Three Years Community-Based Intervention Program to Prevent Non-communicable Disease Risk Factors in Sousse, Tunisia

Aim： To evaluate the effectiveness of a community-based intervention program for NCD （non-communicable disease） risk factors prevention. Materials and methods： The authors conducted a quasi experimental study with a control group over five years between 2009 and 2014 in the region of Sousse, Tunisia. In each group, the sample size required was 1,000 participants. For this, the authors randomly selected 500 households from two areas and they included all adults aged 18 years and older in these households. They evaluated the habits of participants from both intervention and control groups at baseline in 2009-2010 and after three years of intervention in 2013-2014. They focused on the main lifestyle risk factors represented by poor diet, physical inactivity, and tobacco use. Results： Tobacco use decreased significantly among men by 7.3% （/9 = 0.03） in the intervention group, versus an increase of 3.5% in the control group （p = 0.4）. Physical activity and fruits and vegetables intake increased significantly respectively by 25% （p 〈 0.001） and 19% （p 〈 0.001） in intervention group. Conclusion： The results suggest that a combined community-based lifestyle intervention in a developing country, can significantly improve some NCD risk factors. However, a supportive environment or a gender approach is required to maximize the effectiveness and maintain the sustainability of the health intervention.

Relation of Nausea and Vomiting in Acute Myocardial Infarction to Location of Infarct

Nausea and vomiting occur frequently in patients with acute myocardial infarction （AMI）. To determine whether the incidence of nausea and vomiting in patients with AMI varies with infarct location, the authors studied 80 patients who had been admitted to the hospital for ST-segment elevation AMI. Data were prospectively collected from patients with AMI and nausea or vomiting for one year. Patients were enrolled if the diagnosis of AMI with nausea or vomiting is retained. The infarct location （i.e., inferior vs. anterior） in the patients with STEMI was determined using the established World Health Organization electrocardiographic criteria. Of the 80 patients included, nausea was reported in 44 patients （55%） and vomiting in 36 patients （45%）. The mean age was 58, 1 ± 11, 8 years old, and males comprised 62 patients （77.5%）. Inferior AMI was present in 47 patients （58.8%） and anterior AMI in 33 patients （41.2%）. The peak serum troponin I concentrations was significantly greater in those with anterior AMI than in those with inferior AMI respectively 0.52 ± 0.28 ng/mL and 0.50 ± 0.34 ng/mL. In conclusion, nausea and vomiting are common presenting symptoms in patients with either inferior or anterior wall AMI, but their frequency is unrelated to the infarct location.

Epilepsy Aspects and EEG Patterns in Neuro-Metabolic Diseases

Neurometabolic diseases (NMD) are a frequent cause of epilepsy in children. Epilepsy is more frequently part of a complex clinical picture than a predominant symptom and may be of different types and various EEG patterns. The primary goal of this article is, departing from a large personal series, to describe the seizure type, EEG patterns and response to antiepileptic drugs in NMD and to discuss clinical value of epilepsy type in the setting of specific NMD. We found epilepsy was associated to NMD in 43.1%. Disorders of energy metabolism were the most frequent cause of epilepsy (61.3%). We observed generalized epilepsy in 75% of the patients with partial epilepsy in 25%. EEG was abnormal in only 71% of cases with variable patterns. Resistance to antiepileptic drugs was observed in 75% of cases. Valproate acid was incriminated in seizure worsening in 22.7% of the patients, all of them affected by mitochondriopathies.

Localized bronchial Amyloidosis is an uncommon disease of unknown origin. Clinical signs are not specific. Tracheobronchial symptoms are the most frequent. We report a case of a 31 year-old man complaining of asthma-like dyspnea. Bronchoscopy was performed because of ineffectiveness of antiasthmatic treatment, showed a submucosal infiltration with stenosis of both right and left upper bronchi and a complete stenosis of intermediate troncus. Multiple biopsies were performed and concluded to Amyloidosis of AL type. Oral corticosteroids were indicated with clinical improvement.

Cost Effectiveness Analysis of Filgrastim versus Placebo in Post AIIogentic Bone Marrow Transplantation

Filgrastim is used to accelerate hematopoietic recovery after ABMT （allogeneic bone marrow transplantation）. Its impact on the total cost of patient care remains to be explored. We therefore undertook a cost effectiveness analysis in the context of a randomized single blinded clinical trial of Filgrastim versus placebo in post ABMT. A primary endpoint, duration of myelosuppression, and three secondary end points （number of days of fever, length of hospital stay, survival at one hundred days） were used to assess efficacy. Direct costs were evaluated and allowed the calculation of the ICER （incremental cost-effectiveness ratios） for the major endpoint of the trial. Sixteen patients were included in the study. The duration of myelosuppression was significantly decreased in the Filgrastim arm with medians of 15 days vs. 19 days in the placebo arm （p = 0.023）. Cost analysis showed no statistically significant difference between the two arms. According to the calculation of ICER, Filgrastim was more costly and more effective than placebo for the number of days of aplasia avoided and the number of days with fever avoided. Placebo strictly dominated filgrastim for days of hospitalization avoided. Filgrastim has proven effective in reducing the duration of aplasia without increasing costs.
