Immediate effect of high-intensity exercise on brain-derived neurotrophic factor in healthy young adults:A systematic review and meta-analysis

Background Although brain-derived neurotrophic factor(BDNF)has been identified as a molecular biomarker of the neurophysiological effects induced by exercise,the acute effects of high-intensity exercise(HIE)on BDNF levels are inconclusive.This study aims to estimate the immediate effects of HIE on BDNF levels in healthy young adults.Methods A systematic search was conducted in the MEDLINE,Scopus,Cochrane CENTRAL,and SPORTDiscuss databases up to December 2020.Randomized controlled trials(RCTs)and non-RCTs reporting pre-post changes in serum or plasma BDNF after an acute intervention of HIE compared to a control condition were included.Pooled effect sizes(p-ESs)and 95%confidence intervals(95%CIs)were calculated for RCTs using a random effects model with Stata/SE(Version 15.0;StataCorp.,College Station,TX,USA).The Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines were followed.PROPERO registration number:CRD42020221047.Results A total of 22 studies with 552 individuals(age range:20-31 years;59.1%male)were included.The meta-analysis included 10 RCTs that reported valid outcome data.Higher BDNF levels were observed when HIE interventions were compared with non-exercise(p-ES=0.55,95%CI:0.12-0.98;I^(2)=25.7%;n=4 studies)and light-intensity exercise(p-ES=0.78,95%CI:0.15-1.40;I^(2)=52.4%;n=3 studies)but not moderate-intensity exercise(p-ES=0.93,95%CI:-0.16 to 2.02;I^(2)=88.5%;n=4 studies)conditions.Conclusion In comparison to non-exercise or light-intensity exercises,an immediate increase in BDNF levels may occur when young adults perform HIE.Given the benefits obtained maximizing circulating BDNF when performing HIE and its potential effects on brain health,our findings suggest that HIE could be recommended by clinicians as a useful exercise strategy to healthy adults.

Background:Volcanic eruptions have large effects on forest ecosystems and create new substrates,triggering primary succession processes.The Paricutín volcano,born in central-western Mexico,erupted between 1943 and 1952.After the cessation of the eruptive activity,plant colonization began on the lava flows and tephra deposits,including the conifer species that dominate the surrounding mature forests.This study aims to reconstruct the history of the establishment of conifer trees on the substrates created by the Paricutín eruption.Methods:16 sampling plots were established along three transects with northern,southern,and south-western aspects,located every 250 m from the preserved forest to the volcanic cone.Increment cores from 400 conifer trees were extracted and their age was determined by cross-dating annual tree rings.The order of the species colonization and the tree establishment,abundance,and dominance patterns were characterized.Also,the influence of the distance from the mature forests and the inter-annual climatic conditions on the temporal tree establishment pattern was evaluated.Results:Eight pine and one fir species have been established since 1970,only 18 years after the end of the eruptive period.However,tree establishment increased by 12.9%annually after 1995,with the youngest tree in our sample getting established in 2015.We did not find a well-defined temporal and spatial pattern of species arrival,which suggests that colonization occurred randomly,although the four pine species that were established early became the most abundant and dominant.Tree establishment was not influenced by the distance from the mature forest,and wet inter-annual conditions did not enhance pulses of tree recruitment,exhibiting a continuous tree establishment pattern.Conclusions:Conifer species have shown a great capacity for colonizing volcanic substrates created by the Paricutín eruption,which suggests that tropical montane conifers can regenerate rapidly under high-magnitude disturbances.These findings support the use

Comparison of clinical outcomes between unrelated single umbilical cord blood and"ex-vivo"T-cell depleted haploidentical transplantation in children with hematological malignancies

Background Over the last two decades,umbilical cord blood(UCB)and haploidentical transplantation(HaploHSCT)have emerged as alternative sources of hematopoietic stem cell for allogeneic transplantation.There are few retrospective studies and no prospective studies comparing both types of alternative transplantation in pediatric patients.Results We analyzed the data of 134 children with hematological malignancies who received a hematopoietic stem cell transplantation from a single umbilical cord blood(UCB)(n=42)or an"tex-vivo"T-cell depleted transplant from a haploi-dentical-related donor(HaploHSCT)(n=92)between 1996 and 2014.Hematological recovery was faster after HaploHSCT than the UCB transplant group(median times to neutrophil and platelet recovery:13 vs.16 days,10 vs.57 days,respectively)(P<0.001).The HaploHSCT group had a significantly early immune reconstitution based on NK and CD8+T cells compared with the UCB group.However,after the first year post-transplantation.HaploHSCT had a lower number of CD4+ T and B lymphocytes compared with the UCB transplant recipients.The cumulative incidence of TRM was 29±8%in the HaploHSCT group versus 40±5%in the UCB group.Relapse incidence was 21±7%in the HaploHSCT group and 19±8%in the UCB group.Probability of DFS was 58±8%in the HaploHSCT group versus 40±9%in the UCB group(P=0.051).Conclusions TCD haploidentical transplant is associated with advantages in terms of engraftment and early immune reconstitution kinetics.TCD haploidentical transplant was associated with lower incidence of infectious and non-infectious complications,especially in the early phases of the transplant compared with UCB transplant recipients.However,there are no advantages in transplant outcomes compared with UCB transplant.

Nutrient drink test: A promising new tool for irritable bowel syndrome diagnosis

BACKGROUND Irritable bowel syndrome(IBS) is a highly prevalent condition. It is diagnosed on the basis of chronic symptoms after the clinical and/or investigative exclusion of organic diseases that can cause similar symptoms. There is no reproducible noninvasive test for the diagnosis of IBS, and this raises diagnostic uncertainty among physicians and hinders acceptance of the diagnosis by patients.Functional gastrointestinal(GI) syndromes often present with overlapping upper and lower GI tract symptoms, now believed to be generated by visceral hypersensitivity. This study examines the possibility that, in IBS, a nutrient drink test(NDT) provokes GI symptoms that allow a positive differentiation of these patients from healthy subjects.AIM To evaluate the NDT for the diagnosis of IBS.METHODS This prospective case-control study compared the effect of two different nutrient drinks on GI symptoms in 10 IBS patients(patients) and 10 healthy controls(controls). The 500 kcal high nutrient drink and the low nutrient 250 kcal drink were given in randomized order on separate days. Symptoms were assessed just before and at several time points after drink ingestion. Global dyspepsia and abdominal scores were derived from individual symptom data recorded by two questionnaires designed by our group, the upper and the general GI symptomquestionnaires, respectively. Psycho-social morbidity and quality of life were also formally assessed. The scores of patients and controls were compared using single factor analysis of variance test.RESULTS At baseline, IBS patients compared to controls had significantly higher levels of GI symptoms such as gastro-esophageal reflux(P = 0.05), abdominal pain(P =0.001), dyspepsia(P = 0.001), diarrhea(P = 0.001), and constipation(P = 0.001) as well as higher psycho-social morbidity and lower quality of life. Human cytomegalovirus(CMV)represents the most common opportunistic infection in liver transplant recipients.CMV infections in post liver transplant patients cause significant morbidity and mortality,directly affecting posttransplant outcomes.This review will provide the framework for the surveillance,diagnosis,prophylaxis and treatment of CMV in the liver transplant population.

Suitability and limitations of mesenchymal stem cells to elucidate human bone illness

Functional impairment of mesenchymal stem cells(MSCs),osteoblast progenitor cells,has been proposed to be a pathological mechanism contributing to bone disorders,such as osteoporosis(the most common bone disease)and other rare inherited skeletal dysplasias.Pathological bone loss can be caused not only by an enhanced bone resorption activity but also by hampered osteogenic differentiation of MSCs.The majority of the current treatment options counteract bone loss,and therefore bone fragility by blocking bone resorption.These socalled antiresorptive treatments,in spite of being effective at reducing fracture risk,cannot be administered for extended periods due to security concerns.Therefore,there is a real need to develop osteoanabolic therapies to promote bone formation.Human MSCs emerge as a suitable tool to study the etiology of bone disorders at the cellular level as well as to be used for cell therapy purposes for bone diseases.This review will focus on the most relevant findings using human MSCs as an in vitro cell model to unravel pathological bone mechanisms and the application and outcomes of human MSCs in cell therapy clinical trials for bone disease.

Beta-blockers and physical frailty in patients with endstage liver disease

AIM To investigate beta-blocker(BB) use in patients with cirrhosis and determine their effects on physical frailty and overall survival.METHODS Adult outpatients with cirrhosis listed for liver transplantation underwent testing of physical frailty using the performance-based Liver Frailty Index, comprised of chair stands, grip strength, and balance testing, as well as self-reported assessments of exhaustion and physical activity. BB use was assessed from medical chart review. Univariable and multivariable logistic regression were performed to determine BB use and their association with measures of physical frailty. Competing risk analyses were performed to determine the effect of BB use on wait-list mortality, as defined by death or delisting for being too sick for transplant.RESULTS Of 344 patients, 35% were female, median age was 60, median model for end stage liver disease was 15, and 53% were prescribed a BB. Compared to those not on BB, patients on BB were similar except for percentage female(25% vs 46%; P < 0.001) and BMI(29 vs 28; P = 0.008). With respect to tests of physical frailty, BB use was not associated with increased odds of frailty(by the Liver Frailty Index), exhaustion, or low physical activity. BB use was, however, significantly associated with a decreased adjusted risk of mortality(SHR 0.55; P = 0.005).CONCLUSION In patients with cirrhosis awaiting liver transplantation, BB use is not associated with physical frailty. We confirmed the known survival benefits with BB use, and concerns about adverse effects should not deter their utilization when indicated.

新冠肺炎给世界各地的教育系统带来了危机,威胁着人们的受教育权和学习过程的连续性。因此,各国出台了基于网络技术的政策来保证教育。但是,许多国家的技术基础设施并不能够胜任疫情期间在线教育的要求。委内瑞拉制定了“居家上大学（University at Home,Universidad en Casa）政策,以保证大学部门在新冠大流行期间的运行。本研究以委内瑞拉中央大学为例,探究疫情期间委内瑞拉为保证大学的正常教学活动而实施的“居家上大学”政策,研究目的在于明确委内瑞拉于疫情下的教育政策对保障大学正常教学的作用,借助混沌和复杂理论解释战略性政策帮助优化远程教学的实践效果,提出保证教育权的教育政策建议。研究对委内瑞拉中央大学的管理层（包括3名院长,1名学校主任,1名教职学术协调员,1名学校学术协调员和1名平台专家）进行了半结构性访谈,并采用内容分析方法对这些质性资料进行了归纳分析。研究结果表明,由于线上教育的条件,国家宪法要求的受教育权难以保障,而且政策的制定未征询大学管理者意见。混沌和复杂性理论原则也没有得到合理应用,因为该理论要求适应性、弹性和计划性,但疫情下委内瑞拉的事实并非如此。大学依靠自己的资源来完成教学任务,不乏有一些好的教学实践,诸如学习、信息和协作等。源于线上教学的特性,教学活动也需要更多弹性和分析。总结而言,此项研究考察了疫情期间委内瑞拉如何采取教育保障政策,以保证大学的正常教学,并提供了相关建议。委内瑞拉在疫情期间制定了“居家上大学”的教育保障政策,但政策的实施未取得理想效果。

In educational environments an improvement in the quality of interior lighting has a direct benefit in increasing productivity and alertness of students and teachers, as well as very important implications for the energy efficiency of the education facilities but when a replacement with different lighting is analyzed, research may be tarnished by users’ pre-formed opinions, influenced by manufacturers and advertising. Consequently, it is necessary to understand the users’ point of view, even before of being subjected to any change in the lighting stimulus.Based on the Kansei Engineering framework, the general objective of this paper is to evaluate and compare the subjective evaluation of students’ pre-formed opinions to lighting provided by two types of lamps (fluorescent and LED). The subjective assessment of 427 university students has been compared over four years. The results show significant differences in students’ subjective evaluation. This finding highlights the existence of symbolic or functional attributes of the usefulness perceived by the student that could influence on investigations in which different types of lighting are compared.

BACKGROUND Diabetes mellitus type 2(T2DM)is a state of hyperglycemia due to a defect in the secretion and/or action of insulin,and it represents the most common form of diabetes worldwide.In Mexico,10.3%of the adult population have been diagnosed with T2DM and it is expanding to the pediatric population.AIM To evaluate and compare the prevalence of T2DM in the pediatric population at the Children’s Hospital,“Hospital Infantil de México Federico Gómez(HIMFG)”,at two time periods:2013 vs 2018.METHODS A comparison of two cross-sectional studies was done(2013 and 2018).The study population was comprised of children and adolescents 8-17 years old,from the Diabetes Clinic at the aforementioned institution.A comprehensive interrogation regarding family history and perinatal antecedents was performed.Complete blood work after 12 h of fasting was obtained to determine serum levels of glucose,glycated hemoglobin,lipid profile,C-peptide,and insulin.The data were analysed using the statistical software package SPSS v.23.0.A P value of<0.05 was considered statistically significant.RESULTS A total of 151 patients were included:47 from 2013,and 104 from 2018.There were age differences noted between the two periods with younger patients presenting T2DM in 2013.Also,T2DM predominated in the male sex in 55.36%in 2013 vs 32.7%in 2018.An increased prevalence of T2DM was noted from 2013 to 2018(20.2%vs 33.0%,respectively),which was a statistically significant 12.8%increase(P<0.0001).The illness phenotype was more aggressive in the 2018 group with the presence of a higher proportion of obesity,insulin resistance,and adverse lipid profiles.CONCLUSION The prevalence of T2DM at the HIMFG institution from 2013 to 2018 increased by 12.8%(20.2%vs 33.0%,respectively).The study results demonstrate the need for vigilance in T2DM trends,and to strengthen programs of healthy nutrition and physical activity as well as early detection and risk factors for obesity,data on insulin resistance,and metabolic syndrome,with the aim of preventing the deve
