BACKGROUND Since its discovery in Wuhan,China in December of 2019,the novel coronavirus has progressed to become one of the worst pandemics seen in the last 100 ***,there has been an increased interest in the hepatic manifestations of coronavirus disease 19(COVID-19).AIM To describe the demographic and clinical characteristics of COVID-19 positive patients and study the association between transaminitis and all-cause *** This is a descriptive retrospective cohort study of 130 consecutive patients with a positive COVID pCR test admitted between March 16,2020 to May 14,2020 at a tertiary care University-based medical *** Wilcoxon-rank sum test and paired t-test were used for comparing non-parametric and parametric continuous variables respectively and a multivariable logistic regression models to study the association between transaminitis and mortality using SAS version 9.4(SAS Institute,Cary,NC,United States).RESULTS Out of the 130 patients,73(56%)patients were found to have transaminitis and 57(44%)did *** compared to patients without transaminitis,the transaminitis group was found to have a higher median body mass index(30.2 kg/m2 vs 27.3 kg/m2,p=0.04).In the multivariate analysis those with transaminitis were found to have 3.4 times higher odds of dying as compared to those without transaminitis adjusting for gender,the Age-adjusted Charlson Comorbidity Index and admission to the intensive care unit(p=0.03).CONCLUSION Our study showed that transaminitis on admission was associated with severe clinical outcomes such as admission to the intensive care unit,need for mechanical ventilation,and mortality.

Alteration in breathing patterns characterized by cyclic variation of ventilation during rest and during exercise has been recognized in patients with advanced heart failure(HF) for nearly two centuries. periodic breathing(pB) during exercise is known as exercise oscillatory ventilation(EOV) and is characterized by the periods of hyperpnea and hypopnea without interposed apnea. EOV is a non-invasive parameter detected during submaximal cardiopulmonary exercise testing. presence of EOV during exercise in HF patients indicates significant impairment in resting and exercise hemodynamic parameters. EOV is also an independent risk factor for poor prognosis in HF patients both with reduced and preserved ejection fraction irrespective of other gas exchange variables. Circulatory delay, increased chemosensitivity, pulmonary congestion and increased ergoreflex signaling have been proposed as the mechanisms underlying the generation of EOV in HF patients. There is no proven treatment of EOV but its reversal has been noted with phosphodiesterase inhibitors, exercise training and acetazolamide in relatively small studies. In this review, we discuss the mechanistic basis of pB during exercise and the clinical implications of recognizing pB patterns in patients with HF.

We propose a silica-core dispersion-decreasing Bragg fiber （DDBF） of mode effective area as large as 55 μm^2 for supercontinuum （SO） generation at the pump wavelength of 1060 nm. Using a fast and simple matrix method to model propagation in the DDBF,we have presented a general criterion to obtain the shortest length of the DDBF that would result in a broad SO spectrum. The proposed DDBF design should be amenable for reproducible fabrication through the well-developed MCVD fiber manufacturing technology and the concept has potential for realization as a practical device.

目的:①定义和建立尼泊尔人安氏Ⅱ类1分类错(牙合)畸形的颅面特征。②比较分析在尼泊尔人和中国人中,安氏Ⅱ类1分类错(牙合)畸形的各项头影测量指标。③探讨结果有无性别差异。方法:材料为135个安氏Ⅱ类1分类错(牙合)畸形病例(96个尼泊尔病例,39个中国病例)的标准头颅定位片。尼泊尔组(42个男,54个女)的平均年龄为16.34±5.4岁,中国组(12个男,27个女)平均年龄为17.18±7.1岁。观测指标包括10项骨性指标,9项牙性指标及3项软组织指标。对所得数据进行描述性统计学分析和独立样本T检验,确定两组间及性别之间的各项测量指标的平均差和标准差。当p≤0.05时,认为结果具有统计学意义。结果:在尼泊尔组和中国组这两组样本中,所选的22项头影测量项目中有15项的差异具有统计学意义。他们是:面角、颌凸角、GoMe-FH、GoGn-SN、Y轴角、平面角、SNA、ANB、U1-NA角、U1-NA距、L1-NB角、L1-NB距、N1-pr-pg1和Z角。在上下齿槽座角、GoMe-FH、GoGn-SN、SNA、N1-pr-pg1和N1-Sn-pg1等测量项目中,尼泊尔组具有明显的性别差异,而中国组只有下颌平面角才有显著的性别差异。结论:①相对于颅底,两组样本都表现为上颌位置正常,下颌后缩,上下颌关系为深覆盖。当进行两组样本间比较,中国组上颌前突,下颌后缩及深覆盖的表现强于尼泊尔组。②两组样本间比较,相对于基骨,尼泊尔组表现为明显的上、下颌切牙前突及颊向位,而中国组表现为明显下颌切牙前突及颊向位,及上颌切牙颊向位,无前突。③两组样本都表现为高角长面形、Ⅱ类面形和骨骼趋势。当进行两组间比较,中国组表现强于尼泊尔。④两组样本都表现为鼻尖及颏部前突及水平生长型,而尼泊尔组的表现强于中国组。⑤在性别间比较,尼泊尔组中有6项测量指标的差异具有统计学意义,而中国组只有1项。

BACKGROUND There is minimal objective data regarding adverse events related to endoscopic retrograde cholangio-pancreatography(ERCp)in patients with cirrhosis compared to those without cirrhosis and even fewer data comparing complications among cirrhosis patients based on severity of *** To determine if patients with cirrhosis are at increased risk of adverse events related to ERCp:mainly pancreatitis,bleeding,perforation,cholangitis,and mortality;And to see if higher Child-pugh(Cp)score and Model for End-Stage Liver Disease(MELD)score are associated with higher post-ERCp *** We performed a retrospective analysis of 692 patients who underwent ERCp and analyzed the impact of cirrhosis etiology,gender,type of sedation used during procedure,interventions performed,and co-morbidities on the rate of complications in cirrhosis patients as compared to non-cirrhosis *** Overall complications were higher in those with cirrhosis as compared to those without cirrhosis(p=0.015 at significance level of 0.05).Cp class,especially Cp class C,was shown to be associated with a significantly higher rate of ERCp complications as compared to Cp class A and Cp class B(p=0.010 at significance level of 0.05).CONCLUSION The results of our study reaffirm that liver cirrhosis has an impact on the occurrence of complications during *** study shows that Cp class seems to be more reliable as compared to MELD score in predicting complications of ERCp in cirrhosis patients.

背景：近二十余年,心血管病循证医学研究取得长足发展,以循证医学证据为基础编写的各种指南应运而生,但循证医学证据和指南与临床实践之间有较大差距。临床路径是根据指南制定的某种疾病诊治的标准化方案,旨在优化和畅通患者的诊疗程序,其在西方发达国家应用证实能规范医疗行为和改善医疗质量。目的：调查中南大学湘雅三医院在急性冠脉综合征的诊治中实施临床路径前后的情况,并予以分析评价,找出临床工作需要改进的地方。方法：首先我们成立了科室急性冠脉综合征临床路径质量控制小组,由主任担任组长,由一名副主任医师作为专职项目协调人,负责临床路径的实施,全体管组医师和总住院医生参与实行临床路径;然后连续入选从2009年12月开始在本院住院确诊ACS的50例患者的临床数据作为基线资料;全科医生通过学习卫生部急性冠脉综合征的临床路径,寻找差距和改进方法,制定适宜我科医疗环境的诊疗流程图,参照调整后的临床路径,第一周期继续连续收集50例确诊ACS患者的临床数据;与基线资料对比,寻找问题症结,改进实施方法,第二周期再连续收集50例确诊ACS患者的临床数据进行比较,如此周而复始至2011年12月共四个周期,并将临床数据与全国部分“三甲医院”资料进行比较分析。结果：1.与基线水平相比,实施临床路径后本院患者症状出现12小时内到达医院的STEMI患者比例升高(ppp>0.05),本院氯比格雷和他汀类药物使用比例高于全国医院平均水平。3.本院实施临床路径前后STEMI患者的再灌注治疗时间(平均入院到溶栓时间和入院到球囊扩张时间)差异无统计学意义(p>0.05),比指南规定的时间长,但平均入院到溶栓时间短于全国医院平均水平。***及NSTEMI患者的TIMI危险评分与冠脉介入治疗的实施呈负相关(r=-0.312, pp>0.05)。结论：1.临床路径的实施促进了本院急诊科和心内科的有效衔接,规范了本院心肌标志物检测方法。2.临床路径的实施可能改善了患者的临床预后。3.与低危UA及NSTEMI患者比较,高危患者接收介入治疗比例低。4.本院的临床实践和心血管病指南间仍有一定差距,需要改进。
