Multiple paradoxical embolisms caused by central venous catheter thrombus passing through a patent foramen ovale: A case report

BACKGROUND To date,this is the first case of a paradoxical embolism(PDE)that concurrently manifested in the coronary and lower limb arteries and was secondary to a central venous catheter(CVC)thrombus via a patent foramen ovale(PFO).CASE SUMMARY Here,we report a case of simultaneous coronary and lower limb artery embolism in a PFO patient carrier of a CVC.The patient presented to the hospital with acute chest pain and lower limb fatigue.Doppler ultrasound showed a large thrombus in the right internal jugular vein,precisely at the tip of the CVC.Transthoracic and transesophageal echocardiography confirmed the existence of a PFO,with inducible right-to-left shunting by the Valsalva maneuver.The patient was administered an extended course of anticoagulation therapy,and then the CVC was successfully removed.Percutaneous PFO closure was not undertaken.There was no recurrence during follow-up.CONCLUSION Thus,CVC-associated thrombosis is a potential source for multiple PDE in PFO patients.

In this paper,■-dressing method based on a local 3×3 matrix ■-problem with non-normalization boundary conditions is used to investigate coupled two-component Kundu-Eckhaus equations.Firstly,we propose a new compatible system with singular dispersion relation,that is time spectral problem and spatial spectral problem of coupled two-component Kundu-Eckhaus equations via constraint equations.Then,we derive a hierarchy of nonlinear evolution equations by introducing a recursive operator.At last,by solving constraint matrixes,a spectral transform matrix is given which is sufficiently important for finding soliton solutions of potential function,and we obtain N-soliton solutions of coupled two-component Kundu-Eckhaus equations.

Dietary parboiled rice(PR)has a low risk of disease,but little is known about the contribution of PR to the prevention of hyperlipidemia.The potential role and underlying mechanisms of PR in hyperlipidemia were evaluated in this study.Male C57BL/6J mice were fed with a normal diet,high-fat diet(HFD)containing refined rice(HFDRR)or PR(HFDPR).It was found that PR intervention improved lipid accumulation in mice.Transcriptomic data analysis revealed that 27 genes were up-regulated(mostly involved in lipid breakdown)and 86 genes were down-regulated(mostly involved in inflammatory responses)in the HFDPR group compared to the HFDRR group.And 15 differentially expressed genes(DEGs)were validated by quantitative real-time PCR(RT-qPCR),while protein interaction network showed that protein tyrosine phosphatase receptor type C(PTPRC)has a central role.The gut microbiota of mice was also altered after different dietary treatments,with higher ratio of Firmicutes and Bacteroidetes,increased abundances of Ruminococcaceae,Lachnospiraceae,Christensenellaceae,Porphyromonadaceae,Rikenellaceae and Prevotellaceae,and decreased abundances of Lactobacillaceae,Peptostreptococcaceae,Erysipelotrichaceae and Actinobacteria in the HFDRR group.In addition,it was observed that PPAR signaling pathway may act as a bridge between DEGs and differential gut microbiota.These results suggested that PR can prevent hyperlipidemia by modulating liver genes and gut microbiota.

A two-terminal self-powered optoelectronic structure could mimic synaptic functions of a human brain synapse unit(Figure 1(a)) based on the photovoltaic effects [1], demonstrating the potential for decreasing energy consumption for future computing. However, current self-powered optoelectronic synaptic devices face issues of stability and cost,which mainly arise from noble metal electrodes and independent hole transport layers used in these structures. Substituting low-cost carbon for these metal electrodes and carrier transport layers may promise to solve these challenges.

The non-isothermal deformation of soft mudrocks or clay soils is one of the most critical issues in energy and environmental related geotechnics.Clay-related geomaterials hold complex microstructure and mineral composition,which brings difficulty in investigating their thermo-mechanical behaviors.Previous studies pay little attention to the difference between a thermal plastic strain and the strain from clay dehydration.In this study,a new constitutive model is proposed for describing the thermoelastoplastic behaviors of clayey soils under water-saturated condition.The effect of temperature variation and mechanical loading on elastoplastic strains and dehydration are investigated.The thermodynamics laws and the unconventional plasticity are applied to quantify the thermo-mechanical behavior.The irreversible strain is captured by using Cam-Clay plasticity and subloading yield surface concept.The dehydration strain is described by utilizing a novel method based on generalized thermodynamics approach and Helmholtz free energy function.The internal variables,and the first and second laws of thermodynamics are applied in the model.The hardening rule is established by implementing the laws of physical conservation,energy dissipation,and plastic flow.The proposed model is validated using specially designed thermal consolidation tests on laboratory prepared heavily consolidated clayey soils and some published data of clayey soils with different geological origins.

With the development of smart home energy management technology,prosumers are endowed with increased initiative in peer-to-peer(P2P)transactions,bringing new potential for cost savings.In this study,a novel strategic P2P energy trading framework is proposed considering the impact of network constraints on personal transaction strategies.Prosumers can estimate the allowed power injection before engaging in the P2P energy trading,which is solved in a distributed manner based on the sharing form alternating direction method of multipliers(ADMM)algorithm.To quantify the network usage cost for each prosumer and promote local transactions among prosumers at the same bus,a modified continuous double auction(CDA)matching algorithm is proposed including a transaction fee.An adaptive aggressiveness-based bidding strategy is generated considering the risk of uncertainty in real-time energy delivery amount under the limitations of the distribution network.The proposed strategic P2P energy trading framework is tested with the IEEE 37-bus distribution network and it is effective in creating profits for prosumers and supporting distribution network operations.

Aiming at solving the problem of high global warming potential of R134 a,a new mixed refrigerant R13 I1/R152 a(molar fraction ratio of 35:65)with no ozone depletion potential and low global warming potential was proposed as a substitute for R134 a in automotive air conditioning.The computational models for the thermodynamic properties of R13 I1/R152 a were established by using the PR(Peng-Robinson)equation of state combined with the vdW mixing rule.Based on these models,the cycle performance of this working fluid was calculated,which was also compared with that of R134 a and R1234 yf under the different operating conditions.The results show that R13 I1/R152 a is a near azeotropic refrigerant whose temperature glide is approximately 0,and the saturated vapor pressure curve of which is equivalent to that of R134 a.Moreover,compared to R134 a,R13 I1/R152 a has an average 5.7%improvement in coefficient of performance as well as similar volumetric cooling capacity.The average coefficient of performance and volumetric cooling capacity of R13 I1/R152 a are significantly higher than those of R1234 yf by 13.8%and12.0%,respectively.However,the average discharge temperature of R13 I1/R152 a is approximately13.3 K higher than that of R134 a,but it is also within reasonable limits.Hence,the application of the proposed refrigerant R13 I1/R152 a in automotive air conditioning system is technically feasible.

The dressing method based on the 2×2 matrix■-problem is generalized to study the complex modified KdV equation(cmKdV).Through two linear constraint equations,the spatial and time spectral problems related to the cmKdV equation are derived.The gauge equivalence between the cmKdV equation and the Heisenberg chain equation is obtained.Using a recursive operator,a hierarchy of cmKdV with source is proposed.On the basis of the■-equation,the N-solition solutions of the cmKdV equation are obtained by selecting the appropriate spectral transformation matrix.Furthermore,we get explicit one-soliton and two-soliton solutions.

Knitted composites are textile composite materials that consist of knitted textile reinforcement and polymer matrix. Knitted composites exhibit great design flexibility by allowing the customization of shapes, textures, and material properties. These features facilitate the optimization of buildings’ material systems and the creation of buildings with light weight and high material efficiency.To achieve such a lightweight, material-efficient building structure with knitted composites, this research investigates the material properties of knitted composites and proposes a design process for building-scale knitted composite systems. In the material study, this research examines certain mechanical properties of the material and the effects of additional design elements. In the design exploration, this research explores the design workflow of the structural form, element arrangement, and knit distribution of the material system at the macro-, meso-, and microscales. The project of MeiTing serves as proof of the concept and the design workflow.
