复习了 H_1抗组胺药对慢性特发性荨麻疹和物理性荨麻疹瘙痒及风团的治疗效果。为致不多的研究表明,在应用 H_1抗组胺药治疗时,尽管存在着个体差异,但瘙痒及风团同时平均减轻2/3。在某些物理性荨麻疹患者,瘙痒和风团以及红斑的改善不成比例。镇静作用轻微的 H_1抗组胺药疗效等同于或较强于经典的抗组胺药。抗组胺药完全消除组胺风团的剂量仅部分减轻荨麻疹风团,由此推理对瘙痒作用亦相同。因此,有必要寻求针对组胺以外的致痒介质的疗法,以改善荨麻疹的瘙痒情况。

Capnodium berberidis,a sooty mould fungus,previously reported only from Pakistan in 1978,was recently observed on Berberis lycium in Jot Pass,district Chamba of Himachal Pradesh,*** lycium is a well-known small to medium sized spiny,woody,deciduous or evergreen medicinal and ornamental plant distributed throughout temperate and subtropical regions of the *** symptoms appeared as thin,black to dark brown,easily removed superficial layer of hyphae on various parts of host plant including leaves,stem,twigs and *** detailed studies on its morphology and taxonomy revealed it a sooty mould fungus Capnodium *** per literature consulted,this is the first report of Capnodium berberidis from India and probably second from world.

Child-Pugh-Turcott versus Meld score for predicting survival in a retrospective cohort of black African cirrhotic patients

AIM: To compare the performance of the Child-Pugh-Turcott (CPT) score to that of the model for end-stage liver disease (MELD) score in predicting survival of a retrospective cohort of 172 black African patients with cirrhosis on a short and mid-term ***: Univariate and multivariate (Cox model) analyses were used to identify factors related to mortality. Relationship between the two scores was appreciated by calculating the correlation coefficient. The Kaplan Meier method and the log rank test were used to elaborate and compare survival respectively. The Areas Under the Curves were used to compare the performance between scores at 3, 6 and 12 ***: The study population comprised 172 patients, of which 68.9% were male. The mean age of the patient was 47.5 ± 13 years. Hepatitis B virus infection was the cause of cirrhosis in 70% of the cases. The overall mortality was 31.4% over 11 years of follow up. Independent factors significantly associated with mortality were: CPT score (HR = 3.3, 95% CI [1.7-6.2]) (P 1.5 mg/dL versus serum creatine 21 vs MELD black African patients with cirrhosis. Moreover, its handling appears less cumbersome in clinical practice as compared to the latter.

Interest of SAAG in Etiological Diagnosis of Ascites in African black

Purpose: To show the interest of the serum-ascites concentration gradient in albumin (SAAG) in the etiological diagnosis of ascites in African black. Materiel and Methods: This was a monocentric observational and analytical study performed on patients over 15 years old and hospitalized in the Hepato-Gastroenterology department of Cocody from January 1st, 2010 to March 31st, 2017 for ascites. The cytology and chemistry of the ascites fluid and the protidogram were performed. The main outcome measure was the prediction of portal hypertension (PHT) or non-portal hypertension related causes of ascites from SAAG. Results: The hospital prevalence of ascites was 11%. There were 82 women (46.9%) and 93 men (53.1%) with a sex ratio (M/F) of 1.1. SAAG was low in 68.6% of cases and high in 31.4% of cases. The most common etiologies were post-viral cirrhosis (74%), HCC (16.60%) and peritoneal tuberculosis (13.7%) respectively. In the case of high SAAG, 91% of ascites related to PHT were observed, but also 77.5% of a low SAAG corresponded to ascites related to PHT (p = 0.024) with Se = 35% Sp = 84% and accuracy = 44%. All patients with peritoneal tuberculosis had a SAAG low of less than 1.1 g/dl significantly (p = 0.002). SAAG had a diagnostic performance of 89% in peritoneal tuberculosis at the cutoff value of ?1.24 g/dl (AUROC = 0.91, Se = 96%, Sp = 86%) while in post viral hepatitis cirrhosis at a threshold of 0.58 g/dl the diagnostic performance was 60% (AUROC = 0.69, Se = 53%, Sp = 79%). Conclusion: The simple technical SAAG, allowing to distinguish the ascites linked to the PHT and the ascites not bound to the PHT, can be used like a method of diagnosis of the etiologies of the ascites especially in the peritoneal tuberculosis in the African countries low income.
