Diagnosis of asymptomatic atrial septal aneurysms using two-dimensional color Doppler and contrast transthoracic echocardiography

Objective To evaluate the dimensions of atrial septal aneurysm (ASA), the presence and characteristics of interatrial shunt, the movement of the wall of the aneurysm, and correlation between these findings and sign and/or symptoms suggesting embolism in Manisa, a district of a western Anatolian city of Turkey. Methods Two thousand five hundred cases were examined by routine transthoracic echocardiography (TTE) in both pediatric and adult cardiology outpatient clinics. ASA was detected in 20 cases and evaluated by two-dimensional color Doppler echocardiography (CDE). The length of the base, the maximum radius and the maximum displacement of ASA were measured. The shunt between the atria was examined by CDE. In cases where a shunt could not be found, galactose and palmitic acid was injected. Standard 12-lead electrocardiogram (ECG) and exercise stress test were also performed. Results No clinical signs or symptoms were found, suggesting a systemic or cerebral embolism. The maximum displacement of ASA was between 2 and 5 mm. All of the aneurysms were localized in the right atrium, and the walls of the aneurysm did not move beyond the base of the left atrium during the maximum displacement. Interatrial shunt was detected in 14 of 20 patients (70%) by CDE and in the remaining six cases by contrast TTE. frequent ventricular ectopic beats were observed in one patient. conclusions During routine TTE we observed 0.8% asymptomatic ASA in our population. The use of a contrast agent was found to be a valuable additional method in patients with ASA when the shunt could not be detected by CDE. The risk for embolism is not high when the maximum displacement of the wall of ASA was 5 mm or less and no bulge into the left atrium was observed. Based on our experience with this method, TTE is easy to perform, well-tolerated and acceptable.

Stiffness of In-Situ formed Interleaving Polymeric Nanofiber-Epoxy Nanocomposites

This study proposes a facile, but precise method to back-calculate the effective modulus of nanocomposite interleaving plies. Adaptation of a conventional dry-reinforcement resin film infusion (RfI) approach allows interleaving neat epoxy layers (NE) with the epoxy-infused nanofibrous plies (XE) of constant thickness. The final cured nanocomposite laminate thus has the form (NE/XE)n, where “n” denotes the number of the repeats and enables clear distinction of the nanocomposite interlayers through the thickness. Mechanical testing of neat epoxy and laminated nanocomposite specimens can be coupled with the classical lamination theory for back-calculating in-plane elastic modulus of the individual epoxy-infused nanofibrous plies (EXE). finite element analysis (fEA) and testing the laminated nanocomposite subject to flexural loading (3-point bending) are proposed to validate the analytically back-calculated EXE. It is shown that the fEA prediction incorporating EXE and testing for flexural modulus of (NE/XE)20 laminated nanocomposites correlate well and the results are within 5%. This finding suggests that the back-calculation scheme reported herein would be attractive for accurately determining the properties of an individual nanocomposite building block layer. The proposed framework is beneficial for modelling laminated structural composites incorporating XE-like nanocomposite interlayers.

目的:比较儿童患者中散瞳或不散瞳状态下使用Plusoptix A09摄影验光仪与普通自动验光仪的准确性。方法:共评估了90例患儿180眼的屈光状态。在散瞳或不散瞳状态下使用Plusoptix A09摄影验光仪检测屈光度,并与散瞳后使用普通自动验光仪测得的结果进行比较。使用Bland-Altman分析比较等效球镜、球镜度数、柱镜度数和柱镜轴J0、J45值。结果:患者年龄3~13(7.48±3.01)岁。未散瞳状态下的Plusoptix A09摄影验光仪检测所得球镜度数和等效球镜与散瞳后的普通自动验光仪结果之间存在显著差异(P0.05)。散瞳后Plusoptix A09摄影验光仪检测的球镜度数、等效球镜和散瞳后普通自动验光仪结果之间存在显著差异(P0.05)。Bland-Altman相关性分析显示在球镜度数、柱镜度数和等效球镜的测量中未散瞳及散瞳的Plusoptix A09检测结果与散瞳后的普通自动验光仪检测结果有很好的一致性,但在J0和J45测量上一致性较差。结论:为了检测儿童的屈光度,散瞳或不散瞳状态下Plusoptix A09可以得到可靠结果。但是在测量柱镜轴和高屈光度时,这项检测并不合适。这一设备是检测、筛查不合作儿童屈光度时的有效选择。

flexural Strength of Temporary Restorative Materials Stored in Different Solutions

Aim: The aim of this study was to evaluate the effects of different solutions on the flexural strength of one methacrylate-based resin and three bis-acryl resin provisional materials using an in vitro model test system. Methods: The specimens (25 × 2 × 2 mm) were prepared from interim materials and stored for 14 days at 37°C in different solutions (coffee, burn, cola) and distilled water (control group). A standard three-point bending test was conducted on the specimens with an Instron universal testing machine at crosshead speed of 0.5 mm/min. Data were analyzed using the one-way analysis of variance and Tukey HSD multiple comprasion tests. Results: The methacry-late-based resin (Takilon) showed the least fracture strength (61.6 - 85.6 MPa). One of the bis-acryl resins (Protemp 4-3M ESPE) showed the highest fracture strength (112 - 128 MPa). conclusions: All bis-acryl resin composite materials exhibited high fracture strength over the traditional methyl methacrylate resin throughout the 14 day time interval of investigation. The different solutions have not statistically significant effect on the flexure strength values on four temporary materials.

Attitude Stabilization of a Novel flying Robot by Dynamic compensation

This paper gives details about the controller design that aims to stabilize the novel twinrotor flying robot, Toruk. Toruk is an experimental test bench to study center of gravity steering, effect of the location of the center of gravity, controller design and implementation, etc. Physical components are also briefly discussed in this paper. Attitude dynamics of the system is inherently unstable. It is stabilized by a regulator. In addition, an observer is designed and utilized to estimate the unmeasured states. Thrust force generated by the propulsion unit is estimated by using the identified mathematical model of the unit. An experimental setup is employed to identify the mathematical model that expresses the relation between the applied input voltage to the propulsion unit and thrust produced by the propeller. Mathematical model for the attitude dynamics of Toruk is built. Then controllability and observability analysis are carried out for the system. Dynamic compensator composed of a state observer and a regulator, is designed on the mathematical model. Physical implementation on the system will be performed.

Effects of Sleep Disorders on Hemoglobin Alc Levels in Type 2 Diabetic Patients

Background： Studies have reported the presence of sleep disorders in approximately 50-70% of diabetic patients, and these may contribute to poor glycemic control, diabetic neuropathy, and overnight hypoglycemia. The aim of this study was to determine the frequency of sleep disorders in diabetic patients, and to investigate possible relationships between scores of these sleep disorders and obstructive sleep apnea syndrome （OSAS） and diabetic parameters （fasting blood glucose, glycated hemoglobin Alc [HbA lc], and lipid levels）. Methods： We used the Berlin questionnaire （BQ） for OSAS, the Epworth Sleepiness Scale （ESS）, and the Pittsburgh Sleep Quality Index （PSQI） to determine the frequency of sleep disorders and their possible relationships with fasting blood glucose, HbA I c, and lipid levels. Results： The study included 585 type 2 diabetic patients admitted to family medicine clinics between October and December 2014. Sleep, sleep quality, and sleep scores were used as the dependent variables in the analysis. The ESS scores showed that 54.40% of patients experienced excessive daytime sleepiness, and according to the PSQI, 64.30% experienced poor-quality sleep. The BQ results indicated that 50.20% of patients were at high-risk of OSAS. HbAlc levels correlated significantly with the ESS and PSQI results （r = 0.23, P 〈 0.001 and r = 0.14, P = 0.001, respectively）, and were significantly higher in those with high-risk of OSAS as defined by the BQ （P 〈 0.001 ）. These results showed that HbAlc levels were related to sleep disorders. conclusions： Sleep disorders are common in diabetic patients and negatively affect the control of diabetes. conversely, poor diabetes control is an important factor disturbing sleep quality. Addressing sleep disturbances in patients who have difficulty controlling their blood glucose has dual benefits： Preventing diabetic cornplications caused by sleep disturbance and improving diabetes control.

5-year follow-up of a patient with an impacted maxillary canine transplantation and a single-tooth implant:A case report

Background: Despite scientific and technological developments on dentistry, man is still losing their teeth due to several reasons. Tooth loss may result in esthetical and functional discrepancies. Although beginning of dental transplantation is quite old, it started to take attraction on scientific basis recently. In recent years, oral implants are being used nearly in all fields of dentistry. Purpose: The purpose of this study was to evaluate 5 year follow-up of a patient with an impacted maxillary canine transplantation and a single-tooth implant. Materials and Methods: In our study an impacted canine was removed and transplanted immediately. Thereafter the missing premolar was replaced by an implant retained cemented crown. The patient was evaluated clinically and radiographically. Clinical periodontal parameters and implant mobility was evaluated, marginal bone level was measured on radiographs. Results: No significant changes have been observed in these parameters and implant mobility between year one and year five of the study. conclusion: This study has demonstrated that stable long-term results can be achieved with replacement of single teeth with MIS Implant, and cemented crown on abutment with adjacent transplanted canine, with cumulative success rate for five years.

A Rare Case: Multiple Mandibular Arteriovenous Malformation with High Output: May a Tooth Extraction Alone Turn to a Catastrophic condition?

Background: Arteriovenous malformations (AVM) in mandible and neck are potentially life-threatening conditions because of possible massive hemorrhage. They are often first diagnosed by dentists as bleeding. In this case, we report how a tooth extraction turned to a catastrophic condition in a patient with multiple AVMs in the mandibular, submandibular and hyoid region. Case: A 19-year-old male patient diagnosed with AVMs in the mandibular and submandibular regions was admitted to our hospital with the complaint of leaky hemorrhage around the lower left 3rd molar tooth. Tooth extraction was planned after embolization procedure. The patient was intubated uneventfully using video laryngoscope. However, an acute bleeding of more than 1600 ml occurred in one minute just after the mouth opener was inserted. The patient was transferred to the Interventional Radiology and other branches of the high-flow AVM were embolized. The patient was taken back to the operating room and the tooth was extracted uneventfully. conclusion: In AVM cases, trauma should be minimized during induction of general anesthesia and intubation should be performed gently with fiberoptic bronchoscopy or video laryngoscopy. We conclude that a multidisciplinary approach as applied to our patient would be valuable in such cases.

Newly identified Au-Ag-Bi-Te mineralization in the Aydindere skarn fe and Cu deposit， Giresun， NE Turkey: implications of gold mineralization during retrograde skarn evolution

This study investigates the mineralogy and mineral-chemistry of a newly discovered Au-Ag-Bi-Te mineralization at the Aydindere fe-Cu skarn deposit， within the Pontides Tectonic Unit， northeastern Turkey. The mineralization is developed in the skarn zone at the contact between Upper Cretaceous andesite-pyroclastic rocks and a Paleocene-Eocene I-type granitoid. The principal ore minerals of the Aydindere fe-Cu deposit are oxides (magnetite)， sulfides (pyrite-chalcopyrite-galena-sphalerite)， tellurides/sulfotellurides (tetradymite， hessite)， sulfosalts (wittichenite， emplectite， aikinite) and native gold-electrum. Skarn minerals include anhydrous phases (garnet) formed in a prograde stage and hydrous phases (amphibole， epidote， chlorite)， which were formed in a retrograde stage in association with quartz， adularia， apatite and late calcite. Sulfides， tellurides and sulfosalts are introduced during the retrograde stage. The Au-Ag-Bi-Te mineralization was detected for the first time within the western ore body of the Aydindere deposit， and occurs in calcite-bearing sulfide bodies that cut magnetite-garnet-amphibole-epidote skarns with magnetite ores of different grade， including massive magnetite. Chlorite geothermometry indicates formation of the Au-Ag-Bi-Te mineralization at temperatures between 300 and 250 °C， during the retrograde skarn evolution. Assuming the temperature is ～275°C， logfS2= -10.5 to -13， logfO2= -37 to -33， and logfTe2values range from approximately -12 to -8.5 were estimated. The available mineralogical and geological data (presences of magnetite， oxidized-type tellurides/sulfotellurides， and andraditic garnets， and absence of pyrrhotite and arsenopyrite) suggest that Aydindere is an oxidized Au-bearing skarn deposit. The discovery of Au-Ag-Bi-Te mineralization at Aydindere increases its productivity and requires more detailed exploration in the deposit for precious (Au， Ag) and critical (Bi， Te) metals.
