Identification of clinical subphenotypes of sepsis after laparoscopic surgery

Objective:Some patients exhibit septic symptoms following laparoscopic surgery,leading to a poor prognosis.Effective clinical subphenotyping is critical for guiding tailored therapeutic strategies in these cases.By identifying predisposing factors for postoperative sepsis,clinicians can implement targeted interventions,potentially improving outcomes.This study outlines a workflow for the subphenotype methodology in the context of laparoscopic surgery,along with its practical application.Methods:This study utilized data routinely available in clinical case systems,enhancing the applicability of our findings.The data included vital signs,such as respiratory rate,and laboratory measures,such as blood sodium levels.The process of categorizing clinical routine data involved technical complexities.A correlation heatmap was used to visually depict the relationships between variables.Ordering points were used to identify the clustering structure and combined with Consensus K clustering methods to determine the optimal categorization.Results:Our study highlighted the intricacies of identifying clinical subphenotypes following laparoscopic surgery,and could thus serve as a valuable resource for clinicians and researchers seeking to explore disease heterogeneity in clinical settings.By simplifying complex methodologies,we aimed to bridge the gap between technical expertise and clinical application,fostering an environment where professional medical knowledge is effectively utilized in subphenotyping research.Conclusion:This tutorial could primarily serve as a guide for beginners.A variety of clustering approaches were explored,and each step in the process contributed to a comprehensive understanding of clinical subphenotypes.

In situ regulating intimately connected heterostructure by decomposition of solid solution oxides toward high-efficient water oxidation

Heterogeneous interfaces produced by interdomain interactions on a nanoscale performs a crucial role in boosting the properties of an electrocatalyst toward oxygen evolution reaction（OER） process.Herein,a series of dual-phase electrodes with intimately connected heterointerfaces are prepared by in situ decomposing solid solution oxide of NixCoyFe100-x-yO,which grew on Ni foam massively via an ultrafast combustion approach.Particularly,with high-reaction kinetics caused by the reduction treatment at 450℃,the less electronegative Fe and Co are more oxyphilic than Ni,which facilitated their co-exsolution and formation of CoFe2O4/NiO oxide with enriched oxygen vacancies.Benefiting from the nanoporous framework,heterojunction structure,and oxygen defects,the self-supporting electrodes present rapid charge/mass transmission and provide abundant active sites for OER.The optimized sample（R-SNCF4.5） shows low overpotentials of 226 and 324 mV at 10 and100 mA·cm-2,a small Tafel slope(46.7 mV·dec-1),and excellent stability.The assembled R-SNCF4.5//Pt/C/NF electrolyzer demonstrates continuous electrolysis over 50 h at a current density of 10 mA·cm-2,under 1.51 V.Density functional theory（DFT） calculations verify that the strong electronic modulation plays a critical part in the CoFe2O4/NiO hybrid by lowering the energy barriers for the ratedetermining steps,and Fe sites are the most active OER sites.

Intermediate phase-assisted growth of CsPbBr3 for high performance of carbon-based perovskite solar cells

CsPbBr3inorganic perovskites have been regarded as the promising materials in the field of photovoltaics because of the high tolerance against environment The high energy barrier of phase transition from lead bromide （PbBr2） to CsPbBr3perovskite and low solubility of perovskite in organic solvent impede the further improvement of device performance in terms of CsPbBr3solar cells.Herein,an intermediate phase-assisted growth of CsPbBr3perovskite was proposed by introducing tetraphenylphosphonium bromide （TPPB） as additive.The TPPB is expected to react with PbBr2in organic solvent to form an intermediate phase of[TPPB·DMF]·PbBr2,which not only effectively improves the crystallinity of PbBr2crystals,but also greatly reduces the phase transition energy barrier,leading to uniform and compact CsPbBr3perovskite films with large grain size and high crystallinity In combination with carbon electrode,the CsPbBr3solar cells yield a champion device performance of 9.57%in comparison of pristine CsPbBr3solar cells showing a low efficiency of 8.17%.Furthermore,the intermediate phaseassisted growth of CsPbBr3-based solar cells displays an outstanding storage over 720 h.

Objective: IMpower210(NCT02813785) explored the efficacy and safety of single-agent atezolizumab vs.docetaxel as second-line treatment for advanced non-small cell lung cancer(NSCLC) in East Asian patients.Methods: Key eligibility criteria for this phase Ⅲ, open-label, randomized study included age ≥18 years;histologically documented advanced NSCLC per the Union for International Cancer Control/American Joint Committee on Cancer staging system(7th edition);Eastern Cooperative Oncology Group performance status of 0 or 1;and disease progression following platinum-based chemotherapy for advanced or metastatic NSCLC. Patients were randomized 2:1 to receive either atezolizumab(1,200 mg) or docetaxel(75 mg/m^(2)). The primary study endpoint was overall survival(OS) in the intention-to-treat(ITT) population with wild-type epidermal growth factor receptor expression(ITT EGFR-WT) and in the overall ITT population.Results: Median OS in the ITT EGFR-WT population(n=467) was 12.3 [95% confidence interval(95% CI),10.3-13.8] months in the atezolizumab arm(n=312) and 9.9(95% CI, 7.8-13.9) months in the docetaxel arm[n=155;stratified hazard ratio(HR), 0.82;95% CI, 0.66-1.03]. Median OS in the overall ITT population was 12.5(95% CI, 10.8-13.8) months with atezolizumab treatment and 11.1(95% CI, 8.4-14.2) months(n=377) with docetaxel treatment(n=188;stratified HR, 0.87;95% CI, 0.71-1.08). Grade 3/4 treatment-related adverse events(TRAEs) occurred in 18.4% of patients in the atezolizumab arm and 50.0% of patients in the docetaxel arm.Conclusions: IMpower210 did not meet its primary efficacy endpoint of OS in the ITT EGFR-WT or overall ITT populations. Atezolizumab was comparatively more tolerable than docetaxel, with a lower incidence of grade3/4 TRAEs.

Objective:In B-cell acute lymphoblastic leukemia(B-ALL),current intensive chemotherapies for adult patients fail to achieve durable responses in more than 50%of cases,underscoring the urgent need for new therapeutic regimens for this patient population.The present study aimed to determine whether HZX-02-059,a novel dual-target inhibitor targeting both phosphatidylinositol-3-phosphate 5-kinase(PIKfyve)and tubulin,is lethal to B-ALL cells and is a potential therapeutic for B-ALL patients.Methods:Cell proliferation,vacuolization,apoptosis,cell cycle,and in-vivo tumor growth were evaluated.In addition,Genome-wide RNA-sequencing studies were conducted to elucidate the mechanisms of action underlying the anti-leukemia activity of HZX-02-059 in B-ALL.Results:HZX-02-059 was found to inhibit cell proliferation,induce vacuolization,promote apoptosis,block the cell cycle,and reduce in-vivo tumor growth.Downregulation of the p53 pathway and suppression of the phosphoinositide 3-kinase(PI3K)/AKT pathway and the downstream transcription factors c-Myc and NF-κB were responsible for these observations.Conclusion:Overall,these findings suggest that HZX-02-059 is a promising agent for the treatment of B-ALL patients resistant to conventional therapies.

An improved initial rotor position estimation method using high-frequency pulsating voltage injection for PMSM

High frequency pulsating voltage injection method is a good candidate for detecting the initial rotor position of permanent magnet synchronous motor.However,traditional methods require a large number of filters,which leads to the deterioration of system stability and dynamic performance.In order to solve these problems,a new signal demodulation method is proposed in this paper.The proposed new method can directly obtain the amplitude of high-frequency current,thus eliminating the use of filters,improving system stability and dynamic performance and saving the work of adjusting filter parameters.In addition,a new magnetic polarity detection method is proposed,which is robust to current measurement noise.Finally,experiments verify the effectiveness of the method.

Do nocturnal birds use acoustic and visual cues to avoid predation by bats?

Anti-predation strategies are critical to animal survival and are fundamental to deciphering predator–prey interactions.As an important defense strategy,sensory predator detection(such as through acoustic and visual cues)enables animals to assess predation risk and execute predator-avoidance behavior;however,there are limited studies on the anti-predation behavior of nocturnal animals.The prey of bats provides an excellent representative system for examining the anti-predation behavior of nocturnal animals.Here,we broadcasted different types of echolocation calls of the bird-eating bat Ia io to two wild passerine birds,namely,Zosterops japonicus and Sinosuthora webbiana,that are preyed upon by I.io,and presented the birds with individual bats under different light intensities.The results showed that both bird species were able to perceive the low-frequency audible portion of the bats’echolocation calls;however,they did not exhibit escape responses to the acoustic stimuli.In the dark and under moonlit conditions,both bird species were unable to respond to active bats at close range and the birds only exhibited evasive flight behavior when bats approached or touched them.These results suggest that nocturnal passerine birds may not be able to use acoustic or visual cues to detect bats and adopt evasive maneuvers to avoid predation.This work suggests that bat predation pressure may not elicit primary predator-avoidance responses in nocturnal passerine birds.The results provide new insights into the anti-predation behavior of nocturnal animals.

TRIANGLE operation,combined with adequate adjuvant chemotherapy,can improve the prognosis of pancreatic head cancer:A retrospective study

BACKGROUND The TRIANGLE operation involves the removal of all tissues within the triangle bounded by the portal vein-superior mesenteric vein,celiac axis-common hepatic artery,and superior mesenteric artery to improve patient prognosis.Although previously promising in patients with locally advanced pancreatic ductal adenocarcinoma(PDAC),data are limited regarding the long-term oncological outcomes of the TRIANGLE operation among resectable PDAC patients undergoing pancreaticoduodenectomy(PD).AIM To evaluate the safety of the TRIANGLE operation during PD and the prognosis in patients with resectable PDAC.METHODS This retrospective cohort study included patients who underwent PD for pancreatic head cancer between January 2017 and April 2023,with or without the TRIANGLE operation.Patients were divided into the PD_(TRIANGLE)and PD_(non-TRIANGLE)groups.Surgical and survival outcomes were compared between the two groups.Adequate adjuvant chemotherapy was defined as adjuvant chemotherapy≥6 months.RESULTS The PD_(TRIANGLE)and PD_(non-TRIANGLE) groups included 52 and 55 patients,respectively.There were no significant differences in the baseline characteristics or perioperative indexes between the two groups.Furthermore,the recurrence rate was lower in the PD_(TRIANGLE) group than in the PD_(non-TRIANGLE) group(48.1%vs 81.8%,P<0.001),and the local recurrence rate of PDAC decreased from 37.8%to 16.0%.Multivariate Cox regression analysis revealed that PD_(TRIANGLE)(HR=0.424;95%CI:0.256-0.702;P=0.001),adequate adjuvant chemotherapy≥6 months(HR=0.370;95%CI:0.222-0.618;P<0.001)and margin status(HR=2.255;95%CI:1.252-4.064;P=0.007)were found to be independent factors for the recurrence rate.CONCLUSION The TRIANGLE operation is safe for PDAC patients undergoing PD.Moreover,it reduces the local recurrence rate of PDAC and may improve survival in patients who receive adequate adjuvant chemotherapy.

In vivo pilot study into superficial microcirculatory characteristics of colorectal adenomas using novel high-resolution magnifying endoscopy with blue laser imaging

BACKGROUND No studies have yet been conducted on changes in microcirculatory hemody-namics of colorectal adenomas in vivo under endoscopy.The microcirculation of the colorectal adenoma could be observed in vivo by a novel high-resolution magnification endoscopy with blue laser imaging(BLI),thus providing a new insight into the microcirculation of early colon tumors.AIM To observe the superficial microcirculation of colorectal adenomas using the novel magnifying colonoscope with BLI and quantitatively analyzed the changes in hemodynamic parameters.METHODS From October 2019 to January 2020,11 patients were screened for colon adenomas with the novel high-resolution magnification endoscope with BLI.Video images were recorded and processed with Adobe Premiere,Adobe Photoshop and Image-pro Plus software.Four microcirculation parameters:Microcirculation vessel density(MVD),mean vessel width(MVW)with width standard deviation(WSD),and blood flow velocity(BFV),were calculated for adenomas and the surrounding normal mucosa.RESULTS A total of 16 adenomas were identified.Compared with the normal surrounding mucosa,the superficial vessel density in the adenomas was decreased(MVD:0.95±0.18 vs 1.17±0.28μm/μm2,Pboth increased.BFV slowed in the adenomas(709.74±213.28 vs 1256.51±383.31μm/s,P<0.05).CONCLUSION The novel high-resolution magnification endoscope with BLI can be used for in vivo study of adenoma superficial microcirculation.Superficial vessel density was decreased,more irregular,with slower blood flow.

Multi-resonance thermally activated delayed fluorescence(MR-TADF)emitters can enable narrowband emission with high color purity and electroluminescence efficiency.Nitrogen/carbonyl(N/C=O)system is receiving increasing attention while the nitrogen/boron(N/B)system has been widely studied.Donor decoration is an effective approach for N/B type MR-TADF system but always leads to broadening and red-shifting of the emission band in N/C=O MR-TADF system.We attribute these unfavorable phenomena to the formation of intramolecular charge transfer between the MR-core and peripheral donors.To address this issue,we have developed a new strategy by decorating DMQAO(a fused N/C=O MR-core)with a triazine acceptor and a neutral terphenyl group to construct MTDMQAO and MBDMQAO,respectively.The introduction of the triazine acceptor not only realizes efficient narrowband emission in MTDMQAO,but also accelerates the reverse intersystem crossing process through enhanced spin-orbital coupling.As a result,MTDMQAO exhibits a significantly higher external quantum efficiency of 29.4%compared to the referent emitters,validating the rationality of our derivation strategy.This study highlights the potential of the N/C=O system for MR-TADF emitters and provides important insights for understanding the difference between N/B and N/C=O systems.
