Effect of smoking on the risk of gastrointestinal cancer after cholecystectomy: A national population-based cohort study

BACKGROUND The role of smoking in the incidence of colorectal cancer(CRC)or gastric cancer(GC)in populations undergoing cholecystectomy has not been *** To evaluate the effect of smoking on CRC or GC development in cholecystectomy *** A total of 174874 patients who underwent cholecystectomy between January 1,2010 and December 31,2017 were identified using the Korean National Health Insurance Service claims *** patients were matched 1:1 with mem-bers of a healthy population according to age and *** or GC risk after cholecystectomy and the association between smoking and CRC or GC risk in cholecystectomy patients were evaluated using adjusted hazard ratios(HRs)and 95%*** The risks of CRC(adjusted HR:1.15;95%CI:1.06-1.25;P=0.0013)and GC(adjusted HR:1.11;95%CI:1.01-1.22;P=0.0027)were significantly higher in cholecystectomy *** the population who underwent cholecystectomy,both CRC and GC risk were higher in those who had smoked compared to those who had never *** both cancers,the risk tended to increase in the order of non-smokers,ex-smokers,and current *** addition,a positive correlation was observed between the amount of smoking and the risks of both CRC and *** Careful follow-up and screening should be performed,focusing on the increased risk of gastrointestinal cancer in the cholecystectomy group,particularly considering the individual smoking habits.

Random pinhole attenuator for high-power laser beams

The intensity attenuation of a high-power laser is a frequent task in the measurements of optical science. Laser intensity can be attenuated by inserting an optical element, such as a partial reflector, polarizer or absorption filter. These devices are, however, not always easily applicable, especially in the case of ultra-high-power lasers, because they can alter the characteristics of a laser beam or become easily damaged. In this study, we demonstrated that the intensity of a laser beam could be effectively attenuated using a random pinhole attenuator(RPA), a device with randomly distributed pinholes, without changing the beam properties. With this device, a multi-PW laser beam was successfully attenuated and the focused beam profile was measured without any alterations of its characteristics. In addition, it was confirmed that the temporal profile of a laser pulse, including the spectral phase, was preserved. Consequently, the RPA possesses significant potential for a wide range of applications.

The application of Li metal anodes in rechargeable batteries is impeded by safety issues arising from the severe volume changes and formation of dendritic Li ***‐dimensional hollow carbon is receiving increasing attention as a host material capable of accommodating Li metal inside its cavity;however,uncontrollable and nonuniform deposition of Li remains a *** this study,we synthesize metal–organic framework‐derived carbon microcapsules with heteroatom clusters(Zn and Ag)on the capsule walls and it is demonstrated that Ag‐assisted nucleation of Li metal alters the outward‐to‐inward growth in the microcapsule ***‐incorporated microcapsules are prepared via chemical etching of zeolitic imidazole framework‐8 polyhedra and are subsequently decorated with Ag by a galvanic displacement reaction between Ag^(+)and metallic *** introduced Ag significantly reduces the energy barrier and increases the reaction rate for Li nucleation in the microcapsule host upon Li *** combined electrochemical,microstructural,and computational studies,we verify the beneficial role of Ag‐assisted Li nucleation in facilitating inward growth inside the cavity of the microcapsule host and,in turn,enhancing electrochemical *** study provides new insights into the design of reversible host materials for practical Li metal batteries.

Introduction Antibiotic use for acute otitis media(AOM)is one of the major sources of antimicrobial ***,the effective minimal antibiotic duration for AOM remains ***,guidelines often recommend broad ranges(5-10 days)of antibiotic use,yet the clinical impact of such a wide window has not been *** We systematically searched PubMed/MEDLINE,Embase,Scopus,Web of Science,and Cochrane Library from database inception to 6 October *** meta-analysis was conducted on randomized controlled trials that assessed antibiotic treatment for AOM in children(PROSPERO CRD42020196107).Results For amoxicillin and amoxicillin-clavulanate,7-day regimens were noninferior to 10-day regimens in clinical responses[amoxicillin:risk ratio(RR)0.919(95%CI 0.820-1.031),amoxicillin-clavulanate:RR 1.108(0.957-1.282)],except for≤2 *** the third-generation cephalosporins,7-day and 10-day regimens had similar clinical responses compared to placebo[7-day:RR 1.420(1.190-1.694),10-day:RR 1.238(1.125-1.362)compared to placebo].However,5-day regimens of amoxicillin-clavulanate and third-generation cephalosporins were inferior to 10-day *** to amoxicillin,a shorter treatment duration was tolerable with *** Our findings indicated that 1O days of antibiotic use may be unnecessarily long,while the treatment duration should be longer than 5 ***,5-day regimens would be sufficient for a modest treatment *** findings revealed that the current wide range of recommended antibiotic durations may have influenced the clinical outcome of AOM,and a narrower antibiotic duration window should be re-established.

Two-dimensional materials represent a major frontier for research into exotic many-body quantum phenomena. In the extreme two-dimensional limit, electron-electron interaction often dominates over other electronic energy scales, leading to strongly correlated effects such as quantum spin liquid and unconventional *** dominance is conventionally attributed to the lack of electron screening in the third dimension. Here, we discover an intriguing metal to Mott insulator transition in 1T-TaSe2that defies conventional wisdom. Specifically, we find that dimensionality crossover, instead of reduced screening, drives the transition in atomically thin 1T-TaSe2. A dispersive band crossing the Fermi level is found to be responsible for the bulk metallicity in the material. Reducing the dimensionality,however, effectively quenches the kinetic energy of these initially itinerant electrons, and drives the material into a Mott insulating *** dimensionality-driven metal to Mott insulator transition resolves the long-standing dichotomy between metallic bulk and insulating surface of 1T-TaSe2. Our work further reveals a new pathway for modulating two-dimensional materials that enables exploring strongly correlated systems across uncharted parameter space.

目的研究汉语非流畅性失语症患者特指疑问句理解与表达的句法障碍特征。方法以句-图匹配、诱导-复述的形式对25例汉语非流畅性失语症患者和25例正常人进行理解和表达能力测试,比较两组结果差异,分析句式、提问对象和疑问词相关句法障碍特点。结果非流畅性失语症患者特指疑问句理解和表达能力均受损,各种类型特指疑问句的正确率均显著低于对照组(P主语伪分裂疑问句和被动疑问句;各种类型的特指疑问句产出正确率均低于对照组,“谁”主动疑问句、“什么”宾语主动疑问句的正确率无显著性差异(P>0.05),其他类型均存在显著性差异(P伪分裂疑问句和被动疑问句。结论汉语非流畅性失语症患者特指疑问句理解和表达受损模式基本相似。句法结构方面主要影响因素包括提问对象(主宾)和句式类型,有wh-成分移位、非规范顺序的句子障碍更明显。

目的通过对4~6岁电声双模式助听儿童的高兴、中性及难过3种情感韵律进行研究,探讨不同听觉年龄电声双模式助听儿童的韵律识别能力。方法选取2022年9月至2023年12月某幼儿园就读的生理年龄4~6岁,听觉年龄1~4岁的电声双模式助听儿童32例,按照听觉年龄分为1.5岁组4例、2岁组6例、2.5岁组6例、3岁组6例、3.5岁组4例及4岁组6例。采用华东师范大学康复科学系自制的15个日常用句,分别用高兴、中性和难过3种不同的情感语气读出,让电声双模式助听儿童识别,共45句。分析不同听觉年龄对高兴、难过和中性3种情感韵律的识别能力。结果 1.5岁组、2岁组、2.5岁组、3岁组、3.5岁组及4岁组的情感韵律识别能力得分比较,差异无统计学意义(F=1.246,P>0.05)。6组3种不同类型情感韵律识别能力比较,差异无统计学意义(F=1.174,P>0.05),情感韵律类型和听觉年龄组之间无交互作用,差异无统计学意义(F=0.980,P>0.05)。6组不同听觉年龄情感韵律识别的正确率比较,差异有统计学意义(F=4.106,P<0.01)。结论听觉年龄1~4岁的大龄电声双模式助听儿童,情感韵律识别能力随听觉年龄的增长而升高;但不同听觉年龄对高兴、难过和中性3种情感韵律的识别不存在差异。随着听觉年龄的增长,要重视大龄电声双模式助听儿童情感韵律识别能力的训练。

New anti-mesenteric delta-shaped stapled anastomosis: Technical report with short-term postoperative outcomes in patients with Crohn’s disease

BACKGROUND Medical treatment for Crohn’s disease(CD)has continuously improved,which has led to a decrease in surgical recurrence *** these advancements,25%of patients will undergo repeat intestinal *** of CD com-monly occurs on the mesentery side of the anastomosis *** To compare the new anti-mesenteric side-to-side delta-shaped stapled anasto-mosis(DSA)with the conventional stapled functional end-to-end anastomosis(CSA).METHODS This retrospective study included CD patients who underwent ileo-ileal or ileo-colic anastomosis between January 2020 and December *** DSA technique employed a stapler to maintain the concept of anti-mesentery side-to-side ana-stomosis by performing a 90°vertical closure of the open window compared with the CSA *** the corner where the open window is closed,the DSA avoids forming a pouch and creates an anastomosis resembling a delta shape within the intestinal *** compared demographics,preoperative condition,operative findings,and operative outcomes for the two *** The study included 175 patients,including 92 in the DSA group and 83 in the CSA *** two groups were similar in baseline characteristics,preoperative medical treatment,and operative findings except for the Montreal classification *** 30-days postoperative complication rate was signi-ficantly lower in the DSA group compared with the CSA group(16.3%vs 32.5%,P=0.009).Ileus incidence was significantly lower in the DSA group than in the CSA group(4.3%vs 14.5%,P=0.033),and the hospital stay was shorter in the DSA group than in the CSA group(5.67±1.53 days vs 7.39±3.68 days,P=0.001).CONCLUSION The DSA technique was feasible and showed comparable postoperative outcomes with lower short-term complic-ations compared with the CSA *** studies on CD recurrence and long-term complications are warranted.

BACKGROUND Double-chambered left ventricle(DCLV)is an extremely rare congenital disease in which the left ventricle(LV)is divided by abnormal muscle *** to its rarity,there is a lack of data on the disease,including its diagnosis,treatment,and ***,we report a case in which DCLV was diagnosed and followed *** SUMMARY A 45-year-old man presented to our hospital due to abnormal findings on an electrocardiogram recorded during a health *** had no specific cardiac symptoms,comorbidities or relevant past medical *** revealed that the LV was divided into two by muscle *** were no findings of ischemia on coronary angiography and coronary computed tomography angiography performed to exclude differential *** comprehensive analysis of the images,DCLV was *** it seemed to be asymptomatic DCLV,we decided the patient was to be observed without administering any ***,follow-up echocardiography revealed a thrombus in the accessory chamber(AC).Anticoagulant medication was initiated,the thrombus resolved,and the patient is currently undergoing follow-up without any specific *** Asymptomatic,uncomplicated DCLV was diagnosed through multimodal imaging;however,a thrombus in the AC occurred during the *** findings highlight that multimodal imaging is essential in diagnosing DCLV,and that anticoagulation is important in its management.
