms responsible for the increased tolerance were genetically determined. Drosophila subobscura Collin （Diptera： Drosophilidae） pop- ulations were sampled from two localities with different history of heavy metal pollution, and reared for eight generations in the laboratory on a standard medium and on media with different concentrations of lead （Pb）. To determine whether flies from different natural populations exposed to the Pb-contaminated media in the laboratory show population spe- cific variability in fitness components over generations, experimental groups with different concentrations of lead were assayed in three generations （F2, Fs, and Fs） for fecundity, de- velopmental time, and egg-to-adult viability. On the contaminated medium, fecundity was reduced in later generations and viability was increased, irrespective of the environmental origin of populations. For both populations, developmental time showed a tendency of slowing down on media with lead. Faster development was observed in later generations. Preadaptation to contamination, meaning higher fecundity, higher viability, and faster egg to adult development in all studied generations, was found in D. subobscura originating from the locality with a higher level of heavy metal pollution,

Cyanobacterial blooms are global phenomena that can occur in calm and nutrient-rich （eutrophic） fresh and marine waters. Human exposure to cyanobacteria and their biologically active products is possible during water sports and various water activities, or by ingestion of contaminated water. Although the vast majority of harmful cyanobacterial products are confined to the interior of the cells, these are eventually released into the surrounding water following natural or artificially induced cell death. Electrochemical oxidation has been used here to damage cyanobacteria to halt their proliferation, and for microcystin degradation under in-vitro conditions. Partially spent Jaworski growth medium with no addition of supporting electrolytes was used. Electrochemical treatment resulted in the cyanobacterial loss of cell-buoyancy regulation, cell proliferation arrest, and eventual cell death. Microcystin degradation was studied separately in two basic modes of treatment： batch-wise flow, and constant flow, for electrolytic-cell exposure. Batch-wise exposure simulates treatment under environmental conditions, while constant flow is more appropriate for the study of boron-doped diamond electrode efficacy under laboratory conditions. The effectiveness of microcystin degradation was established using high-performance liquid chromatography-photodiode array detector analysis, while the biological activities of the products were estimated using a colorimetric protein phosphatase-1 inhibition assay. The results indicate potential for the application of electro-oxidation methods for the control of bloom events by taking advantage of specific intrinsic ecological characteristics of bloom-forming cyanobacteria. The applicability of the use of boron-doped diamond electrodes in remediation of water exposed to cyanobacteria bloom events is discussed.
