A Binomial Model Approach: Comparing the R0 Values of SARS-CoV-2 rRT-PCR Data from Laboratories across Northern Cyprus

Northern Cyprus has implemented relatively strict measures in the battle against the outbreak of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The measures were introduced at the beginning of the COVID-19pandemic, in order to prevent the spread of the disease. One of these measures was the use of two separate realtime reverse transcription polymerase chain reaction (rRT-PCR) tests for SARS-CoV-2 referred to as the doublescreening procedure, which was adopted following the re-opening of the sea, air and land borders for passengersafter the first lockdown. The rRT-PCR double screening procedure involved reporting a negative rRT-PCR testwhich was carried in 72 to 120 h before departure whilst presenting no known symptoms of the COVID-19and performing a second rRT-PCR test at the point of arrival. This study compares the results of SARS-CoV-2rRT-PCR tests performed on incoming flight passengers from the 1st July to 9th of September 2020 to NorthernCyprus. The rRT-PCR test results collected by the Near East University (NEU) DESAM COVID-19 laboratory werecompared with the rRT-PCR test results collected by the Ministry of Health and/or private COVID-19 laboratoriesin Northern Cyprus. This comparative study was conducted using binomial distribution. In addition, by applyingthe Susceptible-Exposed-Infected-Removed (SEIR) model to Northern Cyprus, overall basic reproduction number(R0) value of the COVID-19 was analysed for the same time period to act as a threshold for this *** both the statistical and SEIR mathematical model, R0 was calculated. It was assumed that, the more similarthe R0 results of NEU DESAM COVID-19 laboratory and other laboratories were with the overall R0 value ofNorthern Cyprus, the more reliable the results would be. Exergy is the amount of work obtainable when some matter is brought to a state of thermodynamic equilibrium with the common components of the natural surroundings by means of reversible processes, involving interaction only with the above mentioned components of nature. This paper presents standard chemical exergy values for 85 elements. Reference species in the atmosphere (air), dissolved in the hydrosphere (oceans), and contained in the lithosphere (minerals) are used for these calculations. Standard chemical exergy values of elements were calculated from tabulated values obtained for standard conditions (an ambient temperature of 298.15 K and an atmospheric pressure of 0.1 MPa). Very low concentrations of elements in the atmosphere and oceans and the abundance of elements in the Earth′ s crust are no longer used in determining reference states for chemical elements. Liquid and gas mixtures generally are not useful as reference states. As a result of the work in this paper, a table of the chemical exergy values of many elements in the periodic table under standard conditions was tabulated.

目的：通过针刺常规穴和宁心安神穴治疗更年期妇女肥胖的临床研究,观察针刺对于减肥的作用,并进一步探讨宁心安神穴对于更年期妇女减肥的功效。方法：将60例患者随机分为2组：常规治疗A组和“宁心安神”B组。A组患者针刺“曲池、天枢、阴陵泉、丰隆、太冲、内庭、三阴交”,B组患者在A组常规治疗穴位的基础上加“安眠、神庭、神门、期门、阳陵泉”,均针刺治疗30次,分别测量和统计治疗前后体重、BMI、腰臀比;并采用麦克奈尔制定的情绪状态量表(POMS),统计分析针刺不同穴位对于更年期妇女精神情绪的影响。结果：(1)通过两组治疗前后体重、BMI及腰臀比的观察对比,经过组间方差分析,A组治疗前后P=0.000,B组治疗前后P=0.000,A、B两组体重变化均有统计学意义(P0.05,无统计学意义。(2)B组患者增加“宁心安神”穴位后,精神情绪均得到明显改善,对于患者改善肥胖有积极作用。结论：针刺对于更年期妇女肥胖有明显改善作用,同时针刺“宁心安神”穴位可明显改善患者精神情绪,对于改善更年期肥胖有积极作用。

Computational Simulations of Bone Remodeling under Natural Mechanical Loading or Muscle Malfunction Using Evolutionary Structural Optimization Method

Live bone inherently responds to applied mechanical stimulus by altering its internal tissue composition and ultimately biomechanical properties, structure and function. The final formation may structurally appear inferior by design but complete by function. To understand the loading response, this paper numerically investigated structural remodeling of mature sheep femur using evolutionary structural optimization method (ESO). Femur images from Computed Tomography scanner were used to determine the elastic modulus variation and subsequently construct finite element model of the femur with stiffest elasticity measured. Major muscle forces on dominant phases of healthy sheep gait were imposed on the femur under static mode. ESO was applied to progressively alter the remodeling of numerically simulated femur from its initial to final design by iteratively removing elements with low strain energy density (SED). The computations were repeated with two different mesh sizes to test the convergence. The elements within the medullary canal had low SEDs and therefore were removed during the optimization. The SEDs in the remaining elements varied with angle around the circumference of the shaft. Those elements with low SED were inefficient in supporting the load and thus fundamentally explained how bone remodels itself with less stiff inferior tissue to meet load demand. This was in line with the Wolff’s law of transformation of bone. Tissue growth and remodeling process was found to shape the sheep femur to a mechanically optimized structure and this was initiated by SED in macro-scale according to traditional principle of Wolff’s law.

Background: After a successful primary percutaneous transluminal coronary angioplasty (PTCA)/stent placement, performing bypass surgery to an artery with a patent stent during semiurgent coronary artery bypass grafting (CABG) surgeries may protect the patient against ischemic complications due to acute stent occlusion. This issue, however, is controversial. Loss of bypass graft function may occur in the early stages due to the competition between native flow and graft flow. In the present study, the effects of bypass surgery to an artery with a patent stent on the outcomes were investigated. Methods: Fifty nine patients who had undergone isolated CABG surgery within 6 weeks after a successful primary PTCA/stent placement were included in the study. Patients who had undergone emergency operation due to suspicion of acute stent occlusion and the redo cases were excluded in the study. Patients were divided into two groups;those with (Group P, protected) and without (Group NP;non-protected) coronary bypass to an artery with a patent stent. Preoperative, operative and postoperative data were collected retrospectively from the patient files. The need for urgent revascularization was recorded postoperatively. Mann-Whitney U test was used for the analysis of continuous variables, whereas categorical variables were analyzed either using Pearson's chisquare test or Fisher's exact test. Results: Of 59 patients, 35 underwent bypass surgery to the artery with patent stent, whereas bypass was not performed on 24 patients. It was observed that dual antiplatelet therapy had been performed on 20% of the patients in Group P and 87.5% of those in Group NP (p = 0.001). No acute ischemic change due to stent occlusion was determined in Group P, whereas it was observed in one patient in Group NP (p = 0.4). The prevalence of hospital mortality was found to be 8.6% and 8.3% in Group P and Group NP, respectively (p = 1). Conclusions: Although it was not statistically significant, it was obse
