Prevalence of hepatitis B in Pointe-Noire is 23%. The objective of this study was to evaluate the level of therapeutic management of patients with hepatitis B in Pointe-Noire. Methods: A retrospective, cross-sectional, descriptive and analytical study. Results: 302 patients included: 217 (72%) men and 85 (28%) women of median age 40 years. The viral load was measured in 140 (46%) patients and in 162 (54%), the viral load was not measured. Their distribution according to the viremia was as follows: 0 - 1 log = 1 (0.7%) patients;1.5 to 2 log = 23 (16%) patients;2.5 to 3 log = 49 (35%) patients;>3log = 55 (39%) patients. Of the 302 patients included, 36 (11%) patients were treated with antivirals and 253 (83%) patients were not. Conclusion: The number of patients treated for chronic viral hepatitis B was low. Our results show interest for improved preventive and curative management of hepatitis B in Congo in order to reduce the existing high prevalence.

Introduction: Lymph node (LN) metastasis are important biomarkers in the resection of colorectal cancer translating into accurate disease staging. It is therefore obvious that lymph node dissection in the curative treatment of colorectal cancer is a prognostic factor that influences survival and guides management strategy. This study aims to evaluate lymph node dissection in colorectal cancers (CRC) managed at the Teaching Hospital, Brazzaville, Congo. Patients and Method: We carried out a descriptive prospective study during a period of 18 months from October 2020 to March 2021 involving patients who underwent curative treatment for colorectal cancer at the visceral surgery department of the Brazzaville Teaching Hospital. Results: Thirty (30) cases of CRC were recorded out of 738 hospitalizations in the visceral surgery departments, which constitute 4.06% of the in-patient activity of the department during the period under study. Mean age was 55.13 years and a male predominance was observed with a sex ratio of 1.14. Colonic involvement was predominant in our series and was found in 22 of the patients. 25 patients underwent surgical resection while 5 patients could not undergo operative treatment. 17 out of the 25 patients underwent curative treatment, 6 were offered palliative care, whereas surgical abstention was decided in 2 patients. The average number of lymphadenectomies per patient was 16.43. Lymphadenectomy was greater than or equal to 12 in 88% of patients. Nine patients had vascular emboli whereas 10 patients had perineural invasion. The tumor was staged as T3 in 15 patients;six patients had a negative lymph node and 11 patients had at least one lymph node invasion. Distant metastasis status could not be determined in 10 patients. Conclusion: Lymphadenectomy is a hallmark in the curative treatment of colorectal cancer as it allows for optimal staging. This represents a strong prognostic factor that defines adjuvant therapy and overall patient outcome.

Late Neonatal Bacterial Infection at the Brazzaville Teaching Hospital

Background: Late Neonatal Bacterial Infection (LNNBI) is a clinical and biological manifestations related to penetration and growth of specific causative bacteria in bloodstream occurring on the 4th-28th day of life. LNNBI still represents an important cause of mortality and morbidity among infants. Objectives: To determine the frequency of late bacterial infections in newborns, to describe the clinical and biological profiles and to identify the main responsible germs. Methods: Descriptive study data collection, conducted over a period of 10 months at the Brazzaville Teaching Hospital, of interest to newborns admitted from the 4th day of life for suspicion of neonatal infection, and those admitted for any other pathology and having presented an infection 48 hours after hospitalization, and in whom a bacterial culture and/or an inflammatory assessment confirmed or suspected infection. Results: During the study period, 1682 newborns were hospitalized, and 86 were hospitalized for a late neonatal bacterial infection, i.e. a frequency of 5.1%. There were 67 (77.9%) community infections and 19 (22.1%) nosocomial infections. The frequency of nosocomial infection was 1.1%. The main signs were fever in 65 cases (75.6%), and respiratory distress in 37 cases (43%). The most frequent localizations were bacteremia 32 (37.2%), pulmonary 21 (24.4%), digestive and meningeal in 11 cases (12.8%) each. The most common germ Klebsiella in 10 (50%) newborns was resistant to the usual antibiotics. The evolution was favorable in 71 cases (82.5%), and death occurred in 12 cases (14%). Conclusion: Late neonatal bacterial infection is common. The main responsible germs are gram-negative bacilli, in particular Klebsiella multi-resistant.

Split hand/foot malformation (SHFM), formerly known as ectrodactyly is a rare congénital anomaly, its incidence varies from 1/8.500 to 1/25.000 live birth. It mainly affects the development of the limbs, its clinical variability is standard, can present as an isolated feature or as a syndrome associated with other congenital anomalies. Our objective was to present the two cases of SHFM, and to review the literature on the clinical aspects and discuss a probable origin. The father went to school and is a driver because the malformations concerned only the fingers, were less severe, and did not prevent the realization of certain simple gestures of the daily life. On the other hand, the malformations of the fingers of the newborn were severe and the absence of the thumbs compromised the later prehension function. Also the association of a microglossia and a cleft palate contributed to a weight loss that justified hospitalization. The clinical presentation of split hands and feet is variable and the prognosis depends on the type of anomaly. Familial cases suggest a probable genetic origin. Genetic testing is necessary to establish genetic counseling.
