OBjECTIVE: To describe the differential diagnosis of periurethral masses is a consecutive series extracted from a single tertiary urogynecologic practice database. METHODS: A patient database of a private urology and urogynecology practice with 1,950 women was searched for patients who were found to have a periurethral mass during the accrual dates of 1994 to 2002, and these records were reviewed for diagnostic testing and results. All women provided a history, completed a questionnaire, and underwent physical examination, voiding diary, cystoscopy, and videourodynamic testing; selected patients then underwent additional imaging. RESULTS: Seventy-nine (4%) patients aged 41.2 ±.14 years were identified. Of these, 72 (91%) had been referred for evaluation of persistent irritative lower urinary tract symptoms or incontinence. Seven patients (9%) had been referred specifically because of a periurethral mass. Sixty-six patients (84%; 95%confidence interval [CI]-73%, 91%) had urethral diverticula, of which 4 (6%; 95%CI 2%, 14.8%) contained malignancies. Six patients (7%; 95%CI 3%, 15%) had vaginal cysts histologically identified as fibromuscular tissue, 4 (5%; 95%CI 1%, 12%) had leiomyomata, and 2 (2.5%; 95%CI 0.03%, 8.8%) had ectopic ureteroceles. Two patients had vaginal squamous cell carcinomas (2.5%; 95%CI 0.03%, 8.8%), and 1 had an infected granuloma. Masses were palpable in 42 patients (53.8%; 95%CI 42%, 64%) and in 37 patients either were encountered at surgery (n = 5) or were urethral diverticula diagnosed by voiding cystourethrogram (n= 32). CONCLUSION: Periurethral masses were encountered in less than 4%of our patient sample. Most masses were urethral diverticula; however, the differential diagnosis included leiomyoma, vaginal cysts, and malignancy. Masses were generally either palpable or seen at imaging studies performed during evaluation of lower urinary tract symptoms.

Innovative pathways allow safe discharge of mild acute pancreatitis from the emergency room

Acute pancreatitis(AP)is a leading cause of gastrointestinal-related hospitalizations in the United States,resulting in 300000 admissions per year with an estimated cost of over$2.6 billion annually.The severity of AP is determined by the presence of pancreatic complications and end-organ damage.While moderate/severe pancreatitis can be associated with significant morbidity and mortality,the majority of patients have a mild presentation with an uncomplicated course and mortality rate of less than 2%.Despite favorable outcomes,the majority of mild AP patients are admitted,contributing to healthcare cost and burden.In this Editorial we review the performance of an emergency department(ED)pathway for patients with mild AP at a tertiary care center with the goal of reducing hospitalizations,resource utilization,and costs after several years of implementation of the pathway.We discuss the clinical course and outcomes of mild AP patients enrolled in the pathway who were successfully discharged from the ED compared to those who were admitted to the hospital,and identify predictors of successful ED discharge to select patients who can potentially be triaged to the pathway.We conclude that by implementing innovative clinical pathways which are established and reproducible,selected AP patients can be safely discharged from the ED,reducing hospitalizations and healthcare costs,without compromising clinical outcomes.We also identify a subset of patients most likely to succeed in this pathway.

Semi-analytical investigation of heat transfer in a porous convective radiative moving longitudinal fin exposed to magnetic field in the presence of a shape-dependent trihybrid nanofluid

The thermal examination of a non-integer-ordered mobile fin with a magnetism in the presence of a trihybrid nanofluid(Fe_3O_4-Au-Zn-blood) is carried out. Three types of nanoparticles, each having a different shape, are considered. These shapes include spherical(Fe_3O_4), cylindrical(Au), and platelet(Zn) configurations. The combination approach is utilized to evaluate the physical and thermal characteristics of the trihybrid and hybrid nanofluids, excluding the thermal conductivity and dynamic viscosity. These two properties are inferred by means of the interpolation method based on the volume fraction of nanoparticles. The governing equation is transformed into a dimensionless form, and the Adomian decomposition Sumudu transform method(ADSTM) is adopted to solve the conundrum of a moving fin immersed in a trihybrid nanofluid. The obtained results agree well with those numerical simulation results, indicating that this research is reliable. The influence of diverse factors on the thermal overview for varying noninteger values of γ is analyzed and presented in graphical representations. Furthermore, the fluctuations in the heat transfer concerning the pertinent parameters are studied. The results show that the heat flux in the presence of the combination of spherical, cylindrical, and platelet nanoparticles is higher than that in the presence of the combination of only spherical and cylindrical nanoparticles. The temperature at the fin tip increases by 0.705 759% when the value of the Peclet number increases by 400%, while decreases by 11.825 13% when the value of the Hartman number increases by 400%.

Rodent management in Aotearoa New Zealand: approaches and challenges to landscape-scale control

Aotearoa—New Zealand has only four rodent species,all introduced.In order of arrival,they are Pacific rat Rattus exulans,brown rat R.norvegicus,house mouse Mus musculus,and black rat R.rattus.Rodent management in New Zealand aims mainly to conserve indigenous biodiversity rather than to protect crops or manage diseases,as is usual elsewhere.We describe four major“regimes”and one major vision for rodent control in New Zealand to meet ecological restoration objectives.Current challenges for island eradications are for large islands that are remote or populated by people.Aerial 1080 is the only large-scale(tens of thousands of hectares)option for black rat control,but its application requires adjustment to counter subsequent rapid black rat repopulation.Unfenced“ecosanctuaries”(mean 720 ha)use ground-based traps and poisons to target mainly black rats and face constant reinvasion.Ecosanctuaries with mammal-resistant fences(up to 3500 ha)limit reinvasion and target more pest species and have enabled the return of previously extirpated taxa to the main islands.Predator Free 2050 aims to eradicate the rat species(but not mice)plus some other introduced mammals from New Zealand by 2050.This vision is not attainable with current tools,but research and experimental management is exploring techniques and technologies.The large scale(to 100000 ha)at which black rats are now targeted for control to extremely low abundance seems to be unique to New Zealand.

Supposition of graphene stacks to estimate the contact resistance and conductivity of nanocomposites

In this study,the effects of stacked nanosheets and the surrounding interphase zone on the resistance of the contact region between nanosheets and the tunneling conductivity of samples are evaluated with developed equations superior to those previously reported.The contact resistance and nanocomposite conductivity are modeled by several influencing factors,including stack properties,interphase depth,tunneling size,and contact diameter.The developed model's accuracy is verified through numerous experimental measurements.To further validate the models and establish correlations between parameters,the effects of all the variables on contact resistance and nanocomposite conductivity are analyzed.Notably,the contact resistance is primarily dependent on the polymer tunnel resistivity,contact area,and tunneling size.The dimensions of the graphene nanosheets significantly influence the conductivity,which ranges from 0 S/m to90 S/m.An increased number of nanosheets in stacks and a larger gap between them enhance the nanocomposite's conductivity.Furthermore,the thicker interphase and smaller tunneling size can lead to higher sample conductivity due to their optimistic effects on the percolation threshold and network efficacy.

A new approach to pedestal differentiation for soil loss estimation-a case study from a burnt area in north-central Portugal

Soil pedestals have long been used as qualitative indicators of soil splash erosion.In rangelands,plant-capped pedestals,generally grass tussocks,have also been used to quantitatively estimate soil loss since the first half of the twentieth century.In agricultural lands,forests,and bad-lands,stone-capped pedestals have been used as qualitative and semi-quantitative indicators of active,'extreme'erosion.Little work has been reported on using capstone pedestal data for quantifying soil loss.We postulate that three distinct capstone pedestal types may be present in any given location and that a detailed analysis of a pedestal height histogram may be used to recognize their populations.This analysis can subsequently inform if soil loss can be reliably estimated and if so,which of the existing methods using pedestal height data will provide more accurate results.The three proposed capstone pedestal types are:(1)neo-pedestals formed underneath surface stones exposed by(partial)removal of the soil surface cover;(2)endo-pedestals formed underneath stones that were buried in the soil but have been exposed by erosion;and(3)phoenix-pedestals formed underneath stones from collapsed pedestals.In the pedestal height histogram of any given location,a skew to smaller heights may indicate the existence of endo-and/or phoenix-pedestals,which may be revealed as a bi-(or tri)modal distribution when using a smaller bin size.This concept was applied to a case study where soil loss had been monitored for control plots and mulched plots during a 5-year period following wildfire in a eucalypt plantation.We measured pedestal heights and used methods to quantitatively assess soil loss from soil pedestal data in the available literature.Soil pedestal data at the end of the 5-year period under or overestimated soil loss in the control treatment,with results ranging from 60 to 115%of measured soil loss,depending on the method.It is postulated that phoenix-and endo-pedestals may be a driving factor behind the observed discrepancies.We discuss

Optimization of chemistry and process parameters for control of intermetallic formation in Mg sludges

Intermetallic formation in sludge during magnesium(Mg)melting,holding and high pressure die casting practices is a very important issue.But,very often it is overlooked by academia,original equipment manufacturers(OEM),metal ingot producers and even die casters.The aim of this study was to minimize the intermetallic formation in Mg sludge via the optimization of the chemistry and process parameters.The Al8Mn5 intermetallic particles were identified by the microstructure analysis based on the Al and Mn ratio.The design of experiment(DOE)technique,Taguchi method,was employed to minimize the intermetallic formation in the sludge of Mg alloys with various chemical compositions of Al,Mn,Fe,and different process parameters,holding temperature and holding time.The sludge yield(SY)and intermetallic size(IS)was selected as two responses.The optimum combination of the levels in terms of minimizing the intermetallic formation were 9 wt.%Al,0.15 wt.%Mn,0.001 wt.%(10 ppm)Fe,690℃ for the holding temperature and holding at 30 mins for the holding time,respectively.The best combination for smallest intermetallic size were 9 wt.%Al,0.15 wt.%Mn,0.001 wt.%(10 ppm)Fe,630℃ for the holding temperature and holding at 60 mins for the holding time,respectively.Three groups of sludge factors,Chemical Sludge(CSF),Physical Sludge(PSF)and Comprehensive Sludge Factors(and CPSF)were established for prediction of sludge yields and intermetallic sizes in Al-containing Mg alloys.The CPSF with five independent variables including both chemical elements and process parameters gave high accuracy in prediction,as the prediction of the PSF with only the two processing parameters of the melt holding temperature and time showed a relatively large deviation from the experimental data.The Chemical Sludge Factor was primarily designed for small ingot producers and die casters with a limited melting and holding capacity,of which process parameters could be fixed easily.The Physical Sludge Factor could be used for mass production with a single type of Mg allo

We obtain an exact solution for spherically symmetric Letelier AdS black holes immersed in perfect fluid dark matter(PFDM).Considering the cosmological constant as the positive pressure of the system and volume as its conj.g.te variable,we analyze the thermodynamics of our black holes in the extended phase space.Owing to the background clouds of strings parameter(a)and the parameter endowed with PFDM(β),we analyze the Hawking temperature,entropy,and specific heat.Furthermore,we investigate the relationship between the photon sphere radius and phase transition for the Letelier AdS black holes immersed in PFDM.Through the analysis,with a particular condition,non-monotonic behaviors are found between the photon sphere radius,impact parameter,PFDM parameter,temperature,and pressure.We can regard the changes in both the photon sphere radius and impact parameter before and after phase transition as the order parameter;their critical exponents near the critical point are equal to the same value,1/2,similar to that in ordinary thermal systems.This indicates that a universal relation of gravity may exist near the critical point for a black hole thermodynamic system.

Mid-spatial frequency wavefront deformation can be deleterious for the operation of high-energy laser systems. When fluid cooled high-repetition-rate amplifiers are used, the coolant flow is likely to induce such detrimental mid-spatial frequency wavefront deformations. Here, we describe the design and performance of a 90 mm × 90 mm aperture, liquid-cooled Nd:phosphate split-slab laser amplifier pumped by flash-lamps. The performance of the system is evaluated in terms of wavefront aberration and gain at repetition rates down to 1 shot per minute. The results show that this single cooled split-slab system exhibits low wavefront distortions in the medium to large period range, compatible with a focus on target, and despite the use of liquid coolant traversed by both pump and amplified wavelengths. This makes it a potential candidate for applications in large high-energy laser facilities.

BACKgROUND Unhealthy maternal diet leads to heavy metal exposures from the consumption of ultra-processed foods that may impact gene behavior across generations,creating conditions for the neurodevelopmental disorders known as autism and attention deficit/hyperactivity disorder(ADHD).Children with these disorders have difficulty metabolizing and excreting heavy metals from their bloodstream,and the severity of their symptoms correlates with the heavy metal levels measured in their blood.Psychiatrists may play a key role in helping parents reduce their ultra-processed food and dietary heavy metal intake by providing access to effective nutritional epigenetics education.AIM To test the efficacy of nutritional epigenetics instruction in reducing parental ultra-processed food intake.METHODS The study utilized a semi-randomized test and control group pretest-posttest pilot study design with participants recruited from parents having a learning-disabled child with autism or ADHD.Twenty-two parents who met the inclusion criteria were randomly selected to serve in the test(n=11)or control(n=11)group.The test group participated in the six-week online nutritional epigenetics tutorial,while the control group did not.The efficacy of the nutritional epigenetics instruction was determined by measuring changes in parent diet and attitude using data derived from an online diet survey administered to the participants during the pre and post intervention periods.Diet intake scores were derived for both ultra-processed and whole/organic foods.Paired sample t-tests were conducted to determine any differences in mean diet scores within each group.RESULTS There was a significant difference in the diet scores of the test group between the pre-and post-intervention periods.The parents in the test group significantly reduced their intake of ultra-processed foods with a preintervention diet score of 70(mean=5.385,SD=2.534)and a post-intervention diet score of 113(mean=8.692,SD=1.750)and the paired t-test analysis showing a significance of
