Immunovirological discordance and Associated Factors among People Living with HIV under Antiretroviral Treatment at Hôpital de Jour de Donka, Guinea

The antiretroviral treatment (ART) has significantly reduced the number of new HIV/AIDS infections and related deaths. However, cases of immunovirological discordance (IVD) are found in various locations. The objective of this study was to determine the profile of People living with HIV (PLHIV) with IVD and to identify associated factors. We conducted a cross-sectional study based on the records of PLHIV under ART for at least 6 months, followed at Hpital de Jour Donka from 2015 to 2017, and having both viral load (CV) and CD4 T-cell count. Prevalence of IVD was 34.57%, with 23.87% for immunological discordance (ID) and 10.7% for virological discordance (VD). Females were predominant (66.26%), and male gender influenced IVD with a statistically significant difference (p = 0.006) and was associated with VD (p = 0.007). The average age was 38.77 11.30 years. PLHIV were classified at WHO stages 3 and 4 (86.01%). The median initial haemoglobin level was 11.5 g/L [3.2 - 12]. The mean initial CD4 T-cell count was 272.84 cells/mm3 201.6. The median initial viral load (VL) was 147,337 copies/mL [1092 - 31,675,000]. The initial CD4 T-cell count 3 was associated with IVD with a statistically significant difference (p = 0.0009) and correlated with ID (p = 0.000). Prurigo was associated with IVD with a statistically significant difference (p = 0.003). Cerebral toxoplasmosis was not associated with IVD but was associated with ID (p = 0.04). This study allowed us to describe the profile of PLHIV with IVD. The main associated factors were male gender, initial CD4 T-cell count 3, toxoplasmosis, prurigo, and herpes zoster.

Connective tissue diseases (CTDs) are Autoimmune diseases (AIDs) characterized by the appearance of autoantibodies, which are diagnostic markers. Investigations of these autoantibodies play a major role in the management of several autoimmune diseases. The objective of this study was to describe the profile of anti-ENA antibodies according to the clinical symptoms of mixed CTDs in Conakry teaching Hospital. We performed a cross-sectional study during six months. A total of 20 patients was recruited and we measured antibodies using the ELISA technique. The mean age of our patients was 36.5 years, with a predominance of females. Cutaneous and rheumatological signs were the main clinical manifestations. SLP was the most frequent CTDs;the threshold of ENA antibodies positivity was higher in scleroderma with and SLP. Anti-ENA identification reveals the frequency of anti-SSA (83.33%), anti-U1RNP (66.66%) and anti-histone (50%) antibodies. Antinuclear antibodies (ANA) react with various components of the cell nucleus. Their detection is of major interest in the diagnosis of CTDs. Our results highlight the importance of determining the specificity of these antibodies to guide differential diagnosis.

Purpose: diet and eating habits are major risk factors for the health and the development of disease, such as, for example, metabolic disorder leading to cardiovascular pathology and cancer, decreased immunity exposing to infections. This study of the physico-chemical and nutritional properties of a soumara-based food broth was carried out with the aim of promoting the consumption of organic broth made from nere seeds (soumara). That is to alleviate certain metabolic diseases, which is a matter of food safety, and also to limit the risk for the health about the consumption of some cooking stocks on the market. Methods: Several natural ingredients such as nere seeds (soumara), ginger, black pepper, parsley and garlic were used to create a nere-based stock. All these ingredients were freeze-dried and the powder obtained was used to make the broth, regarding their physical and chemical properties. Results: The broth had a good protein content of 17.41 ± 0.367 g/100g, a lipid content of 16.80 ± 0.08 g/100g and a fiber content of 8.66 ± 0.04 g/100g. In terms of nutritional values, the broth showed good levels of calcium 184.21 ± 0.09 mg/100g, potassium 50.04 ± 1.45 mg/100g and iron and zinc. In terms of antioxidant activity, the broth also showed good antioxidant activity. Conclusion: Regarding the properties of our food broth, whose composition is based on natural ingredients, could be recommended for consumption and, its properties, could play an important role in preventing and combating certain metabolic diseases.

Determination of the sensible heat effectiveness and pressure loss of a rotary regenerative heat exchanger using CFD

The paper presents a new method to determine the sensible effectiveness and pressure loss of rotary regenerative heat exchangers using computational fluid dynamics(CFD).It is based on CFD simulations of a single thermal wheel microchannel with a small cross-sectional area and thin walls,with cyclic inlet and outlet boundary *** unique measurement set-ups were designed and built for the experimental measurement of the heat exchanger *** different types of thermal wheels were manufactured,measured,and simulated in ANSYS *** wheels achieve an effectiveness greater than 73%under certain(air flow)conditions,which is the minimum effectiveness required by Ecodesign(in the EU).For the examined exchangers,the effectiveness ranges from 66.5%to 83.3%,depending on the boundary conditions and geometric parameters of the *** highest sensible effectiveness is achieved by heat exchangers with the largest heat exchange surface Ac;on the other hand,these wheels have the largest pressure *** paper discusses the use of a simplifiedε-NTU correlation model for the HVAC systems(typically C*=1).The correlation model and CFD results were compared and found to be different from each *** results of the CFD simulation were compared with measurements to prove that the proposed simulation method can predict the behaviour of the real heat exchanger as a *** was demonstrated that the sensible effectiveness and pressure loss of the rotary heat exchanger predicted by the CFD simulations correspond well to the measured values within the measurement uncertainty±1.3%.The proposed method can be used for the comparison of different rotary regenerative heat exchangers before their manufacturing and for verification that they meet the EU Ecodesign requirements set by the current *** reduces the cost of the initial optimisation and testing of new designs.

Scleroderma in Guinea’s African: Epidemiological, Clinical and Therapeutic Aspects

Introduction: Scleroderma is a generalized condition of the connective tissue, arterioles and micro-vessels characterized by tissue fibrosis, vascular obliteration and abnormal humoral and cellular immune responses. The aim of this study was to describe the epidemiological, clinical, therapeutic and evolutionary aspects of scleroderma at the Department of Dermatology-Venerology at Donka National Hospital. Methods: This was a descriptive cross-sectional study with a retrospective collection of data for a period of 11 years, from January 1, 2000, to December 31, 2010, covering all inpatient records of Scleroderma in the service. Results: We collected 17 cases of scleroderma out of 3289, a frequency of 0.04%. The average age of patients was 33.33 years with extremes of 10 and 60 years. A female predominance was noted with a sex ratio M/F of 0.13. Cutaneous induration was the main symptom associated with the phenomenon of Raynaud, and disorders of pigmentation. The extra-skin manifestations were mainly dominated by gastrointestinal involvement (32%), pulmonary involvement (24%), cardiac (20%), articular (20%) and renal (12%) involvement. The systemic form was the most common (99.45%). Corticotherapy was the most prescribed treatment (24.59%). Clinical improvement was noted in (93.2%) of the cases. Conclusion: Scleroderma, although rare, deserves special attention at all levels for early diagnosis.

The objective of this study was to describe Thrombotic manifestations and therapeutic management during HIV infection and tuberculosis in a 19-year-old male student. The objective of this study was to describe Thrombotic manifestations and therapeutic management during HIV infection and tuberculosis in a 19-year-old student. Observation: It is Mr. S. C., aged 19, admitted for pain and swelling of the right lower limb, physical asthenia and notion of fever. Evolution 5 days, with no particular history of cardiovascular disease, anti-retroviral treatment for 1 year and anti-tuberculosis treatment for 3 months. On clinical examination heart sounds are irregular with a heart rate at 115 bpm, blood pressure at 110/70 mmhg. A febrile red painful swelling at right lower limb with positive Homans sign. 0°C (Temperature 37.7°C), Weight at 58 Kg. The rest of the clinical examination is without particularity. Venous Doppler echo of the lower limbs: shows the presence of acute deep vein thrombosis of the right lateral vein. Mr. S. C. to benefit the following Medical Treatment: lovenox 0.6 UI subcutaneously morning and evening, Sintrom 4 mg 1 comprimed at night, Tramadol 50 mg morning and evening;with a good clinical evolution. CONCLUSION: TB and HIV are chronic infections that result in widespread inflammation predisposing patients to a MTVE (Venous thromboembolic disease) table as well as rifampicin and anti-proteases.

Lyell Syndrome with Fatal Outcome in Two Pregnant Women Receiving Antiretroviral Therapy in Guinea Conakry

An old woman, 28 years old, weighing 74 kg was referred for skin detachment associated with bubbles and mucosal erosions. This clinical picture had occurred 15 days after the institution of a triple combination: AZT (300 mg) + 3TC (150 mg) and nevirapine (200 mg), during a pregnancy of 20-week gestation. The woman was in her third gestation. The second case is aged 26, weighing 65 kg. This clinical picture had occurred 20 days after the administration of a triple therapy combination: AZT (300 mg) + 3TC (150 mg) and nevirapine (200 mg), during a pregnancy of 32-week gestation. It was the fourth gestation. The birth of a stillborn had occurred the day before his admission into our service. The diagnosis of Toxic Epidermal necrolysis with nevirapine was selected on the basis of the clinical and anamnestic arguments. The biological tests showed the following results: transaminases (ALT and AST) were within normal limits, blood glucose, and urea > 10 mmol/l. The evolution was marked by the patient death to Day 9 (D9) and at D2 of hospitalization. discussion: neT and the SJS have been reported in pregnant women as potentially dangerous complications that are often associated with suspected drugs. Among these drugs, antiretroviral drugs, prescribed to pregnant women to prevent mother to child transmission of HIV are often reported. Our case is an illustration of the problem of HIV management in pregnant women. It also focuses on the association between pregnancy and Toxic Epidermal necrolysis. nevirapine is an effective drug in the regimens proposed in the management of HIV patients in limited resources countries. But the high frequency of toxidermia with this drug should lead to its replacement by other antiretrovirals least providers of toxidermia as anti proteases in the context of PMTCT programs.

Effect of NaCl-Induced Salinity and Human Urine Fertilization on Substrate Chemical Properties

We evaluated the effect of NaCl-induced salinity and successive urine fertilization on changes in cultivation substrate chemical properties in a greenhouse study. The substrate was composed of an equal volume ratio mixture of bio-waste compost, quartz sand and silty loam soil. Salinity was imitated by adding NaCl solutions to a known substrate weight achieving three target salinity treatments of ECe 1.3 (S0—no NaCl), 4.6 (S1) and 7.6 (S2) dS·m-1. Cultivation substrate had been cropped with two cycles of maize (Zea mays L.) (crop cycles I and II) and fertilized with human urine at N amounts of 0 (U0—no urine), 180 (U1) and 360 (U2) mg·kg-1 substrate in the first cycle and half of the urine-N dosages in cycle II. Substrate samples collected at the end of each cycle were analyzed for pHKCl, ECe, exchangeable and water extractable cations (Na+, K+, Ca2+, Mg2+), cation exchange capacity, water extractable anions (Cl-, , , ) and exchangeable sodium percentage (ESP). Exchangeable Na+, K+ and Ca2+ were significantly (p ne interaction. ECe significantly increased by 7.3, 5.3 and 7.6 dS·m-1 in the S0, S1 and S2 treatments following an increase in urine from U0 to U2. In the S0 treatment, ESP increased in the order U0 and Cl- were significantly affected by crop cycle, salinity and urine interactions (p ne fertilizer on extractable and depended on crop cycle alone. There was a tendency towards increasing soil sodicity with mounting urine fertilization. The level of NaCl salinity and the amount of urine applied are important determinants of substrate chemical properties. Adoption of appropriate management techniques to avoid salinity/sodicity build up should be included in urine fertilization planning.

Effect of the seeds provenance and treatment on the germination rate and plants growth of four forest trees species of Cote d’Ivoire

The rapid rate of deforestation in Cote d’Ivoire has led to loss of plant species diversity and also threatens some commercial tree species with *** reforestation and aff orestation has been ***,for some species,the numbers of surviving transplants has ***,it is necessary to develop properly adapted and resilient genotypes that can eff ectively support the programs of forest *** this context we evaluated the eff ects of the provenances of seeds and of various treatments on the germination rates and on plant growth of four commercial trees species(Pycnanthus angolensis,Terminalia superba,Mansonia altissima and Pterygota macrocarpa).We analyzed these parameters for seeds of three provenances(Daloa,Akoupéand Daoukro)of four species and for thirty plants aged 6 months per *** to begin germination and time to reach maximum germination were similar among species,and were not infl uenced by either treatment or *** seeds and those soaked for 24 and 48 h in water at room temperature yielded the highest germination rates,irrespective of species or *** one exception was ***,for which no germination was *** species,some provenances produced higher germination percentages than did *** clear relationship was computed between the germination rate and the morphometric characteristics of the seeds which varied signifi cantly between *** growth and morphological variability also depended greatly on *** factors might be implicated,therefore provenance trials should be undertaken and ***,molecular analysis should be undertaken to confi rm the implications of genetic factors and allow for genetic selection.
