Chinese herbal medicine decreases incidence of hepatocellular carcinoma in diabetes mellitus patients with regular insulin management

BACKGROUND Type 2 diabetes mellitus(DM)is an independent risk factor for hepatocellular carcinoma(HCC),while insulin is a potent mitogen.Identifying a new therapeutic modality for preventing insulin users from developing HCC is a critical goal for researchers.AIM To investigate whether regular herbal medicine use can decrease HCC risk in DM patients with regular insulin control.METHODS We used data acquired from the Taiwan,Chinaese National Health Insurance research database between 2000 and 2017.We identified patients with DM who were prescribed insulin for>3 months.The herb user group was further defined as patients prescribed herbal medication for DM for>3 months per annum during RESULTS We initially enrolled 657144 DM patients with regular insulin use from 2000 to 2017.Among these,46849 patients had used a herbal treatment for DM,and 140547 patients were included as the matched control group.The baseline variables were similar between the herb users and nonusers.DM patients with regular herb use had a 12%decreased risk of HCC compared with the control group[adjusted hazard ratio(aHR)=0.88,95%CI=0.80–0.97].The cumulative incidence of HCC in the herb users was significantly lower than that of the nonusers.Patients with a herb use of>5 years cumulatively exhibited a protective effect against development of HCC(aHR=0.82,Pbination of single herbs.Meanwhile,Ji Sheng Shen Qi Wan plus Dan Shen was the most frequent combination of herbs and formulas.CONCLUSION This large-scale retrospective cohort study reveals that herbal medicine may decrease HCC risk by 12%i

Impact of short-term ambient air pollution exposure on the risk of severe COVID-19

Ecological studies suggested a link between air pollution and severe COVID-19 outcomes,while studies accounting for individual-level characteristics are limited.In the present study,we aimed to investigate the impact of short-term ambient air pollution exposure on disease severity among a cohort of 569 laboratory confirmed COVID-19 patients admitted to designated hospitals in Zhejiang province,China,from January 17 to March 3,2020,and elucidate the possible biological processes involved using transcriptomics.Compared with mild cases,severe cases had higher proportion of medical conditions as well as unfavorable results in most of the laboratory tests,and manifested higher air pollution exposure levels.Higher exposure to air pollutants was associated with increased risk of severe COVID-19 with odds ratio(OR)of 1.89(95%confidence interval(CI):1.01,3.53),2.35(95%CI:1.20,4.61),2.87(95%CI:1.68,4.91),and 2.01(95%CI:1.10,3.69)for PM_(2.5),PM_(10),NO_(2)and CO,respectively.OR for NO_(2)remained significant in two-pollutant models after adjusting for other pollutants.Transcriptional analysis showed 884 differentially expressed genes which mainly were enriched in virus clearance related biological processes between patients with high and low NO_(2)exposure levels,indicating that compromised immune response might be a potential underlying mechanistic pathway.These findings highlight the impact of shortterm air pollution exposure,particularly for NO_(2),on COVID-19 severity,and emphasize the significance in mitigating the COVID-19 burden of commitments to improve air quality.

BACKGROUND Testicular torsion is the most common acute scrotum worldwide and mainly occurs in children and adolescents.Studies have demonstrated that the duration of symptoms and torsion grade lead to different outcomes in children diagnosed with testicular torsion.AIM To predict the possibility of testicular salvage(TS)in patients with testicular torsion in a tertiary center.METHODS We reviewed the charts of 75 pediatric patients with acute testicular torsion during a 12-year period from November 2011 to July 2023 at the Suzhou Hospital of Anhui Medical University.Univariate and multivariate logistic regression analyses were used to determine independent predictors of testicular torsion.The data included clinical findings,physical examinations,laboratory data,color Doppler ultrasound findings,operating results,age,presenting institution status,and follow-up results.RESULTS Our study included 75 patients.TS was possible in 57.3%of all patients;testicular torsion occurred mostly in winter,and teenagers aged 11-15 years old accounted for 60%.Univariate logistic regression analyses revealed that younger age(P=0.09),body mass index(P=0.004),torsion angle(P=0.013),red blood cell count(P=0.03),neutrophil-to-lymphocyte ratio(P=0.009),and initial presenting institution(P<0.001)were associated with orchiectomy.In multivariate analysis,only the initial presenting institution predicted TS(P<0.05).CONCLUSION The initial presenting institution has a predictive value for predicting TS in patients with testicular torsion.Children with scrotal pain should be admitted to a tertiary hospital as soon as possible.

A Review of Intelligent Configuration and Its Security for Complex Networks

Complex networks are becoming more complex because of the use of many components with diverse technologies. In fact, manual configuration that makes each component interoperable has breed latent danger to system security. There is still no comprehensive review of these studies and prospects for further research. According to the complexity of component configuration and difficulty of security assurance in typical complex networks, this paper systematically reviews the abstract models and formal analysis methods required for intelligent configuration of complex networks, specifically analyzes, and compares the current key technologies such as configuration semantic awareness, automatic generation of security configuration, dynamic deployment, and verification evaluation. These technologies can effectively improve the security of complex networks intelligent configuration and reduce the complexity of operation and maintenance. This paper also summarizes the mainstream construction methods of complex networks configuration and its security test environment and detection index system, which lays a theoretical foundation for the formation of the comprehensive effectiveness verification capability of configuration security. The whole lifecycle management system of configuration security process proposed in this paper provides an important technical reference for reducing the complexity of network operation and maintenance and improving network security.

Hydralazine represses Fpn ubiquitination to rescue injured neurons via competitive binding to UBA52

A major impedance to neuronal regeneration after peripheral nerve injury (PNI) is the activation of various programmed cell death mechanisms in the dorsal root ganglion. Ferroptosis is a form of programmed cell death distinguished by imbalance in iron and thiol metabolism, leading to lethal lipid peroxidation. However, the molecular mechanisms of ferroptosis in the context of PNI and nerve regeneration remain unclear. Ferroportin (Fpn), the only known mammalian nonheme iron export protein, plays a pivotal part in inhibiting ferroptosis by maintaining intracellular iron homeostasis. Here, we explored in vitro and in vivo the involvement of Fpn in neuronal ferroptosis. We first delineated that reactive oxygen species at the injury site induces neuronal ferroptosis by increasing intracellular iron via accelerated UBA52-driven ubiquitination and degradation of Fpn, and stimulation of lipid peroxidation. Early administration of the potent arterial vasodilator, hydralazine (HYD), decreases the ubiquitination of Fpn after PNI by binding to UBA52, leading to suppression of neuronal cell death and significant acceleration of axon regeneration and motor function recovery. HYD targeting of ferroptosis is a promising strategy for clinical management of PNI.

Garnet-type solid-state batteries(SSBs)are considered to be one of the most promising candidates to realize next-generation lithium metal batteries with high energy density and safety.However,the dendrite-induced short-circuit and the poor interfacial contact impeded the practical application.Herein,interface engineering to achieve low interfacial resistance without high temperature calcination was developed,which Li_(6.4)La_(3)Zr_(1.4)Ta_(0.6)O_(12)(LLZTO)was simply coated with complex hydride(Li_(4)(BH_(4))_(3)I(3L1L))in various mass ratios n(Li_(4)(BH_(4))_(3)I)-(100−n)LLZTO(10≤n≤40).The interfacial conductivity increases by more than three orders of magnitude from 8.29×10^(−6)S·cm^(−1)to 1.10×10^(−2)S·cm^(−1).Symmetric Li cells exhibit a high critical current density(CCD)of 4.0 mA·cm^(−2) and an excellent cycling stability for 200 h at 4.0 mA·cm^(−2).SSBs with polymeric sulfur-polyacrylonitrile(SPAN)cathode achieve a high discharge capacity of 1149 mAh·g^(−1) with a capacity retention of 91%after 100 cycles(0.2 C).This attempt guides a simple yet efficient strategy for obtaining a stable Li/LLZTO interface,which would promote the development of solid-state batteries.

海洋内部的大幅内孤立波可能会威胁到潜器的航行稳定性和安全性,为探究潜器迎面遭遇内孤立波时的运动特性,建立两层流中细长潜器结构在内孤立波作用下的大幅运动时域求解模型。考虑潜器各运动自由度耦合引起的非线性作用力,潜器受到的内孤立波力采用Morison方程和Froude-Krylov力计算,由内孤立波传播引起的静力变化通过对潜器表面静压强积分计算。通过与计算流体力学数值仿真结果和物理模型试验结果对比验证模型的精度,并计算SUBOFF潜器在两层流中不同潜深、不同航速时迎面遭遇KdV型内孤立波后的运动轨迹和姿态。数值仿真结果表明:内孤立波作用下,位于密度分层界面上方和分层界面处的潜器沿内孤立波传播方向运动,位于密度分层界面下方的潜器逆着内孤立波传播方向运动;垂向上潜器会发生大幅掉深,且先下沉再上浮;不同潜深、航速下潜器表现出不同的运动轨迹和姿态。

Numerical examination of wave power absorption by the Edinburgh Duck wave energy converter device

A two-dimensional computational fluid dynamics(CFD)numerical model is developed to study the power takeoff(PTO)efficiency of an Edinburgh duck wave energy converter(WEC)device by using the OpenFOAM source package.After the numerical validations and the convergent verifications,the characteristics of the duck WEC device for the power takeoff in various incident wave periods are examined.The present numerical investigation indicates that the maximum absorbed power of the duck WEC device is attained in its resonant period,while the maximum efficiency is achieved in a lower period.The influence of the damping coefficient on the PTO is also investigated.The optimal damping coefficient for acquiring more power and better efficiency is determined from a numerical examination.

为了厘清我国住房租赁政策的变迁规律和作用机理,对1998—2023年的63份中央层面住房租赁政策文件进行文本分析,研究发现:住房租赁政策变迁存在三个均衡期和两个间断点,呈现出政策目标不断扩面、政策主体逐渐多元、政策工具使用结构合理的趋势。政策目标由注重住房租赁保障转向市场与保障并重,住房供给主体从政府包揽转变为多方参与。在前两个政策均衡期内,政府为实现管控效果偏重于使用强制型政策工具,在第三个政策均衡期则更多运用混合型政策工具。注意力作为中介变量,受内外情境结构因素影响推动了政策变迁。政府系统回应和冲突扩大机制对两次政策间断起到关键作用,政府在解决夹心层群体和新市民住房问题过程中赋予住房租赁政策以新的意义。未来应注重住房租赁政策目标接续,发挥政策工具的叠加效应,重视保障性租赁住房建设并规范住房租赁市场,确保租赁住房供需均衡。

基于映像理论将部分反射直墙前物体的散射问题,等效于开敞水域中原物体的散射和关于直墙映像体散射的线性叠加进行求解。采用高阶边界元方法建立了部分反射直墙前二维任意形状物体波浪绕射和辐射问题的数值分析模型,通过与已发表的海底方箱和淹没圆柱结果的对比验证了数值模型的准确性。应用该模型研究了直墙反射系数幅值及相位、方箱与墙间距离等参数对水面方箱上波浪激振力、附加质量和辐射阻尼的影响。结果表明:直墙反射系数幅值越大,波浪激振力、附加质量和辐射阻尼的波动越大,附加质量在一些频率下出现负值;相位角的变化会改变波浪激振力、附加质量和辐射阻尼曲线的偏移,在低频区对升沉附加质量有显著影响;方箱距离直墙越远,方箱上的波浪激振力、附加质量和辐射阻尼随波数振荡的频率越快,峰值频率向低频侧移动。
