Gastroenteric tube feeding plays a major role in the management of patients with poor voluntary intake,chronic neurological or mechanical dysphagia or gut dysfunction,and patients who are critically ***,despite the benefits and widespread use of enteral tube feeding,some patients experience *** review aims to discuss and compare current knowledge regarding the clinical application of enteral tube feeding,together with associated complications and special *** conducted an extensive literature search on PubMed,Embase and Medline using index terms relating to enteral access,enteral feeding/nutrition,tube feeding,percutaneous endoscopic gastrostomy/jejunostomy,endoscopic nasoenteric tube,nasogastric tube,and refeeding *** literature showed common routes of enteral access to include nasoenteral tube,gastrostomy and jejunostomy,while complications fall into four major categories:mechanical,e.g.,tube blockage or removal;gastrointestinal,e.g.,diarrhea;infectious e.g.,aspiration pneumonia,tube site infection;and metabolic,e.g.,refeeding syndrome,*** the type and frequency of complications arising from tube feeding vary considerably according to the chosen access route,gastrointestinal complications are without doubt the most *** associated with enteral tube feeding can be reduced by careful observance of guidelines,including those related to food composition,administration rate,portion size,food temperature and patient supervision.

The relative value of education of emergency physicians in patient outcome: A retrospective analysis at a single center in developing India

BACKGROUND: There is a considerable paucity with regards to the research available on the quality and quantity of clinical teaching in the national emergency department(ED) setups. With the onset of the age of modern medicine, the outlook towards to the time worn tradition of triage and detailed medical evaluation must be revoked. Despite the variety of programs being conducted in the country, a comparable entity common to all is patients' clinical outcomes which can be measured using simple parameters which can be easily acquired compiling hospital registry ***: A retrospective observational study was conducted in the emergency department of Max Hospital, Saket, New Delhi. A period of 22 months prior to the start of the program and like-wise 22 months after initiation of the program was collected from the hospital registry. The Emergency Medicine program in consideration was the Masters in emergency Medicine(MEM) Program affiliated with George Washington University, NY, USA. Patients of all age groups and gender registering in the Emergency Department and so were all the doctors working in the ED before and after initiation of the ***: An improvement was noted in terms of total admissions through the ED per month, average length of stay of admitted as well as discharged patients; return to ED within 24 hours; leave against medical advice and patient complaints. A reduction was noted in number of discharges from the ED. Despite a numerical worsening on the patient's death in ED a graphical improvement can be noted considering the month wise representation of ***: We can make a coherent conclusion that there is an improvement in the outcome of the entire patient related aspects in the Emergency Department considering the all two time frames included in the study. Particles dispersion on fluid-liquid interfaces

This paper is concerned with the dispersion of particles on the fluid-liquid interface. In a previous study we have shown that when small particles, e.g., flour, pollen, glass beads, etc., contact an air-liquid interface, they disperse rapidly as if they were in an explosion. The rapid dispersion is due to the fact that the capillary force pulls particles into the interface causing them to accelerate to a large velocity. In this paper we show that motion of particles normal to the interface is inertia dominated; they oscillate vertically about their equilibrium position before coming to rest under viscous drag. This vertical motion of a particle causes a radially-outward lateral （secondary） flow on the interface that causes nearby particles to move away. The dispersion on a liquid-liquid interface, which is the primary focus of this study, was relatively weaker than on an air-liquid interface, and occurred over a longer period of time. When falling through an upper liquid the particles have a slower velocity than when falling through air because the liquid has a greater viscosity. Another difference for the liquid-liquid interface is that the separation of particles begins in the upper liquid before the particles reach the interface. The rate of dispersion depended on the size of the particles, the densities of the particle and liquids, the viscosities of the liquids involved, and the contact angle. For small particles, partial pinning and hysteresis of the three-phase contact line on the surface of the particle during adsorption on liquid-liquid interfaces was also important. The frequency of oscillation of particles about their floating equilibrium increased with decreasing particle size on both air-water and liquid-liquid interfaces, and the time to reach equilibrium decreased with decreasing particle size. These results are in agreement with our analysis.

Use of the liquisolid compact technique for improvement of the dissolution rate of valsartan

The aim of this study was to improve the dissolution rate of the poorly soluble drug valsartan by delivering the drug as a liquisolid *** compacts were prepared using propylene glycol as solvent,Avicel PH102 as carrier,and Aerosil 200 as the coating *** crystallinity of the newly formulated drug and the interaction between excipients was examined by X-ray powder diffraction and Fourier-transform infrared spectroscopy,*** dissolution studies for the liquisolid formula-tion and the marketed product were carried out at different pH *** results showed no change in the crystallinity of the drug and no interaction between *** dissolution efficiency of valsartan at 15 min was increased from 4.02% for plain drug and 13.58% for marketed product to 29.47% for the liquisolid *** increase in the dissolution rate was also found to be significant compared to the marketed product at lower pH values,simulating the gastric environment where valsartan is largely *** liquisolid technique appears to be a promising approach for improving the dissolution of poorly soluble drugs like valsartan.

TO THE EDITORWe read with great interest and a bit of surprise the article by Tonus et al. We would share herein our comments about it. The specificity of the fecal tumor M2 pyruvate kinase 0VI2-PK） reported by Tonus et al for diagnosing colorectal cancer （CRC） is 93%. This is considerably higher than in any of the previously published studies. In our work, the maximum specificity of fecal M2- PK is 78% （Table 1）. There is a vast difference of 15% between the previous studies and the current study of Tonus et al, which is difficult to explain. We are skeptical about such a high specificity reported by the authors.

Cellulose based polymers in development of amorphous solid dispersions

Cellulose derivatives have gained immense popularity as stabilizers for amorphous solid dispersion owing to their diverse physicochemical properties. More than 20 amorphous solid dispersion-based products that have been approved for marketing consist of cellulose derivatives as stabilizers, thus highlighting their importance in generation of amorphous solid dispersions. These polymers offer numerous advantages like drug solubilization, crystallization inhibition and improvement in release patterns of drugs. Exploring their potential and exploiting their chemistry and p H responsive behaviour have led to the synthesis of new derivatives that has broadened the scope of the use of cellulose derivatives in amorphous formulation development. The present review aims to provide an overview of different mechanisms by which these cellulose derivatives inhibit the crystallization of drugs in the solid state and from supersaturated solution. A summary of different categories of cellulose derivatives along with the newly explored polymers has been provided. A special segment on strengths, weaknesses, opportunities, and threats(SWOT) analysis and critical quality attributes(CQAs) which affect the performance of the cellulose based amorphous solid dispersion will aid the researchers in identifying the major challenges in the development of cellulose based solid dispersion and serve as a guide for further formulation development.

Determination of asenapine in presence of its inactive metabolites in human plasma by LC-MS/MS

A highly selective and sensitive liquid chromatography-tandem mass spectrometry （LC-MS/MS） assay has been described for the determination of asenapine （ASE） in presence of its inactive metabolites N-desmethyl asenapine （DMA） and asenapine-N-glucuronide （ASG）. ASE, and ASE 13C-d3, used as internal standard （IS）, were extracted from 300 μL human plasma by a simple and precise liquid-liquid extraction procedure using methyl tert-butyl ether. Baseline separation of ASE from its inactive metabolites was achieved on Chromolith Performance RPse （100 mm ×4.6 mm） column using acetonitrile- 5.0 mM ammonium acetate-10% formic acid （90：10：0.1, v/v/v） within 4.5 min. Quantitation of ASE was done on a triple quadrupole mass spectrometer equipped with electrospray ionization in the positive mode. The protonated precursor to product ion transitions monitored for ASE and ASE 13C-d3 were m/z 286.1 → 166.0 and m/z 290.0 → 166.1, respectively. The limit of detection （LOD） and limit of quantitation （LOQ） of the method were 0.0025 ng/mL and 0.050 ng/mL respectively in a linear concentration range of 0.050-20.0 ng/mL for ASE. The intra-batch and inter-batch precision （% CV） and mean relative recovery across quality control levels were 〈 5.8% and 87.3%, respectively. Matrix effect, evaluated as IS-normalized matrix factor, ranged from 1.03 to 1.05. The stability of ASE under different storage conditions was ascertained in presence of the metabolites. The developed method is much simpler, matrix free, rapid and economical compared to the existing methods. The method was suc- cessfully used for a bioequivalence study of asenapine in healthy Indian subjects for the first time.

Highly sensitive LC–MS/MS method to estimate doxepin and its metabolite nordoxepin in human plasma for a bioequivalence study

A selective, sensitive and rugged liquid chromatography–tandem mass spectrometry(LC–MS/MS) assay has been developed for the simultaneous determination of doxepin(Dox) and its pharmacologically active metabolite, nordoxepin(NDox) in human plasma. The analytes and their internal standards(IS)were extracted from 500 m L of human plasma by liquid-liquid extraction using methyl tert-butyl *** separation was achieved on Hypurity C8 column(100 mm ? 4.6 mm, 5 mm) using a mixture of acetonitrile-methanol(95:5, v/v) and 2.0 mM ammonium formate in 93:7(v/v) ratio. Detection was accomplished by tandem mass spectrometry in the positive ionization and multiple reaction monitoring acquisition mode. The protonated precursor to product ion transitions studied for Dox, NDox,and their corresponding ISs, propranolol and desipramine, were m/z 280.1-107.0, 266.0-107.0,260.1-116.1 and 267.1-72.1, respectively. A linear dynamic range of 15.0–3900 pg/mL for Dox and 5.00–1300 pg/mL for NDox was established with mean correlation coefficient(r2) of 0.9991 and 0.9993, respectively. The extraction recovery ranged from 86.6%–90.4% and 88.0%–99.1% for Dox and NDox, respectively. The intra-batch and inter-batch precision(% CV) across quality control levels was r 8.3% for both the analytes. Stability evaluated under different storage conditions showed no evidence of degradation and the % change in stability samples compared to nominal concentration ranged from 4.7% to12.3%. The method was successfully applied to a bioequivalence study of 6 mg doxepin hydrochloride orally disintegrating tablet in 41 healthy Indian subjects under fasting and fed conditions.

Does dyslipidemia worsen the hearing level in diabetics?

Objective: To identify the effect of dyslipidemia on auditory function detected by Pure Tone Audiometry. To check if dyslipidemia worsens the hearing level in ***: This was a comparative study where 120 subjects between the age group of 20 and 50 years underwent pure tone audiometry, lipid profile and blood sugars. Group 1 consisted of 30 subjects with type 2 diabetes and dyslipidemia; Group 2 had 30 subjects with isolated diabetes;Group 3 had 30 with isolated dyslipidemia and Group 4 included 30 normal subjects as ***: Significant hearing loss was seen only in the group with isolated diabetes(63%). The most common type of hearing loss was high frequency sensorineural hearing loss. When comparison was made between the combinations of different lipid profiles, no association was found to the level of ***: Diabetics are more prone to high frequency hearing loss. Altered lipid profile has no role in causing hearing loss.
