BACKGROUND The technique of percutaneous endoscopic lumbar discectomy(PELD)as a transforaminal approach has been used to treat highly migrated lower lumbar disc ***,due to the different anatomic characteristics of the upper lumbar spine,conventional transforaminal PELD may fail to remove the highly migrated upper lumbar disc nucleus ***,the purpose of this study was to describe a novel surgical technique,two-level PELD,for the treatment of highly migrated upper lumbar disc herniations and to report its related clinical *** SUMMARY A 60-year-old male presented with a complaint of pain at his lower back and right lower *** patient received 3 mo of conservative treatments but the symptoms were not *** examination revealed a positive femoral nerve stretch test and a negative straight leg raise test for the right leg,and preoperative visual analog scale(VAS)score for the lower back was 6 points and for the right leg was 8 *** resonance imaging(MRI)demonstrated L2-L3 disc herniation on the right side and the herniated nucleus pulposus migrated to the upper margin of L2 vertebral *** to physical examination and imaging findings,surgery was the primary ***,the patient underwent surgical treatment with two-level *** pain symptom was relieved and the VAS score for back and thigh pain was one point *** patient was asymptomatic and follow-up MRI scan 1 year after operation revealed no residual nucleus *** Two-level PELD as a transforaminal approach can be a safe and effective procedure for highly migrated upper lumbar disc herniation.

A novel type of ZnO-Al2O3-B2O3-SiO2 glass-ceramics sealing to Kovar in electronic packaging was developed, whose thermal expansion coefficient and electrical resistance are 5.2× 10^-6/℃ and over 1×10^13 Ω·cm, respectively. The major crystalline phases in the glass-ceramic seals were ZnAl2O4, ZnB2O4, and NaSiAl2O4. The dielectric resistance of the glass-ceramic could be remarkably enhanced through the control of alkali metal ions into crystal lattices. It was found that crystallization happened first on the surface of the sample, leaving the amorphous phase in the inner, which made the glass suitable for sealing. The glass-ceramic showed better wetting on the Kovar surface, and sealing atmosphere and temperature had great effect on the wetting angle. Strong interracial bonding was obtained, which was mainly attributed to the interracial reaction between SiO2 and FeO or Fe3O4.

目的分析人工智能(artificial intelligence,AI)应用于老年人跌倒的研究热点和趋势,以期为我国老年跌倒研究提供思路。方法在Web of Science与中国知网中检索AI应用于老年人跌倒的相关文献,使用Excel分析发文趋势、CiteSpace对关键词进行可视化分析。结果最终获得515篇英文文献、96篇中文文献。年发文量总体呈上升趋势。研究热点显示AI在老年人跌倒预测、检测及预防方面备受关注。结论近年来AI应用于老年人跌倒的研究热度持续增加,我国在该领域的发展滞后于国外。未来可在把握国际热点基础上,深化护理与AI领域合作,探索适宜我国国情的老年人精准跌倒预防与管理策略。

Objective To analyze the effect of age on the ECG QT interval, an important predictor of cardiovascular mortality and drug-induced cardiac arrhythmias, and determine whether QT-heart rate correction formulae （QTc） have differential relationships with age and sex. Methods Data were examined from the US National Health and Nutrition Examination Survey （NHANES） II and III, civilian population aged 25 to 90 years. QT weighted means and standard deviations were calculated for all ages. The QTc were evaluated for six QTc： proposed by Bazett （QTcBZT）, Fridericia （QTcFRD）, Hodges （QTcHDG）, Dmitrienko （QTcDMT）, Rautaharju （QTcRTHa） and Framingham （QTcFRM）. Results QTc was strongly related to age and gender, for all formulae except for QTcBZT for women. The relationship between QTc and age was significant regardless of whether the relationship was approximated by a linear or non-linear （quadratic or cubic spline） model. QTc increased more dramatically with age in men. There was a significant （P 〈 0.001） positive relationship between QTc variance and age for each QTc formula for both men and women. There were a greater proportion of individuals with longer QTc with older ages especially age 80 years and above. Conclusion QTc and its variance increase with age. Prolonged QTc is more prevalent in older individuals, especially men.

The effect of Mg and Si additon to Al matrix on infiltration kinetics and rates of Al alloys pressureless infiltration into porous SiCp preform was investigated by observing the change of infiltration distance with time as the Al alloys infiltrate into SiCp preforms at different *** results show that infiltration of SiCp preforms by Al melt is a thermal activation process and there is an incubation period before the infiltration becomes *** the increase of Mg content in the Al alloys from 0wt% to 8wt%,the infiltration will become much easier,the incubation period becomes shorter and the infiltration rate is faster,but these effects are not obvious when the Mg content is higher than 8wt%.As for Si addition to the Al alloys,it has no obvious effect on the incubation period,but the infiltration rate increases markedly with the increase of Si content from 0wt% to 12wt% and the rate has no obvious change when the content is bigger than 12wt%.The effect of Mg and Si on the incubation period is related to the infiltration mechanism of Al pressureless infiltration into SiCp preforms and their impact on the infiltration rate is a combined result from viscosity and surface tension of Al melt and SiC-Al wetting ability.

The angular distribution of gamma-ray burst（GRB）jets is not yet *** observed luminosity of GRB 170817A is the lowest among all known short GRBs,which is best explained by the fact that our line of sight is outside of the jet opening angle,θ_（obs）〉θ_j,whereθ_（obs） is the angle between our line of sight and the jet *** inferred by gravitational wave observations,as well as radio and X-ray afterglow modeling of GRB 170817A,it is likely that θ_（obs）~20°–28°.In this work,we quantitatively consider two scenarios of angular energy distribution of GRB ejecta：a top-hat jet and a structured jet with a power law index *** the top-hat jet model,we get a large θ_j（e.g.,θ_j〉10°）,a rather high local （i.e., z 〈 0.01） short GRB rate ~8–15×10~3 Gpc^（-3）yr~（-1（（estimated to be 90~1850 Gpc^（-3）yr^（-1） in Fong et al.）and an extremely high（on-axis,V（~500 ke V for a typical short GRB）.For the structured jet model,we use θ_（obs） to give limits on s and θ_j for typical on-axis luminosity of a short GRB（e.g.,10^（49）erg s（-1） 1051erg s（-1））,and a low on-axis luminosity case（e.g.,1049erg s（-1））gives more reasonable values of *** structured jet model is more feasible for GRB170817A than the top-hat jet model due to the rather high local short GRB rate,and the extremely high on-axis E_（peak,0） almost rules out the top-hat jet *** 170817A is likely a low on-axis luminosity GRB（1049erg s（-1））with a structured jet.
