Diagnostic accuracy of the multi-target stool DNA test in detecting colorectal cancer:A hospital-based study

BACKGROUND The multi-target stool DNA test(MT-sDNA)has potential utility in the detection of colorectal cancer(CRC),but validation of its clinical accuracy has been limited in China.AIM To evaluate the diagnostic performance of MT-sDNA and investigate the combined diagnostic value of alpha-fetoprotein(AFP),carcinoembryonic antigen(CEA),and carbohydrate antigen 199(CA199)with MT-sDNA in CRC and adenomas.METHODS We evaluated the performance of the MT-sDNA kit based on a hospital clinical trial.In this case-control study,135 participants from the Affiliated Hospital of Medical School of Ningbo University,including 51 CRC patients,23 patients with adenomas,and 61 healthy controls were enrolled.We used a risk scoring system to determine the positivity of tests with histological diagnosis or colonoscopy as the reference standard.RESULTS The main indices of sensitivity,specificity and accuracy were evaluated.The sensitivity and specificity for CRC detection were 90.2%and 83.3%,respectively,with an accuracy of 89.8%.For adenoma,the sensitivity and specificity were 56.5%and 68.9%,respectively,with an accuracy of 73.1%.The sensitivity and specificity of MT-sDNA combined with CEA in the diagnosis of adenoma were 78.3%and 60.7%,respectively.CONCLUSION The MT-sDNA test showed better performance in the detection of CRC,which was superior to AFP,CEA,and CA199 separately,but not for predicting adenomas.The combination of MT-sDNA with CEA further improved the sensitivity for adenoma diagnosis.

Effects of radix curcumae-derived diterpenoid C on Helicobacter pylori-induced inflammation and nuclear factor kappa B signal pathways

AIM:To study effect of diterpenoid C extracted from radix curcumae on Helicobacter pylori(H.pylori)-infected inflammation,intestinal metaplasia,and nuclear factor kappa B(NF-κB)signaling pathway in vitro.METHODS:We used I-type H.pylori to infect human gastric epithelial gastric epithelium cell line(GES-1)cell lines,and then H.pylori-infected GES-1 cells were treated with radix curcumae(RC)-derived diterpenoid C of different concentrations(5,10,20μg/mL)and amoxicillin.The expression of p65,IκB kinase(IKK)αand IKKγproteins was detected with Western blotting,and the expression of interleukin(IL)-8,IL-6 and IL-4 was determined with enzyme-linked immunosorbent assay method.Data were analyzed using SPSS software ver18.0.For comparisons between groups of more than two unpaired values,one-way analysis of variance(ANOVA)was used.If an ANOVA F value was significant,post hoc comparisons were performed between groups.If results were not normally distributed,the Mann-Whitney U test was used to compare two groups of unpaired values,whereas for comparisons between groups of more than two unpaired values,the Kruskal-Wallis H test was used.Statistical significance was established at P<0.05.RESULTS:The MTT assay results revealed the inhibited rate of GES-1,and indicated that the IC5 of RCderived diterpenoid C and amoxicillin all were 5μg/mL for gastric GES-1 cells.The expression of IL-8 was significantly increased,especially at 12 h time point;and the expression of IL-4 was decreased in H.pyloriinfected GES-1 cells.After H.pylori-infected GES-1 cells were treated with RC-derived diterpenoid C of different concentrations and amoxicillin,the expression of IL-8was decreased at 12,24,48,72 h points(P<0.01),especially in high-concentration diterpenoid C(20μg/mL)group;and the expression of IL-4 was increased,especially in moderate and high-concentration diterpenoid C(10 and 20μg/mL)groups.RC-derived diterpenoid C had the inhibitory effects on H.pylori-induced p65 translocation from cytoplasm into cell nucleus,H.pylori-stimulant IkBαdegr

Studies on the separation treatment of high-level liquid waste by bisamide podand(Ⅰ):Extraction and separation of An(Ⅲ)from Ln(Ⅲ)

A process for actinide(Ⅲ)and lanthanum(Ⅲ)extraction separation from high-level liquid waste(HLLW)was proposed,with N,N,N’,N’-tetraoctyl diglycolamide(TODGA)as the extractant,tri-n–butyl phosphate(TBP)as the phase modifier and 2,6-bis[1-(propan-1-ol)-1,2,3-triazol-4-yl]pyridine(PyTri-Diol or PTD)as hydrophilic stripping agent.This‘hot test’was successfully carried out,achieving 99.92%removal of americium-241(^(241)Am)with a separation factor SF(Eu/Am)of 3.8×10^(3)in the actinide(Ⅲ)product solution.The results show that bisamide podand extractants can effectively realize the extraction and separation of actinide(Ⅲ)and lanthanum(Ⅲ)from Chinese commercial HLLW and thus have a bright practical application potential for the treatment of commercial HLLW.

目的：分析某三级中医院医院获得性血流感染（nBSIs）的流行病学和病原学特征，探索影响28天病死率的相关危险因素。创新点：纳入2009-2011年所有确诊的血流感染病例，而非只依靠血培养结果做判定。收集包括患者的基础性疾病、侵入性操作等感染相关危险因素、标本送检及抗菌药物使用情况等全面的临床资料，分析了nBSIs的发生率、病区分布、人群分布、病原体分布、细菌耐药性及病死率，并对影响死亡的危险因素进行了多因素分析。方法：采用回顾性调查的方法，从病历资料中获得预先设置好的相关信息。病原体的分离鉴定和药敏检测由医院微生物实验室专职人员完成。使用WHONET5．6软件对药敏结果进行分析。采用SPSS16．0对所有数据进行统计分析，死亡危险因素判定先用单因素分析，再用多因素Logistic回归分析。结论：某三级中医院nBSIs的发生率为5．7／1000入院人数，大肠埃希菌和凝固酶阴性的葡萄球菌（CONS）是检出最多的两种细菌。约有1／3的肠杆菌对第三代头孢菌素耐药（表4），90．7％的CoNS对甲氧西林耐药（表3）。碳青霉烯类和糖肽类是用于治疗nBSIs使用最广泛的两类抗生素（图1）。感染性休克、血液透析、Pitt菌血症得分〉4和尿路感染是28天死亡的危险因素，而合理的经验性用药可以明显改善预后（表6）。

Case report of primary splenic angiosarcoma with hepatic metastases

Primary splenic angiosarcoma(PSA) is the most unusual type of malignancy with early multifocal metastasis through hematogenous spread. PSA is generally believed to originate from splenic sinusoidal vascular endothelium with a high rate of metastasis and to have a poor prognosis. Its etiology and pathogenetic mechanisms have not yet been clearly described. Thus far, only approximately 200 cases have been reported. PSA has variable symptomatology with the potential to present with life-threatening complications. The diagnosis of PSA is challenging; and often late. PSA should be considered in the differential diagnosis of patients with splenomegaly and anemia of unknown etiology. Surgical treatment with splenectomy is considered the only curative intervention for potential long-term disease-free survival. Early diagnosis and treatment are very important. It is important that clinical doctors improve the understanding of PSA. Herein, we report one rare case of PSA with hepatic metastases, along with a review of the current literature.

Interaction between systemic iron parameters and left ventricular structure and function in the preserved ejection fraction population:a two-sample bidirectional Mendelian randomization study

BACKGROUND Left ventricular(lv)remodeling and diastolic function in people with heart failure(HF)are correlated with iron status;however,the causality is uncertain.This Mendelian randomization(MR)study investigated the bidirectional causal relationship between systemic iron parameters and lv structure and function in a preserved ejection fraction population.METHODS Transferrin saturation(TSAT),total iron binding capacity(TIBC),and serum iron and ferritin levels were extracted as instrumental variables for iron parameters from meta-analyses of public genome-wide association studies.Individuals without myocardial infarction history,HF,or lv ejection fraction(lvEF)lv end-diastolic volume,lv endsystolic volume,lv mass(lvM),and lvM-to-end-diastolic volume ratio(lvMVR).We used a two-sample bidirectional MR study with inverse variance weighting(IVW)as the primary analysis method and estimation methods using different algorithms to improve the robustness of the results.RESULTS In the IVW analysis,one standard deviation(SD)increased in TSAT significantly correlated with decreased lvMVR(β=-0.1365;95%confidence interval[CI]:-0.2092 to-0.0638;P=0.0002)after Bonferroni adjustment.Conversely,no significant relationships were observed between other iron and lv parameters.After Bonferroni correction,reverse MR analysis showed that one SD increase in lvEF significantly correlated with decreased TSAT(β=-0.0699;95%CI:-0.1087 to-0.0311;P=0.0004).No heterogeneity or pleiotropic effects evidence was observed in the analysis.CONCLUSIONS We demonstrated a causal relationship between TSAT and lv remodeling and function in a preserved ejection fraction population.

Mechanism of aqueous fructus aurantii immaturus extracts in neuroplexus of cathartic colons

AIM: To examine the effect of aqueous fructus aurantii immaturus(FAI) extracts on the intestinal plexus of cathartic colons.METHODS: Cathartic colons were induced in rats with dahuang,a laxative used in traditional Chinese medicine. Once the model was established(after approximately 12 wk),rats were administered mosapride(1.54 mg/kg) or various doses of aqueous FAI extracts(1-4 g/kg) for 14 d. Transit function was assessed using an ink propulsion test. Rats were then sacrificed,and the ultramicrostructure of colonic tissue was examined using transmission electron microscopy. The expression of the 5-hydroxytryptamine receptor 4(5-HTR4) and neurofilament-H was assessed in colon tissues using real-time PCR,Western blot,and immunohistochemistry.RESULTS: Mosapride and high dose(4 g/kg) of aqueous FAI extracts significantly improved the bowel movement in cathartic colons compared to untreated model colons as measured by the intestinal transit rate(70.06 ± 7.25 and 72.02 ± 8.74,respectively,vs 64.12 ± 5.19; P < 0.05 for both). Compared to controls,the ultramicrostructure of cathartic colons showed signsof neural degeneration. Treatment with mosapride and aqueous FAI extracts resulted in recovery of ultrastructural pathology. Treatment with mosapride alone upregulated the gene and protein expression of 5-HTR4 compared to untreated controls(P < 0.05 for both). Treatment with aqueous FAI extracts(≥ 2 g/kg) increased 5-HTR4 m RNA levels(P < 0.05),but no change in protein level was observed by Western blot or immunohistochemistry. The m RNA and protein levels of neurofilament-H were significantly increased with mosapride and ≥ 2 g/kg aqueous FAI extracts compared to controls(P < 0.05 for all).CONCLUSION: Aqueous FAI extracts and mosapride strengthen bowel movement in cathartic colons via increasing the expression of 5-HTR4 and neurofilament-H.

In the context of the gradual popularity of electric vehicles(EVs),the development of lithium battery systems with high energy density and power density is regarded as the foremost way to improve the range of EVs.LiNi_(1-x-y)Co_(x)Mn_(y)O_(2)(NCM)cathodes have been the focus of researchers due to their high energy density,excellent power performance,and low-temperature resistance.However,the elaboration of the decay mechanism of NCM cathode based on lithium metal batteries(LMBs)is still being restricted to the primary level.In the past decades,the development and application of advanced in-situ characterization tools have facilitated researchers'understanding of the internal operation mechanism of batteries during charging and discharging.In this minireview,the latest progress of in-situ observation of the NCM cathode by X-ray diffraction(XRD),fourier transform infrared(FT-IR)spectroscopy,Raman spectroscopy,atomic force microscopy(AFM),transmission electron microscope(TEM),optical microscope,and other characterization tools is summarized.The mechanisms of structural degradation,cathode-electrolyte interfaces(CEIs)composition,and dynamic changes of NCM,electrolyte breakdown,and gas production are elaborated.Finally,based on the existing research progress,the opportunities and challenges for future in-situ characterization technology in the study of the mechanism of LMBs are discussed in depth.Therefore,the purpose of this minireview is to summarize recent work that focuses on the outstanding application of in-situ characterization techniques in the mechanistic study of LMBs,and pointing the way to the future development of high energy density and power density LMBs systems.

Point Cloud Classification Using Content-Based Transformer via Clustering in Feature Space

Recently, there have been some attempts of Transformer in 3D point cloud classification. In order to reduce computations, most existing methods focus on local spatial attention,but ignore their content and fail to establish relationships between distant but relevant points. To overcome the limitation of local spatial attention, we propose a point content-based Transformer architecture, called PointConT for short. It exploits the locality of points in the feature space(content-based), which clusters the sampled points with similar features into the same class and computes the self-attention within each class, thus enabling an effective trade-off between capturing long-range dependencies and computational complexity. We further introduce an inception feature aggregator for point cloud classification, which uses parallel structures to aggregate high-frequency and low-frequency information in each branch separately. Extensive experiments show that our PointConT model achieves a remarkable performance on point cloud shape classification. Especially, our method exhibits 90.3% Top-1 accuracy on the hardest setting of ScanObjectN N. Source code of this paper is available at https://***/yahuiliu99/PointC onT.

BACKGROUND Colorectal cancer(CRC)is a major global health burden.The current diagnostic tests have shortcomings of being invasive and low accuracy.AIM To explore the combination of intestinal microbiome composition and multi-target stool DNA(MT-sDNA)test in the diagnosis of CRC.METHODS We assessed the performance of the MT-sDNA test based on a hospital clinical trial.The intestinal microbiota was tested using 16S rRNA gene sequencing.This case-control study enrolled 54 CRC patients and 51 healthy controls.We identified biomarkers of bacterial structure,analyzed the relationship between different tumor markers and the relative abundance of related flora components,and distinguished CRC patients from healthy subjects by the linear discriminant analysis effect size,redundancy analysis,and random forest analysis.RESULTS MT-sDNA was associated with Bacteroides.MT-sDNA and carcinoembryonic antigen(CEA)were positively correlated with the existence of Parabacteroides,and alpha-fetoprotein(AFP)was positively associated with Faecalibacterium and Megamonas.In the random forest model,the existence of Streptococcus,Escherichia,Chitinophaga,Parasutterella,Lachnospira,and Romboutsia can distinguish CRC from health controls.The diagnostic accuracy of MT-sDNA combined with the six genera and CEA in the diagnosis of CRC was 97.1%,with a sensitivity and specificity of 98.1%and 92.3%,respectively.CONCLUSION There is a positive correlation of MT-sDNA,CEA,and AFP with intestinal microbiome.Eight biomarkers including six genera of gut microbiota,MT-sDNA,and CEA showed a prominent sensitivity and specificity for CRC prediction,which could be used as a non-invasive method for improving the diagnostic accuracy for this malignancy.
