Metastases of esophageal carcinoma to skeletal muscle:Single center experience

Metastases of esophageal carcinoma to the skeletal muscle are rare,but the incidence may be increasing because of better diagnosis resulting from widespread use of positron emission tomography/computed tomography(PET/CT).A cohort of 205 patients with esophageal carcinoma treated at our center who had PET/CT between 2006 and 2010 was retrospectively evaluated for the presence of skeletal muscle metastases.Four patients had skeletal muscle metastases of esophageal carcinoma,including two patients with squamous cell carcinoma.In another patient with squamous cell carcinoma of the esophagus and synchronous skeletal muscle metastases,muscle metastases were subsequently shown to be related to second primary pancreatic adenocarcinoma.In all cases,skeletal muscle metastases were the first manifestation of systemic disease.In three patients palliation was obtained with the combination of external beam radiation therapy,systemic chemotherapy or surgical resection.Skeletal muscle metastases are a rare complication of esophageal carcinoma.

AIM To analyze pancreatic cancer patients who developed metachronous pulmonary metastases(MPM) as a first site of recurrence after the curative-intent surgery.METHODS One-hundred-fifty-nine consecutive pancreatic ductal adenocarcinoma(PDAC) patients who underwent radical pancreatic surgery between 2006 and 2013 were included in this retrospective analysis. The clinical data including age, sex, grade, primary tumor location, p TNM stage, lymph node infiltration, microangioinvasion, perineural invasion, lymphovascular invasion, the therapy administered, and follow-up were all obtained from medical records. Further analysis covered only patients with metachronous metastases. Clinical and histopathological data(age, sex, grade, primary tumor location, p TNM stage, lymph node infiltration, microangioinvasion, perineural invasion, lymphovascular invasion, the therapy administered and follow-up) of patients with metachronous non-pulmonary metastases and patients with metachronous pulmonary metastases were statistically assessed. Disease-free survival(DFS) from pancreas resection until metastases onset and overall survival(OS) were calculated. Wilcoxon test, χ~2 test and survival functions computed by the KaplanMeier method were used. Statistical significance was evaluated by the log-rank test using SPSS. A P-value of less than 0.05 was considered statistically significant.RESULTS Metachronous pulmonary metastases were observed in 20(16.9%) and were operable in 3(2.5%) of PDAC patients after a prior curative-intent surgery. Patients with isolated pulmonary metastases(oligometastases and multiple metastases) had estimated prior DFS and OS of 35.4 and 81.4 mo, respectively, and those with metachronous pulmonary metastases accompanied by other metastases had prior DFS and OS of 17.3 and 23.4 mo, respectively. Patients with non-pulmonary metastases had prior DFS and OS of 9.4 and 15.8 mo, respectively. A recent study published in Nature Immunology by Peter Savage and colleagues sought to characterize regulatory T-cell(Treg)-suppressed self-reactive CD4^(+)T cells using an elegant system combining the depletion of Tregs with T-cell receptor(TCR)sequencing of expanded conventional CD4^(+)T-cell clones.By tracking the fate of T cells engineered to express the identified TCRs,Lee et al.discovered a‘new’subpopulation of CD4^(+)T cells bearing antigen receptors with high levels of self-reactivity that take on a T follicular helper(TFH)-like phenotype in the periphery in the absence of immune challenge[1].Antigen-specificαβTCRs are generated through the somatic recombination of V(D)J gene segments during thymic development,giving rise to T cells that can recognize an enormously diverse set of peptides presented by major histocompatibility complex(pMHC)molecules.A risk of this process is that it can also lead to the generation of potentially highly self-reactive TCRs.T cells with TCRs with high affinity for self-pMHC molecules are purged in the thymus by clonal deletion,a key tolerance mechanism that prevents autoimmunity.However,this process is incomplete,and highly self-reactive T cells can escape negative selection and join the peripheral naive T-cell pool.Indeed,while the naive CD4^(+)T-cell pool is heterogeneous with regard to an individual T cell’s reactivity for self-pMHC molecules,the distribution of T cells with differing strengths of self-reactivity leaving the thymus is skewed toward those with relatively higher self-pMHC binding strengths[2].A subset of these highly self-reactive CD4^(+)T cells in the periphery are deleted,some become functionally anergic,and others differentiate into Tregs,which go on to play an important role in peripheral tolerance,holding other CD4^(+)T cells with potentially dangerous levels of self-reactivity in check[3].What is the fate of these Treg-suppressed self-reactive cells?

The consequences of changes planned in the European Union legislation relevant to the disposal of sewage sludges are discussed. A specific municipal waste water treatment plant is analyzed in terms of drying and subsequent combustion or pyrolysis of the produced stabilized sludge, and the respective net energy balances are carried out. A simplified economic analysis of the two disposal options is presented, which suggest that combustion of the sludge would be economically infeasible while pyrolysis of the sludge in a modular, self-sufficient container unit can bring a small financial benefit due to the selling of the produced phosphorus-rich biochar.

The device preconditioning dependent hysteresis and the consequential performance degradation hinder the actual performance and stability of the perovskite solar cells. Ion migration and charge trapping in the perovskite with large contribution from grain boundaries are the most common interpretations for the hysteresis. Yet, the high performing devices often include intermediate hole and electron transporting layers, which can further complicate the dynamical process in the device. Here, by using Kelvin Probe Force Microscopy and Confocal Photoluminescence Microscopy, we elucidate the impact of chargetransporting layers and excess MAI on the spatial and temporal variations of the photovoltage on the MAPbI3-based solar cells. By studying the devices layer by layer, we found that the light-induced ion migration occurs predominantly in the presence of an imbalanced charge extraction in the solar cells, and the charge transporting layers play crucial role in suppressing it. Careful selection and processing of the electron and hole-transporting materials are thus essential for making perovskite solar cells free from the ion migration effect.

Cold forming of high strength materials is accompanied by an undesirable spring-back effect and therefore the automotive industry prefers to produce components from high strength steels by hot stamping.Hot deformation and cooling in a die are applied to obtain shaped components with martensitic microstructure and high yield strength and ultimate tensile strength.This article presents new applications of this forming technology by incorporating another innovative heat treatment by the Q-P process,which improves both strength and ductility of obtained structures at the same time.Ultimate strengths over 2000 MPa with ductility above 10% can be achieved by this processing.To test microstructure development,thin sheet was hot formed and a corresponding FEM simulation was created.This processing was applied to low alloyed AHS steel with 0.42% of carbon and with an alloying strategy based on Mn,Si and Cr.Martensitic microstructure with retained austenite was obtained by this processing with a strength of around 2000 MPa and ductility of 10%.

The influence of different heat treatment strategies on the microstructure and properties of 42SiCr steel was investigated.To compare the benefit of innovative treatment methods with more conventional ones,several strategies based on a new quenching and partitioning (Q-P) process were designed together with several strategies using more traditional quenching and tempering process.Different combinations of mechanical properties were obtained with ultimate strengths in the range of 1818-2120 MPa and ductility between 10% and 24%.Resulting microstructures were martensitic with 0-12% of retained austenite and they were analyzed by laser scanning confocal microscopy,transmission electron microscopy and X-ray diffraction phase analysis.

Barrett's esophagus (BE) is a metaplastic lesion of the distal esophagus arising as a consequence of chronic gastroesophageal reflux disease. Multiple studies show that BE is associated with increased risk of esophageal adenocarcinoma (EAC). Epidemiological studies and animal models demonstrate that chronic inflammation triggered by repeated exposure to refluxate predisposes to the development of BE and EAC. The chronic inflammation is associated with cytokine alterations. Interleukin 6 (IL-6) is a cytokine that stimulates cell proliferation and apoptosis resistance is frequently increased in different cancers. Importantly, IL-6 and transcriptional factor signal transducer and activator of transcription 3 (STAT3) that is activated by IL-6 are also increased in BE and EAC. This review critically appraises the role of IL-6/STAT3 pathway in progression of BE to EAC from the published evidence currently available.

The purpose of this paper is to evaluate control of orientation of foliations and joints on the direction of valley axes in the St?íbrné Hory area in the Bohemian-Moravian Highlands in central part of the Czech Republic. This area is predominantly built by metamorphic rocks. It is situated near the P?ibyslav deep-seated fault. The data concerning of foliations, joints and valley axes were statistically analyzed and then visually compared according to their distribution using direction roses. Directions of valley axes have generally orientation of NNE–SSW, SE, E?W, NW and WNW and coincide with the system of foliations and ordinarily steep joints.
