Is there consensus regarding surgical treatment of bone sarcomas?

AIM To perform an Internet based survey on the surgical management of bone sarcomas in the lower extremity amongst sarcoma surgeons. METHODS All orthopedic surgical members of the Scandinavian Sarcoma Group were invited to participate in an online questionnaire. The questionnaire consisted of a clinical case involving resection of a malignant bone tumor. Several questions were asked, subdivided into categories. Among these, surgical/technical considerations, e.g., choice of implant; choice of antibiotics, dosage, and duration of treatment, choice of antithrombotic drug, initial start-up, dosage, and duration were *** In terms of choice of implant fixation, the majority of surgeons preferred an uncemented prosthesis in younger patients until the age of 50. All participants administer intravenous prophylactic antibiotics for endoprosthetic reconstructive surgery. First choice of antibiotics was cephalosporin. Less common used was glycopeptide, penicillin, or a combination. Duration of prophylactic antibiotics ranged from less than one day to more than four days. All participants used low molecular weight heparins as antithrombotic prophylaxis and 55% of the participants answered that initial treatment was started preoperatively, 3% perioperatively and 42% postoperatively. Duration of the antithrombotic treatment ranged from five days to more than twenty-eight *** The use of resection prosthesis in the treatment of bone sarcomas is a well-established procedure. However, therse is a significant discrepancy in the surgical treatment algorithm between the sarcoma centers. Still the treatment is mainly based on best clinical practice,due to the lack of evidence-based medicine in the surgical management of bone sarcomas.

[背景]全氟烷基物质(PFASs)是持久性污染物,动物实验中具有内分泌干扰性和神经毒性。一些横断面数据显示PFASs与儿童神经行为的问题之间存在正向关联,但是来自前瞻性研究的结果仍然有限。[目的]研究产前暴露于PFASs是否与儿童中的注意力缺陷/多动障碍(ADHD)或儿童期自闭症之间有关联。[方法]在1996—2002年丹麦全国出生队列的83 389对母子中,根据丹麦国家医院登记和丹麦精神病中心登记,确定了890名ADHD病例和301名儿童自闭症病例。从这个队列中,随机各选择220例多动症和自闭症病例,并随机选择550例性别相匹配的对照儿童。测量在妊娠早期或中期采集的母体血浆中16种PFASs,使用广义线性模型计算风险比(RRs),模型中考虑到了抽样权重。[结果]在所有样品中均检测到全氟辛烷磺酸(PFOS)和全氟辛酸(PFOA),在≥90%的样本中可以定量其他4种PFASs。没有发现母亲PFAS血浆水平和ADHD之间关联的一致性证据[浓度(ng/m L)的自然对数值每增加一个单位,PFOS对应的RR=0.87(95%CI:0.74,1.02),PFOA对应的RR=0.98(95%CI:0.82,1.16)],对于自闭症也是如此[浓度(ng/m L)的自然对数每增加一个单位,PFOS对应的RR=0.92(95%CI:0.69,1.22),PFOA对应的RR=0.98(95%CI:0.73,1.31)]。在同时校正所有PFASs的模型中,较高的PFAS分位数和ADHD之间既有阳性关联也有阴性关联,但是这些关联的估计值并不精确。[结论]在这项研究中,没有发现一致性的证据能够提示产前PFAS暴露增加儿童中ADHD或儿童期自闭症儿的风险。

Changes in Brachial and Central Blood Pressure after Short Term Continuous Positive Airway Pressure Treatment of Patients with Moderate-to-Severe Obstructive Sleep Apnoea and Impaired Renal Function

Background: Previous studies of continuous positive airway pressure (CPAP) treatment for obstructive sleep apnoea (OSA) have shown conflicting results on the effect on blood pressure (BP), and patients with chronic kidney disease (CKD) have not been included in these studies. As OSA is a frequent comorbidity in patients with CKD, it is of relevance to evaluate the effect of CPAP treatment on BP in this population. Aim: In this prospective follow-up study, we measured the effect of short term CPAP treatment of moderate-to-severe OSA on brachial and central BP, plasma level of syndecan-1 and vasoactive hormones, renal handling of sodium, subjective sleepiness, and quality of life in patients with impaired renal function. Methods: From December 2015 until March 2017, 25 patients were invited to participate in the study at the University Clinic in Nephrology and Hypertension, Aarhus University and Holstebro Hospital. At baseline and at follow-up after three to four months of CPAP treatment, we performed 24 h brachial and central ambulatory BP measurement, blood sampling measurements of plasma concentrations of syndecan-1, renin, angiotensin II, aldosterone, vasopressin, creatinine, haemoglobin A1c, and cholesterol, cardio respiratory monitoring, 24 h urine collection for measurement of urinary excretion of albumin, aquaporin-2, and epithelial sodium channel, Epworth Sleepiness Scale (ESS), and SF-36 (quality of life). Results: At follow-up, the 17 included patients with mean baseline estimated glomerular filtration rate 66 mL/min/1.73 m2 had a significant decrease in systolic office-, 24 h- and daytime-BP (13, 7, and 8 mmHg, respectively, p Conclusion: Short-term CPAP treatment of patients with moderate-to-severe OSA and reduced renal function decreased 24 h- and daytime-BP significantly and reduced urinary albumin excretion. Our results underline the importance of treatment of OSA in hypertensive patients with impaired renal function.
