Transcatheter embolization therapy in liver cancer:an update of clinical evidences

Transarterial chemoembolization （TAcE） is a form of intra-arterial catheter-based chemotherapy that selectively delivers high doses of cytotoxic drug to the tumor bed combining with the effect of ischemic necrosis induced by arterial embolization. chemoembolization and radioembolization are at the core of the treatment of liver hepatocellular carcinoma （Hcc） patients who cannot receive potentially curative therapies such as transplantation, resection or percutaneous ablation. TAcE for liver cancer has been proven to be useful in local tumor control, to prevent tumor progression, prolong patients＇ life and control patient symptoms. Recent evidence showed in patients with single-nodule Hcc of 3 cm or smaller without vascular invasion, the 5-year overall survival （OS） with TAcE was similar to that with hepatic resection and radiofrequency ablation. Mthough being used for decades, Lipiodol~ （Lipiodol~ Ultra Fluid~, Guerbet, France） remains important as a tumor-seeking and radio-opaque clrug delivery vector in intervendonal oncology. There have been efforts to improve the delivery of chemotherapeutic agents to tumors. Drug-eluting bead （deB） is a relatively novel drug delivery embolization system which allows for fixed dosing and the ability to release the anticancer agents in a sustained manner. Three deBs are available, i.e., Tandem~ （celoNova Biosciences Inc., USA）, Dc-Beads~ （BTG, UK） and HepaSphere~ （BioSphere Medical, Inc., USA）. Transarterial radioembolization （TARE） technique has been developed, and proven to be efficient and safe in advanced liver cancers and those with vascular complications. Two types of radioembolization microspheres are available i.e., SIR-Spheres~ （Sirtex Medical Limited, Australia） and TheraSphere~ （BTG, UK）. Evaluating and quantifying the effect of various spruce budworm intervention strategies on forest carbon dynamics in Atlantic canada

Spruce budworm (SBW) outbreaks are one of the most devastating natural disturbances in spruce-balsam fir forests of eastern North America. Both early intervention strategy (EIS) and foliage protection strategy (FP) are being tested to limit forest losses, but the quantitative impact on forest carbon (c) dynamics is still unclear. In this study, we designed 19 separate scenarios of no intervention or varying success of EIS, FP, and their combination on SBW caused defoliation and mortality. We then used the TRIPLEX-Insect model to quantify their effects on forest c dynamics in the forests of the four provinces of Atlantic canada. A scenario applying FP to 10%of the area with the greatest potential c losses of living biomass, protecting foliage in 10%of the forests is more realistic than higher proportion of FP given the high cost and large areas involved, resulted in reducing average cumulative net ecosystem productivity (NEP) from 2020 to 2039 by 56%–127%compared to a no outbreak *** results showed that FP would have to be applied everywhere to reduce tree mortality and increase NEP more than 8 years of successful EIS applied. However, if EIS can be successfully implemented for 12 years, it will maintain more forest c than FP applied everywhere during a moderate outbreak. We also found that the combination of EIS followed by FP in 10%of the areas disturbed by the SBW could maintain average cumulative NEP at similar levels to no defoliation in every province of Atlantic canada. Black/red spruce forests younger than 60years old underwent the smallest changes in c dynamics whether using EIS, FP, or both. This highlights the importance of forest species, forest age, and their interactions on the effectiveness of a treatment during SBW outbreak. Overall, 31%–76%of the study area in Atlantic canada could convert from a c sink to a source by 2039,if no protective measures are used under the worst-case scenarios, thus contributing to future climate warming.

The renewed interest in donation after cardio-circulatory death (DcD) started in the 1990s following the limited success of the transplant community to expand the donation after brain-death (DBD) organ supply and following the request of potential DcD families. Since then, DcD organ procurement and transplantation activities have rapidly expanded, particularly for nonvital organs, like kidneys. In liver transplantation (LT), DcD donors are a valuable organ source that helps to decrease the mortality rate on the waiting lists and to increase the availability of organs for transplantation despite a higher risk of early graft dysfunction, more frequent vascular and ischemia-type biliary lesions, higher rates of re-listing and re-transplantation and lower graft survival, which are obviously due to theinevitable warm ischemia occurring during the declaration of death and organ retrieval process. Experimental strategies intervening in both donors and recipients at different phases of the transplantation process have focused on the attenuation of ischemia-reperfusion injury and already gained encouraging results, and some of them have found their way from pre-clinical success into clinical reality. The future of DcD-LT is promising. concerted efforts should concentrate on the identification of suitable donors (probably Maastricht category Ⅲ DcD donors), better donor and recipient matching (high risk donors to low risk recipients), use of advanced organ preservation techniques (oxygenated hypothermic machine perfusion, normothermic machine perfusion, venous systemic oxygen persufflation), and pharmacological modulation (probably a multi-factorial biologic modulation strategy) so that DcD liver allografts could be safely utilized and attain equivalent results as DBD-LT.

In côte d’Ivoire, the decline in soil fertility strongly impacts the productivity of maize (Zea mays L.) on heavily leached ferralitic soil. In this study, the general objective was therefore to improve the productivity of maize EV87-28 on the Ferralsols in pre-forested areas during different cropping seasons. Eight (8) micro-plots were set up according to a total randomization device with three repetitions. Two factors were studied: nitrogen fertilizer modalities (main factor) and crop season (secondary factor). Growth, flowering and yield parameters were measured and analyzed. The results showed that there was no interaction between the nitrogen fertilizer factor and the cropping season factor. In addition, this study showed the short rainy season had the most positive impact on growth, flowering and yield parameters than the long rainy season. The results also showed that the different nitrogen fertilizer modalities had no statistically different effects on growth, flowering and yield parameters. However, quantitative differences were reported, highlighting one nitrogen fertilizer modality, which is the combination of urea granule + farm manure (75% urea indorama granules and 25% farm manure). The combination of urea granule + farm manure (75% urea indorama granules and 25% farm manure) had the best effect on corn grain yield. So, the combination of urea (75%) and manure (25%), that resulted in yield gain, could be recommended for corn fertilization during the small rainy season.

Effects of calcium and Magnesium on Phosphorus Availability in Ferralsols and Rice Production in Forest Zones of côte d’Ivoire

Phosphorus bioavailability has long been a recurring problem in tropical acid soils. A pot experiment was carried out during three (3) successive rice production cycles at Adiopodoumé to evaluate the response of the NERIcA 5 rice accession to various doses of calcium, magnesium and phosphorous. The experiment was conducted using a randomized split-plot design. The interactive effects of calcium carbonate (0, 25, 50 and 75 kg·ca·ha-1) and magnesium sulfate (0, 25, 50 and 75 kg·Mg·ha-1) and Togo natural phosphate (0, 25, 50 and 75 kg·P·ha-1) were determined at each production cycle. The results showed that single-dose natural phosphate supplementation for three cropping cycles resulted in an average enrichment of around 2 mg·P·kg-1 after each trial following its continuous dissolution, with an increase in DSP (33.31% to 70.52%). The study revealed one strategy for managing and enhancing native P with cations and another for exogenous P: there would be a synergy of ca/Mg on native P, whereas an antagonism would characterize the two parameters in phosphate fertilization.

Introduction: Anemia in people living with human immunodeficiency virus (PLHIV) is a major health problem. Although anemia often responds to combination antiretroviral therapy, many patients remain anemic despite treatment, and such persistent anemia continues to adversely affect prognosis, regardless of drug response. Scientists have identified some of the factors involved. However, the mechanisms put in place have not been effective in overcoming them. Examples include the withdrawal of zidovudine from antiretroviral treatment lines, iron and folate supplementation, etc. Anemia is still a major concern in HIV-positive patients. The aim of this study is to assess the prevalence of anemia and its associated factors among PLHIV followed up at the outpatient treatment centre (cTA) of the Panzi General Reference Hospital (HGR) in South Kivu, democratic Republic of congo (DRc). Method: We conducted a cross-sectional, comparative study of 276 HIV-infected adults on antiretroviral therapy (ART) followed up at the cTA of Panzi HGR. Socio-demographic and nutritional parameters were collected using a survey questionnaire, and clinical assessment and nutritional status were performed at the centre. Hemoglobin, seric albumin and viral load determinations were performed at the HGRP laboratory. We constructed univariate and multivariate logistic regression models to assess factors associated with anemia in people living with HIV/AIDS. Results: We found a prevalence of anemia of 39.4%, including 4.1% severe anemia, 17.7% moderate anemia and 17.5% mild anemia. After multivariate adjustment, the factors associated with anemia in our PLHIV were: moderate undernutrition (aOR = 1.26;95% cI: 1.50 - 4.20;p = 0.001), severe undernutrition (aOR = 115.4;95% cI: 2.04 - 164.52;p = 0.021), hypoalbuminemia (aOR = 2.11;95% cI: 1.87 - 5.10;p = 0.004) and the lower degree of dietary diversity (aOR = 1.56;95% cI: 1.10 - 4.32;p = 0.034). conclusion: The prevalence of anemia in PLHIV on ART is high.

Introduction: In côte d’Ivoire, there is a scarcity of data on children’s purulent pleurisies. Objective: This study aims to elucidate the epidemiological, diagnostic, therapeutic, and evolutionary facets of non-tuberculous purulent pleurisies in pediatric patients. Methods: A retrospective analysis was conducted using the medical records of children aged one month to fifteen years with purulent pleurisies at Bouaké University Hospital center from January 2017 to december 2021. Results: The study identified 124 cases of purulent pleurisies, constituting 18% of lower respiratory tract infections and 0.8% of all hospitalizations. The majority of these cases (69%) were in children between 1 and 24 months of age. Prominent symptoms included dyspnea (85.5%), O2 saturation below 95% in room air (76.6%), respiratory distress (68.5%), cutaneous-mucosal pallor (63.7%), and fever (43.5%). Radiological findings predominantly showed right-sided pleurisy (62.1%). The pleurisy was often extensive (78.2%), accompanied by pneumothorax (37.1%), alveolo-interstitial opacities (8.1% of cases), and abscess formations (1.6%). Pleural fluid cultures were positive in 46.9% of cases, with Staphylococcus aureus (75%, methi-S) identified among 32 bacteria. Initial antibiotic treatment was empirical, favoring oxacillin (53.2%) or amoxicillin-clavulanic acid (53.2%) in dual (42%) or triple therapy (33%) with gentamicin (64.1%) and/or metronidazole (21.8%). Treatments also included pleural drainage (68.5%) or repeated evacuation punctures (33.1%), and blood transfusion (39%). The mortality rate was 18.8%. conclusion: Non-tuberculous purulent pleurisy remains a significant concern in pediatric hospitalizations at the cHU of Bouaké, marked by high mortality rates.

The increasing pace of urbanization means that cities and global organizations are looking for ways to increase energy efficiency and reduce emissions. combined cooling, heating, and power （ccHP） systems have the potential to improve the energy generation efficiency of a city or urban region by providing energy for heating, cooling, and electricity simultaneously. The purpose of this study is to estimate the water consumption for energy generation use, carbon dioxide （cO2） and NOx emissions, and economic impact of implementing ccHP systems for five generic building types within the Atlanta metropolitan region, under various operational scenarios following the building thermal （heating and cooling） demands. Operating the ccHP system to follow the hourly thermal demand reduces cO2 emissions for most building types both with and without net metering. The system can be economically beneficial for all building types depending on the price of natural gas, the implementation of net metering, and the cost structure assumed for the ccHP system. The greatest reduction in water consumption for energy production and NOx emissions occurs when there is net metering and when the system is operated to meet the maximum yearly thermal demand, although this scenario also results in an increase in greenhouse gas emissions and, in some cases, cost. ccHP systems are more economical for medium office, large office, and multifamilv residential buildings.
