Background:Unresectable disease is sometimes diagnosed during surgery in patients with pancreatic ductal adenocarcinoma(PDAC).This study aimed to identify preoperative risk factors for metastatic disease diagnosed at surgical exploration and to investigate and compare survival in resected and nonresected ***:Patients were identified from the Swedish National Pancreatic and Periampullary Cancer Registry *** of metastatic disease were evaluated with a multivariable logistic regression model,and survival was evaluated with Kaplan-meier estimates and log-rank ***:In total,1938 patients with PDAC were scheduled for *** unresectable situation was diagnosed intraoperatively in 399 patients(20.6%),including 234(12.1%)with metastasized *** risk factors for metastasis were involuntary weight loss(OR=1.72;95%CI:1.27-2.33)and elevated carbohydrate antigen 19-9(CA19-9)(35-599 U/m L,OR=1.79,95%CI:1.11-2.89;≥600 U/m L,OR=3.24,95%CI:2.04-5.17).Overall survival was lower among patients with metastasized disease than that among patients with a resectable tumor(Pmetastasized disease during surgical exploration.

AIm: To investigate whether accelerated catabolism of ganglioside and decreased ganglioside content contribute to the etiology of pro-inflammatory intestinal disease. mETHODS: Intestinal mucosa from terminal ileum or colon was obtained from patients with ulcerative colitis or inflammatory Crohn's disease(n = 11) undergoing bowel resection and compared to control samples of normal intestine from patients with benign colon polyps(n = 6) and colorectal cancer(n = 12) in this observational case-control study. Gangliosides and phospholipids of intestinal mucosa were characterized by class and ceramide or fatty acid composition using liquid chromatography triple-quad mass spectrometry. Content and composition of ganglioside classes Gm1, Gm3, GD3, GD1 a, GT1 and GT3 were compared among subject groups. Content and composition of phospholipid classes phosphatidylcholine(PC) and phosphatidylethanolamine were compared among subject groups. Unsaturation index of individual ganglioside and phospholipid classes was computed and compared among subject groups. Ganglioside catabolism enzymes beta-hexosaminidase A(HEXA) and sialidase-3(NEU3) were measured in intestinal mucosa using western blot and compared among subject groups. RESULTS: Relative Gm3 ganglioside content was 2-fold higher(P m patients with inflammatory bowel disease(IBD) compared to control intestine. The quantity of Gm3 and ratio of Gm3/GD3 was also higher in IBD intestine than control tissue(P more than two unsaturated bonds was 30% lower in IBD intestine than control intestine(P < 0.05). The relative content of HEXA in IBD intestine was increased 1.7-fold(P < 0.05) and NEU3 was increased 8.3-fold

Background: Previous studies of continuous positive airway pressure (CPAP) treatment for obstructive sleep apnoea (OSA) have shown conflicting results on the effect on blood pressure (BP), and patients with chronic kidney disease (CKD) have not been included in these studies. As OSA is a frequent comorbidity in patients with CKD, it is of relevance to evaluate the effect of CPAP treatment on BP in this population. Aim: In this prospective follow-up study, we measured the effect of short term CPAP treatment of moderate-to-severe OSA on brachial and central BP, plasma level of syndecan-1 and vasoactive hormones, renal handling of sodium, subjective sleepiness, and quality of life in patients with impaired renal function. methods: From December 2015 until march 2017, 25 patients were invited to participate in the study at the University Clinic in Nephrology and Hypertension, Aarhus University and Holstebro Hospital. At baseline and at follow-up after three to four months of CPAP treatment, we performed 24 h brachial and central ambulatory BP measurement, blood sampling measurements of plasma concentrations of syndecan-1, renin, angiotensin II, aldosterone, vasopressin, creatinine, haemoglobin A1c, and cholesterol, cardio respiratory monitoring, 24 h urine collection for measurement of urinary excretion of albumin, aquaporin-2, and epithelial sodium channel, Epworth Sleepiness Scale (ESS), and SF-36 (quality of life). Results: At follow-up, the 17 included patients with mean baseline estimated glomerular filtration rate 66 mL/min/1.73 m2 had a significant decrease in systolic office-, 24 h- and daytime-BP (13, 7, and 8 mmHg, respectively, p Conclusion: Short-term CPAP treatment of patients with moderate-to-severe OSA and reduced renal function decreased 24 h- and daytime-BP significantly and reduced urinary albumin excretion. Our results underline the importance of treatment of OSA in hypertensive patients with impaired renal function.

The phylogeny of the Acarosporaceae(Lecanoromycetes,Acarosporomycetidae,Acarosporales)is investigated using data from three molecular markers;nuclear ITS-LSU rDNA,mitochondrial SSU andβ-*** is shown to be constituted by six main clades;myriospora,Timdalia,Pleopsidium,a clade composed by BAcarospora^rhizobola and BA.^terricola,the poorly supported Sarcogyne clade(including several Polysporina and Acarospora species)and the Acarospora clade(including the type of Polysporina,***,and several other Polysporina species).The common ancestor of the Acarosporaceae did not produce strongly black pigmented(carbonized or melanized)ascomata,but this trait has arisen secondarily and independently numerous times in the evolution of the *** number of changes in character states of both carbonized epihymenium and carbonized exciple are considerably more than the minimum *** genera Sarcogyne and Polysporina—largely circumscribed based on the presence of black pigmented ascomata—are shown to be distinctly *** presence of green algae in the ascoma margin(lecanorine or lecideine ascomata)may vary even within single species.

[背景]全氟烷基物质(PFASs)是持久性污染物,动物实验中具有内分泌干扰性和神经毒性。一些横断面数据显示PFASs与儿童神经行为的问题之间存在正向关联,但是来自前瞻性研究的结果仍然有限。[目的]研究产前暴露于PFASs是否与儿童中的注意力缺陷/多动障碍(ADHD)或儿童期自闭症之间有关联。[方法]在1996—2002年丹麦全国出生队列的83 389对母子中,根据丹麦国家医院登记和丹麦精神病中心登记,确定了890名ADHD病例和301名儿童自闭症病例。从这个队列中,随机各选择220例多动症和自闭症病例,并随机选择550例性别相匹配的对照儿童。测量在妊娠早期或中期采集的母体血浆中16种PFASs,使用广义线性模型计算风险比(RRs),模型中考虑到了抽样权重。[结果]在所有样品中均检测到全氟辛烷磺酸(PFOS)和全氟辛酸(PFOA),在≥90%的样本中可以定量其他4种PFASs。没有发现母亲PFAS血浆水平和ADHD之间关联的一致性证据[浓度(ng/m L)的自然对数值每增加一个单位,PFOS对应的RR=0.87(95%CI:0.74,1.02),PFOA对应的RR=0.98(95%CI:0.82,1.16)],对于自闭症也是如此[浓度(ng/m L)的自然对数每增加一个单位,PFOS对应的RR=0.92(95%CI:0.69,1.22),PFOA对应的RR=0.98(95%CI:0.73,1.31)]。在同时校正所有PFASs的模型中,较高的PFAS分位数和ADHD之间既有阳性关联也有阴性关联,但是这些关联的估计值并不精确。[结论]在这项研究中,没有发现一致性的证据能够提示产前PFAS暴露增加儿童中ADHD或儿童期自闭症儿的风险。
