目的:观察微囊化兔嗅球组织细胞移植对大鼠脊髓全横断损伤后细胞凋亡及功能恢复的影响,并探讨其对脊髓继发性损伤的保护作用及其机制。方法:实验于2004-10/2005-07在南昌大学基础医学院应用解剖研究室完成。选择清洁级Sd大白鼠120只,按随机数字表法分为4组:正常对照组、损伤对照组、单纯细胞悬液移植组、微囊化移植组,每组30只。制作T10节段脊髓全横断损伤模型。正常对照组仅打开椎管,暴露脊髓不作移植;损伤对照组仅用明胶海绵填塞脊髓损伤腔隙;单纯细胞悬液移植组植入吸附细胞悬液的明胶海绵块;微囊化移植组用与断端腔隙大小相吻合的明胶海绵块吸附10μL的微囊化细胞,植入洞腔。分别于术后12h,1,3,7,21d5个时间点对动物进行BBB评分后处死。取损伤区1cm范围脊髓节段,常规石蜡包埋,水平切片进行苏木精-伊红染色、TUNEL染色,观察凋亡细胞的数量及分布变化。按BBB评分法对大鼠术后运动功能的恢复情况进行评分,共分22级,最低分0分,最高分21分,分数越高,运动功能恢复越完善。结果:纳入大鼠120只,存活96只,存活率为80%。其中以损伤对照组及单纯细胞悬液移植组死亡率较高,损伤对照组大鼠死亡的主要原因可能是脊髓损伤导致损伤平面以下神经功能的丧失所引起的一系列并发症所导致的;而单纯细胞悬液移植组是因为免疫排斥反应所导致。①术后3d内各组间BBB评分差异无显著性意义(P>0.05);术后7,21d微囊化移植组和单纯细胞悬液移植组大鼠BBB评分高于损伤对照组,差异均有显著性意义[分别为(7.25±1.17),(4.58±0.38),(2.67±0.61)分;(10.42±1.63),(6.08±0.80),(3.33±0.68)分,F=4.528,3.821,Pd微囊化移植组TUNEL阳性细胞数及凋亡指数低于损伤对照组,差异有显著性意义(以术后3d为例,TUNEL阳性细胞数分别为(8.83±1.33),(14.50±1.05)个/视野,P<0.01;凋亡指数分别为10.45±1.58,17.06±1.23,P<0.01)。结论:微囊化兔嗅球组织细胞移植对大鼠脊髓全横断损伤后细胞凋亡具有保护作用,能促进脊髓功能的恢复。

Proximal sensor-enhanced soil mapping in complex soil-landscape areas of Brazil

Portable X-ray fluorescence(pXRF) spectrometry and magnetic susceptibility(MS) via magnetometer have been increasingly used with terrain variables for digital soil mapping. However, this methodology is still emerging in many countries with tropical soils. The objective of this study was to use proximal soil sensor data associated with terrain variables at varying spatial resolutions to predict soil classes using the Random Forest(RF) algorithm. The study was conducted on a 316-ha area featuring highly variable soil classes and complex soil-landscape relationships in Minas Gerais State, Brazil. The overall accuracy and Kappa index were evaluated using soils that were classified at 118 sites, with 90 being used for modeling and 28 for validation. digital elevation models(dEMs) were created at 5-, 10-, 20-, and 30-m resolutions using contour lines from two sources. The resulting dEMs were processed to generate 12 terrain variables. Total Fe, Ti, and SiO_(2) contents were obtained using pXRF, with MS determined via a magnetometer. Soil class prediction was performed using the RF algorithm. The quality of the soil maps improved when using only the five most important covariates and combining proximal sensor data with terrain variables at different spatial resolutions. The finest spatial resolution did not always provide the most accurate maps. The high soil complexity in the area prevented highly accurate predictions. The most important variables influencing the soil mapping were MS, Fe, and Ti. Proximal sensor data associated with terrain information were successfully used to map Brazilian soils at variable spatial resolutions.

The multi-frequency light curves of bl Lacertae during 1997.5 - 1999.5 have been modeled by four outbursts, each having a 3-stage evolution in the （Sm, vm） plane with distinct rising-plateau-decaying phases. It is shown that the observed light curves can be well fitted for the eight frequencies from 350 GHz to 4.8 GHz. The main characteristics of the model-fitting are; （1） the outbursts are found to have very flat spectra with an optically thin spectral index α （defined as Sv α u^-α） of about 0.15. This is consistent with the results previously obtained by Valtaoja et al. （1992）; （2） it is found that there is no spectral flattening between the rising-plateau phase and the decay phase. In other words, the optically thin spectral index does not change from the rising-plateau phase to the decay phase. These features are in contrast to the 3-stage shocked-in-jet model proposed by Marscher ＆ Gear （1985） for submm- IR-optical flares, in which a spectral flattening of △α = 0.5 is predicted when a transition occurs from the Compton/synchrotron phase （or rising-plateau phase） to the adiabatic phase （or decay phase） with α≥ 0.5 for the shock being non-radiative. We propose a new model to interpret the fitting results, suggesting that the 3-stage evolution of the mm-cm outbursts in bl Lacertae may be related to the process of shock formation and propagation in a highly collimated jet （for example, a ＇parabolic＇ jet）. In particular, during the rising phase, the thickness of the synchrotron-radiating region created by the shock may rapidly increase with time （relative to the jet width） due to the rapid injection of relativistic electrons and a magnetic field, and this leads to the observed behavior that the turnover flux density Sm rapidly increases while the turnover frequency um decreases. In the decay phase, the emitting plasma enters into a free expansion regime without further injection of relativistic electrons and a magnetic field （for example, when a transition from a co

目的:通过分析心肌梗塞后患者(PMI患者)运动中功能、生理、知觉和临床的反应,检测他们运动终止的主要原因(RET),目的在于确定RET是否可被预测得出;方法:112名男性近期PMI患者在活动跑台上进行递增负荷运动实验,运动终止依美国运动医学会推荐的标准。随后,受试者依据不同的运动终止原因被细分为4个组(生理组,身体组,感觉组和临床组)。运动中记录主观用力感觉、血压、血乳酸浓度(bl)、摄氧量、心率和呼吸困难记分,并且连续监控12导联心电图。运动前测量血胆固醇浓度(BC)。结果显示,在运动能力方面,身体组和知觉组的运动终止原因优于临床组,但劣于生理组。患者的分布在BC为5.4mmol·L-1和bl为1.6mmol·L-1这两点出现了显著的组间差异(Pbl,病人的RET便可被预测出来,且预测具有较高的敏感性和特异性。结论:利用本研究得出的方法对RET进行预测,在心脏康复的运动实验和处方制定中,都可发挥出有益的作用。

Neuroendocrine tumors of the small bowel are on the rise. In the US they have increased by 300%-500% in the last 35 years. At the same time their prognosis is much improved. Today,most neuroendocrine tumors (NETs) of the duodenum are detected "incidentally" and therefore recognized at an early stage. duodenal NETs which are well differentiated,not larger than 10 mm and limited to the mucosa/submucosa can be endoscopically resected. The management of duodenal NETs ranging between 10 and 20 mm needs an interdisciplinary discussion. Endoscopic ultrasound is the method of choice to determine tumor size and depth of infiltration. Surgery is recommended for well-differentiated duodenal NET tumors greater than 20 mm,for localized sporadic gastrinomas (of any size) and for localized poorly differentiated NE cancers. Surgery is recommended for any ileal NET. Advanced ileal NETs with a carcinoid syndrome are treated with longacting somatostatin analogs. This treatment significantly improves (progression-free) survival in patients with metastatic NETs of the ileum. For optimal NET management,tumor biology,type,localization and stage of the neoplasm,as well as the patient's individual circumstances have to be taken into account.

Neuroendocrine neoplasms (NENs) of the stomach, duo- denum, appendix or rectum that are small (≤ 1 cm) and well differentiated can be considered "early" tumors, since they generally have a (very) good prognosis. In the new WHO classification of 2010, these neoplasms are called neuroendocrine tumors/ carcinoids (NETs), grade (G) 1 or 2, and distinguished from poorly differentiated neuroendocrine carcinomas (NECs), G3. NETs are increasing, with a rise in the age-adjusted incidence in the U.S.A. by about 700 % in the last 35 years. Improved early detection seems to be the main reason for these epidemiological changes. Both the better generalavailability of endoscopy, and imaging techniques, have led to a shift in the discovery of smaller-sized (≤ 10-20 mm) intestinal NETs/carcinoids and earlier tumor stages at diagnosis. Endoscopic screening is therefore effective in the early diagnosis, not only of colorectal adenocarcinomas, but also of NETs/carcinoids. Endoscopic removal, followed up with endoscopic surveillance is the treatment of choice in NETs/carcinoids of the stomach, duodenum and rectum that are ≤ 10 mm in size, have a low proliferative activity (G1), do not infiltrate the muscular layer and show no angioinvasion. In all the other intestinal NENs, optimal treatment generally needs surgery and/or medical therapy depending on type, biology and stage of the tumor, as well as the individual situation of the patient.

Introduction: Musculoskeletal injuries are common in children but rarely severe. Their treatment varies according to the type of injury observed. The aim was to describe the epidemiological and therapeutic aspects of severe musculoskeletal injuries in children during play. Patients and Methods: This was a retrospective descriptive study conducted in the Orthopaedic-Traumatology department and the Paediatric Surgery Unit of the University Hospital of Bouaké between January 2018 and december 2020. It concerned patients less than 16 years of age hospitalised for more than 24 hours for a musculoskeletal trauma following a gambling accident. The variables studied were epidemiological and therapeutic. Results: A total of 53 patients were collected. The frequency was 2.9%. The mean age was 9 ± 3.99 years. There were 38 (71.7%) boys and 15 (28.3%) girls. The sex ratio was 2.5. The patients were attending school in 79.3% (n = 42). The circumstances of occurrence were dominated by falls during play accidents (n = 35;66%). Football was observed in 93.5% (n = 49). The thoracic limb (n = 35;66%) was the preferred site of these injuries. The injuries were closed (86.7%) with a predominance of forearm fractures (n = 22;41.6%). The average hospital stay was 2.8 ± 1.4 days. Most of the patients were treated orthopedically (n = 37;69.9%). The evolution was favourable without complications. Conclusion: Musculoskeletal injuries during play were infrequent (2.9%). The circumstances were dominated by falls during play. The injuries were mainly closed fractures, most of which were treated orthopedically.

目的探讨基质金属蛋白酶-2／9（MMP-2／9）及其组织抑制剂（TIMP-1／2）在高氧所致急性肺损伤（ALI）发病过程中的作用。方法将54只小鼠置于含体积分数大于98％氧气的密闭室中，以18只呼吸正常空气的小鼠作为对照组。分别于暴露24、48和72h活杀各组小鼠，取肺组织标本，测定肺湿／干重（W／d）比值、支气管肺泡灌洗液（BALF）中蛋白含量及胸腔积液，以评价肺损伤程度。用逆转录-聚合酶链反应（RT—PCR）测定肺组织MMP-2／9和TIMP-1／2的mRNA表达；用酶联免疫吸附法（ELLSA）测定BALF中MMP-2／9及TIMP1／2的蛋白含量。结果高氧能引起ALI，各实验组的肺W／d比值、BALF中蛋白浓度及胸腔积液均明显高于对照组（P〈0．05或P〈0．01）。RT—PCR结果显示，高氧组肺组织MMP-2／9及TIMP-1的mRNA表达均较对照组明显增高（P〈0．05或P〈0．01），TIMP-2 mRNA表达无明显变化；MMP-2／TIMP-2的mRNA比值在高氧组中均明显升高（P均〈0．01）。ELISA结果显示，高氧组BALF中MMP-2／9和TIMP-1蛋白含量较对照组明显升高（P〈0．05或P〈0．01），而TIMP-2蛋白含量同样无明显变化；MMP-2／TIMP-2的蛋白比值在高氧组中也明显升高（P均〈0．01）。结论高氧能引起ALI伴MMP-2／9和TIMP-1的表达增高，而MMP-2／TIMP-2平衡失调导致细胞外基质降解在奠发生过程中起重要作用。
